This study was conducted to provide the baseline data for preparing an educational program for mothers of pediatric asthma patients by identifying the knowledge about asthma, mother's educational demand and the perceived educational performance. This study used survey design. The subjects were chosen from the mothers whose children have received pediatric asthma treatment or who have admitted in the pediatric unit of major hospitals using selection criteria. The total number of subjects were 63 mothers. The data collection period was from May 1st, 2001 to April 17th, 2001. Instruments used for this study were knowledge examination, educational demand evaluation, and educational performance. The data were analysed using t-test, ANOVA with SPSS PC(Version 10.0). The results of this study were as follows. 1. Mean score of knowledge about pediatric asthma was 17.95, which suggests the mothers of children with asthma have a medium knowledge level. The highest grade was knowledge about treatment and follow management and the lowest grade was knowledge about diet. 2. Demand for education showed 4.23, which suggests the mothers of children with asthma have high educational demand. The highest score was about exercise and activity in daily life and the lowest score was medication. 3. Perceived educational performance score of themselves showed 2.40, which suggests the mothers of children with asthma thought that health team do not give enough education to them. The highest score was knowledge about pediatric asthma itself and the lowest score was exercise and activity in daily life. 4. Demand for education and perceived educational performance about pediatric asthma showed significant difference in all areas. 5. There were no statistically significant difference noted between general characteristics and degree of knowledge, educational demand and perceived educational performance about pediatric asthma. In conclusion, there needed a systematic educational program development for the mothers of children with asthma. Especially, an education program for mothers in the beginning period of pediatric asthma should be emphasized.

Asthma is a prevalent and serious health problem in Korea. Recently, the Korean Asthma Guideline has been updated by The Korean Academy of Tuberculosis and Respiratory Diseases (KATRD) in an effort to improve the clinical management of asthma. This guideline focuses on adult patients with asthma and aims to deliver up to date scientific evidence and recommendations to general physicians for the management of asthma. For this purpose, this guideline was updated following systematic review and meta-analysis of recent studies and adapting some points of international guidelines (Global Initiative for Asthma [GINA] report 2014, National Asthma Education and Prevention Program [NAEPP] 2007, British Thoracic Society [BTS/SIGN] asthma guideline 2012, and Canadian asthma guideline 2012). Updated issues include recommendations derived using the population, intervention, comparison, and outcomes (PICO) model, which produced 20 clinical questions on the management of asthma. It also covers a new definition of asthma, the importance of confirming various airflow limitations with spirometry, the epidemiology and the diagnostic flow of asthma in Korea, the importance and evidence for inhaled corticosteroids (ICS) and ICS/formoterol as a single maintenance and acute therapy in the stepwise management of asthma, assessment of severity of asthma and management of exacerbation, and an action plan to cope with exacerbation. This guideline includes clinical assessments, and treatment of asthma-chronic obstructive pulmonary disease overlap syndrome, management of asthma in specific conditions including severe asthma, elderly asthma, cough variant asthma, exercise-induced bronchial contraction, etc. The revised Korean Asthma Guideline is expected to be a useful resource in the management of asthma.

Histological examination of biopsy or postmortem lung tissue from patients with asthma usually reveals thickened airway walls. This change is called airway remodeling, which is characterized by airway eosinophilia, hyperplasia of goblet cells and smooth muscle, and subepithelial fibrosis [1,2]. In this study, we investigated the time-course functional, morphological, biochemical changes of remodeling in a ovalbumin (OVA)-induced murine chronic asthma model. (omitted)
Objectives : Based on the changes of quality of life and pulmonary function test, we aimed to identify the effects and side-effects of Gamichuongsangboha-tang and in asthmatic patients. Methods : The subjects consisted of 30 patients with asthma who were treated with Gamichuongsangboha-tang extract for four weeks. Gamichuongsangboha-tang extract is the extracted powder form of the modified herbal decoction Chuongsangboha-whan, which has been used as the traditional therapeutic agent for asthma. Pulmonary function test (PFT) was checked before and after 4 weeks of treatment. Quality of Life QuestionnaireQ for Adult Korean Asthmatics (QLQAKA) was checked before and after 2 and 4 weeks of treatment. Unpleasant symptoms were checked in all patients at 2 weeks and 4 weeks from the beginning of treatment. Results : Treatment with Gamichuongsangboha-tang extract for four weeks resulted in significant increase in $FEV_{1.0}%$, PEFR%, and QLQAKA. The total efficacy rate of QLQAKA in the patient group was 53.3% after 4 weeks. The QLQAKA of the step 2, step 3, and step 4 groups (n=18, 55.6%) classified by global initiative for asthma (GINA) consistently increased, and significant improvement of QLQAKA occurred in the step3 group. In the step 4 group, FEV1.0%, and PEFR% were significantly increased. A total of 15 patients experienced unpleasant symptoms during the first 2 weeks of treatment, but these symptoms disappeared in all but 2 cases with no need of further treatment during the 2nd 2-week period. Conclusions : This study shows that Gamichuongsangboha-tang extract has effects on improvement of pulmonary function and quality of life, especially in persistent symptomatic asthmatics. Obviously further study concerning all these is still necessary.

Background: Asthma and chronic obstructive pulmonary disease (COPD) both require long-term management, and patients with asthma-COPD overlap (ACO) need comprehensive and prolonged care beyond pharmacotherapy. However, due to limited research on ACO, treatment strategies and long-term outcomes remain unclear. This highlights the need for further multidisciplinary research to improve ACO treatment and prognosis. Case Presentation: This case report describes a patient who presented with symptoms of cough, sputum, wheezing, dyspnea, and oral dryness. The patient was diagnosed with ACO and received a combination of Korean medicine treatments alongside standard asthma medication for 15 days. Standardized tools were used to evaluate symptoms and quality of life, respectively. After treatment, Medical Research Council scale, Leicester cough questionnaire Korean-Version and Numerical Rating Score demonstrated clinically meaningful improvement, and Total IgE and Eosinophils were slightly decreased. Conclusion: This case suggests Korean medicine treatments might be effective in managing symptoms in patients with ACO and enhancing quality of life.

