Effects of the Nei-Guan (P6) Acupressure on Nausea, Vomiting, and Pain in Geriatric Patients after Total Knee Replacement Arthroplasty (내관지압이 슬관절 전치환술 받은 노인 환자의 오심, 구토 및 통증에 미치는 효과)

  • Purpose: Patient-controlled epidural analgesia (PCEA) is an effective method for controlling post surgical pain. However, it is associated with adverse drug reactions such as nausea and vomiting. In this, study we tested the effects of Nei-Guan (P6) acupressure on PCEA-associated nausea, vomiting, and pain in geriatric patients after total knee replacement arthroplasty (TKRA). Methods: Ninety-nine patients who had TKRA for the first time were randomly assigned to either, experiment group (n=50) or control groups (n=49). All patients received PCEA immediately after surgery, but acupressure on Nei-Guan (P6) point was applied to experiment group only. Ten minutes of finger acupressure on Nei-Guan (P6) acupressure point was applied twice with 15 minute-interval in the experiment group. The incidence of nausea, vomiting, and retching as well as pain intensity were assessed at 12 and 24 hours after surgery. Nausea, and vomiting were assessed by Index of Nausea, Vomiting and Retching (INVR) questionnaire. Pain intensity was measured with frequency of analgesics. Results: Vomiting both was significantly different between two groups at 12 hours (t=-2.18, p=0.03) and 24 hours (t=-2.64, p=0.01) after surgery. Total scores of nausea, vomiting and retching in experimental group was significantly lower than control group 24hours after surgery (t=-2.18, p=0.03). However, pain was not different between two groups. Conclusion: Nei-Guan (P6) acupressure may be considered as an effective nursing intervention to decrease PCEA-associated nausea and vomiting in older patients after TKRA.

Conservative Treatment for Injured Anterior Cruciate Ligament - Two Cases Report - (손상된 전방십자인대의 보존적 치료 - 증례 보고 -)

  • While the cruciate ligament has a profuse vascular response following injury, spontaneous repair does not occur. This may result from the fact that synovial fluid dilution of the hematoma following injury prevents the formation of a fibrin clot and thus the initiation of the healing mechanism. Another theory suggests that the dynamic nature of the fascicles of the anterior cruciate ligament(ACL) through even small ranges of motion prohibits spontaneous union to these fibers. But we experienced two cases of spontaneous healing of partially injured ACL. Initially they showed more than grade II anterior instability. 6 mm difference by stress roentgenographs(pull view) and difference of 8 mm by KT 1000TM arthrometer between the ACL injured knee and normal side knee. Lax, nearly complete tear of ACL and synovial bleeding were noted during arthroscopic examination but the continuity of synovial membrane was seemed to be intact. These cases were treated by conservative management rather than reconstructive procedure. Postoperatively they showed excellent clinical results, no anterior instability and unlimited athletic activity. Based on our clinical experience. we think that cruciate ligament has the spontaneous healing potential in acute stage and middle aged patient. We consider the microfracture technique and initial immobilization for accelleration of healing response of the ACL.

MRI Diagnosis of the Tear Pattern of Menisci (반월상 연골판 파열 양상의 자기공명영상 진단)

  • In the diagnosis of the meniscal tear of the knee, the high accuracy of the MRI diagnosis is well-known, but the accuracy of the MRI in the diagnosis of the very pattern of the tear of menisci is not well-established. The purpose of this study is to give some informations to consider in the diagnosis and therapeutic planning of torn menisci. The authors performed a retrospective study comparing the MRI and arthroscopic findings of 141 knees which had undergone arthroscopic surgery from Mar. 1997 to Mar. 1998. The results are as follows. In the diagnosis of tear of the menisci, MRI had sensitivity of 91%, specificity of 96%, accuracy of 95%. The mismatch of the tear patterns of the menisci between MRI and arthroscopic findings was identified in 59%, especially high in flap tear(100%), complex tear(84%), peripheral tear(55%). In conclusion, MRI is very accurate in the diagnosis of the tear of menisci, but the tear patterns of the menisci cannot be accurately determined by MRI. In the cases of flap tears, complex tears and peripheral tears, it is more difficult to determine the tear patterns of the menisci by MRI. False-negative rate of MRI was especially high(44%) in the peripheral tear type. So, other clinical correlations should be made in determination of the presence of the tear in the diagnosis of peripheral tear of the menisci.

