옹벽파괴에 의한 사면붕괴 사례연구 (A case study on the landslide resulted from earth retaining wall failure)

  • This paper presents the example of landslide triggered by the failure of earth retaining wall. Close examinations such as visual inspections and non-destructive testings revealed that the earth retaining wall does not have enough strength to resist active earth pressure and ground water pressure. This fact is proved to be a direct initiation of landslide. Numerical studies including slope stability analyses and seepage analyses were performed with material properties obtained by geophysical explorations and laboratory tests. The results of numerical studies show that the overturning of the earth retaining wall affects the slope stability, leading to landslide consequently.

Strongyloidiasis associated with amebiasis and giardiaisis in an immunocompetent boy presented with acute abdomen

  • Strongyloides stercoralis (SS) is an intestinal nematode that is mainly endemic in tropical and subtropical regions and sporadic in temperate zones. SS infection frequently occurs in people who have hematologic malignancies, HIV infection and in individuals undergoing immunosuppressive therapy. In this study, we report a 12year-old immunocompetent boy who was admitted to our hospital with acute abdomen. Laboratory evaluation showed strongyloidiasis, amebiasis and giardiasis. Clinical and laboratory findings immediately improved with albendazole therapy. Therefore, when diarrhea with signs of acute abdomen is observed, stool examinations should be done for enteroparasitosis. This approach will prevent misdiagnosis as acute abdomen. Complete clinical improvement is possible by medical therapy without surgical intervention.

Prognostic Significance of Basic Laboratory Methods in Non-Small-Cell-Lung Cancer

  • Background: In our study, the LDH, albumin, hemoglobin, neutrophile, thrombocyte, lymphocyte counts and prognostic significance of neutrophile-lymphocyte and thrombocyte-lymphocyte ratios in NSCLC derived from these counts obtained during regular examinations of patients were examined. Materials and Methods: Histopathologically diagnosed non-small-cell-lung cancer patients between 2008 and 2010 were included in the study. Before the treatment, full blood count including routine lymphocyte count, blood biochemistry examinations including liver (AST, ALT, total protein, Albumin), LDH and kidney (BUN, Cre) function tests were performed. Results: A total of 156 patients, 76 of whom (48.7%) were female and 80 of whom (51.3%) were male were included. Mean hemoglobin level was determined as 12. Overall survival was found to be significantly dependent on whether patients were anemic or not (p: 0.005). Mean LDH level was determined as 233.4. There was nosurvival difference between patients with and without high LDH (p: 0.532). In patients where NLR showed systemic inflammatory response, overall survival was 10.8 months whereas this duration was 19.6 months in patients where the systemic inflammatory response was negative (p: 0.012). In patients where TLR showed systemic inflammatory response, overall survival was 13.6 months whereas this duration was 21.9 months in patients where the systemic inflammatory response was negative (p: 0.04). Conclusions: Molecular methods have been changing rapidly in today's world and they manage the treatment besides defining the prognosis of patients. However, easily accessible and cheap laboratory parameters should be considered in the prognosis of patients besides these new methods.

Estimating Organ Doses from Pediatric Cerebral Computed Tomography Using the WAZA-ARI Web-Based Calculator

  • Background: The use of computed tomography (CT) device has increased in the past few decades in Japan. Dose optimization is strongly required in pediatric CT examinations, since there is concern that an unreasonably excessive medical radiation exposure might increase the risk of brain cancer and leukemia. To accelerate the process of dose optimization, continual assessment of the dose levels in actual hospitals and medical facilities is necessary. This study presents organ dose estimation using pediatric cerebral CT scans in the Kyushu region, Japan in 2012 and the web-based calculator, WAZA-ARI (https://waza-ari.nirs.qst.go.jp). Materials and Methods: We collected actual patient information and CT scan parameters from hospitals and medical facilities with more than 200 beds that perform pediatric CT in the Kyushu region, Japan through a questionnaire survey. To estimate the actual organ dose (brain dose, bone marrow dose, thyroid dose, lens dose), we divided the pediatric population into five age groups (0, 1, 5, 10, 15) based on body size, and inputted CT scan parameters into WAZA-ARI. Results and Discussion: Organ doses for each age group were obtained using WAZA-ARI. The brain dose, thyroid dose, and lens dose were the highest in the Age 0 group among the age groups, and the bone marrow and thyroid doses tended to decrease with increasing age groups. All organ doses showed differences among facilities, and this tendency was remarkable in the young group, especially in the Age 0 group. This study confirmed a difference of more than 10-fold in organ doses depending on the facility and CT scan parameters, even when the same CT device was used in the same age group. Conclusion: This study indicated that organ doses varied widely by age group, and also suggested that CT scan parameters are not optimized for children in some hospitals and medical facilities.

