Treatment of Transverse Patella Fracture with Minimally Invasive Load-Sharing Patellar Tendon Suture and Cannulated Screws (최소 침습 기법 슬개건 부하 분산 봉합술과 유관 나사못을 이용한 슬개골 횡골절의 치료)

  • A transverse fracture is the most common type of displaced patella fracture requiring surgery. These fractures are commonly fixed with parallel Kirschner wires or screws that cross the fracture line, often with an additional tension band. Nevertheless, conventional fixation methods of patella fractures have prevalent complications caused by the protrusion of wires or pins. These complications necessitate additional surgery for hardware removal, increase medical cost, and can limit the function of the knee joint. This paper reports cases treated with a minimally invasive load-sharing percutaneous suture of the patella tendon. The procedure provides reliable fixation for transverse patella fractures, minimizes soft tissue injuries, preserves blood flow, and reduces postoperative pain. In addition, the procedure also reduces the irritation and pain caused by the internal fixture, thereby reducing the risk of restricted knee joint movement.

Analysis of Major Physical Health Conditions and Disabilities that Limit Activity in Later Stage Elderly (후기 노인의 활동을 제한하는 주요 신체적 건강 상태와 장애 분석)

  • PURPOSE: This study is a data analysis study to determine the physical health problems and reasons for disabilities and activity limitation rates that limit daily life and social activities among the elderly population aged 75 or older in Korea. METHODS: This study data was extracted from the raw data of the 7th National Health and Welfare Survey (2016-2018). The subjects of this study were 1,995 elderly people (823 men, 1,172 women) aged 75 years or older. The collected data were analyzed using frequency analysis and logistic regression analysis. RESULTS: From 2016 to 2018, the activity limitation rate among the elderly population aged 75 or older in Korea was 20.6% for men, 24.6% for women, and 23.1% overall. The three major diseases with the highest frequency of activity limitations were back and neck problems (36.5%), arthritis and rheumatism (28.7%), and knee and leg pain (14.7%). Activity limitation due to old age was found to be 13.1%, making it the fourth most frequent reason. The rate of activity limitations due to mental retardation and obesity was found to be 0%. The three major activity limitation rates were significantly related to gender. CONCLUSION: The main diseases causing activity limitations among the elderly population aged 75 or older in Korea were back and neck problems, arthritis and rheumatism, and musculoskeletal diseases such as knee and leg pain. Therefore, it is believed that it can be used as basis data for reducing the activity limitation rate of the elderly population in the aging era.

The Effect of Arthroscopic Medial Meniscectomy in Degenerative Arthritis of the Knee (슬관절 퇴행성 관절염에서 관절경적 내측 반월상 연골 절제술의 효과)

  • Purpose: In patients with meniscal tear with degenerative arthritis, controversy remains as to whether arthroscopic menisectomy is worthwhile or not. The purpose of this study was to evaluate the effect of arthroscopic medial meniscectomy in degenerative arthritis of the knee with meniscal tear. We also intended to identify pertinent indications and risk factors. Materials and Methods: 287 patients underwent arthroscopic medial meniscectomy from 2006 to 2008; 103 patients who had Kellgren-Lawrence grade II, III arthritis of the knee, were over 50 years old, and had minimum 1 year follow-up, were analyzed in this study. Clinical assessment was performed retrospectively using the arthroscopic surgery database, medical records, questionnires and interviews. Assessment included visual analogue scale (VAS) scores and Lysholm scores. Results: The mean Lysholm score increased from 69 to 85 after surgery. The mean VAS score improved from 7 to 3.1 after surgery. Kellgren-Lawrence grade II group and group with trauma history showed significant improvement of pain and function compared with grade III and group without trauma history. Outerbridge grade I showed significantly more improvement of pain than grade III and IV. Multiple regression analysis showed that trauma history and Outerbridge grade affect the improvement of Lysholm score. Conclusion: In one year follow-up, arthroscopic medial menisectomy can improve pain and function of patients in Kellgren-Lawrence grade II, III degenerative arthritis of the knee. We could expect good results especially in group with low Kellgren-Lawrence grade, trauma history, and mild articular cartilage lesion.

Osteoid Osteoma Around the Hip Joint (고관절 주위에 발생한 유골 골종)

  • Purpose: We experienced 8 cases of osteoid osteoma arising around hip joint which had some characteristics that differ from those arising on long bone. we reports the characteristics of osteoid osteoma arising around the hip joint. Materials and Methods: 8 cases of osteoid osteoma were diagnosed during 1985 to 2004 at hanyang university hospital. all cases were comfirmed patholgically. 6 cases were male, 2 cases were female patients. The mean age was 17 years old (ranged from 8 to 29). They occurred in intertrochanteric area (4 cases), subtrochanteric area (2 cases), acetabulum (1 case) and femoral neck (1 case). We used radiologic tools including magnetic resonance image, computed tomography, bone scintigraphy. clinicopathologic test including erythrocyte sedimentation rate and Creactive protein. Results: The patients expressed various symptoms including thigh pain, knee pain, low back pain and radiating pain respectively. 2 patients had experienced operation on knee joint. 3 patients showed limping gait. Aspirin relieved the pain in 3 patients. The difference in circumference was 1cm between both thighs in 2 cases. Conclusion: Patients with osteoid osteoma arising around hip joint which have various symptoms such as severe knee pain and claudication, differ from infectious disease by clinicopathologic test including erythrocyte sedimentation rate and C- reactive protein and had better diagnostic result in computed tomography.

