Development and validation of an instrument to measure the health status of healthy but unsatisfied people : Mibyeong index(未病 index) (이상증상 기반 미병 분류도구의 신뢰도 및 타당도 연구)

  • Objectives : The number of people in Mibyeong state that complain of physical and mental discomfort but without a clear medical diagnosis has rapidly increased, but the conventional medical system is insufficient to care for these people. By establishing an evaluation instrument for Mibyeong state, it will be possible to provide a research base for Mibyeong management system and expand the clinical area of integrative medicine. Methods : Mibyeong Index was designed to measure inconvenience and resilience of subject's complains including four physical symptoms (fatigue, pain, low sleep quality, indigestion) and mental distress include anxiety, anger, depression. A 21-item quality of healthy measure was developed and tested. We used results of a nation-wide stratified sampled adult data in Korea. Results and Conclusions : Mibyeong index had adequate internal consistency, Cronbach's alpha was 0.88 in general population(N=1,110). The correlation between establised quality of life questionnaires (including SF-12 and EQ-D5 VAS) and the Mibyeong index were from 0.468 to 0.493. The national promotion of advanced health for an aging society and original Mibyeong care technology based on traditional Korean medicine can be developed by a self-care system that enhancing health before suffering illness. We expect that this instrument could be contribute to health management of people in Mibyeong state.

A Study on Stress and Blood Stasis Pattern for 55 Uterine Myoma, Adenomyosis, Endometriosis Patients (자궁근종, 자궁선근증, 자궁내막증 환자 55명의 스트레스 정도와 어혈증(瘀血證) 연구)

  • Purpose: The purpose of this study is to know stress and blood stasis pattern for the patients who had hysterectomy related with uterine myoma, adenomyosis, endometriosis. Methods: We got questionnaires from 55 patients among 243 patients who had hysterectomy related with uterine myoma, adenomyosis, endometriosis in the department of obstetrics and gynecology, OO medical center from April 11th 2010 to October 14th 2010, and analyzed them. We conducted correlation analysis between age, pain(VAS), duration of pain and blood stasis score, and between duration of pain, blood stasis score and the sum of Stress Response Index. We compared the sum of Stress Response Index, blood stasis score, VAS according to the result of pathology. For statistics, we used Pearson's correlation coefficient, Mann-Whitney test, oneway analysis of variances(ANOVA) and SPSS version 17.0 for windows. Results: The correlation between duration of pain and blood stasis score showed statistical significance. The correlation between sum of the Stress Response Index and blood stasis score showed statistical significance. There is no statistically correlation between age, VAS, duration of the pain and blood stasis score. There is no statistically significant difference in sum of Stress Response Index, blood stasis score, VAS according to the result of pathology. Conclusion: The result showed that Stress is related with static blood, and the longer patients suffer from pain, the easier patients' blood to be static. Further study will be needed.

A Study on Correlation of Transvaginal Sonography Findings, CA-125 and Blood Stasis Pattern for 15 Adenomyosis Patients (자궁선근증 환자 15명의 초음파 검사 소견, CA-125와 어혈증(瘀血證) 설문지 점수와의 상관성 규명)

  • Objectives: The purpose of this study is to know the correlation of transvaginal sonography findings, CA-125 and blood stasis pattern for 15 adenomyosis patients. Methods: We got questionnaires from 15 patients among 243 patients who had hysterectomy related with adenomyosis in the department of obstetrics and gynecology, OO medical center from April 11th 2010 to November 15th 2010, and analyzed them. For statistics, we used Spearman correlation coefficient and SPSS version 17.0 for windows. Results: The correlation between sum of uterine walls and CA-125 showed statistical significance. The correlation between CA-125 and blood stasis score showed statistical significance. There is no statistically correlation between age, VAS, duration of the pain and blood stasis score. There is no statistically correlation between difference of uterine walls and CA-125, and between difference and sum of uterine walls and blood stasis score. Conclusion: The result showed that sum of uterine walls is related with CA-125. We need a further study to develop blood stasis indicators with a validated questionnaire, and more subjects involved at multiple institutions.

E-mail Survey for Developing Clinical Guideline Protocol on Acupuncture Treatment for Low Back Pain (요통에 관한 침구임상 진료지침 프로토콜 개발을 위한 전자우편 설문조사)

  • Objectives: This survey was done in order to find out how Korean medical doctors derive pattern identification for acupuncture prescriptions in treating low back pain in real clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society from March 26th to April 14th in 2009. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results : 1.57 Korean medical doctors selected meridian pattern identification based on the course of the meridians(44.6%), visceral pattern identification(32.1%), pattern identification based on cause of disease(14.3%) as the most commonly used pattern identification methods for acupuncture prescription when treating low back pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, gallbladder meridian of hypochondriac region(13.0%), bladder meridian of lateral low back region(11.2%), governer vessel of central low back region(11.7%) were selected 3. In visceral pattern identification, yang deficiency of kidney(20.2%), deficiency of kidney(19.3%), liver(16.7%), yin deficiency of kidney(14.0%), violence qi of kidney(8.8%), small intestine(7.9%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, meridian pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for low back pain diagnosis in real clinical practice.