Background : Bronchial asthma is an inflammatory disease of the airways that is associated with airway remodeling. The vascular endothelial growth factor (VEGF) is a potent, multifunctional cytokine that contributes to angiogenesis and inflammation. Matrix metalloproteinase-9 (MMP-9) is a major proteolytic enzyme that in duces bronchial remodeling in asthma. However, there is no data available on the possible role of the VEGF or on the potential relationship between the VEGF and MMP-9 in acute asthma. Therefore, the VEGF was studied to determine whether or not it participates in airway inflammation during acute asthma. An additional aim of this study was to determine whether or not the VEGF levels correlated with the MMP-9 levels in the sputum of acute asthma patients. Methods: Both the VEGF and MMP-9 levels were measured by an enzyme immunoassay and zymographic analysis in the sputum of patients with either stable asthma or with acute asthma. The VEGF and MMP-9 levels were also evaluated during a spontaneous asthma attack. Results : The VEGF levels were significantly higher in the sputum of acute asthmatic patients than in either the stable patients the control subjects. The VEGF levels in the sputum during asthma exacerbation were significantly higher than those on the remission days, and those levels decreased after asthma therapy. In acute asthmatic patients, the VEGF levels in the sputum correlated with the number of neutrophils and eosinophils. In addition, a significant correlation was established between the VEGF and MMP-9 levels in the sputum. Conclusion : These results suggest that VEGF overproduction is associated with airway inflammation during acute asthma and is related to the MMP-9 function.

Background: Although airway hyper-responsiveness is one of the characteristics of asthma. bronchial hyper-responsiveness has also been observed to some degree in patients with chronic obstructive pulmonary disease (COPD). Moreover, several reports have demonstrated that a number of patients have both COPD and asthma. The methacholine bronchial challenge test (MCT) is a widely used method for the detecting and quantifying the airway hyper- responsiveness, and is one of the diagnostic tools in asthma. However, the significance of MCT in differentiating asthma or COPD combined with asthma from pure COPD has not been defined. The aim of this study was to determine the role of MCT in differentiating asthma from pure COPD. Method: This study was performed prospectively and was composed of one hundred eleven patients who had undergone MCT at Chonbuk National University Hospital. Sixty-five asthma patients and 23 COPD patients were enrolled and their MCT data were analyzed and compared with the results of a control group. Result: The positive rates of MCT were 65%, 30%, and 9% in the asthma, COPD, and control groups, respectively. The mean $PC_{20}$ values of the asthma, COPD, and control groups were $8.1{\pm}1.16mg/mL$, $16.9{\pm}2.21mg/mL$, and $22.0{\pm}1.47mg/mL$, respectively. The sensitivity, specificity, positive predictive value, and negative predictive value of MCT for diagnosing asthma were 65%, 84%, 81%, and 69%, respectively. The sensitivity, specificity, positive predictive value, and negative predictive value of MCT (ed note: please check this as I believe that these values correspond to the one $PC_{20}$ value. Please check my changes.) at the new cut-off points of$PC_{20}{\leq}16mg/ml$, were 80%, 75%, 78%, and 78%, respectively. Conclusion: MCT using the new cut-off point can be used as a more precise and useful diagnostic tool for distinguishing asthma from pure COPD.
Objectives: This study aimed to report the effects of Jaeumganghwa-tang (Ziyinjianghuo-tang) and Gyeongok-go on a patient with asthma. Methods: A 54-year-old female patient was treated with herbal medicine, including Jaeumganghwa-tang, Gyeongok-go, acupuncture, and moxibustion treatment. The effects of treatments were evaluated using the modified medical research council dyspnea scale (mMRC), St. George's Respiratory Questionnaire (SGRQ), and Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA). Results: Following treatment, the mMRC score decreased from 2 to 1, the SGRQ score decreased from 60.53 to 25.61, and the QLQAKA score increased from 44 to 72. In the SGRQ, symptom scores decreased from 70.67 to 57.49, activity scores from 36.22 to 25.96, and impact scores from 72.15 points to 15.44 points. In QLQAKA, symptom scores increased from 18 to 21, emotional scores from 7 to 13, environmental scores from 6 to 14, and activity scores from 13 to 24. Conclusion: The results suggest that Jaeumganghwa-tang and Gyeongok-go could be effective in treating patients with asthma.

Purpose: This study was done to investigate the relationship of knowledge related to disease, awareness and practice of inhaler use in asthmatic patients. Method: The participants in this research were 124 patients with asthma who used inhalers, and were admitted to hospital or attended outpatient asthmatic clinics. Data were collected using a self-administered questionnaire which consisted of demographic characteristics, illness-related characteristics, and tools to measure knowledge of disease, awareness and practice related to inhaler use. Results: Knowledge related to disease was statistically significantly different for age (p<.001) and marital status (p=.017). The mean score for awareness of inhaler use was significantly higher than the mean score for practice in inhaler use (p<.001). There were significant correlations between awareness and practice of inhaler use (p<.001), knowledge related to disease and awareness of inhaler use (p<.001), and knowledge related to disease and practice of inhaler use (p<.001). Conclusion: Results indicate that due to the variation in degree of illness in patients with asthma, there is a need to develop appropriate education programs for people of different ages and also precise ${\alpha}$ II content for use of inhalers method of checking capacity washing and storing as well as directions about different kinds of the inhalers.