Mutual Inhibitory Effects of Electroacupuncture Applied to Combination of ST36 and PC7 in the Rat Model of Arthritis (흰쥐 관절염 모델에서 족삼리와 대릉 배오 전침의 상호억제 효과)

  • Objectives : We examined mutual inhibitory effects of combined acupoints in arthritic pain induce by carrageenan(CA). Electroacupuncture(EA) is considered a potentially useful treatment for arthritis. Although the analgesic effect of acupuncture is well documented, little is understood about its biological basis. There are many previous studies of positive effect of combined acupoint, this study was conducted to see the mutual inhibitory effects produced by combined acupoint(ST36 and PC7) on arthritic rats. Methods : For the induction of inflammatory pain rat model, CA was injected into the knee joint cavity. There are four groups; EA was applied to bilateral PC7 acupoints(PC7 group), ST36 acupoints(ST36 group), and both PC7 and ST36 acupoints(ST36+PC7 group) except for the control group. The pain level were assessed to determine the change in weight bearing force. We also examined the COX-2 expression in dorsal horn using immunohistochemistry and western blot analysis. Results : The ST36+PC7 group data showed the significant reduction of weight bearing force and the induction of COX-2 protein expression compared with the ST36 group. Conclusions : Simultaneous EA applied to the ST36 and PC7 acupoints reduced the analgesic effect of the ST36 group on knee inflammatory pain.

The Relationship between Experience of Workplace Violence and Musculoskeletal Pain among Wage Workers in South Korea (한국 임금 근로자의 직장 내 폭력 경험과 근골격계 증상과의 연관성에 관한 연구)

  • Objectives: We aimed to examine the association between experience of workplace violence and musculoskeletal pain among wage workers in South Korea. Methods: We analyzed a cross-sectional survey of 29,601 wage workers from the third wave Korean Working Conditions Survey in 2011. Experience of workplace violence was assessed through three questions, "Over the past 12 months, have you ever experienced: (1) physical violence, (2) bullying, or (3) sexual harassment at workplace?" Musculoskeletal pain was measured using the three questions, "Over the past 12 months, have you ever experienced: (1) low back pain, or (2) upper limb pain(i.e. shoulder, neck, and arm), or (3) lower limb pain(i.e. hip, leg, knee, and foot)?" Wage workers could answer 'Yes' or 'No' to each of the three questions. Multivariate negative binomial regression was applied to examine the association between workplace violence and musculoskeletal pain after adjusting for confounders including self-reported physical work factors. All analyses were performed using STATA/SE version 13.0. Results: Physical violence was associated with low back pain(PR: 2.16, 95% CI: 1.77, 2.65), upper(PR: 1.65, 95% CI: 1.45, 1.88) and lower limb pain(PR: 1.81, 95% CI: 1.52, 2.15) among male wage workers whereas it was related to upper(PR: 1.86, 95% CI: 1.53, 2.26) and lower limb pain(PR: 2.95, 95% CI: 2.47, 3.53) among female wage workers. Significant association was observed between sexual harassment and upper(PR: 1.25, 95% CI: 1.01, 1.56) and lower limb pain(PR: 2.41, 95% CI: 1.97, 2.93) among female wage workers whereas the association was only significant in the analysis with lower limb pain(PR: 1.87, 95% CI: 1.17, 2.97) among male wage workers. Bullying was associated only with lower limb pains among both male(PR: 1.77, 95% CI: 1.32, 2.37) and female(PR: 2.10, 95% CI: 1.69, 2.61) wage workers. Conclusions: This study found that experience of workplace violence, particularly physical violence and sexual harassment, was associated with musculoskeletal pain among Korean wage workers.

Experience with the Application of Magnetic Resonance Diagnostic $Analyser^{(R)}$ -A case of reflex sympathetic dystrophy- (자기공명분석기에 의한 반사성 교감신경성 위축증의 치험)

