조선후기 의서 『낙산당신집의방금낭지보(樂山堂新集醫方錦囊至寶)』 수재(收載) 약성가(藥性歌)에 대한 연구 (A Study on Rhymes of Herbal Medicine in Yosandangsinjipeuibangkeumnangjibo during Late Joseon Period)

  • The medical book Yosandangsinjipeuibangkeumnangjibo (『樂山堂新集醫方錦囊至寶』, below as Yosandang, 『樂山堂』) is housed in the Handok museum of Medicine and Pharmacy. At the end of the Yosandang, written in manuscript, contains a rhymes of herbal medicine (藥性歌). The rhymes of herbal medicine is a record of song form about herbal medicine. Song-forms were widely used because they were easy to sing and memorize. In particular, there are many records of song forms in Korean medical books, which have been used in various fields such as herbal medicines, acupuncture points, and diagnosis. Although Yosandang is not a widely known medical book, it is meaningful in that it shows a cross section of late Joseon Korean medicine. So, this paper considers rhymes of herbal medicine in Yosandang. Yosandang is a medical book of the late Joseon period written by doctor Byun Gwangwon in 1806. This book consists of 6 books and 14 volumes, 13 of which are rhymes of herbal medicine. The rhymes of herbal medicine in Yosandang is 7 words-2 phrases form for the first time in Korea. This is almost 80 years earlier than the same form of Bangyakhabpyeon (『方藥合編』). The first part of rhymes of herbal medicine in Yosandang is considered to refer to the rhymes of herbal medicine in Jejungsinpyeon (『濟衆新編』), and the last part seems to be based on the contents of Donguibogam (『東醫寶鑑』). In other words, rhymes of herbal medicine in Yosandang could be considered as the result of trying to contain herbal medicine knowledge as a new 7 words-2 phrases form based on the Jejungsinpyeon (『濟衆新編』) and Donguibogam (『東醫寶鑑』). Unlike the previous rhymes of herbal medicine made during the compilation of medical books led by the late Joseon government, the rhymes of herbal medicine in Yosandang is a new type of rhymes created based on individual efforts in the early 19th century. It has a medicine historical significance in that it can show some aspects of Korean medicine in the late Joseon period.

편두통 환자의 한약치료 임상연구에 대한 고찰 (Review of Clinical Research on Herbal Medicine Treatment of Migraine)

  • Objectives: The purpose of this study was to investigate the effects of herbal medicine treatment for patients with migraine and to suggest research methods for herbal medicine treatment on migraine. Methods: In this study, a search was conducted through several academic sites using a combination of terms '편두통', 'Migraine', '한약', '한약치료', 'Herbal medicine', 'Herbal medicines', 'Herb', 'Traditional Chinese Medicine', and 'TCM'. Randomized controlled trials using herbal medicine treatments for adult patients with migraine were selected. Results: Ultimately, 46 papers were selected and analyzed. A statistically significant improvement was noted in the treatment group in terms of clinical migraine symptoms and other migraine evaluation tools before and after the herbal medicine treatment. Herbal medicines were administered in decoction, pill, and granule formulations. Many kinds of medicinal herbs, such as 解表藥類, 補益藥類, 淸熱藥類, 活血祛瘀藥類, and 平肝藥類, have been used for migraine. Among them, 川芎, belonging to 活血祛瘀藥類, is mentioned 36 times and is the most frequently used medicine. Herbal medicine was used safely for migraine treatment, without major adverse reactions, and the recurrence rate was significantly lower in the treatment group than in the control group. Conclusions: In conclusion, the herbal medicine treatment for patients with migraine showed a statistically significant improvement in 46 papers. Future studies should utilize standardized and objective evaluation tools, along with appropriate experimental design. The relevant articles should be increased to a significant level to verify the effect of herbal medicine treatments on migraine.

구안와사에 대한 훈증요법 복합치료 효과 (The Clinical Study for Peripheral Facial Paralysis Treated with Herbal Stream Therapy)

  • Objectives : This study was performed to investigate the effect of herbal steam therapy on peripheral facial paralysis. Methods : 55 patients with peripheral facial paralysis were treated with Korean Medicine therapy including herbal steam therapy. We evaluated the effect of Korean Medicine therapy including herbal steam therapy by Yanagihara grading system. Results : 1. 55 patients were treated with Korean Medicine therapy and 23.27±14.11 times of herbal steam therapy. 2. After treatment, Y-system score was significantly increased(p-value<0.001) from 18.00±6.5514 to 35.00±6.3404. Conclusions : Herbal stream therapy seem to be effective to improve symptoms of peripheral facial paralysis. Further studies will be needed to identify the beneficial of herbal steam therapy on peripheral facial paralysis.

