Alternative medicine is defined as the practices used for the prevention and the treatment of diseases that are not taught widely in medical schools, nor generally available inside hospitals. Alternative medicine or therapy is of growing interest to the general public. We conducted a survey to determine the prevalence and patterns of using alternative medicine such as herbal medicine acupuncture, folk medicine, health food and diet, yoga, qi therapy, shiatsu, chiropractics, homeopathy etc.. Based on 794 completed questionnaires, 484 respondents(61%) reported using at least one form of alternative medicine for their medical problems. The major medical conditions for which they used alternative medicine were back pain(85.6%), arthritis(85.4%), hypertension(85.2%), cerebrovascular disorders(86.4%), and insomnia(84.6%). The types of alternative medicine frequently used were herbal medicine, acupuncture, folk medicine, and health food. The reasons why the vast majority of people sought after alternative medicine was that: less side-effect, it is not harmful, it is more effective, there is a shorter waiting time, and a better explanation fur the conditions and a kinder therapist. We found that the frequency of using alternative medicine in the health care system was high. Physicians need to be more aware that many patients may be using alternative medicine. Further survey of the nation-wide prevalence of alternative medicine, and scientific study into the efficacy of this medicine should be followed. In the meantime, we suggest paying attention to possible harmful effects caused by some sorts of alternative medicine.
Purpose: This study aimed to investigate healthcare consumers' interest in patient safety on social media using structural topic modeling (STM) and to identify changes in interest over time. Methods: Analyzing 105,727 posts from Naver news comments, blogs, internet cafés, and Twitter between 2010 and 2022, this study deployed a Python script for data collection and preprocessing. STM analysis was conducted using R, with the documents' publication years serving as metadata to trace the evolution of discussions on patient safety. Results: The analysis identified a total of 13 distinct topics, organized into three primary communities: (1) "Demand for systemic improvement of medical accidents," underscoring the need for legal and regulatory reform to enhance accountability; (2) "Efforts of the government and organizations for safety management," highlighting proactive risk mitigation strategies; and (3) "Medical accidents exposed in the media," reflecting widespread concerns over medical negligence and its repercussions. These findings indicate pervasive concerns regarding medical accountability and transparency among healthcare consumers. Conclusion: The findings emphasize the importance of transparent healthcare policies and practices that openly address patient safety incidents. There is clear advocacy for policy reforms aimed at increasing the accountability and transparency of healthcare providers. Moreover, this study highlights the significance of educational and engagement initiatives involving healthcare consumers in fostering a culture of patient safety. Integrating consumer perspectives into patient safety strategies is crucial for developing a robust safety culture in healthcare.
Korea has very easy access to tertiary hospitals, including university hospitals, among OECD countries, and patients can reach the emergency room of a university hospital within 1-2 hours. However, there are many so-called 'essential medical' blind spots where people die because they do not receive surgery in time. In particular, in the case of essential medical care in the surgical field, despite basic surgery, there is a shortage of medical staff to perform outpatient surgery in emergency situations at university hospitals. Although this lack of manpower has a problem with low insurance premiums for surgery, it also has a very large impact on the burden of criminal liability for medical malpractice, which increases the incidence in case of emergencies. Here, we propose crime immunity to solve the manpower shortage of converged smart surgical essential medical (SES) hospitals. Currently, the medical community agrees on the need for crime immunity, but it is an ambiguous scope of immunity that has not reached a national consensus. We would like to present clear standards for essential medical practices (surgery) that require criminal immunity.

Objective : The purpose of this study is to understand trends of "Gi-Gong" related studies from 1950 to 2016 and to help guide the research direction of Gigong study. Methods : The computerized Korean database was searched from 1950 to 2016. The search terms used were only Korean language terms such as '기, '기공', '도인', '명상', '생체에너지', '수련', '양생'. Results : 1. A total of 310,184 (155,565(1950~2007) + 13,705(2016) + 140,914(2008~2016)) studies were searched in this study. A total of 6,240 studies were finally selected as Gigong related studies. 2. In the results according to the Main Subject Categories(KCI;Korea Citation Index), the 'Humanities' was the largest with 2,686 studies and the 'Marine Agriculture, Fishery' was the least with 10 studies. 3. In the results according to the Middle Subject Categories(KCI;Korea Citation Index), the 'Philosophy' was the largest with 1,014(16.26%) studies. 4. A result of the Gigong Categories showed that the 'Philosophy of Gi' was the highest with 2,151(33.82%) not including duplication. 5. As a result of Research Method, the most frequently used method was 'Analysis Research' with 4,199(67.33%) studies. 6. In the author search, 'Kim Nak Pil' was searched 112 times and was the most.. 7. The journal that published the most relevant studies with Gigong was 『Conference Proceedings OF The Korean Society OF Jungshin Science』 (189), and the university that had the largest number of studies related to Gigong was 'Wonkwang University'(212). Conclusions : 1. The number of Gigong related studies had increased rapidly from the late 1990s to the early 2000s, and reached to a peak in 2007 and has been gradually increasing since 2008. 2. The three major academic fields of Gigong related studies are Thought Field, Korean Medicine Field, and Kinesic Field. 3. In the Gigong related studies, the best authority is 'Kim Nak-pil' and the most active academy is the 'Korean Society OF Jungshin Science'. 4. Although most categories of Gigong have been generally studied evenly, studies on External Gi Therapy and Taoist Sexual Practices are very insufficient.

