Purpose: This study aimed to investigate the effects of eccentric training applied to the calf muscles on muscle tone, muscle strength, and gait variables in patients with chronic stroke. Methods: Twenty-two participants were divided into experimental (n=12; eccentric training) and control (n=10; static stretching and stretching board) groups. The participants completed 30-minute physical therapy sessions five times a week for three weeks. Calf muscle tone, muscle strength, and gait variables were measured using MyotonPRO, a hand-held dynamometer, and Optogait, respectively, before and after each intervention. Results: Two-way analysis of variance (ANOVA) indicated a significant interaction effect between measurement points and groups in frequency, stiffness, and decrement of the lateral gastrocnemius, medial gastrocnemius, and soleus muscles (p<.05). Paired t-tests showed that the experimental group exhibited significantly decreased frequency and stiffness scores for the lateral gastrocnemius, medial gastrocnemius, and soleus muscles (p<.05), as well as significantly increased decrement and muscle strength scores, gait speed, step length, and stride length (p<.05). Conclusion: The application of eccentric training to the calf effectively reduced muscle tone, increased muscle strength, and improved the gait speed, step length, and stride length of patients with chronic stroke.
During the deep dissection of the front of the forearm, an anomalous accessory muscle in relation to the flexor digitorum profundus (FDP) muscle was observed in the right forearm. The accessory muscle consisted of a spindle-shaped muscle belly with a long tendon underneath the flexor pollicis longus muscle. When followed distally, the accessory muscle tendon was found lateral to the FDP tendon for the index finger and entered the palm deep to the flexor retinaculum. In the palm, we encountered the first lumbrical muscle as a bipennate muscle taking origin from the adjacent sides of the middle of the tendons of FDP and accessory muscle tendon. After giving origin to first lumbrical muscle, the accessory muscle got merged with the tendon of FDP for index finger. Understanding this kind of variation is required for radiologists and hand surgeons for diagnostic purposes and while performing corrective surgical procedures.
Abdominal muscle plays a crucial role in postural control and respiration control. However, thickness of abdominal muscle in the paretic side of a hemiplegic patient has not been reported in previous studies. The purpose of this research was to compare lateral abdominal muscle thickness between the nonparetic and paretic side in patients with chronic stroke using rehabilitative ultrasound imaging. Twenty two patients with chronic stroke participated in this study. Absolute thickness of transversus abdominis (TrA), internal oblique (IO) and external oblique (EO) was measured at the end of inspiration and expiration during quiet breathing, and relative thickness was calculated (thickness of each muscle as a percentage of total muscle thickness). Ultrasound imaging was recorded three times and the average value was determined for statistical analysis. Differences in absolute and relative lateral abdominal muscle thickness between the nonparetic and paretic side were assessed with paired t-tests. Absolute muscle thickness of the paretic side TrA was thinner than that of the nonparetic side at the end of inspiration and expiration during quiet breathing. Relative muscle thickness of the paretic side TrA was thinner than the paretic side only at the end of expiration during quiet breathing (p>.05). Therefore, it is necessary to strength TrA in patients with chronic stroke during physical therapy intervention. Further study is needed whether physical therapy intervension will induce TrA thickness in patients with chronic stroke in prospective study design.
This study was performed to investigate the influence of lateral guidance pattern and chewing pattern on masticatory muscle activity and occlusal contact pattern. Twenty-five patients with temporomandibular disorders and thirty students without temporomandibular disorders were selected for this study. Electromyographic examination and occlusal contact examination were performed simultaneously with Bio-Pak System (Bioresearch Inc.,U.S.A.) and T-Scan System(Tekscan Co., USA). The obtained results were as follows : 1. The EMG activity of masseter and anterior temporal muscle in patient group with canine guidance was higher than with non-canine guidance. but there was no significant difference (p>0.05). 2. The EMG activity of anterior temporal muscle in normal group with non-canine guidance was higher at preferred chewing side than at opposite side (p<0.05). 3. The EMG activity of masseter muscle was higher in normal group than in patient group (p<0.05). 4. The number of occlusal contact in patient group with canine guidance was higher than with non-canine guidance, but there was no significant difference (p>0.05). 5. The occlusal contact force at preferred chewing side was higher in patient group than in normal group(p<0.05).
This study is to provide details of the unique anatomical features on the alar lobule region in Korean nose. We hypothesized that the anatomy of this area differs according to the shape of the alar lobule. Based on the prominence and roundness of alar lobules, they were classified into horizontal and vertical types. A total of 20 fresh cadaver noses(10 for each type) were dissected. The anatomical differences between the horizontal and vertical types were investigated by gross and histologic studies. The alar lobule is composed of three layers, i.e., external skin, muscle, and vestibular skin. Profound differences between the two alar lobule types were evident in terms of the volume of the dilator naris anterior muscle, the insertion of the dilator naris posterior muscle, and the thickness of the external skin at the lateral end of the alar circumference. The horizontal type has a greater volume of dilator naris anterior muscle, an additional insertion of the dilator naris posterior muscle, and thicker external skin at the lateral end of the alar circumference than the vertical type. The Korean nose differs anatomically and morphologically from the Caucasian nose. This study shows that there are anatomic differences between the horizontal and vertical types of alar lobules in Korean nose.