Systems pharmacology approaches in herbal medicine research: a brief review

  • Herbal medicine, a multi-component treatment, has been extensively practiced for treating various symptoms and diseases. However, its molecular mechanism of action on the human body is unknown, which impedes the development and application of herbal medicine. To address this, recent studies are increasingly adopting systems pharmacology, which interprets pharmacological effects of drugs from consequences of the interaction networks that drugs might have. Most conventional network-based approaches collect associations of herb-compound, compound-target, and target-disease from individual databases, respectively, and construct an integrated network of herb-compound-target-disease to study the complex mechanisms underlying herbal treatment. More recently, rapid advances in high-throughput omics technology have led numerous studies to exploring gene expression profiles induced by herbal treatments to elicit information on direct associations between herbs and genes at the genome-wide scale. In this review, we summarize key databases and computational methods utilized in systems pharmacology for studying herbal medicine. We also highlight recent studies that identify modes of action or novel indications of herbal medicine by harnessing drug-induced transcriptome data.

건강보험 급여 한약제제 56종 처방의 계량적 문헌 근거 현황 분석 (Analysis of the current status of quantitative literature evidence for the prescription of 56 herbal medicines covered by health insurance)

  • Objectives: The purpose of this study is to analyze the current state of quantitative literature evidence for the prescription of 56 herbal medicines covered by health insurance that have been studied in Korea for the past 30 years, to evaluate the reliability of the evidence, and to find out the research direction of herbal medicine prescription in the future. Methods: 56 kinds of herbal medicine prescriptions were searched in domestic literature search databases OASIS, DBpia, and overseas PubMed, classified into chemistry, toxicity, cells, animals, clinical cases, and clinical trial studies, and built into an EBM pyramid structure. Results: When classified according to research contents, there were 61 cases (7.5%) of physicochemical analysis to identify constituent substances, 80 cases (9.8%) of toxicity evaluation, and 672 cases (82.7%) of efficacy evaluation. The efficacy evidence was classified according to the evidence-based medical pyramid structure: 196 cell trials (29.1%), 372 animal trials (55.4%), 89 case and case reporting series (13.3%), 7 comparative case studies (1.1%), and 8 randomized control clinical trials (1.2%). In the pyramid composition, the basis for the validity of 56 kinds of herbal medicines prescribed was 568 cases (84.5%) in cell and animal units, which could not be said to be highly reliable. There was no relationship between the ranking of quantitative literature evidence for herbal medicine prescriptions and the ranking of salary administration. Conclusions: In an era that continues to require scientific evidence for herbal medicine, traditional herbal medicine should secure the basis for safety validity even for the 10th most frequent prescription among 56 herbal medicine prescriptions for consumers. In particular, traditional herbal medicine should increase the quantitative and qualitative level of case reports on related herbal medicine prescriptions, focusing on each clinical society, and move toward comparative case studies and randomized clinical trial so that traditional herbal medicine is positioned as Evidence-based medicine.

베트남 약용식물 추출물의 알도즈 환원 효소 억제 효능 검색(XI) (Screening of Vietnamese Herbal Medicines with Inhibitory Effect on Aldose Reductase (XI))

  • Aldose reductase (AR) has been indicated the critical enzyme of the polyol pathway in the development of the diabetic complications. In this study, 119 Vietnamese herbal medicines have been investigated for inhibitory activities on AR. Among them, 7 herbal medicines, Synedrella nodiflora (stems and leaves), Combretum sundaicum (stems and leaves), Argyreia acuta (stems and leaves), Platea latifolia (whole plants), Linociera sangda (whole plants), Rhaphiolepis indica (whole plants), Harrisonia perforata (stems and leaves) and 2 fractions from Harrisonia perforata (stems and leaves) with EtOAc and BuOH, exhibited a significant inhibitory activity against AR. Particularly, S. nodiflora, A. acuta and H. perforata (stems and leaves) showed 1.0-1.8 times more potent inhibitory activity than the positive control, 3,3-tetramethyleneglutaric acid (TMG).

