Effects of body weight-supported treadmill training combined with ball-kicking on balance and gait of subacute stroke patients

  • Objective: This study focused on subacute stroke patients who were asked to kick a ball while walking on a treadmill. The aim of the study was to determine the effect of a body weight-supported treadmill training (BWSTT) combined with ball-kicking on muscle strength, balance, and gait. Design: Single blind, randomized controlled trial. Methods: Twenty stroke patients who volunteered to participate in this study were randomly assigned to either the BWSTT combined with ball-kicking (BWSTT-BK; 10 participants) group or the BWSTT group (10 participants). Participants in the BWSTTBK group performed treadmill walking combined with simultaneous ball-kicking for 30 minutes daily for 5 weeks. Participants from the BWSTT group performed only treadmill walking. The muscular strength, balance, and gait ability were measured before and after the 5-week training. To assess for muscular strength, a digital muscle tester was used to measure hip flexor, knee extensor, and dorsiflexor strength. To assess for balance, the Berg Balance Scale (BBS) and Timed Up and Go Test (TUG) was used. To assess for gait, the 10 meter walk test (10MWT) and Functional Gait Assessment (FGA) was used. Results: The BWSTT-BK group showed significantly improved muscular strength, balance, and gait according to BBS, TUG, 10MWT, FGA, and digital muscle testing scores compared to the BWSTT group (p<0.05). In addition, within-group comparison showed significant improvement in all variables (p<0.05). Conclusions: These findings suggest that BWSTT-BK results in more favourable outcomes for stroke patients. Therefore, BWSTT-BK may be useful for the recovery of gait ability of stroke patients.

The Effect of Exercise Intensity on Muscle Activity and Kinematic Variables of the Lower Extremity during Squat

  • Objective: The purpose of this study was to determine how exercise intensity affects muscle activity and kinematic variables during squat. Method: Fifteen trainers with >5 years of experience were recruited. For the electromyography (EMG) measurements, four surface electrodes were attached to both sides of the lower extremity to monitor the rectus femoris (RF) and biceps femoris. Three digital camcorders were used to obtain three-dimensional kinematics of the body. Each subject performed a squat in different conditions (40% one-repetition maximum [40%1RM], 60%1RM, and 80%1RM). For each trial being analyzed, three critical instants and two phases were identified from the video recording. For each dependent variable, one-way analysis of variance with repeated measures was used to determine whether there were significant differences among the three different conditions (p<.05). When a significant difference was found, post hoc analyses were performed using the contrast procedure. Results: The results showed that the average integrated EMG values of the RF were significantly greater in 80%1RM than in 40%1RM during the extension phase. The temporal parameter was significantly longer in 80%1RM than in 40%1RM and 60%1RM during the extension phase. The joint angle of the knee was significantly greater in 80%1RM than in 40%1RM at flexion. The range of motion of the knee was significantly less in 80%1RM than in 40%1RM and 60%1RM during the flexion phase and the extension phase. The angular velocity was significantly less in 80%1RM than in 40%1RM and 60%1RM during the extension phase. Conclusion: Generally, the increase of muscle strength decreases the pace of motion based on the relation between the strength and speed of muscle. In this study, we also found that the increase of exercise intensity may contribute to the increase of the muscle activity of the RF and the running time in the extension phase during squat motion. We observed that increased exercise intensity may hinder the regulation of the range of motion and joint angle. It is suitable to perform consistent movements while controlling the proper range of motion to maximize the benefit of resistance training.

  • Currently, Intelligent femoral prostheses that support the corresponding mode in walking and specific movements are being studied. Certain controls such as upstairs, sitting, and standing require a technique to classify control commands based on the user's intention because the mode must be changed before the operation. Therefore, in this paper, we propose a technique that can classify various control commands based on the user's intention in the intelligent thigh prosthesis system. If it is determined that the EMG signal needs to be compensated, the proposed technique compensates the EMG signal using the correlation between the strength and frequency components of the normal EMG signal and the muscle volume estimated by the pressure sensor. Through the experiment, it was confirmed that the user's intention was accurately detected even in the situation where muscle fatigue was accumulated. Improved intention detection techniques allow five control modes to be distinguished based on the number of muscle contractions within a given period of time. The results of the experiment confirmed that 97.5% accuracy was achieved through muscle tone compensation even if the strength of the muscle signal was different from normal due to muscle fatigue after exercise.

Effects of Energy System Contribution on Isokinetic Muscle Strength in Various Sport Events Athletes (무산소, 유산소 운동종목별 엘리트선수의 등속성 근기능에 미치는 영향)

  • This study was conducted to compare the muscle strength, muscle power, and H/Q ratio according to energy system contribution in athletes participating in various sports. Subjects of the study were assigned into an Anaerobic Exercise Group (AEG, n=60; Short-Distance, Weight Lifting, Jumping, Throwing, Bowling, Golf) and an Endurance Exercise Group (EEG, n=60; Modern Pentathlon, Field Hockey, Handball, Cycle, Boxing, Rowing) groups. Isokinetic peak torque/body weight% and flexor/extensor ratio at 60, 180 deg/sec of knee extension and flexion were measured using an cybex 770. Data analysis was conducted using an independent t-test and one-way ANOVA. Based on the results of this study, there was higher extension, flexion strength and flexion power in the AEG than the EEG (p<0.05). We also confirmed higher muscle strength and muscle power in short distance and jumping athletes than other athletes participating in other events (p<0.05). However, there was no significant difference within the endurance exercise group. The HS ratio was within a stable range of 50% to 60% in all events. Collectively, the outcomes of this study indicate that routine physiological and performance testing can provide measurable benefits for elite athletes and their coaches.