The present experiment was carried out, in the rabbit and cat, in order to explore functional interrelationship between the vestibular semicircular canals and extraocular muscles, which are involved in the vestibulooculomotor reflex as the receptor and effector organ respectively. Semicircular canals were subjected to electrical stimulation, lymphatic fluid flow or acute freezing, and responses of the extraocular muscles were recorded in terms of changes in electromyographic activity and isometric tension. Electrical stimulation of a unilateral canal elicited contraction of the superio-medial muscle group (superior oblique, superior rectus and medial rectus muscles) in the ipsilateral eye and the inferio-lateral muscle group (inferior oblique, inferior rectus and lateral rectus muscles) in the contralateral eye. Thus a simple and distinct axiom was found in the pattern of the reflex-response of the extraocular muscles. Inhibition of the unilateral canals elicited the extraocular muscle responses contrary to those observed by excitation of the canal. Based on the present experimental results, it was demonstrated that the functional interrelations between the semicircular canals and extraocular muscles are rather equivalent in the frontal eyed cats (with binocular vision) and lateral eyed rabbits (with monocular vision). Therefore the previous thesis that the vestibuloocular relations vary from species to species awaits experimental reevaluation.
The purpose of this study is to report the treatment effects of Korean medicine for patients with lateral epicondylitis. This study was done on 12 cases of patients with lateral epicondylitis who visited in Korean Medicine Clinic of Ganghwa-gun Public Health Care Center. We used muscle contraction/relaxation strengthen technique, Instrument assisted soft tissue mobilization (IASTM), acupuncture on patients and mesured numerical rating scale (NRS), pain disability index (PDI), and grip strength test to evaluate treatment effects. Among twelve patients, eleven cases showed decreasing in NRS and all cases showed decreasing in PDI. Ten cases showed increasing in grip strength test. Korean medical treatments including muscle contraction/relaxation strengthen technique and IASTM are thought to be one of the effective treatment for patients with lateral epicondylitis.
Purpose: The purpose of this study was to describe the adjustment process of patients with Amyotrophic Lateral Sclerosis(ALS). Method: The data were collected from May 2007 to February 2008 through individual in-depth interviews with 4 ALS patients. The data collection and analysis were performed according to grounded theory methodology, as suggested by Strauss and Corbin. Results: 'Accepting the sick role' was the central phenomenon, which was derived from 'perceiving an intial symptom'. Therefore, the adjustment pattern was represented by using the different strategies, which were 'raising hope' and 'hoping to dye comfortably'. For taking a view of the future, ALS patients adopted the strategies of 'living positively', 'being treated hardly', 'joining in the experience', 'depending on the absolute being', 'recognizing the dying process'. Conclusion: This study provides guidance for the development of nursing interventions for patients with ALS.
Background: Motor unit number estimation (MUNE) can directly assess motor neuron populations in muscle and quantify the degree of physiologic and/or pathologic motor neuron degeneration. A high degree of reproducibility and reliability is required from a good quantitative tool. MUNE, in various ways, is being increasingly applied clinically and statistical MUNE has several advantages over alternative techniques. Nevertheless, the optimal method of applying statistical MUNE to improve reproducibility has not been established. Methods: We performed statistical MUNE by selecting the most compensated compound muscle action potential (CMAP) area as a test area and modified the results obtained by weighted mean surface-recorded motor unit potential (SMUP). Results: MUNE measures in amyotrophic lateral sclerosis (ALS) patients showed better reproducibility with sizeweighted modification. Conclusions: We suggest size-weighted MUNE testing of "neurogenically compensated"CMAP areas present an optimal method for statistical MUNE in ALS patients.

PURPOSE: This study examined the effects on the upper extremity function, muscle strength, and hand function of a task-oriented training approach using a percussion instrument for patients with chronic stroke. METHODS: Twenty-four chronic stroke patients accompanied with upper extremity hemiplegia were selected for research and were classified randomly into 12 experimental groups and 12 control groups. The experimental group performed a task-oriented approach, and the control group performed upper extremity occupational therapy. Stroke upper extremity test, Jebsen-Taylor Hand Function test, upper extremity muscle strength test, and hand muscle strength test were measured before and after training in the evaluation process. RESULTS: In the upper extremity test and Jebsen-Taylor test, there were no significant differences between the groups. In the upper extremity muscle strength test, there were significant differences in shoulder flexion, internal rotation, and elbow flexion in the experimental group. In the hand muscle strength test, there were significant differences in the grip, tip Pinch, lateral Pinch, and 3-jaw chuck in the experimental group and significant differences in only grip, tip pinch, and lateral pinch in the control group. In addition, there were significant differences in the lateral pinch compared to the amount of change. CONCLUSION: Task-oriented approach using percussion instruments for upper extremity rehabilitation in stroke patients is effective in the upper extremity function and strength, hand function, and strength.
