  • Posttransplant lymphoproliferative disease(PTLD) has emerged as a potential life-threatening complication of immunosuppressive therapy after organ transplantation. The occurrence of PTLD is usually associated with an Epstein-Barr virus(EBV) infection in patients who are treated by aggressive immunosuppressive therapy. PTLD is represented by diverse manifestations ranging from reactive lymphoid hyperplasia to high grade malignant lymphoma. This is a case report of a late PTLD in a child. The patient is a 14-year-old girl, who presented as malignant lymphoma 44 months after successful renal transplantation. There was no evidence of EBV infection. On bone marrow study, many neoplastic lymphoid cells were defected. Aggressive chemotherapy for PTLD had resulted in clinical remission. However the patient expired from uncontrolled sepsis and septic shock after 77 days.

Continuous Renal Replacement Therapy in a 4-year-old Child with Rhabdomyolysis Following Parainfluenza Virus Infection and Hyperammonemia due to Isovaleric Acidemia (Parainfluenza virus 감염 후 발생한 횡문근융해증과 isovaleric acidemia로 인한 고암모니아혈증을 가진 소아에서의 지속적 신대체요법)

  • Parainfluenza virus infection is one of the causes of fatal rhabdomyolysis. Rhabdomyolysis can be aggravated by mitochondrial fatty acid ${\beta}$-oxidation disorders during prolonged periods of fasting. Moreover, in patients with late-onset isovaleric acidemia, hyperammonemia may occur following catabolic stress. In the present report, we describe a case of a 4-year-old boy with parainfluenza virus infection and late-onset isovaleric acidemia that rapidly progressed to coma, seizures, and cardiorespiratory collapse. His serum ammonia and creatinine kinase (CK) levels were $385{\mu}Mol/L$ and 23,707 IU/L, respectively. Continuous renal replacement therapy (CRRT) was initiated using continuous venovenous hemodiafiltration, after which the ammonia and CK levels returned to normal. Thus, we recommend the immediate initiation of CRRT in the management of patients with life-threatening rhabdomyolysis and hyperammonemia.

Assessment of the Availability of Health Insurance Data for Epidemiologic Study of Childhood Aseptic Meningitis (소아 무균성 뇌막염의 역학적 연구를 위한 건강보험자료원의 유용성 평가)

  • Objectives : Aseptic meningitis is a major cause of Korean childhood morbidity late spring and early summer. However, the nationwide incidences of the disease have not been reported. This study was conducted to evaluate the availability of National Health Insurance data (NHID) for the study of an epidemiological trend in the surveillance of aseptic meningitis in children. Methods : All the claims, under A87, A87.8, and A87.9 by ICD-10, among children below 15 years of age, to the National Health Insurance Corporation, between January and December 1998, were extracted. A survey of the medical record of 3,874 cases from 136 general hospitals was peformed. The availability of the NHID was evaluated by the three following methods: 1) The diagnostic accuracy (the positive Predictive value : proportion of the confirmed aseptic meningitis among the subjects registered as above disease-codes in NHID) was evaluated through a chart review, and according to age, gender, month and region of disease-occurrence. 2) The distribution of confirmed cases was compared with the distribution of total subjects from the NHID, for subjects in General hospitals, or the subjects surveyed. 3) The proportion of confirmed CSF test was confirmed, and the relating factor, which was the difference in CSF-test rate, analyzed. Results : Among 3,874 cases, CSF examinations were peformed on 1,845 (47.6%), and the CSF-test rates were different according to the medical utility (admission vs. OPD visit) and the severity of the symptoms and signs. The diagnostic accuracy for aseptic meningitis, and during the epidemic (May-Aug) and sporadic (Sept-Apr) periods, were 85.0 (1,568/1,845), 86.0 (1,239/l,440) and 81.2% (329/405), respectively. The distributions by age, sex, month or period (epidemic/sporadic) and region, in the confirmed cases, were similar to those in the NHID, in both the subjects at General hospitals and in those surveyed, to within ${\pm}7%$. Conclusions : In this paper, the NHID for the subjects registered with an aseptic meningitis disease-code might be available for an epidemiological study on the incidence-estimation of childhood aseptic meningitis, as the NHID could include both the probable and definite cases. On the basis of this result, further studies of time-series and secular trend analyses, using the NHID, will be peformed.

