Objectives: Postoperative paresthesia is a common complication after sagittal split osteotomy (SSO). This study aimed to compare paresthesia among different fixation methods one year postoperative. Materials and Methods: This prospective cohort study assessed subjects in four groups: class II with miniplate fixation (Group 1), class II with three-screw fixation (Group 2), class III with miniplate fixation (Group 3), and class III with three-screw fixation (Group 4). Paresthesia was evaluated one year postoperative based on a 0-10 visual analogue scale. Pearson correlation was used to evaluate associations of age and mandibular movement with paresthesia. ANOVA was used to compare paresthesia among groups. Results: A total of 80 subjects were enrolled, with 20 subjects in each of the four groups. The Pearson correlation test demonstrated a significant correlation between mandibular movement and paresthesia (P=0.001). Comparison of paresthesia among the groups showed significant differences among groups 1 and 2, 2 and 3, and 3 and 4 (P<0.05). Conclusion: The three-screw fixation method led to more paresthesia one year postoperative compared with miniplate fixation. In addition, the magnitude of mandibular movement had a positive correlation with paresthesia.
Purpose: Extremely hazardous pesticides are classified as World Health Organization (WHO) hazard class Ia. However, data describing the clinical course of WHO class Ia OP (organophosphate) poisonings in humans are very scarce. Here, we compare the clinical features of patients who ingested hazard class Ia OPs. Methods: This retrospective observational case study included 75 patients with a history of ingesting ethyl p-nitrophenol thio-benzene phosphonate (EPN), phosphamidon, or terbufos. The patients were divided according to the chemical formulation of the ingested OP. Data regarding mortality and the development of complications were collected and compared among groups. Results: There were no differences in the baseline characteristics and severity scores at presentation between the three groups. No fatalities were observed in the terbufos group. The fatality rates in the EPN and phosphamidon groups were 11.8% and 28.6%, respectively. Patients poisoned with EPN developed respiratory failure later than those poisoned with phosphamidon and also tended to require longer mechanical ventilatory support than phosphamidon patients. The main cause of death was pneumonia in the EPN group and hypotensive shock in the phosphamidon group. Death occurred later in the EPN group than in the phosphamidon group. Conclusion: Even though all three drugs are classified as WHO class Ia OPs (extremely hazardous pesticides), their clinical courses and the related causes of death in humans varied. Their treatment protocols and predicted outcomes should therefore also be different based on the chemical formulation of the OP.
Objectives : The purpose of this study was to investigate the satisfaction of students who participated in a role play using the syndrome differentiation CPX (Clinical Performance Examination) scenario in Korean Medicine Classics class, and to find out whether the results were correlated with self-assessment of treatment skills and subject achievement. Methods : In the Korean Medicine Classics class in the first semester of 2022, 44 first-year students in the Department of Korean Medicine completed theoretical education and formative evaluation on the subject of internal damage fever, and then role-played using CPX scenarios. Among them, 41 students consented to the study. Students who agreed to the study answered the satisfaction questionnaire consisting of 13 questions in 2 areas on learning methods and effects, and self-evaluation was conducted according to the evaluation items of medical technology consisting of 23 questions. Satisfaction and self-assessment results were analyzed for correlation, and additionally, correlation with Korean Medicine Classics subject achievement was also analyzed. Results : The result of student satisfaction with the role play was 4.87±0.06 while the result of student self-assessment of clinical skill was 4.73±0.16. The student satisfaction with the role play showed statisticallysignificant correlations with the self-assessment of clinical skill and quiz score of Korean Medical Classics while it did not show correlations with the total score of Korean Medical Classics and paper test. Also, the self-assessment of clinical skill did not show correlations with the total score of Korean Medical Classics, paper test, and quiz. Conclusions : Even though the students who performed the role play using the syndrome differentiation based CPX scenario highly assessed their clinical skill and showed high satisfaction, it did not show significant correlation with the achievement of Korean Medical Classics.
