Electrochemical dehalogenation of disinfection by-products and iodine-containing contrast media: A review

  • This paper summarizes results of research on the electrochemical (EC) degradation of disinfection by-products (DBPs) and iodine-containing contrast media (ICMs), with the focus on EC reductive dehalogenation. The efficiency of EC dehalogenation of DBPs increases with the number of halogen atoms in an individual DBP species. EC reductive cleavage of bromine from parent DBPs is faster than that of chlorine. EC data and quantum chemical modeling indicate that the EC reduction of iodine-containing DBPs (I-DBPs) is characterized by the formation of active iodine that reacts with the organic substrate. The occurrence of ICMs has attracted attention due to their association with the generation of I-DBPs. Indirect EC oxidation of ICMs using anodes that produce reactive oxygen species can result in a complete degradation of these compounds yet I-DBPs are formed in the process. Reductive EC deiodination of ICMs is rapid and its overall rate is diffusion-controlled yet I-DBPs are also produced in this reaction. Further progress in practically feasible EC methods to remove DBPs, ICMs and other trace-level organic contaminants requires the development of novel electrocatalytic materials, elimination of mass transfer limitations via innovative design of 3D electrodes and EC reactors, and further progress in the understanding of intrinsic mechanisms of EC reactions of DBPs and TrOC at EC interfaces.

Bactericidal Effect of Waterless Alcohol Gel Hand Washing Agent (물 없이 사용하는 알코올 젤의 손소독 효과에 관한 연구)

  • The purpose of this study was to compare the hand disinfection effect of waterless alcohol gel hand washing agent with that of soap and water, 4% chlorhexidine gluconate, and 10% povidone-iodine. Hands of fourty subjects were artificially contaminated with Acinetobacter baumannii $5m{\ell}$ and randomly distributed to each hand washing methods. Samples were collected from gloved hand by glove juice sampling procedure. Mean log reduction after hand washing were compared with baseline values. Number of microorganisms were converted to log and tested by ANOVA in SPSSWIN 10.0. Mean log reduction of soap and water, alcohol gel, 4% chlorhexidine gluconate, 10% povidone-iodine were $2.76{\pm}0.62$, $2.97{\pm}0.56$, $4.66{\pm}1.70$, $4.60{\pm}0.91$, respectively. The bactericidal effect of alcohol gel was similar to that of soap and water, but the effect was much less than chlorhexidine gluconate and povidone-iodine(p<0.001). In terms of microorganism reduction, the efficacy of waterless alcohol gel was almost the same as soap and water hand washing. Further evaluation of the bactericidal effect of waterless alcohol gel is needed because waterless alcohol gel is simple, convenient, and non-irritating hand washing agent and also very effective in busy hospital environment.

Disinfection State and Effective Factors of Foodservice Facilities and Utilities of Elementary Schools in Busan -Based on the Characteristics of Dietitian, Employee and Foodservice- (부산지역 초등학교 급식시설,설비의 소독실태 및 영향요인 -영양사, 조리종사원 및 급식소 특성에 따른-)

  • The purpose of this study was to investigate disinfection state and effective factors of foodservice facilities and utilities of elementary schools in Busan area. Foodservice facilities and utilities investigated in this study were ceiling, wall, floor, trench, greasetrap, hood, pest and dumb waiter. The questionnaire which was administered to 196 dietitians was used as a survey method. The results were as follows. The washing method of foodservice facilities and utilities was mainly used by detergent washing-natural dry. The disinfection method of foodservice facilities and utilities was mainly sanitized by sodium hypochlorite. Dumb waiter and hood were sanitized by 70% ethyl alcohol and iodine. In washing and disinfection frequencies of foodservice facilities and utilities ceiling was twice/year, wall, hood and pest control facilities were once/week, floor, trench, greasetrap and dumb waiter were everyday, respectively. Floor, trench, greasetrap and dumb waiter were disinfected properly, whereas ceiling, wall, hood and pest were not disinfected properly. The washing method was related to dietitian's marital status and education. The disinfection state was related to dietitian's age and career, having of sanitation check list. The disinfection method was also affected dietitian's marital status, number of employee, duration of foodservice, number of total serving and having of sanitation check list. Therefore based on the results of this study, it should be given to the microbiological study on disinfection of facilities and utilities such as ceiling, wall, floor, trench, greasetrap, hood, pest and dumb waiter and the dietitian and employee's sanitation training also should be conducted continuously.