A Study on Blood Stasis Pattern for 159 Hysterectomy Patients (자궁절제술 시행 환자 159명의 어혈증 연구)

  • Purpose: The purpose of this study is to know the blood stasis pattern for the patients who had hysterectomy. Methods: Among the patients who had hysterectomy in the department of obstetrics and gynecology, OO medical center from March 3rd 2009 to October 21st 2009, subjects who met inclusion criteria and submitted appropriate questionnaire. We compared blood stasis score and operational method, age, BMI, past operation history of abdominal and pelvic site, parity, whether adhesiolysis, oriental medicine intervention was done. For statistics, we used independent samples t-test, oneway analysis of variances(ANOVA) and SPSS version 13.0 for windows. Results: Blood stasis score of the subtotal hysterectomy group ($44.43{\pm}15.01$) is higher than that of the total hysterectomy group($39.04{\pm}12.46$). Blood stasis score under the age 40($51.88{\pm}12.68$) is higher than 40-49 age patients($40.34{\pm}14.24$) and over the age 50 patients($41.62{\pm}13.46$). There is no statistically significant difference between laparoscopic and abdominal trial, whether adhesiolysis, oriental medicine intervention was done, BMI, past operation history abdominal and pelvic site, parity, chief complaint before hysterectomy, oriental medicine intervention. Conclusion: Through the results showed, when traditional Korean medical doctors use static blood medication, history of hysterectomy and age should be considered as the history of subtotal hysterectomy and younger age tend to have a higher rate of a blood stasis pattern. We need a further study to develop blood stasis indicators with a validated questionnaire, and more subjects involved at multiple institutions.

A Survey on Usages of Decision Support Functionalities in Korean Medicine Electronic Charts (한의전자차트에서 진단 지원 기능의 활용도에 대한 설문조사)

  • Objectives : A Survey was conducted to find out usages of Korean medicine electronic charts and requirements of clinical decision support functionalities in the charts. Methods : An e-mail was sent to about 12,000 Korean medicine doctors that was affiliated to the Association of Korea Oriental Medicine. 250 doctors answered the questionnaires during one week. Results : Most doctors of 83% answered in use the electronic charts and use it mainly to insurance claims. 46% of them felt that diagnosis functions need to be improved first in the electronic charts. Moreover, 66% of them answered that expert systems to support diagnosis is required if provided. Conclusions : The clinical decision support systems help doctors diagnosis patients in a desirable manner. Many researches have been proposed about them in modern medical science, while a few studies suggested in Korean medicine. In the future, more researches in the field of diagnosis of electronic charts should be proceeded.

  • 1. Objectives This study focuses on the Revision and Translation of the Mongolian's Sasang constitution Diagnosis Questionnaire (SDQ-M) 2. Methods 03'SDQ's questionnaire analysis study have been performed based on the absolutely diagnosed group of 423 cases who have respond to the Sasang constitution Diagnosis Questionnaire (SDQ) from June to November, 2003. 3. Results and Conclusions Revised 05'SDQ-M's traits, which result from on the basis of 03'SDQ's questionnaire analysis study results and socio-cultural understanding via Mongolian physical constitution diagnosis as well as Mongolian translators' interchange, are as follows. (1) 05'SDQ-M is mainly adjusted in terms of external appearances and the nature of illness symptoms. (2) 05'SDQ-M's physical constitution questionnaires are equally adjusted. (3) 05'SDQ-M is re-used, deleted, or created on the basis of 03'SDQ's questionnaire analysis results. (4) 05'SDQ-M is translated to be easy for Mongolians.

A Study of Blood Stasis Pattern for Frozen Shoulder Using Blood Stasis Pattern Questionnaires (BSPQ) and the Relationship of Pain, Shoulder Range of Motion and BSPQ Score (어혈변증 설문지를 통한 오십견의 어혈변증 평가 및 통증, 견관절 운동범위와 어혈변증과의 관계)