한약이 골다공증 혹은 골감소증 환자의 골밀도에 미치는 영향: 체계적 문헌고찰 및 메타분석을 위한 프로토콜 (Effects of Herbal Medicines on Bone Mineral Density Score in Osteoporosis or Osteopenia: Study Protocol for a Systematic Review and Meta-Analysis)

  • Objectives The impact of social and economic losses on society due to the occurrence of osteoporosis patients is serious. The purpose of this study is to identify the types and effects of herbal medicine for osteoporosis and osteopenia. Methods We will investigate 8 online databases (MEDLINE/PubMed, Cochrane library, EMbase, China National Knowledge Infrastructure [CNKI], J-stage, National Digital Science Library [NDSL], Research Information Sharing Service [RISS], Koreanstudies Information Service System [KISS]) without language, publication date limitation for the clinical study of herbal medicine for osteoporosis and osteopenia published between the start of the database and 2021. The frequency, duration and pattern of the any type of herbal medicine utilization will be analyzed. Results Randomized controlled trials about herbal medicine or herbal medicine extracts for osteoporosis and osteopenia should be included in the study. Cochrane risk in bias tools will be used to evaluate the methodological quality of the study. A risk ratio or mean difference with a 95% confidence interval will show the effects of herbal medicine or herbal medicine extracts for osteoporosis and osteopenia. Conclusions The results of the systematic review will reflect the current status of herbal medicine treatment for osteoporosis and osteopenia, and it can be expected to verify the validity of the effectiveness, and provide it as a basic data for the use of herbal medicine in clinical utilization.

한약을 사용한 암환자 대상 임상시험에서의 항암 효능 평가변수 고찰 (A Review of Anticancer Efficacy Outcome Measures in Clinical Trials of Herbal Medicine for Cancer Patients)

  • Objectives : The objective of this review is to examine the variety of evaluation parameters utilized in clinical trials that assess the anticancer efficacy of herbal medicine, focusing on the importance of including both symptomatic management and direct anticancer effectiveness. Methods : A detailed literature review was conducted across PubMed, Embase, and the Cochrane Library to identify clinical trials investigating the antitumor efficacy of herbal medicine. The search was performed on February 22, 2024. This review specifically examined the employed outcome measures, which were then categorized and analyzed to understand their relevance and application in evaluating the anticancer properties of herbal medicine. Results : From an initial search of 900 records, 15 clinical trials were selected for in-depth analysis after deduplication and screening. These studies evaluated the efficacy of herbal medicine across various cancers, including hepatocellular carcinoma, colorectal cancer, and breast cancer, using outcome measures such as survival rates, disease control rates, and quality of life improvements. The research spanned multiple countries, primarily in East Asia and the United States, reflecting a global interest in herbal medicine as a complementary approach to cancer treatment. The present study demonstrated that herbal medicine, especially when used alongside standard treatments, potentially improved clinical outcomes and patient well-being. Conclusions : The findings of this review highlight the need for a broader focus on the full range of therapeutic capabilities of herbal medicine, including its direct anticancer effects, in the management of cancer patients. Future oncology research involving herbal medicine should integrate a wide spectrum of clinical endpoints to fully ascertain its impact on cancer treatment and patient health.

HPV감염질환으로 6개월 이상 한약을 복용한 환자 121명의 간기능검사(AST/ALT)에 대한 후향적 고찰 (Clinical Study of Liver Function Tests (AST/ALT) of 121 HPV Disease Patients taking Herbal Medicine over 6 Months)

  • There are some controversies about the safety of herbal medicine. In order to examine the safety of herbal medicine, this study investigated the level of liver function test(AST/ALT) in patients taking herbal medicine for more than 6 months We checked liver function in 121 patients who took herbal medicine for more than 6 months. AST/ALT were measured before treatment and every 3 months (from 3 month to 9 month). In 121 patients taking herbal medicine for more than 6 months, mean AST level after 6 months was lower than before treatment and mean ALT level after 6 months was lower than before treatment. In 20 patients with abnormal AST/ALT before herbal treatment, 18 patients's AST/ALT changed to normal after 6 month herbal treatment. 2 patients's AST/ALT was slightly higher than normal. One woman patient met the criteria for herb-induced liver injury(HILI) with RUCAM scores 4 after taking herbal medicine for 6 months. Although her RUCAM score decreased to zero after taking herbal medicine for 9 months. This study suggests that long-term herbal medicine for 6 months or longer is very unlikely to injury liver function. Individual-specific liver damage may occur, it can be recovered.

본초서(本草書)의 계통(系統)과 본초학(本草學) 발전사(發展史) (The Lineage of Herbal Medicine Books and the Development of Herbal Medicine Studies)

  • This paper proposes to examine how herbal medicine studies developed amidst the historical changes, by studying the publications of herbal medicine books. For this reason, centering on the most eminent herbal medicine books of each period, the characteristic of the times shown by their publication and the development of herbal medicine were summarized. Also, traces of herbal medicine books among the eminent classics that were imported into our medicine and made a deep impact, and whether or not a Chosun edition was published were researched and introduced in order to show the influence it had on Korean Medical History.