  • Reflex sympathetic dystrophy is a syndrome characterized by persistent, burning pain, hyperpathia, allodynia & hyperaesthesia in an extremity, with concurrent evidence of autonomic nervous system dysfunction. It generally develops after nerve injury, trauma, surgery, et al. The most successful therapies are directed towards blocking the sympathetic intervention to the affected extremity by regional sympathetic ganglion block or Bier block with sympathetic blocker; other traditional treatments include transcutaneous electrical stimulation, immobilization with cast & splint, physical therapy, psychotherapy, administration of sympathetic blocker, calcitonin, corticosteroid and analgesic agents. The purpose of this report is to evaluate and describe the effects of magnetic resonance following unsatisfactory results with traditional treatments of RSD. A 17 year old female patient, 1 year earlier, had received excision and drainage of pus at the right femoral triangle due to an injury caused by a stone. Afterwards, she experienced burning pain, knee joint stiffness, and muscle dystrophy of the right thigh, especially when standing and walking. Despite a year of number of traditional treatments such as: lumbar sympathetic block, continuous epidural analgesia, transcutaneous electrical stimulation, & administration of predisolone, her pain did not improve. Surprisingly, the patients was able to walk free from pain and difficulty after just one application of magnetic resonance. The patient has been successfully treated with further treatment of two to three times a week for approximately ten weeks. More recently, magnetic resonance has been demonstrated to produce effective results for the relief of pain in a variety of diseases. From our experiences we recognize magnetic resonance as a therapeutic modality which can provide excellent results for the treatment of RSD. It has been suggested that polysynaptic reflex which are disturbed in RSD may be modulated normally on the spinal cord level through the application of magnetic resonance.

Inhibition of Articular Sensory Activities to Mechanical Stimulation by Aqua-acupuncture in an Animal Model of Arthritic Pain (관절통에 관한 동물모델에서 약침에 의한 기계적 자극에 대한 관절 감각신경 활동의 억제)

  • The aim of this study was to examine the effects of aqua-acupuncture a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferus on an animal model of arthritic pain. Under halothane anesthesia, arthritic pain was induced by the injection of $2\%$ carrageenan into the left knee joint cavity of male Sprague-Dawley rats. The responses of afferents to a movement cycle were recorded before and after aqua-acupuncture. The aqua-acupuncture at acupoints reduced neural responses to noxious movement stimulation. Aqua-acupuncture at Zusanli inhibited neural responses of articular afferents to noxious stimulation more than at Hegu. These results indicate that aqua-acupuncture of a mixture of bos taurus domesticus and selenarctos thiberanus, and bos taurus domesticus, selenarctos thiberanus and Moschus moschiferusmay provide a potent strategy in relieving arthritic pain.

Effects of the Application of Elastic Compression Stockings on Edema and Pain of Lower Extremity in Hospital Nurses (근무 중 탄력압박스타킹 착용이 간호사의 하지부종 및 통증에 미치는 효과)

  • Purpose: This study was to identify the effects of the application of elastic compression stockings during the hours of work on edema and pain of lower extremity in hospital nurses. Method: The participants were categorized into three groups, 15 for an experimental group of the application of Thigh-Length (T-L) elastic compression stocking, 15 for another experimental group of the application of Knee-Length(K-L) elastic compression stocking and 15 for a control group. The data were analyzed by $X^{2}-test$, ANOVA, Repeated Measures ANOVA and Sheffe-test using SPSS Win 12.0 program. Result: As a result of the study "There are significant differences in lower extremity edema among three groups." was partly supported. "There are significant differences in girth of ankle after work among three groups." was supported(Rt.:F=4.56, p=.016; Lt.: F=5.82, p=.006). "There are significant differences in girth of calf after work among three groups." was not supported(Rt. : F=1.76, p=.183; Lt. : F=1.94, p=.155) "There are significant differences in lower extremity pain after work among three groups." was supported(Rt. : F=95.33, p=.000; Lt. : F=81.33, p=.000). Conclusion: The results of this study revealed that the application of elastic compression stockings during the hours of work is effective for the relief of edema and pain of lower extremity in hospital nurses.

Effects of the Self-Help Program on Pain, Fatigue, Difficulty in Physical Activity, Joint Stiffness, Flexibility of the Joints in Arthritis Patients (관절염 자조관리과정이 통증, 피로, 일상활동 어려움, 관절뻣뻣함, 유연성에 미치는 효과)

  • Purpose: The purpose of this study was to determine if osteoarthritis patients would benefit in terms of pain, fatigue, difficulty with physical activity, joint stiffness, and flexibility of the joints from a structured self-help program. Method: This self-help program was carried out 2-3 hours once a week for 6 weeks in 2005-2006, and evaluated in one group pretest-posttest pre-experimental design. The subjects of this study who were diagnosed osteoarthritis were recruited at two different Community Health Centers in Kangwon. The subjects who agreed with the purpose of this study and participated both pretest and post-test were 55 patients. Mean age is 63.48 (9.48) years, mean duration of disease is 7.95 (7.66) years. The self-help program was consisted of weekly health contract, exercise, health education, group discussion, group counseling, and recreation. At every meeting, researcher and trained public health nurse evaluated the program, and prepared the next program. The measurement tools were pain rating scale (0-10), fatigue rating scale (0-10), Korean WOMAC (Western Ontario and McMaster University Osteoarthritis) Index, ruler, and goniometer. Results: At the completion of 6 weeks of self-help program, the subjects reported significantly less pain and difficulty with physical activity and more flexibility in both shoulder and knee joints compared to pretest. Conclusion: The self-help program would be helpful on pain, physical activity, and joint flexibility for arthritis patients.