Arthroscopic Treatment of an Acute Septic Arthritis after Meniscal Allograft Transplantation - A Case Report - (반월상 연골 동종 이식술 후 발생한 화농성 관절염의 관절경적 치료 - 증례 보고 -)

  • We report the case of a 21-year-old man with acute septic arthritis of the knee after meniscal allograft transplantation, which was successfully treated with repeated arthroscopic debridement and irrigations. Our procedures included arthroscopic debridement and irrigation with 10L normal saline, repeated arthroscopic irrigations (5 times), and intravenous antibiotics. Our decision to repeat the debridement was based on clinical and laboratory results. The significance of this case is that early aggressive arthroscopic debridement and repeated irrigations as part of a treatment protocol of acute septic arthritis after meniscal allograft transplantation can be an effective treatment option in selected cases.

Clinical Efficacy of a Modified Surgical Procedure in the Treatment of Incompetent Great Saphenous Veins

  • Background: The aim of this study was to evaluate the clinical efficacy of a modified surgical procedure for the treatment of varicose veins. Methods: This retrospective analysis was conducted on lower extremities with symptomatic great saphenous vein (GSV) incompetence that underwent stripping from the groin to the knee, with preservation of the superficial epigastric vein (SEV), between January 2015 and April 2022. Follow-up assessments were performed using Doppler ultrasound, Venous Clinical Severity Score (VCSS), and the Aberdeen Varicose Vein Questionnaire (AVVQ) at 6 and 12 months after surgery. Results: The study included 179 limbs from 120 patients (47 men and 73 women). The mean patient age was 56.5 years (range, 20-78 years), and the distribution of preoperative Clinical-Etiology-Anatomy-Pathophysiology clinical classes was 8% C0-C1, 88% C2, and 4% C3-C6. The preoperative diameter of the saphenofemoral confluence averaged 6.9 mm (range, 2.7-15.8 mm). After a mean postoperative follow-up period of 24 months, evidence of neovascularization around the stump of the saphenofemoral junction (SFJ) was observed in 2 limbs (1.1%). Additionally, varicose vein recurrence was found in 1 limb (0.6%) and was associated with an incompetent thigh perforator. At postoperative follow-up, both VCSS and AVVQ scores were significantly lower than the preoperative scores. Conclusion: Modified surgical treatment of GSV incompetence, involving preservation of the SEV and stripping of a short segment up to the knee, demonstrated favorable clinical results in terms of postoperative complication rate, neovascularization rate around the SFJ stump, varicose vein recurrence rate, and improvement in lower extremity symptoms.

Hindfoot Alignment Change after High Tibial Valgization Osteotomy in a Patient with an Ipsilateral Fused Ankle: A Case Report (족관절이 유합되어 있는 상태에서 동측 슬관절 고위경골외반절골술을 시행한 이후 후족부 정렬의 변화: 증례 보고)

  • Ankle arthrodesis was performed on a 55-year-old male patient with an active lifestyle who developed severe arthritis in the left ankle. Over the follow-up period, high tibial valgization osteotomy was conducted for painful medial knee joint arthritis with genu varum deformity to correct overall lower limb alignment from varus to valgus with respect to the fused ankle. This study was conducted to investigate how hindfoot alignment would change when the overall alignment of the lower limb shifted from varus to valgus with the ipsilateral ankle in a fused state. Conclusively, while no intrinsic changes in the hindfoot alignment were observed following the alteration of lower limb alignment, the hindfoot naturally adjusted to valgus deviation in response to limb valgus realignment. Moreover, symptoms changed in line with this adjustment. Given the absence of similar case studies or reports, a review of relevant literature is included to contribute to knowledge of this subject.

Effect of Position Sense in Used Taping and Icing on Muscle Fatigue Occurred on Knee Joint (근피로 유발 후 테이핑과 냉 적용이 슬관절 위치감각에 미치는 영향)

  • The purpose of this study is to define the effect of taping and icing on muscle fatigue at knee joint of ordinary adult. The 30 adults meeting research standard were chosen to be observed. The period of the research was 9 days and N-K table was used as equipment to measure position sense of knee joint. Heart rate monitor was also used to measure heart beat during both when in comfortable state and in exercises. For the method of this study, error in position sense of knee joint was measured right before fatigue occurs. Then volunteers were to be exercised for 10 minutes in treadmill aiming to raise the heart beat to 90%. If muscle fatigue occurs after the exercise, let the comparative group rest for 20 minutes while applying taping for group 1 and icing for group 2. For the icing, it was applied for 3 minutes followed by 1 minute rest. 5 sets of icing were applied on thighs and calf each, Then error of position sense of knee joint was measured same way it was done before muscle fatigue. As the result, there were similar differences among comparative group(p<.05), group 1 and group 2 before the muscle fatigue and after the muscle fatigue. There were similar differences in comparative group with muscle fatigue, group 1 and group 2 as the result of one-way ANOVA and with further examination, similar differences were only found in between comparative group and group 2(p<.05). With the result, we can see there wasn't clear effect of rest, icing or taping, however, group 2 with icing showed better condition compared to comparative group who have rested without any application.