Affecting Factors on Hospital Nurses' Practice of Disinfection: Focused on Alcohol, Chlorhexidine Gulconate, and Povidone Iodine (병원 간호사의 주요 피부소독제 인지도와 수행도에 영향을 미치는 요인 - 알코올, 클로르헥시딘, 베타딘을 중심으로 -)

  • Purpose: This study was performed to identify the affecting factors on hospital nurses' practice of disinfection focused on alcohol, chlorhexidine gluconate (CHG), and povidone iodine (PVI). Methods: The participants in this study were 196 nurses of 3 general hospitals and 2 upgrade general hospitals in 5 cities in Korea. Data were collected through self-reported questionnaires in January, 2011. The collected data were analysed by ANOVA, Pearson correlation and stepwise multiple regression using SPSS/WIN 18.0. Results: The subjects used alcohol and PVI daily for intravenous site care and surgical wound dressing respectively. The mean percentage of awareness in practicing main disinfectants were 80.0% and 72.5% respectively. The awareness and practice were highest in alcohol, but lowest in CHG. The mean percentage of practice of disinfectants was 72.5%. There was positive correlation among awareness and practice of main disinfectants. The awareness and nurses' salary explained 34.6% of variance in practice of disinfectants. Conclusion: An educational program focusing on strategy to enhance hospital nurses' awareness would be effective in improving their practice of disinfectants.

Clinical Study Comparing the Skin Antiseptics of Povidone-iodine and Chlorhexidine-ethanol on Port Site (투관침 부위에 적용하는 포비돈 아이오딘과 클로르헥시딘-에탄올의 소독제 효과 연구)

  • The purpose of this study was to compare in antiseptic effect between povidone-iodine and chlorhexidine-ethanol for laparoscopic cholecystectomy on the port site. A total of 46 patients admitted to surgery at one university hospital in Seoul were included in the study. A study group was randomly assigned using a random number table. The antiseptic effect was evaluated and compared to povidone-iodine and chlorhexidine-ethanol by using a culture and identification of bacteria, measurement of the number of bacteria (CFU/ml) and incidence of surgical site infection. Our results show that povidone-iodine and chlorhexidine-ethanol have no statistically significant difference in the incidence rate of bacteria after 3 minutes of disinfection and before removal of the trocar and in the number of bacteria and the incidence of surgical site infection. Thus, in order to select an efficient and appropriate skin antiseptics, we suggest that it is necessary to consider not only the disinfectant effect but also cost and convenience. In this research, our results about effect and usage of skin antiseptics can be used as basic data and educational resource, and this is expected to improve comprehension of skin antiseptics.

Production of Healthy Shrimp(Penaeus chinensis, Penaeus japonicus) Seedling Free from White Spot Baculovirus(WSBV) (White Spot Baculovirus(WSBV) 미감염 새우(Penaeus chinensis, Penaeus japonicus) 종묘 생산)

  • For the production of healthy shrimp(Penaeus chinensis, Penaeus japonicus) seedlings free from the white spot baculovirus(WSBV), we tried to disinfect shrimp eggs with iodine. A relative hatching rate of over 50% has resulted from the treatments of fertilized eggs of fresh shrimps(P. chinensis) with an effective povidon-iodine concentration of 20-200 ppm for 30 seconds and 60 seconds, respectively. The 60 sec treatment group with a 20 ppm effective povidon-iodine concentration showed nearly a 50% relative hatching rate. The over 50 ppm treatment group, however, did not result in hatching show a relative hatching rate. These hatching rates were similar to those in the fertilized eggs of kuruma prawns treated with povidon-iodine solutions. In case of washing fertilized eggs with UV-irradiated seawater 33.3% relative hatching rate in fresh shrimps was much lower than 55.2% in kuruma shrimps. Treatment fertilized eggs of 2 species of shrimp with 200 ppm iodine for 30 sec did not affect the survival rate of the larvae until the early stage of the post larvae even though the hatching rates were low. In order to produce healthy shrimp seedlings non-infected by WSBV, the shrimp eggs should be washed with UV-irradated seawater and sterilized with a low povidon-iodine concentration for a short time.

Bactericidal Efficacy of a Disinfectant Composed of Povidone-iodine Against Clostridium Perfringens and Mycobacterium Fortuitum

  • Clostridium perfringens (C. perfringens) and Mycobacterium fortuitum (M. fortuitum) are associated with considerable diseases in animals and human. In this study, the disinfection efficacy of a commercial disinfectant composed to povidone-iodine (PVI) was evaluated against C. perfringens and M. fortuitum. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to C. perfringens and M. fortuitum for 30 min at $4^{\circ}C$. The disinfectant and test bacteria were diluted with hard water (HW) or organic matter suspension (OM) according to treatment condition. On HW condition, the bactericidal activity of the disinfectant against C. perfringens and M. fortuitum was 50 and 80 fold dilutions, respectively. On OM condition, the bactericidal activity of the disinfectant against both C. perfringens and M. fortuitum was 15 fold dilutions. As the disinfectant composed to PVI possesses bactericidal efficacy against C. perfringens and M. fortuitum, the disinfectant solution can be used to control the spread of bacterial diseases.

