Effects of a Self-Help Program including Stretching Exercise on Reduction of Symptom in Patients with Fibromyalgia (신장운동을 포함한 자조관리프로그램이 섬유조직염환자의 증상완화에 미치는 효과)

  • This study was an quasi-experimental study, done to identify factors Influencing the reduction of symptoms in patients with fibromyalgia. The primary purpose of the study was to develop a Self-Help Program suitable for patients with fibromyalgia in Korea. The secondary purpose was to identify the effects of a Self-Help Program which included stretching exercise. This study was carried out between Feb. 24 and July 8, 1997 and patients in the study Included out patient diagnosed with fibromyalgia based on the criteria developed by the American College of Rheumatology(1990) and H, University which is a tertiary patient care clinic for Rheumatism. The experimental group included 38 patients who were residents of Seoul or Kyungi province, and a control group of 38 patients who were residents of other areas. The control patients were matched to the experimental group patients and they were selected considering the number of tender points on the Fibromyalgia Impact Questionnaire score and a score of self-efficacy. The experimental group participated in a Self-Help Program based on the American Arthritis Foundation(1995) guidelines. The program participants participated in a small group which consisted of 12-15 members attending the program once a week, for 6 weeks with each program lasted two to two and a half hours. The stretching exercise was carried out in each patient's home every day following the video tape exercise provided by the researcher, and the researcher provided encouragement and concern to the patients by calling them once a week. The number times the exercise was performed was divided by the number of participants to calculate the percentage of performance and determine the amount of exercise. Self-efficacy was measured by the Self-Efficacy Scale developed by Lorig et al. (1989) for arthritis patients. The degree of pain was converted to scores based on the Visual Analog Scale, the number of tender points was converted to scores based on the criteria of the ACR(1990) and of Yunus. Depression was measured by CES-D and physical disability, sleep disturbance, fatigue, and anxiety of patients with fibromyalgia were measured by the Fibromyalgia Impact Questionnaire. The level of the exercise performance was converted to scores using the number of times the exercise was performed following the video tape prepared by Association of Rheumatology Health Professionals. Data were analyzed by SPSS windows and the results ire described below. 1. The experimental group which participated in the Self-Help Program showed higher efficacy scores than the control group when both groups were analyzed for depression and the number of tender points as common variables(F=9.146, p=.003). 2. The experimental group which participated in the Self-Help Program showed lower scores than the control group, for pain, the number of tender points, depression, physical disability, fatigue, sleep disturbance and anxiety. These symptoms of fibromyalgia can all be seen to have subsided(F=9.483, p=.003 : F=32.680, p=.001 ; F=11.104, p=.001, F=5.344 : p=.024, F=7.630 : P=.007, F=15.6512, p=.003 : F= 7.5412, p=.008). 3. In the experimental group, the self-efficacy score for the first three weeks showed a positive correlation with the exercise-performance score for four to six weeks (r=.387, p=.043). 4. In the experimental group, the relationship between the level of exercise-performance and the reduction of symptoms showed a significant correlation only to physical disability(r= -.500, p=.001). 5. In the experimental group, the relationship between the self-efficacy score and pain, the number of tender points, depression, physical disability, fatigue, sleep disturbance and anxiety score showed inverse correlations and thus, a reduction of symptoms occured when the self-efficacy score increased(r=-.325, p=.004 ; r= -.253, p=.027, r=-.452, p=.001 : r=-.434, p=.001 ; r=-.316, p=.005 ; r=-.460, p=.001 ; r=-.397, p=.014). Therefore, self-efficacy improved following the Self-Help Program including the stretching exercise. It was also found that physical symptoms (pain, number of tender points, level physical disability) and psychological symptoms (depression, fatigue, sleep disturbance, anxiety) were reduced. Moreover, It was found that the higher the self-efficacy, the the higher the degree of achievement of goals set for the stretching exercises. In addition, the level of exercise-performance influenced the level of physical disability, one of the symptoms of fibromyalgia. Accordingly, the conclusions from this study are that exercise-performance and the reduction of symptoms is achieved through promotion of self-efficacy. Therefore, it is proposed that are the Self-Help Program including stretching exercises is an appropriate nursing intervention for the reduction of symptoms of fibromyalgia.