Background : Coding of principal diagnosis is essential component for producing reliable health statistics. We performed this study to evaluate the current practice of principal diagnoses determination and coding, and to give some basic data to improve coding of principal diagnosis. Method : Nineteen medical record administrators (MRAs) of 3 university hospitals participated in coding principal Dx. from August 1, 2001 to August 31, 2001. From each hospital, 10 medical records of patients with high frequency disease were selected randomly. Each 10 medical records were grouped into three (A. B, C). Then, these 30 medical records were given to each MRAs for coding. At the same time questionnaire was given to each of them. Questions were to prove how they decide and code the principal diagnosis among many current diagnoses; how they decide and code the principal diagnosis when they see irrelevant diagnosis recorded as the principal diagnosis in medical record, when only tentative diagnoses were recorded without final diagnosis, and when different diagnoses were recorded in different sheets of same record. Agreement of coding among 3 hospitals were compared and survey results were analysed with SAS 6.12. Results : Agreement of coding was found in medical records 5-6 of each 10 medical records. Causes of disagreement were as follows. Difference of clinician's opinion from each hospital; mixed use of guideline from KCD-3 and guideline from DRG; difference in 4th digit classification according to the absence of pathology report in the medical record; difference of abbreviations among hospitals. 57.9% of MRAs selected the principal diagnosis recorded by physician, 42.1% of MRAs decided principal diagnosis after consulting to KCD-3 guideline. When there were difficulties in determining the principal diagnosis, 42.1% of MRAs decided principal diagnosis after discussion with the physician, 26.3% after discussion with fellow MRAs. Conclusion : There were differences in codings among hospitals. To minimize the difference, we suggest the development of disease-specific guidelines for coding in addition to the current general guideline such as KCD-3. To do this, Coding Clinic which can produce guidelines is needed.
Background: In vitro culture of preantral follicles is a promising technology for fertility preservation. Objectives: This study aims to investigate an optimized three-dimensional (3D) fetal bovine serum (FBS)-free preantral follicle culture system having a simple and easy operation. Methods: The isolated follicles from mouse ovaries were randomly divided in an ultra-low attachment 96-well plates supplement with FBS or bovine serum albumin (BSA) culture or encapsulated with an alginate supplement with FBS or BSA culture. Meanwhile, estradiol (E2) concentration was assessed through enzyme-linked immunosorbent assay of culture supernatants. The diameter of follicular growth was measured, and the lumen of the follicle was photographed. Spindle microtubules of oocytes were detected via immunofluorescence. The ability of oocytes to fertilize was assessed using in vitro fertilization. Results: The diameters were larger for the growing secondary follicles cultured in ultra-low attachment 96-well plates than in the alginate gel on days 6, 8, and 10 (p < 0.05). Meanwhile, the E2 concentration in the BSA-supplemented medium was significantly higher in the alginate gel than in the other three groups on days 6 and 8 (p < 0.05), and the oocytes in the FBS-free system could complete meiosis and fertilization in vitro. Conclusions: The present study furnishes insights into the mature oocytes obtained from the 3D culture of the preantral follicle by using ultra-low attachment 96-well plate with an FBS-free system in vitro and supports the clinical practices to achieve competent, mature oocytes for in vitro fertilization.
This study is designed to find out some intra-clinic factors affecting the content of practice provided by primary care physicians in Korea, and proposed factors in this study are characteristcs of each private clinc --- physician-related variables(age, sex, specialty), bfed-related variables for inpatient care, laboratory-related variables for precise diagnosis. We have tried to estimate the difference of disease entities cared by each primary care physician according to above factors by analyzin gdisease data claimed during one month(April, 1992) to National Federation of Medical Insurance. The diagnosis codes by ICD-9 in the research disease data were reclassified to 'diagnosis clusters' by virtue of clinical similarities for effective analyses. We have converted frequent-tsing ICD-9 codes to 86 diagnosis clusters, which incorporated 97.4 percents of all ambulatory visits to private clinics. This result means proposed diagnosis-cluster method is effective tool for analysis of the content of ambulatory medical care carried out by primary care physicians. Comparisons and analyses of multiple diagnosis-clusters made on the basis of presented factors were done and the results were as follows; - Major factors affecting the difference between diagnosis-cluster pattern by each variables were phyusician's age, sex, specialty and bed counts of each private clinic for inpatient care and the size of laboratories of each clinic. - Middle aged(30th to 40th) group physicians are providing more comprehensive care than 20th or above 50th aged groups. Male physicians are more adequate for comprehensive care than female physicians, because woman-doctors are providing narrow-spectrum care. The content of practice of obstetricians and gynecologists shows much difference from primary medical practice, and they cannot be included in primary care physician, this study suggested. Pediatricians are also providing short-spectum acre, and nearly all visits to pediatricians were incorporated only 2-3 diagnosis-clusters. General surgeons' practices are very similar to general practioners' or family physicians' practices, the means they are providing primary care rather than special surgical care. And small number of beds(under 5 beds) and only basic(2-3 sorts of)diagnostic apparatuses are sufficient for primary physicians' clinic to carry out primary care. In conclusion, to reinforce primary care department in Korea, there must be support with health policy to expand office-based primary care practice-- with small number of beds for inpatient care and only basic laboratories-- provided by general practitioner of family physician.