  • Purpose: This study was undertaken to investigate the effects of foot reflexo-massage on physical, psychological, and physiological parameters in patients with osteoarthritis of the knee. Method: This study was employed non-equivalent control group pretest-posttest time series experimental design. The subjects of this study were 47 elderly patients with osteoarthritis of the knee from two different nursing homes in Deajeon. The patients were divided into two groups; 26 patients in experimental group, the rest in control group. Each patient in experimental group received the 30-minute foot reflexo-massage, 3 times per week for 4 weeks. The dependent variables of each patient were measured before treatment, after 2 weeks and after 4 weeks during the treatment session. The collected data was analyzed using the Social Package Social Science(version 10.0) software program. Result: There were significant differences in the flexion angle of both knees; the flexion and extension muscle strength of both knees; systolic blood pressure; POMS; plantaris skin temperature and blood velocity of dorsalis pedis artery between the two groups over different the three measurement times. Conclusion: The results suggest that foot reflexo-massage could be an effective intervention to improve physical, psychological and physiological parameters for patients with knee osteoarthritis.

  • Purpose : The main purpose of this study was to investigate the effect on Balance Ability of Knee Osteoarthritis(OA) by modality. Methods : The subjects were consisted of 30 women patients with knee OA. All subjects were randomly assigned to modality group. Each group had a treatment for 45 minutes per day and three times a week during 8 weeks period. Was used to measure recovery or worse of patient's condition, muscle assessment questionnaire(MAQ) was used to measure patient's muscular strength, Endurance, coordination/balance, KWOMAC was used to pain, stiffness, and physical function, and BPM was used to measure velocity, anterior-posterior. Results : This study results in following conclusions. 1. MAQ score was significantly decreased in modality group(p<.05). 2. KWOMAC score was significantly decreased in modality group(p<.05). 3. BPM were score was significantly decreased in modality group(p<.05). Conclusions : From this result the modality treatment retrogression characteristic will be effective in treatment of patient.

Effects of 8-Week Elastic Resistance Exercise on Knee Isokinetic Rate of Velocity Development and Balance by Age

  • Ten women in their 20s and 10 women in their 50s were selected to investigate the effect of an elastic band exercise program for 8 weeks according to age on women's knee speed expression rate and balance. Knee isokinetic muscle strength measurement, single-legged standing with eyes closed, and YBT were performed 1 week before and after the exercise program. The measured data were analyzed through a mixed design two-way ANOVA, and if there was a significant difference, post hoc verification was performed using the bonferoni method. In our study, as a result of an 8-week elastic band exercise program, there was no difference in the speed development rate according to the measurement period, and the speed development rate according to age was found to be higher in people in their 50s than in their 20s. In our study, there was no difference in balance ability depending on the measurement period, and there was also no difference in balance ability depending on age. Discussing the results of our study, we found that 8 weeks of elastic band exercise cannot bring about significant changes in speed development ability and balance ability, which become more accurate with age.

Effects of Sit-to-Stand Training on Unstable Surface on Balance in Subject With Stroke (불안정지지면에서 일어서기 동작훈련이 뇌졸중 환자의 균형능력에 미치는 영향)

  • The purpose of this study was to determine the effectiveness of sit-to-stand training on unstable surfaces in individuals with stroke. Nineteen subjects with chronic stroke were divided into two groups: an experimental group (10 subjects) and a control group (9 subjects). They received 30 minutes of Neuro-developmental therapy (NDT) treatment, and sit-to-stand exercise for 15 minutes three times a week for four weeks. During the sit-to-stand training, the experimental group performed on an unstable AIREX balance pad, but the control group performed on a stable surface. Balance ability and weight-bearing distribution during quiet standing were measured before and after training period using the 7-item Berg balance scale-3P (BBS-3P) and the Five-times-sit-to-stand test (FTSST). In addition, the muscle strength of the knee extensor was evaluated before and after the training period. The results were as follows: 1) The weight-bearing distribution forward of the affected leg, increased significantly in the experimental group after the four-week intervention (p<.05), 2) The 7-item BBS-3P and FTSST increased significantly in the experimental group after the four-week intervention (p<.05), 3) The knee extensor muscle strength in both groups increased significantly after the four-week intervention (p<.05). In conclusion, the results of this study did not show that the sit-to-stand training on an unstable surface was more effective than on a stable surface. However, the results suggested that sit-to-stand training is effective in the balance training of stroke patients.

The Effects of Lumbar Stabilization Exercise and Strengthening Exercise of Lower Extremity on Pain and Muscle Strength of Leg in Patients with Chronic Low Back Pain (요부 안정화운동과 하지 근력강화운동이 만성 요통환자의 통증과 하지근력에 미치는 영향)

  • Purpose: This study was to investigate the effects of lumbar stabilization exercise and lower extremity strengthening program on pain, lower extremity muscle in patients with chronic low back pain. Method: The subject were randomly divided two groups. 15 people who were conducted lower extremity exercises and lumbar stabilization exercises called the combined exercise group and other(15 people) who were only conducted lower extremity exercise group. The assessment tools were the pain level and the led muscle power. Exercise was conducted for eight weeks. Result: Pain of the combined exercise group showed significant differences in the change in pain level(p<.05). The leg muscle power showed significant differences within group which hip flexor, extensor muscles and abductor muscles. there were significant differences within combined exercise group (p<.05). But the knee joint in each group showed a significant difference within group (p<.05). Conclusion: This study suggest that the lumbar stabilization exercises and lower extremity exercises showed more efficient results in the pain levels and leg muscles power than only the lower extremity exercise for patients with lumbar instability.