Effects of Cotton Balls of Alcohol and Povidone for Disinfectoin of Skin in Newborns (신생아에서 알코올 솜 및 포비돈 솜의 피부 소독효과 비교)

  • Purpose : Despite of development of neonatal intensive care, infections, especially catheter-related infection, have been a major problem in newborns on intensive care. Catheter-related infection is closely related to skin colonization so that skin disinfection is crucial to protect newborns from the infection. Cotton balls of seventy percent alcohol made by hospital frequently open to air so that composition of alcohol may be easily evaporated. We hypothesized that 10% povidone would have better disinfection compared to 70% alcohol and normal saline. Methods : The effect of skin disinfection was evaluated in newborn infants who were admitted to the neonatal intensive care unit(NICU) of Hanyang University Kuri Hospital by a skin swab culture before and after disinfectant exposure. Before skin disinfective preparation, the peripheral intravenous catheter insertion sites were swabbed for culture with sterile cotton moistened with normal saline. Cotton balls of 70% alcohol(Alcohol group, n=20) or 10% povidone and 70% alcohol(povidone group, n=20) were randomly selected. Normal saline swabs were used for controls(n=10). After swabbing, the skin swab culture was done on same sites with same procedure. Results : Before skin disinfection, culture-positivity was shown in 9 of 20 leases of alcohol and povidone groups, respectively and in 8 of 10 cases of the controls. After skin disinfection, culture-positivity was still shown in 3 of 9 cases of the Alcohol group, in 2 of 9 of the Povidone group, and in 6 of 8 cases of the controls. Disinfective effect was significantly different among the 3 groups(p=0.0387). The povidone group had significantly better disinfection than the controls(p=0.0156), but the alcohol group was not significantly different from the controls. Conclusion : The cotton balls of povidone had similar effect of disinfection compared to those of alcohol, but better disinfection than the controls. A disinfective effect of the cotton balls of alcohol was not, however, significantly different from the controls. We suggest that purity of alcohol, quality control of cotton balls of alcohol, and disinfection method should be reevaluated.

  • Salmonella spp. and Brucella spp. are associated with considerable diseases of both humans and animals. In addition, these microorganisms cause the economic loss in animal farming and food industry. In this study, the disinfection efficacy of a commercial disinfectant, composed to povidone-iodine was evaluated against S. typhimurium and B. ovis. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to test bacteria for 30 min at $4^{\circ}C$. The disinfectant and test bacteria were diluted with hard water (HW) or organic matter suspension (OM) according to treatment condition. On HW condition, the bactericidal activity of the disinfectant against S. typhimurium and B. ovis was 400 and 150 fold dilutions, respectively. On OM condition, the bactericidal activity of the disinfectant was 5 and 20 fold dilutions against S. typhimurium and B. ovis, respectively. As the disinfectant composed to povidine-iodine possesses bactericidal efficacy against animal pathogenic bacteria such as S. typhimurium and B. ovis, the disinfectant solution can be used to control the spread of bacterial diseases.

A STUDY ON DIMENSIONAL STABILITY OF IMPRESSION MATERIALS FOLLOWING IMMERSION DISINFECTION (수종 인상재의 침지 소독이 경석고 모형의 크기 안정성에 미치는 영향에 관한 연구)

  • Dental practice can produce and spread some infectious diseases from patients to dentist, dental assistant, and dental labors. One possible method for preventing these cross-contamination is to immerse dental impression in chemical disinfectants. So for many investigators studied on the dimensional changes of dental impressions and on the surface qualities of stone casts made from impression following immersion in disinfectants. This study was proposed to evaluate some popular impression disinfectant combination from the point of dimensional stability. Impression was taken from dental arch-shaped metal model. Irreversible hydrocolloid and 3 elastomers(polyvinyl siloxane, polysulfide, polyether) were immersed in 3 disinfectants (2% glutaraldehyde, 1% povidone-iodine, 0.5% sodium hypochlorite) for 10 minutes and measured both cross-arch and anterior-posterior distance under stereo microscope to evaluate dimensional change. The results obtained were as follows: 1. Dimensional changes of irreversible hydrocolloid impression was statistically different in cross-arch and anterior-posterior distance when immersed in 2% glutaraldehyde solution and in anterior-posterior distance when immersed in 0.5% sodium hypochlorite solution from control group (p<0.05). 2. Dimensional changes of polyvinyl siloxane and polysulfide impression were not statistically different from control group (p>0.05). 3. Dimensional changes of polyether impression was statistically different in cross-arch distance when immersed in 0.5% sodium hypochlorite solution and in anterior-posterior distance when immersed in 1% povidone-iodine solution from control group (p<0.05). 4. In all cases, dimensional changes were less than 0.1% from the original dimension and concluded clinically acceptable.