Predictors of massive transfusion protocols activation in patients with trauma in Korea: a systematic review

  • Purpose: Massive transfusion protocols (MTPs) implementation improves clinical outcomes of the patient's resuscitation with hemorrhagic trauma. Various predictive scoring system have been used and studied worldwide to improve clinical decision. However, such research has not yet been studied in Korea. This systematic review aimed to assess the predictors of MTPs activation in patients with trauma in Korea. Methods: The PubMed, Embase, Cochrane Library, Research Information Sharing Service databases, KoreaMed, and KMbase were searched from November 2022. All studies conducted in Korea that utilized predictors of MTPs activation in adult patients with trauma were included. Results: Ten articles were eligible for analysis, and the predictors were assessed. Clinical assessments such as systolic and diastolic blood pressure, shock index (SI), prehospital modified SI, modified early warning system (MEWS) and reverse SI multiplied by the Glasgow Coma Scale (rSIG) were used. Laboratory values such as lactate level, fibrinogen degradation product/fibrinogen ratio, and rotational thromboelastometry (ROTEM) were used. Imaging examinations such as pelvic bleeding score were used as predictors of MTPs activation. Conclusions: Our systematic review identified predictors of MTPs activation in patients with trauma in Korea; predictions were performed using tools that requires clinical assessments, laboratory values or imaging examinations only. Among them, ROTEM, rSIG, MEWS, SI, and lactate level showed good effects for predictions of MTPs activation. The application of predictors for MTP's activation should be individualized based on hospital resource and skill set, also should be performed as a clinical decision supporting tools.

근로자건강진단 폐활량검사에서 새로운 신뢰성기준 적용 결과 (New Reliability Criteria for Korean Workers' Health Examination Spirometry Results)

  • 산업안전보건연구원에서는 현재 2005년 미국흉부학회(American Thoracic Society, ATS)와 유럽호흡기학회(European Respiratory Society, ERS)의 폐활량검사 표준화가이드를 준용하여 근로자건강진단 폐활량검사를 평가하고 있으며, 2019년 ATS/ERS가이드의 적용을 검토하고 있다. 새로운 평가기준에 따른 결과를 현재 결과와 비교해보고 우리나라 근로자건강진단에 적용하는 것이 적절한지 확인하기 위해 특수건강진단기관으로부터 수집한 325건의 폐활량검사 결과를 검토하였다. 외삽용적, 보정오차, FIVC 등의 평가기준을 더 엄격하게 적용하였음에도 검사의 신뢰성 점수가 상승한 기관이 더 많았다. 주요원인은 FEV1, FVC의 적합성과 재현성을 분리하여 판단함과 동시에 점수를 ATS 등급에 따라 차별 부여하므로 감점 폭이 감소했기 때문으로 파악된다. 새로운 기준을 적용하면 현재 부적합하다고 판단하는 자료의 활용이 가능하므로 검사자와 판정의사의 결과선별과 해석에 대한 이해도를 높인다면 검사자와 수검자 모두가 검사에 대한 부담감을 줄이면서 신뢰성 있는 결과를 얻을 수 있을 것으로 기대한다.

산부인과 포괄수가제 도입에 따른 일개 대학병원의 진료행태 변화 모의실험 연구 (A study of Simulations on the Changes of Physician's Practice Patterns in University Hospitals after the Introduction of DRG in Obstetrics and Gynecology)

  • 본 연구는 2013년 7월부터 대학병원에서도 시작되는 산부인과 포괄수가제를 대비하여 일개 대학병원 산부인과에서 2012년 4월부터 2012년 9월까지 6개월 동안 산부인과 DRG에 속한 건강보험 환자군을 대상으로 진료행태의 변화를 모의실험을 통해 재원일수의 변화 가능성, 의료 서비스 제공량의 전이와 변화, 항생제를 포함한 약 주사 사용 및 각종 검사 등 항목별 처방량의 변화, 진료재료의 단가 조정 및 대체 효과의 가능성을 예측하였다. 약제비가 가장 변화폭을 보였으며 진료재료와 검사의 변화가 그 뒤를 이었다. 의료 서비스량 분석에서 재원일수의 단축은 의료비용의 감소로 혹은 병원의 수익성을 증가시킬 수 있는 요인으로 판단된다. 이러한 결과로 향후 산부인과에서 DRG 질병군에 대한 표준진료지침 개발에 기초 자료로 사용할 수 있을 것으로 사료되며, 진료의 효율성을 극대화시키고 비효율적인 병원자원 소모를 감소시켜 의료비용 절감 등 의료수익을 최대한 증대시키는 경영전략수립의 기초 자료 토대 마련이 가능할 것으로 사료된다.