This study is to prepare basic data to improve the clinical practice satisfaction of nursing students in Mongolia and Korea by comparatively studying subjective happiness, clinical practices stress and clinical practice satisfaction between Korean nursing students and Mongolian nursing students. A survey was conducted with 187 nursing students from one college in Gyeongnam and 100 nursing students from the National Medical University of Mongolia and data analysis were using the SPSS 21.0 program. The result was Korean nursing students had lower subjective happiness and clinical practice satisfaction than Mongolian nursing students, but they were not statistically significant and clinical practice stress was statistically significantly higher in Mongolian nursing students. In addition, it was found that the lower the clinical practice stress and the higher the subjective happiness, the higher the satisfaction with clinical practice among Korean nursing students and in Mongolian nursing students, the lower the clinical practice stress, the higher the clinical practice satisfaction. Therefore, it is necessary to conduct a comparative study on clinical practice education in nursing education institutions in Korea and Mongolia to develop a joint program to improve clinical practice satisfaction and to verify the effectiveness.
Objectives : The uterus plays an important role in the woman's body. In Korean Medical literature, the uterus is mentioned in various contexts according to different perspectives on its meaning and function. An examination of these various contexts is crucial in understanding the meaning of the uterus and to better understand and approach woman's body. Methods : Aside from the most widely used term Jagung(子宮), there were various terms used to refer to the uterus. Based on a list of these terms, the Siku Qianshu collection of medical literatures was investigated. Contents related to the definition, shape, location, function were extracted and examined. Results : Among the findings, first, there were various terms referring to the uterus similar to contemporary understanding. Some of them referred to the uterus as a whole, while others referred to specific parts, one of which is the placenta. Some reflected a broader perspective on the meaning of the uterus. Second, the functions of the uterus could be summarized as gate keeping, and the maintenance of uterine environment that is related to menstruation, pregnancy, and childbirth. Third, based on the examination of the meaning of the uterus and its functions, perspectives on the uterus in Korean Medicine could be summarized into two. Conclusions : One viewed uterus as an organ dedicated to reproduction, similar to today's common understanding. According to this view, uterus is a special organ specific to women, which functions as an incubator for the fetus. The other viewed the uterus as an intangible source of life in the woman's body. As a general source of life to all human beings, it is not a passive organ but functions as an active source in woman's life phenomena. The two perspectives are not in conflict, but rather reflect the broad range of thought on the concept of the uterus in Korean Medicine. In today's biomedical society, the diversity and flexibility of these perspectives could shed light on medical practices that have resulted from extreme views on the woman's body.

Background and Objectives: Cryoballoon catheter ablation for the treatment of patients with symptomatic atrial fibrillation (AF) has been adopted globally, but there are limited multicenter reports of 12-month outcomes in the Korean patient population. This analysis evaluated the clinical performance and safety of cryoballoon ablation (CBA) according to standard-of-care practices in Korea. Methods: This evaluation of Korean patients with AF was conducted within the larger Cryo Global Registry, which is a prospective, multicenter, post-market registry. Freedom from a ≥30-second recurrence of atrial arrhythmias (after a 90-day blanking period until 12 months) and procedural safety were examined in subjects treated with CBA at 3 Korean centers. Results: Overall, 299 patients with AF (60±11 years old, 24.7% female, 50.5% paroxysmal AF) underwent CBA using the Arctic Front Advance cryoballoon. Of those, 298 were followed-up for at least 12 months. Mean procedure-, left atrial dwell- and fluoroscopy time was 76±21 minutes, 56±23 minutes, and 27±23 minutes, respectively. Freedom from AF recurrence at 12 months was 83.9% (95% confidence interval [CI], 76.9-88.9%) in the paroxysmal and 61.6% (95% CI, 53.1-69.0%) in the persistent AF cohort. Rhythm monitoring was performed on average 4.7±1.4 times during the follow-up period. Serious device- or procedure-related adverse events occurred in 2 patients (0.7%). The 12-month Kaplan-Meier estimate of freedom from repeat ablation and cardiovascular-related hospitalization was 93.8% (95% CI, 90.4-96.1%) and 89.7% (95% CI, 85.6-92.7%), respectively. Conclusions: CBA is an efficient, effective, and safe procedure for the treatment of AF patients when used according to real-world practices in Korea.