  • Objectives : The purpose of this study was 1) to investigate the possibility of taking blood stasis pattern as a clinical parameter of frozen shoulder, using blood stasis pattern questionnaire (BSPQ) to frozen shoulder group comparing with normal group, and 2) to find out the relationship of the severity of main frozen shoulder symptoms (pain and limited range of motion of shoulder) and the level of blood stasis pattern through BSPQ analysis. Methods : During the period of January 2010 to July 2010, fifty five frozen shoulder patients who visited outpatient clinic of department of acupuncture and moxibustion in East-West Neo Medical Center, Kyung Hee University and fifty five normal people without shoulder pain nor limited ROM in shoulder joint were evaluated through BSPQ, and to the frozen shoulder group, also evaluated three different visual analogue scales (VAS) of pain (pain on average, pain at night, and pain on motion) and active / passive range of motion (ROM) of shoulder joint. Results : Mean blood stasis pattern score of frozen shoulder patients group was significantly higher than the score of normal group (patients vs normal group : $4.85{\pm}1.68$ vs $3.49{\pm}1.54$). Three different types of pain VAS (on average, at night and on motion) showed low to very low positive correlation with BSPQ scores when analyzed with Pearson's correlation coefficient. ROM levels in shoulder joint were not significantly related to BSPQ scores, though active external rotation range showed low positive correlation with BSPQ scores. Conclusions : Patients with frozen shoulder showed higher blood stasis pattern score in BSPQ but the level of blood stasis pattern is not significantly related to the severity of pain or limitation of ROM in shoulder joint.

A Survey of the Recognition on the Practice Pattern, Diagnosis, and Treatment of Korean Medicine of Dementia and Mild Cognitive Impairment - Focusing on the Differences between Neuropsychiatrists of Korean Medicine and General Physicians - (치매, 경도인지장애의 한의진료 현황, 진단 및 치료에 대한 한의사의 인식도 조사 연구 - 한방신경정신과 전문의와 일반의의 차이를 중심으로 -)

  • Objectives: To identify the current status of Korean medical practice pattern, diagnosis and treatment of dementia through recognition survey, and to use it as a preliminary data for various dementia research. Methods: Questionnaires were developed through expert meetings. The disease was defined as dementia and mild cognitive impairment, and areas were designated to practice pattern, diagnosis and treatment. From December 18, 2016-January 18, 2017, 221 respondents, including 36 neuropsychiatrists of Korean Medicine and 185 general physicians (including other medical specialists) were included. Results: 1. In both groups, the most commonly used KCD (Korean standard classification of disease and cause of death) were in the order of Unspecified Alzheimer's Dementia (F00.9), Mild Cognitive Impairment (F06.7), and Unspecified Dementia (F03). 2. The most commonly used pattern identification were zang-fu and qi-blood-yin-yang in both groups. 3. Diagnostic evaluation tools were mainly conducted by MMSE, radiologic examination, K-DRS, GDS and CDR in both groups. 4. Both groups reported using acupuncture and herbal medicine mainly. 5. In both groups, the acupuncture method was used extensively in the order of Body, Scalp, and Sa-Am. 6. Neuropsychiatrists used a variety of herbal medicines such as Wonjiseokchangpo-san (Yuanzhushichangpu-san), Yukmijihwang-tang (Liuweidihuang-tang), Palmijihwang-won (Baweidihuang-won), Sunghyangjungki-san (Xingxiang Zhengqi-san) and Ondam-tanggami (Wendan-tangjiawei). General physicians used a variety of herbal medicines such as Ondam-tanggami (Wendan-tangjiawei), Bojungikgi-tang (Buzhongyiqi-tang), Yukmijihwang-tang (Liuweidihuang-tang). 7. Neuropsychiatrists used a variety of Korean herbal preparation products (benefit and non-benefit) such as Ekgan-sangajinpibanha (Yigan-sanjiachenpibanxia), Yukmijihwang-tang (Liuweidihuang-tang), Jodeung-san (houteng-san), Palmijihwang-won (Baweidihuang-won). General physicians used a variety of Korean herbal preparation products such as Bojungikgi-tang (Buzhongyiqi-tang), Banhabaegchulcheonma-tang (banxiabaizhutianma-tang), Yukmijihwang-tang (Liuweidihuang-tang), Ekgan-sangajinpibanha (Yigan-sanjiachenpibanxia), Palmijihwang-won (Baweidihuang-won). Conclusions: By confirming awareness of Korean medical doctors treating dementia in clinical fields and understanding differences between neuropsychiatrists of Korean medicine and general physicians, it can be used to understand guideline users' needs and confirm clinical questions during development of future clinical practice guidelines for dementia.

Changes of Nursing Activities after DNR(do-not-resuscitate) Decision (심폐소생술금지 결정에 대한 간호활동의 변화)

  • Purpose: The purpose of this study was to examine hospital nurses' nursing activities changes after DNR(do-not-resuscitate) decision. Methods: The subjects were 120 registered nurses working in two university hospitals. The data were collected from September 1 to September 15, 2015 using self-report questionnaires. The data were analyzed using SPSS 20.0 program. Results: DNR awareness of the educational needs was very high(91.7%). But DNR educational experience was low(59.2%). Nursing activity change was classified as the physical, emotional, spiritual, and social areas. There were significant difference among religion(F=3.459, p=.010), working unit(F=3.410, p=.036), DNR awareness of the educational needs(t=5.048, p=.027), DNR educational experience(t=-2.816, p=.006) and nursing activities changes. Conclusion: Nurses are needed DNR educational programs to take care of DNR patients. And the criteria for nursing activities related to DNR is required.