A New Reference Range of Serum Anion Gap (혈청 Anion gap의 새로운 범위에 관한 연구)

  • Purpose: The old reference range of serum anion gap(AG) may be excessive compared with value measured by new electrolyte analyzers. Therefore, we studied to establish a new reference range of AG using an autoanalyzer. Methods: With the use of analyzer(Hitachi 747 by enzymatic methods), serum Na and Cl were measured, and with the use of analyzer(CX-3 by differental rage pH), serum $TCO_2$ was measured. We measured AG(=Na-($Cl+HCO_3$)) in 395 stable patients with normal serum albumin and creatinine levels of the pediatric in-patients and out-patients for preoperative examination from march 1997 to July 1997. Results: The normal serum$ AG(mean{\pm}SD)$ were neonate, $11.2{\pm}3.2$ mEq/L; infancy, $11.8{\pm}2.7$ mEq/L; early childhood, $12{\pm}2.7$ mEq/L; late childhood, $11.7{\pm}3.2$ mEq/L; adolescence, $9.6{\pm}2.7$ mEq/L; adult, $9.0{\pm}2.7$ mEq/L. Normal serum AG in more than 10 years of age was significantly lower than the previous normal value and also the difference of AG between more than 10 years and less than 10 years was statistically significant(P<0.05). Conclusions: We suggest to measure serum AG according to each type of analyzers.

Juvenile idiopathic arthritis: Diagnosis and differential diagnosis

  • Juvenile idiopathic arthritis (JIA) is comprised of a heterogeneous group of several disease subtypes that are characterized by the onset of arthritis before the age of 16 years and has symptoms lasting at least 6 weeks. The previous classification of JIA included seven different categories, whereas its current classification was compiled by the International League of the Association for Rheumatology, and replaced the previous terms of "juvenile chronic arthritis" and "juvenile rheumatoid arthritis," which were used in Europe or North America, respectively, with the single nomenclature of JIA. As mentioned above, JIA is defined as arthritis of unknown etiology that manifests itself before the age of 16 years and persists for at least 6 weeks, while excluding other known conditions. The clinical symptoms of JIA can be quite variable. Several symptoms that are characteristic of arthritis are not necessarily diagnostic of JIA and may have multiple etiologies that can be differentiated with careful examination of patient history. The disease may develop over days or sometimes weeks, thereby making the diagnosis difficult at the time of presentation. To make a clinical diagnosis of JIA, the first step is to exclude arthritis with known etiologies. Of note, late treatment due to excessive delay of diagnosis can cause severe damage to joints and other organs and impair skeletal maturation. Therefore, early detection of JIA is critical to ensure prompt treatment and to prevent long-term complications including the likelihood of disability in childhood.

The Effects of Paternal and Maternal Control on Self-Esteem in School-Age Boys and Girls : The Mediating Role of Autonomy (부와 모의 통제가 남녀 아동의 자아존중감에 미치는 영향 : 자율성의 매개적 역할)

  • This study examined the effects of parental control and children's autonomy on self-esteem, using a sample of 415 school-age children (208 boys and 207 girls) recruited in Seoul. Ordinary Least Square regressions revealed a variety of gender-based differences in the associations among these three variables. Boys with high maternal behavioral control had high levels of self-esteem, whereas girls under low paternal psychological and high behavioral control reported high levels of self-esteem. Boys with high maternal behavioral control and girls with high paternal behavioral control also reported high levels of autonomy. Autonomy was found to exert a positive impact on self-esteem for both boy and girls. Autonomy played a mediating role in the relationships between maternal behavioral control and self-esteem for boys, and between paternal behavioral control and self-esteem for girls. These findings highlight the differential influences of psychological control and behavioral control on autonomy and self-esteem, as well as the relative impact of the opposite sex parent on the development of autonomy and self-esteem in late childhood.