The Algorithm-Oriented Management of Nasal Bone Fracture according to Stranc's Classification System

  • Background: Nasal bone fracture is one of the most common facial bone fracture types, and the surgical results exert a strong influence on the facial contour and patient satisfaction. Preventing secondary deformity and restoring the original bone state are the major goals of surgeons managing nasal bone fracture patients. In this study, a treatment algorithm was established by applying the modified open reduction technique and postoperative care for several years. Methods: This article is a retrospective chart review of 417 patients who had been received surgical treatment from 2014 to 2015. Using prepared questionnaires and visual analogue scale, several components (postoperative nasal contour; degree of pain; minor complications like dry mouth, sleep disturbance, swallowing difficulty, conversation difficulty, and headache; and degree of patient satisfaction) were evaluated. Results: The average scores for the postoperative nasal contour given by three experts, and the degree of patient satisfaction, were within the "satisfied" (4) to "very satisfied" (5) range (4.5, 4.6, 4.5, and 4.2, respectively). The postoperative degree of pain was sufficiently low that the patients needed only the minimum dose of painkiller. The scores for the minor complications (dry mouth, sleep disturbance, swallowing difficulty, conversation difficulty, headache) were relatively low (36.4, 40.8, 65.2, 32.3, and 34 out of the maximum score of 100, respectively). Conclusion: Satisfactory results were obtained through the algorithm-oriented management of nasal bone fracture. The degree of postoperative pain and minor complications were considerably low, and the degree of satisfaction with the nasal contour was high.

  • Recently, with increasing the number of patients with head injury and cerebrovascular accident, there has been an increasing need for the useful assessment tools of brain dysfunction and it's localization. With the advances in the neuroscience since the mid-1970s, particularly in the areas of neuroanatomical tracing, neuroimaging, and improved behavioraltest design, it has been possible to develop a more precise understanding and localization of brain dysfunction. However, these equipments are not readily available in the private clinics and too expensive to use as a screening tool to all suspected patients with brain dysfunction. Although several screening tests such as Mini-Mental States Examination(MMSE) or Brief Cognitive Rating Scale(BCRS) are simple in use and useful for the brief assessment of brain dysfunction, these are also limited in using for localization of brain dysfunction because of their simplicity. With increasing need of the assessment tool which is able to localize the dysfunction more precisely in the clinical practice, we planned to develop the new assessment tool, the Bedside Neurocognitive Function Localization Test(BNLT) which is suitable for this purpose. The BNLT was designed to be utilized for localizing brain dysfunction effectively and readily in the clinical practice. We introduced the whole process of designing the BNLT in this manuscript.

Development and Evaluation of Allergic Rhinitis-Specific Quality of Life (ARSQOL) Scale for Adults (성인 알레르기비염 특이형 삶의 질 측정도구 개발 및 평가)

  • Purpose: The purpose of this study was to develop an Allergic Rhinitis-Specific Quality of Life (ARSQOL) scale and verify its validity and reliability. Methods: ARSQOL was developed in 5 steps. Items for the preliminary instrument of ARSQOL were developed through a literature review and deep interviews with allergic rhinitis patients. Face validity with Content Validity Index (CVI), construct validity using factor analysis, and known group comparison, criterion validity test using correlation between ARSQOL and total nasal symptoms score (TNSS) were conducted to evaluate the validity of ARSQOL. Cronbach's ${\alpha}$ was used to evaluate the reliability of ARSQOL. Results: CVI for the items in the final ARSQOL were. 92. Five factors including discomfort associated with nasal symptoms (4 items), physical function (7 items), mental function (5 items), sleep disorder and social function (4 items), and problems of daily life (6 items) were identified through factor analysis and these five factors explained 66.6% of the total variance. The correlation coefficient between TNSS and the total score of life quality was -.69. In the group comparison, the persistent allergic rhinitis group showed lower ARSQOL scores than the intermittent patient group, and moderate to the severe allergic rhinitis patient group presented poorer ARSQOL than the mild symptom patient group. The Cronbach's ${\alpha}$ reliability coefficient was .95. Conclusion: Results show that the ARSQOL has good reliability and validity and thus ARSQOL is a useful scale for clinical practices and research as a measure of quality of life in adults with allergicr hinitis.

