I participated in Academic Exchange Program(Action plan II) between KADH(Korean Association for Disability and Oral Health) and JSDH(Japanses Society for Disability and Oral Health) for 2 months from 3rd July 2012 to 2nd september 2012 in the Department of Hygiene and Oral Health, School of Dentistry, Showa University at Tokyo, Japan. I have observed their operation process and learned what dysphagia is and how it is consulted and taken care of as a therapy for patients with eating and swallowing disorders for two months in The department of special needs dentistry at Showa University Dental Hospital, Jonan Branch of Tokyo Metropolitan Kita Medical Rehabilitation Center for the Disabled, Smile Nakano Center, Tokyo metropolitan center for persons with disabilities in Lidabashi for one week, Eating and swallowing functional therapy workshop for disabled children, Tokyo metropolitan Tobu medical center for Persons with Developmental/Multiple Disabilities located in Minamisunamitchi for one week and on The 17-18th JSDR(Japanese Society of Dysphagia rehabilitation) in Sapporo. Through Action Plan II program, I learned how precious eating, drinking and swallowing with ease are and observed how they do and what they do as a dentist or a dental hygienist in Japan for dysphagia patients. Therefore, I want to present the dental approaches of children with dysphagia in Japan, based on my experience for two months.

To harmonize with the remaining natural teeth a dentist and technician make an effort to do. Dental ceramic perfectly reproduce the functionality and esthetic is so moved that will deliver to the patient. However It is not easy to overcome the problem. Actually, it can't have the same spectrum curve between different object. The spectrum curve and reflectance is a unique feature of an object like fingerprints. So it is not that the identification of spectral curves that we usually focuses color. We need to understand the process of matamerism makes something like a combination of color perception. In other word that will tell in our field with ceramic teeth of the patient wish to match the color matching process to simulate the cone in our retinas with the same combination.
In recent years, precision machining of the dental prosthesis by computer assisted system is becoming pervasive in clinical dentistry. Prosthesis fabricating system that is designed by computer software and made by computer devices is called as a CAD/CAM (Computer-Aided Design/Computer-Aided Manufacturing) system. By the use of dental CAD/CAM system, the improvement of marginal compatibility and mechanical properties in prosthesis can be obtained more effectively, an aesthetic quality by using new materials such as zirconia can be increased. Also, the restoration process can be simple and efficient, the production time can be shortened, the process of manufacture can be standardized, and the mass production is possible. What is clear is that these benefits are theoretically possible, but the dentist or dental technician must understand the CAD/CAM basic principles and limitations for obtaining the maximum advantages of CAD/CAM system. For this reason, this article will be presented about the basic principles of CAD/CAM system and the factors of error that might occur in the CAD/CAM process based on my empirical study.