Objectives : The purpose of this paper is to clarify the social characteristics of the authors of Huangdi Neijing such as status, class, and political ideology. Methods : We analyze the Neijing text and the social order and historical situations in the Han dynasty. Results : Some authors of the Neijing were the local medical officers whose salary was 100~400shi. Their positions were medical craftsmen(yigong) or chief medical craftsmen(yigongchang). They would have published the Neijing after the administrative reforms(146-145 BCE) that began after the suppression of the Rebellion of the Seven Kingdoms. The bureaucrat yigong(chang) would have expected to participate in the public health policy of the empire or kingdom as an acupuncture expert. They would have also expected to contribute to the welfare and health of the privileged intellectual group and the public, hoping to ascend in status and class. Conclusions : By investigating the social characteristics of the authors who composed the Neijing, its various aspects would be newly understood.
We examined the immunogenicity of H-2 class I-restricted and HLA-A2-restricted epitopes through peptide immunization of HLA-A2-transgenic mice that also express mouse H-2 class I molecules. All four of the tested epitopes restricted by H-2 class I robustly elicited T-cell responses, but four of seven epitopes restricted by HLA-A2 did not induce T-cell responses, showing that HLA-A2-restricted peptide epitopes tend to be poorly immunogenic in HLA-A2-transgenic mice. This finding was confirmed in HLA-A2-transgenic mice infected with a recombinant vaccinia virus expressing hepatitis C virus proteins. We examined the precursor frequency of epitope-specific naïve $CD8^+$ T cells in HLA-A2-transgenic and conventional C57BL/6 mice and found that the poor immunogenicity of HLA-A2-restricted peptide epitopes is related to the paucity of naïve $CD8^+$ T-cell precursors in HLA-A2-transgenic mice. These results provide direction for the improvement of mouse models to study epitope repertoires and the immunodominance of human T-cell responses.
Purpose: The aim of this study is to identify the legal concept of a license and a qualification for health care personnel. We analyze the appropriateness of the qualification for the first-class paramedics and the sufficiency of 3-year job experience in substitution for college level academic major. Method: We reviewed Laws on Healthcare which stipulate academic and career qualifications for healthcare personnel for whom college-level academic training and job experience to get a license. Results: There is no legal consideration which permits major and educational background substituted by job experience. Only third-class hygienist who graduated from a high school or the equivalent with 3 year job experience can apply for first-class hygienist' national license examination without educational requirements. However, it is nothing to compare with first-class paramedics because the work of hygienist is not directly related to patients care and there is no need of doctor's supervision for hygienist' job. Paramedics have a major role in saving emergency patients in making prompt decision, applying appropriate emergency treatment, securing intravenous route, managing intubation, applying defibrillator, and etc. Conclusion: Ministry of Health & Welfare should grant license to paramedics and annul replacement of academic major background. In addition, it is imperative to broaden the scope of paramedics' job so that they can guarantee patient's life saving in emergency situation with quality major curriculum of college level.
Objectives : The purpose of this study is to survey the needs of oriental medical students about the existing teaching methods and to investigate satisfaction of student-oriented teaching & learning method on the oriental medical ophthalmology & otolaryngology class. Methods : 1. Oriental medical ophthalmology & otolaryngology were studied by student-oriented teaching method with 36 persons( 4th grade, school of Oriental medicine). 2. The satisfaction of school lesson was surveyed with questioning paper before & after applying student-oriented teaching & learning method. They replied that Problem based learning(36.1%) & Team based learning(22.2%) are more effective teaching methods than lecture(19.4)%). 3. 66.6% students replied about the necessity of improvement of major subject's teaching method. Conclusions : 1. The interest & understanding degree were improved compare with other class by lecture. 2. It was positive about the interaction with professor and students(64.8%). 3. It was positive about the diagnosis and treatment of patients in the future(64.7%). 4. It was negative about the present national examination(67.6%). 5. Meditation was helpful at classwork(64.7%).