Women's Level of Fatigue after Delivery (산부의 피로정도에 관한 연구)

  • Fatigue after delivery affect women's birth experience and interrupt the process of labor. Finally woman cannot have a positive birth result and will experience a postpartum fatigue. But researches about fatigue during the labor are lacked. Nurse help adapting a mother's role, bonding with new baby, recovering after birth, and improving woman's quality of life through decreasing fatigue during the labor and intercepting a continued postpartum fatigue. So it is very important that measuring a fatigue and confirming relationships between fatigue and factors affecting fatigue. The purpose of this study was measuring the level of fatigue within 4hours after delivery and identifing factors affecting fatigue. The ultimate goal was to contribute to improving a birth experience and adapting after birth through decreasing the level of fatigue and interventions. The data was collected for this study at the hospital of two universities and the third hospital in Seoul from Aug. 15. to Nov. 10. 2000. The subjects were 106 of mothers who deliveried a normal newborn and were tested within four hours after birth. The instruments were The Visual Analogue Scale for fatigue, The State Anxiety Inventory, and The Labor Support Inventory. The data were analyzed by using percentage, mean, SD, t-test, ANOVA, Pearson correlation. The results of this study were as follows; (1) The level of fatigue during the labor was 61.48point. (2) The deferences according to general and obstetric character affecting fatigue founded that there were Significant differences according to job(t=2.659, p=0.009), and the type of delivery(t=-2.035, p=0.044). (3) The deferences according to factors affecting fatigue revealed that there was significant difference according to quality of sleep(F=2.935, p=0.037). The significant fatigue and the fatigue after delivery was anxiety(r=0.343, p=0.000). The above findings indicate that the level of fatigue during the labor is higher than during pregnancy and postpartum. Woman having a job, delivering by vacuum was more fatigued. The level of fatigue according to a quality of sleep was significant difference. The poor quality of sleep, higher level of fatigue. And the more anxiety after delivery, the more fatigue. So, the variable nursing interventions for lessening the level of fatigue through appling the situation for rest, relaxation during the labor to reserve energy, and decreasing anxiety should be provided for mothers.

Psychosomatic Symptoms of Rotating Shift and Daytime Working Nurses in a General Hospital (일 병원의 순환교대근무와 주간고정근무 여성 간호사의 근무형태에 따른 정신신체 증상)

  • Objectives : Shift work disorder occurs when you have difficulties adjusting to a work schedule that takes place during a time which most people sleep. Some people may have diverse psychosomatic symptoms, such as sleep problems, depression, anxiety, and headaches even after the shift work schedule ends. The aims of this study are to compare difference of psychosomatic symptoms between rotating shift and daytime working nurse groups. Methods : Volunteer nurses working in a general hospital were recruited in a general hospital. We collected sociodemographic data. We used questionnaires for headache type, headache frequency, and VAS(Visual Analog Scale) for headache intensity, BDI(Beck depression Inventory) and GSAQ(Global Sleep Assessment Questionnaire). We used Mann-Whitney test and Chi-square test for hypothesis testing. Results : Data collected from 84 women nurses. Rotating shift(N=37) working nurses showed younger, more unmarried, and shorter work periods than daytime working nurses (N=47). Also rotating shift group showed more problems of headache, insomnia, excessive daytime sleepiness, depression and anxiety than daytime group. The above results were statistically significant. Conclusions : The rotating shift work women nurses produced more headache, insomnia, excessive daytime sleepiness, depression, and anxiety than daytime working ones.