강원도내 광견병 발생실태 (Report on The Incidence of rabies in Kangwon-do)

  • During the period of 2 years from 1993 to 1994, epidemiological investigation were carried out each time the outbreak of rabies suspected cases were informed and all specimens were transported to the Kangwon-do Veterinary Service Lab. At the Lab., autopsy and laboratory examinations were conducted as well. The results obtained as follows 1. The dogs and cattles were confirmed to be infected with rabies virus by routine histopathological findings, indirect flulrescent antibody technique, and mouth inoculation test. 2. The total numbers of rabies occured in dogs and cattles were 9, 5 heads, respectively. The regional distribution of total of 14 heads were 9 heads in Chulwon, 2 heads in Hwachun, 1 head Yangku, 2 heads in Inje and the highest number of outbreaks were orrured in Chulwon. 3. Most of dogs showed furious type and dumb type found in only one dog. 4. Cattles showed the symtoms of uneasiness feeling, excitement, disturbance howling, the change of voice, sexual accelaration, foreign body swatting and attacked to human and ani-mal and found to be died after two or four days. 5. In the seasonal outbreaks of rabies, 8 heads of the highest incidence were occured in win-ter followed by 4 heads in autumn, 1 head in spring and 1 head in summer. 6. The first outbreaks of rabies was reported in Chulwon and then transmitted into Hwachun, Yangku and Inje in turn. 7 The infectious source was supposed to be wild animals such as laccon dog, badger etc. living around the D. M. Z.

Interaction of Microtubule-associated Protein 1B Light Chain(MAP1B-LC1) and p53 Represses Transcriptional Activity of p53

  • The tumor suppressor and transcription factor p53 is a key modulator of cellular stress responses, and can trigger apoptosis in many cell types including neurons. In this study, we have shown that Microtubule-associated protein 1B(MAP1B) light chain interacts with tumor suppressor p53. MAP1B is one of the major cytoskeletal proteins in the developing nervous system and essential in forming axons during elongation. We also demonstrate that both p53 and MAP1B-LC1 interact in the nucleus in HEK 293 cells. Indeed, we show that the MAP1B-LC1 negatively regulates p53-dependent transcriptional activity of a reporter containing the p21 promoter. Consequently, MAP1B light chain binds with p53 and their interaction leads to the inhibition of doxorubicin-induced apoptosis in HEK 293 cells. Furthermore, these examinations might be taken into consideration when knock-down of MAP1B-LC1 is used as a cancer therapeutic strategy to enhance p53's apoptotic activity in chemotherapy.

Classical swine fever disease in Cheolwon

  • Two cases of classical swine fever (CSF) disease have broken out in Cheolwon (7 April, 2002). The suspected pig herds were huddled together because of high fever (over $40^{\circ}C$) and showed remarkable decrease of the leukocytes. The staggering gait related to posterior weakness, constipation and lethargy, hyperemia, hemorrhagic lesions (on the skin, muzzle, ears, limbs, tail and inner part of legs) and conjunctivitis with dirty streaks below the eyes were observed. The inflammation in the lung, infarction in the spleen, swelling and hemorrhage in lymph nodes, kidney, intestine, heart and cheese like purulent inflammation of the tonsil were observed. The ulcers of the colon were also detected. Several clinical and laboratory techniques including blood test, histo-pathological examinations, indirect fluorescent antibody (IFA) test and RT-PCR test were applied to diagnose the disease. Inoculation test on PK-15 cell was also performed. The necrosis of the lymphatic cells and infiltration of the vessel circumferential cells in the brain and lymph organs were commonly viewed. The proliferation of the glia cell (gliosis) in the lymph was particular. Cytopathogenic effect (CPE) and specific fluorescent-bright-green areas (with IFA) appeared in PK-15 cells inoculated with suspected blood plasma. The IFA test on the epithelial and mucous membrane cells of tonsil was positive. RT-PCR technique required more working hours and labor than other techniques in this examination but it was useful because of the sensitivity to the CSF viral gene.