One of the many dilemmas that the clinical restorative dentist must face is treating young adolescent patient who prematurely loses his permanent teeth. Temporary prosthetic replacement can be achieved with removable denture, orthodontic band-wire fixed denture, adhesion bridge, composite resin splint with reinforcing material until the patients go through growth and development. But, all of these have limitations. Advances in restorative materials and reinforcement materials have made possible new techniques which are as much esthetic, conservative and more economic and stronger than adhesion brides. Two cases are being presented where gas-plasma treated, woven polyethylene fabric to reinforce composite resin was used to fabricate a temporary prosthetic restoration to replace a missing maxillary central incisor. This relatively noninvasive and basically reversible procedure allows the patient to decide the final restoration as he or she goes thorough maturation of the hard and soft tissues.
FRC/ceromer system provides the clinician with a durable, flexible, and esthetic alternative to conventional porcelain fused to metal crowns. FRC is the matrix which is silica-coated and embedded in a resin matrix. The ceromer material which is a second generation indirect composite resin contains silanized, microhybrid inorganic fillers embedded in a light-curing organic matrix. FRC/ceromer restoration has a several advantages: better shock absorption, less wear of occluding teeth, translucency, color stability, bonding ability to dental hard tissues, and resiliency. It has versatility of use including inlay, onlay, single crown, and esthetic veneers. With adhesive technique, it can be used for single tooth replacement in forms of inlay adhesion bridge. In single tooth missing case, conventional PFM bridge has been used for esthetic restoration. However, this restoration has several disadvantages such as high cost, potential framework distortion during fabrication, and difficulty in repairing fractures. Inlay adhesion bridge with FRC/ceromer would be a good alternative treatment plan. This article describes a cases restored with Targis/Vectris inlay adhesion bridge. Tooth preparation guide, fabrication procedure, and cementation procedure of this system will be dealt. The strength/weakness of this restoration will be mentioned, also. If it has been used appropriately in carefully selected case, it can satisfy not only dentist's demand of sparing dental hard tissue but also patient's desire of seeking a esthetic restorations with a natural appearance.
Objectives: This study evaluated the outcomes of continuing professional education in implant dentistry using Kirkpatrick's four-level evaluation model. Material and methods: The study was carried out through a questionnaire distributed to dentists who attended a continuing professional education in implant dentistry, 2008~2012. They were asked to fill out questionnaires at least 6 months after its completion. Results: Mean ages of total 23 dentist was $44.8^{\circ}{\ae}8.2$. Mean period after completion of education was $2.7^{\circ}{\ae}1.2$. Knowledge (level 2) (r=0.71, p<0.01) and behavior (level 3) (r=0.68, p<0.01) was positively correlated with topic and methods of education in reaction (level 1). Behavior was positively correlated with knowledge (r=0.79, p<0.01). Result (level 4) was positively correlated with knowledge (r=0.64, p<0.01) and behavior (r=0.86, p<0.01). Conclusion: Reaction affects on knowledge, behavior and result in order.
Objective: Teeth discoloration is a rare orthodontic complication. The aim of this study was to report the clinical progression of discoloration during orthodontic treatment. Methods: Discolored teeth, detected during orthodontic treatment between January 2003 and December 2012 by a single dentist using similar techniques and appliances, were analyzed. Results: The total number of teeth that showed discoloration was 28. Progression of discoloration was evaluated in only 24 teeth that were observed without any treatment. During the observation period, the discoloration "improved" in 8 of the 24 teeth (33.3%) and was "maintained" in 16 (66.6%). The electric pulp test performed at the time of initial detection of discoloration showed 14.3% positivity, which improved to 21.4% at the final follow-up. None of the initial and final follow-up radiographic findings showed any abnormalities. Conclusions: When teeth discoloration is detected during orthodontic treatment, observation as an initial management is recommended over immediate treatments.

In most persons with disabilities had poor oral hygiene because of less attention and ability to perform a dental care. So the increased prevalence and severity of dental disease were common oral state. Although most persons with disabilities need a adequate dental treatment, it is often very difficult to treat because of noncooperativity and involuntary muscle movements. Dental treatments under sedation and general anesthesia were make to provide a high-quality dental service because of decreased anxiety and fear associated dental treatment in persons with disabilities. The dental professionals must be able to select and apply the proper sedation methods in agreement with the characteristics of the disabilities, general conditions, sedation experience and capacity of dentist, type and time of dental treatment, equipments of dental clinic, consent of patient's protector. The proshodontic treatment procedures, such as abutment preparation, dental impression taking process, try-in process of prosthesis and adjustment of occlusion, are difficult even for patients without disabilities. Those procedures are more difficult to patients with disability because it's too hard to control breathing and muscle. In this report, we performed prosthetic dentistry procedures to three patients with disabilities under dental sedation and general anesthesia.
Dental implant has recently become one of the most viable treatment options for regaining the oral function and dental aesthetics compared with conventional fixed or removable dentures. Dental implants vary in material, dimensions, geometries, surface properties, and interface geometries. It has been reported that there has been a proliferation of manufacturers who produce implants using various materials and surface treatments, and the dentist needs to select from over 2,000 different dental implants and abutments in a specific treatment situation. Unfortunately, however, no metrics have been specifically identified for the purpose of quality assessment and selection of an appropriate dental implant. This study aims to provide practical guidelines for quality assessment of dental implant based on clinical data. Like other medical devices and materials, the superiority related to specific characteristics of the dental implant needs to be verified through extensive clinical studies. The procedures of clinical monitoring for dental implants have been proposed along with a case study to exemplify the usefulness of clinical monitoring for the purpose of continuous quality improvement in medical industry.

A general treatment is to restore abfraction lesions with dental filler materials to reduce stress concentration. A material should be selected from various dental products based on long term experiences of dentist or personal preference concerning filler methods. A quantitative criterion is necessary to make an evaluation of the results as dentists decide treatment methods and dental materials relying on their clinical experiences. The purpose of this study is to find an optimal restoration method and material for noncarious cervical lesions using the finite element method. An objective function was defined to minimize the sum of tension or compression stress. Trial-and-error and approximation were used to find an optimal restoration method. An optimal solution was to fill TetricFlow inside the lesion and Z100 in the remaining region. The most desirable thickness ratio of the two filler materials was 0.125 with trial-and-error and it was similar to the results of approximation, 0.121 and 0.132.