A Case Report of Non-Motor Symptoms Evaluated Using the Non-Motor Symptom Scale in a Patient with Secondary Parkinsonism Presumed to be Probable Lewy Body Dementia and Improved with Combined Treatment with Herbal Medicine and Acupuncture (루이소체 치매로 추정되는 이차성 파킨슨증 환자의 Non-Motor Symptom Scale(NMSS)로 평가한 비운동성 증상을 한약과 침의 복합치료로 호전시킨 증례보고 1례)

  • Parkinson's syndrome is a degenerative brain disease that presents characteristic motor symptoms of tremor, rigidity, and gait disturbance. In addition to these motor symptoms, Parkinson's syndrome also presents non-motor symptoms (NMSs) such as sleep disturbance and cognitive decline. NMSs reduce patient's quality of life and psychosocial functioning and cause economic burden on the patient, so appropriate evaluation and treatment are required. Lewy body dementia is one of the several diseases belonging to Parkinson's syndrome. Its symptoms such as cognitive function, memory impairment, and hallucinations occur with Parkinsonism. Although drug therapy is being used with drug treatment to treat non-motor symptoms, it has limitations such as side effects, which stimulated interest in other complementary treatment methods such as oriental medicine treatment, dance, and yoga. The patient in this case complained of tremor in the right upper extremity, muscle hypertension and pain, and persistent vision, memory, and cognitive decline. The patient was diagnosed with probable Lewy body dementia. The patient was hospitalized for 4 months and received acupuncture and herbal medicines. After treatment, the patient's NMS scale scores decreased from 90 to 63, and the Unified Parkinson's Disease Rating Scale scores (summed I, II, and III) decreased from 17 points to 8 points. The Beck Depression Inventory score decreased from 22 points to 13 points. In addition, the patient's subjective evaluation revealed improvement. In this case, a patient diagnosed with probable Lewy body dementia who did not respond to the standard treatment and did not want to take medications showed improvement in not only motor symptoms but also NMSs after integrative Korean medicine treatment.

The Determinants of Health Promoting Lifestyle in Students of Women's High School (여고생의 건강증진 생활양식 결정요인에 관한 연구)

  • This descriptive correlational study was conducted to identify the variables related to a health promoting lifestyle in women student of high school. 286 students were selected by a convenience sampling method. The data were collected by self - reported questionnaires from 10 to July 13, 2002. The data were analyzed by using descriptive statistics, pearson correlation coefficient, stepwise multiple regression with SPSS program. The result of this study were as follows: 1. The average score of health promoting behavior was 2.46 of a 4 point scale. Sanitary life(3.05), interpersonal support(2.80), alcohol & smoking(2.79) Self actualization(2.76), habit of meal(2.52), rest & sleeping(2.33) regulation of meal(2.30) stress management(2.05), health responsibility(1.91) were also analyzed. 2. Health promoting lifestyle showed significant positive correlations with self-esteem (r=0.446, p<0.00l), life orientation test(r=.493, P<0.0l) and score(F=6.157, P=0.002) 3. Perceived health state showed significant correlation with alcohol & smoking subscale(r=-.134, P<0.05) of health promoting lifestyle. Self-esteem showed significant correlation with total health promoting lifestyle(r=.538, P<0.01), and showed significant correlation self-actualization subscale(r=.657, P<0.01) health responsibility subscale (r=.272, P<0.01), food regulation subscale(r=.126, P<0.05), interpersonal relationship subscale (r=.468, P<0.0l), stress management subscale(r=.349, P<0.01) sanitary life subscale (r=.282, P<0.01) sleep & rest subscale(r=.123, P<0.05). Life orientation test showed significant correlation with total health promoting lifestyle(r=.493, p<0.01), self­actualization subscale(r=.643, p<0.01) health responsibility subscale(r=.192, P<0.0l), habit of meal subscale(r=.215, p<0.01), interpersonal relationship subscale(r=.423, p<0.01), stress management subscale(r=.345, p<0.01) sanitary life subscale(r=.191, p<0.01) sleep & rest subscale(r=.137, P<0.05). Stress response showed significant correlation with sleep & rest subscale(r=-.137, P<0.05) of health promoting lifestyle. 4. The combination of self-esteem. life orientation test explained $33.2\%$ of the variance in health promoting lifestyle. On the basis of the above findings. self-esteem and Life Orientation Test were identified as the variables which explained most of Pender's health promoting model. Nursing strategies enhancing self-esteem and Life Orientation Test which have a more significant effect on health promoting lifestyles should be developed.