Evaluation of O-MAR XD Technique for Reduction of Magnetic Susceptibility Artifact of Knee Implant (인공 무릎관절에서 자화율 인공물의 감소를 위한 O-MAR XD 기법의 평가)

  • Magnetic Resonance Imaging for patients with metallic implant has poor image quality, and signal loss and artifacts including distortion can occur. The purpose of this study is to carry out a comparative evaluation on high receive bandwidth(hiBW), O-MAR, O-MAR XD to reduce artifacts in knee implant. To take MRI, 3.0T scanner and dual-source radiofrequency transmission were used. O-MAR XD technique's strong option showed a significant difference (p<0.001) with O-MAR XD technique's weak option, O-MAR and hiBW excluding the medium option. O-MAR XD's medium option had a significant difference (p<0.01) with O-MAR XD's weak, O-MAR and hiBW. O-MAR XD technique's weak option had a significant difference (p<0.01) with O-MAR XD's strong and medium options, O-MAR and hiBW. O-MAR technique had a significant difference (p<0.001) with strong, medium, weak options of O-MAR XD technique except for hiBW. HiBW had a significant difference (p<0.001) with strong, medium and weak options of O-MAR XD technique except for O-MAR. The results showed that O-MAR XD technique was more useful for MRI scan for patients with knee replacement surgery than traditional techniques such as hiBW or O-MAR, and susceptibility artifacts decreased more when O-MAR XD technique's strong or medium option was applied. Based on the results above, it is considered that it will be possible to acquire images whose susceptibility artifacts were highly decreased by using O-MAR XD technique's strong or medium option when conducting MRI for artificial knee joint and it will be helpful for checking and monitoring patients with knee joint replacement.

  • This paper presents the three dimensional gait analysis of the patients with osteoarticular knee allograft reconstruction. The gait analysis has been performed in some medical fields such as orthopedics and neurosurgery for the purpose of the rehabilitation of patients. However, to the author's knowledge, the analysis of gait for the patients with osteoarticular knee allograft reconstruction caused by tumor has not been reported. In this work, In this work, we confirmed the validity of this method by analyzing 50 samples per one gait cycle obtained from each of 3 patients and 3 normal persons. The motion capture was performed using six infrared cameras. The symmetry and stability of the gait patterns are investigated (patients' r=0.39, p<0.05, normal persons' r=0.65, p<0.05) respectively using the correlation coefficients and the standard deviations of the joint angles of the left and right legs. It also would be applied to the comparison analysis where artificial knee joint is transplanted.

Systemic Review of Hyaluronate for the Treatment of Osteoarthritis (골관절염 치료제 히알우론산 임상연구결과의 Systemic Review)

  • Background: The multitude of the therapeutic usefulness of intra-articular injection of hyaluronate on oateoarthritis of the knee is still in question. The objective of this systemic review was to elucidate both the therapeutic efficacy and the safety of intra-articular administration of hyaluronic acid for degenerative osteoarthritis of the knee joints. Methods: I searched MEDLINE and Korea Medical Database (KMbase) from January 1990 to April 2007 using a combination search terms for knee osteoarthritis and hyaluronic acid and a filter for randomized controlled trials. I extracted data on pain at rest, and during or just after movement, on joint function, and on adverse events. Results: Ten trials that reported usable quantitative information on any of the predefined end points were identified and included in the systemic review. Intra-articular injection of hyaluronic acid can decrease symptoms of osteoarthritis of the knee. The study revealed significant improvements in pain and functional outcomes with few adverse effects. However, there was significant between-study heterogeneity in the estimates of the efficacy of hyaluronic acid. Sub-group analysis showed that lower methodological quality such as a single-blind or single-center design resulted in higher estimates hyaluronic acid efficacy, and that patients older than sixty years of age and those with the most advanced radiographic stage of osteoarthritis were less likely to benefit from intra-articular injection of hyaluronic acid. Conclusion: According to the currently available evidence, intra-articular hyaluronic acid has been proven clinically effective for the patients bearing the knee osteoarthritis with NSAID-induced GI troubles or inapplicable to any surgery, and may be associated with lower risk of adverse events.