Objective : This study was conducted to examine the scope and learning objective of Medical classics in the field of the education of Korean Medicine. Method : This study was analyzed and figured out list of classes which was taken by department of medical classics in eleven College of Korean Medicine and one school of Korean Medicine. Results & Conclusions : 1. Now, 14 subjects out of 16 subjects which were taken by whole department of medical classics in Korea can be a proper area of education of medical classics. Now, Hwangjenaekyung and Nangyeong are the only aim of the lessons at the medical classics. Therefore, we present to modify the aims inclusively. 2. The subject of the class have to change as follows. 'Wonjeon' changes into 'Hwangjenaekyung', 'Medical Chinese character' changes into 'Korean medical chinese character', 'medical informatics' changes into 'korean medical informatics'. 3. As we consider the condition that 'Nangyeong' is educating in just four departments of medical classics, we have to discuss about the stature of Nangyeong and to extend education of Nangyeong. 4. In the department of medical classics, we can improve the level of understanding and reading skills by educating the class of 'Sanghanron' and 'Donguibogam'. 5. This study is actively involved in trying to include 'Korean Medical Informatics' and 'Korean medical terminology ' in the education field of the medical classics.
Patients of chronic hepatic diseases(n=107) including chronic hepatitis caused by hepatitis B virus infections(n=31), liver cirrhosis(n=53), and hepatocellular carcinoma(n=23) were examined to ascertain genetic relationship between chronic hepatic diseases and histocompatibility antigen. Peripheral blood lymphocytes were separated from whole blood by the method of Ficoll/Hypaque gradient. Total 54 histocompatibility antigens(class I antigens: 41, class II antigens: 13) were analysed by performing of complement dependent microlymphocytotoxicity method using Terasaki's and Catholic Medical College tissue typing plates. HLA antigen frequencies were compared with those of 661 normal controls. The following results were obtained: 1. HLA antigen frequencies of HLA-Bw46, -Bw76, -Cw1, -Cw6, and HLA-DR8 in chronic hepatitis patients were shown to be higher than those in controls(P<0.01). 2. HLA antigen frequencies of HLA-Bw46, -Cw7(P<0.01), and HLA-B37, -Bw58, -Cw1, -MT1(P < 0.05) in liver cirrhosis patients were shown relatively higher frequencies than those in controls. 3. In patients with hepatocellular carcinoma, antigens of HLA-A1, -A26, -Cw3, -Cw7 and HLA-DR6 were dominantly detected. 4. There were negative associations with HLA-Cw4, and -DR4 in patients of chronic hepatic diseases(P < 0.05).
In the present study, we investigated if priming of autoreactive $CD8^+T$ cells would be inhibited by competitive peptides for major histocompatibility complex (MHC) class I binding. We used a mouse model of vitiligo which is induced by immunization of $K^b$-binding tyrosinase-related protein 2 (TRP2)-180 peptide. Competitive peptides for $K^b$ binding inhibited IFN-${\gamma}$production and proliferation of TRP2-180-specific $CD8^+T$ cells upon ex vivo peptide restimulation, while other MHC class I-binding peptides did not. In mice, the capability of inhibition was influenced by T-cell immunogenicity of the competitive peptides. The competitive peptide with a high T-cell immunogenicity efficiently inhibited priming of TRP2-180-specific $CD8^+T$ cells in vivo, whereas the competitive peptide with a low T-cell immunogenicity did not. Taken together, the inhibition of priming of autoreactive $CD8^+T$ cells depends on not only competition of peptides for MHC class I binding but also competitive peptide-specific $CD8^+T$ cells, suggesting that clonal expansion of autoreactive T cells would be affected by expansion of competitive peptide-specific T cells. This result provides new insights into the development of competitive peptides-based therapy for the treatment of autoimmune diseases.