A Study of Symptom of Health and Fatigue-regulation Behavior in Middle-aged Women (중년여성의 건강증상호소와 피로조절행위)

  • The purpose of this study were to identify a relationship between symptom of health and fatigue-regulation behavior in middle-aged women. The subjects were 241 women living in Seoul and near the city, ranging in ages of 30-59(mean= 42.1 years) being interviewed during the month of Oct. to Dec., 2000. The following questionnaires were utilized in this study: a self reported symptom of health and a fatigue-regulation behavior scale originated from Kwon(1997). The analysis for the data was done by SAS program, t-test, ANOVA, and Pearson correlation. The results of this study were as follows 1. Total mean scores of health status were shown 29.1, physical symptom 17.9 and psychological symptom 11.2. According to symptom of body system were higher in muscular-skeletal sympom 5.5, neurologic symptom 3.5, and digestive symptom 2.7. 2. The mean scores of symptom of health were significantly different in duraion of marriage, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Physical symptom was significantly different in years of educational, status of menstruation, perceived fatigue, perceived cause of fatigue, perceived help of family, hours of sleep. Psychological symptom was significantly different in parity, whether or not having past illness, perceived fatigue. 3. The neurologic symptom was significantly different in years of education, status of menstruation, perceived fatigue, time to rest The respiratory symptom was significantly different in years of education, status of menstruation and hours of physical exercise. The muscular-skeletal symptom was significantly different duration of marriage, status of menstruation, BMI. perceived fatigue, perceived cause of fatigue, perceived help of family. The cardiovascular symptom was significantly different in whether or not having religion, number of children, duration of marriage, number of pregnancy & abortion, parity, status of menstruation, BMI. The digestive symptom was significantly different in whether or not having religion and the number of children. The urologic symptom was significantly different in whether or not having job, type of family, status of menstruation, BMI, perceived fatigue, perceived cause of fatigue, perceived help of family. The score of fatigue was significantly different in number of pregnancy. 4. There were significantly positive relationships in the area of symptom on health, especially fatigue was significantly positive relationships in score of symptom of health, physical symptom and psychological symptom. 5. They choose fatigue-regulation behavior physical rest, method of diversion, management of stress, enough sleep, and psychological rest in order. In the near future, it is required that further studies investigate socio-environmental factors related to symptoms of health and develop programs motivating fatigue-regulation behaviors actively.

Analgesia After Subacromial Arthroscopy: Prospective Double-blind Study of Continuous Mixed Intra-bursal Infusion with Morphine and Bupivacaine and placebo (견봉하 관절경 수술 후 진통 작용: 모르핀과 국소마취제 혼합제와 위약의 점액낭 내 지속 주입에 대한 전향적 이중 맹검 연구)

  • The aim of this study is to determine if intra-bursal morphine and Bupivacaine mixed infusion provided useful analgesia after subacromial arthroscopic operation. At the end of the subacromial arthroscopy, continuous intra-bursal infusion catheter was inserted. In a double-blind randomized trial, Sixty patients were allocated to one of two groups: placebo group(n=30) received continuous saline infusion. Study group received mixed 5㎖ of 0.5% Bupivacaine, 2㎎ of morphine and 0.05㎖ of 1/1000 epinephrine as bolus and mixed solution of 40㎖ of 0.5% Bupivacaine and 8㎎ of morphine as maintenance dose with continuous infusion pump(0.5㎖ hourly). In the placebo group, two patients were discontinued due to leakage and catheter coming out each. Intensity of pain was evaluated preoperatively and postoperatively for 3 days by 10 graded visual analogue scale. Night pain, pain on motion, sleep disturbance, lying on painful shoulder and amounts of intramuscular pain killer were recorded. Analgesic effect for pain was cleared at 1st and 2nd postoperative day and there was less sleep disturbance for 3 days postoperatively in study group. There was no difference in pain on motion postoperatively. In study group, less pain killers were used in the first 48 hours postoperatively. The continuous intra-bursal infusion decreased perception of pain at resting stage and reduced supplemental analgesic requirement for 2 days postoperatively.