A Structural Model for the Practice of Life Safety Behavior in School-age Children (학령기 아동의 안전생활 실천행동에 관한 구조모형)

  • Purpose: This study is an examination of the paths in which the primary factors of anxiety, impulsiveness, knowledge of life safety practice, attitudes towards life safety practice, interpersonal support, and self-efficacy from Pender's Health Promotion Model influence the practice of life safety behavior in school-age children. Methods: The sample consisted of 489 5th and 6th grade students recruited from five elementary schools in Seoul City and four provinces, South Korea. Data were analyzed using descriptive statistics, correlations, factor analysis, and structural equation modeling. Results: Attitudes towards life safety practice, interpersonal support, self-efficacy and impulsiveness directly influenced practice of life safety behavior. Anxiety did not have a direct influence on practice of life safety behavior, but indirectly affected it. In this modified model, 52.0% of the practice of life safety behavior was explained by the primary factors. Conclusion: To facilitate the practice life safety behaviors in late childhood, a positive attitude towards life safety needs to be developed along with decreasing impulsiveness and enhancing self-efficacy.

Elementary School Children's Perception of Proverbs and Characteristic of the Perceived Proverbs (초등학교 아동의 속담인지수준과 인지속담의 특성)

  • The purpose of this study is to explore the number and type of proverbs known by elementary school children and explain what variables might influence the proverbs' perception. From Seoul and Chonbuk areas, 632 forth, fifth, and sixth graders at elementary school participated. They completed an open-ended questionnaire asking them to list as many proverbs as they know and probing their experience in the use of proverbs with grandparents, parents, and peers. The results showed that the participants listed a total of 185 proverbs, at an average of six proverbs each, and that 63% were a concrete proverb consisting of only concrete and visible nouns. Children reported relatively different ratios of concrete/abstract/complex proverbs according to their grade. Sixth graders perceived more proverbs which included nouns than other graders. Multiple regression analysis revealed that children's grade and using proverbs with peers influenced their proverb perception. These results may suggest a possibility of relationship between figurative language and cognitive development related to thinking of late school-age children.

  • Bartter syndrome is a rare disorder characterized by the association of hypokalemic hypochloremic metabolic alkalosis, hyperreninemia, hyperaldosteronemia, short stature and nephrocalcinosis. This disorder presents with hyperplasia of juxtaglomerular apparatus on renal biopsy. We experienced a case of late-onset Bartter syndrome with nephrocalcinosis in a 9-year-old boy, whose chief pictures were muscle weakness, short stature, persistent sterile pyuria and microscopic hematuria. We report this case with a brief review of related literatures.

The Effect of Mother's Facilitative Communication on Children's Social Adjustment and the Mediating Effect of Self-Regulation (어머니의 촉진적 의사소통이 아동의 사회적 적응에 미치는 영향과 자기조절 능력의 매개효과)

  • This study investigated the relationship between maternal facilitative communication perceived by children and the social adjustment which was measured as pro-social behaviors and aggressive behaviors in children. Special attention was given to the mediating effect of the child's self-regulation on the relationship. The subjects, selected by convenience sampling method, were 193 fourth and fifth grade elementary school students. Data was analyzed through Pearson's correlation, reliability analysis, and multiple regression using the SPSS 12.0. The findings of the study were as follows. First, the mother's facilitative communication perceived by the child was positively associated with the pro-social behavior, while, it was negatively correlated with the aggressive behavior of the child. Second, the mother's facilitative communication had direct effect on the prosocial behavior and the aggressive behavior of the child. Third, the direct impact of facilitative communication on social adjustment was mediated by the self-regulation variable. Based on these findings, this study proposes strategies for the improvement of children's adjustment during late childhood.
