Dilemmas of Oral Cancer Screening: An Update

  • Oral cancer is a global health burden with high mortality and morbidity. Advances in treatment have failed to improve the relatively poor survival rate due to late-stage diagnosis. Early detection and screening have been shown to be effective in reducing mortality and morbidity of most common cancers. Several studies have evaluated the effectiveness of oral cancer screening programs but clear results were not obtained. This narrative commentary aimed to give a critical insight into the dilemma of oral cancer screening and to suggest recommendations for future trends. Conventional oral examination still constitutes the gold standard screening tool for potentially malignant oral lesions and cancer. Interestingly, the findings of the most lasting (15-year) randomized controlled trial on oral cancer screening using visual examination (Kerala) supported the introduction of a screening program in high-risk individuals. Several screening adjuncts exist but are still not at the introduction stage. Further research to find an appropriate adjunct reliable tool for oral cancer screening is needed. In conclusion, oral cancer fulfills most of the essential principles of cancer screening but still many points need to be clarified. Therefore, there is a striking need to establish a global consortium on oral cancer screening that will oversee research and provide recommendations for health authorities at regular intervals.

시상 및 시상하부 뇌경색 후에 수면과다와 기억력 장애가 발생한 환자의 증례보고 (A Case Report of Thalamic and Hypothalamic Infarction with Hypersomnolence and Memory Impairment)

  • Introduction: We report a rare case in which the symptoms of hypersomnia were caused by cerebral infarction that occurred in the hypothalamus and hypothalamus. Case Presentation: The patient underwent Korean medicine treatments using Gami-seonghyangjeonggi-san, acupuncture, electroacupuncture, and physical therapy for 37 days. We evaluated the degree of improvement in hypersomnolence based on the amount of sleeping time and the degree of memory impairment with the patient's suggestive conditions as observed by the patient's guardian, and one-sided facial palsy using the Yanagihara Scale. As treatment was applied, sleeping time was decreased from 22 to 9 hours a day. Memory impairment did not show clear improvements as the patient could not recognize others when they visited during the period in which the patient underwent treatment. One-sided facial palsy showed no improvements and there were no changes in the Yanagihara Scale results. Conclusion: This case showed that Korean medicine treatment could be effective to relieve some of the symptoms of thalamus and hypothalamus infarction. Further research is needed to confirm the effectiveness of Korean medicine treatment.

An introduction to the recently excavated Chunggang Medical Records and research on their medical value

  • This study is a report on recently discovered medical records based on traditional medicine in the 1900s. First, the contents of the records and their significance are described in detail. Next, a simple example of the research follows, in order to explain the medical and historical significance the records contain and to answer the question of how this historical document can contribute to future medical and historical studies. The documents dealt with in this study, the Chunggang Medical Records, are medical records compiled by a Korean doctor of oriental medicine by the name of Younghun Kim who practiced in the center of Seoul for a period of over 60 years. The records, which eventually amounted to over 1,500 books, were made known to the academic world when the descendents recently donated them to Kyunghee University. The reason these medical records attract so much attention from academic circles, even though they are the work of one individual, is that they contain abundant information on general public medical health at the time, in addition to the fact that Kim Younghun was a well known figure among Oriental Medicine doctors in Korea. The medical records start in 1915 and continue until Kim Younhun's death in 1974, though they have some damaged or missing parts. Kim's medical records are a gold mine not only for scholars studying the medical history of the early 1900s, but also for doctors trying to emulate the techniques embedded in a great predecessor's medical practice.

사물안신탕가미방과 침 치료로 임상 증상에 호전을 보인 완전방실차단 환자 치험 1례 (A Case Report of Complete Atrioventricular Block Treatment with Samulanshin-tang-gamibang and Acupuncture)

  • Introduction: This study reports the effect of herbal medicine (Samulanshin-tang-gamibang) and acupuncture on complete atrioventricular (AV) block. Case presentation: A 63-year-old female with complete AV block was experiencing dyspnea, palpitation, dizziness, headache, bradycardia, and insomnia, and she was treated with Samulanshin-tang-gamibang and acupuncture for 12 days. To evaluate the treatment, a numeric rating scale (NRS) and the New York Heart Association (NYHA) functional classification was used. The patient's NRS scores decreased from 6 to 2 for dyspnea and palpitation and from 5 to 1 for dizziness and headache. Her NYHA Class improved from Class II to Class I. No side effects were observed during treatment. Conclusion: This study suggests that herbal medicine and acupuncture may be effective in relieving symptoms caused by complete AV block. However, the long-term effects of the treatment were not observed, and so further studies are still needed.

Synthesis of a PEGylated tracer for radioiodination and evaluation of potential in tumor targeting

  • Radiopharmaceuticals are important for tumor diagnosis and therapy. To deliver a radiotracer at the desired target excluding non-targeted tissues is difficult The development of a targeted tracer that has a good clearance profile while maintaining high biostability and biocompatibility is key to optimizing its biodistribution and transport across biological barriers. Improving the hydrophilicity of radiotracers by PEGylation can reduce serum binding, allowing the tracer to circulate without retention and reducing its affinity for non-targeted tissues. In this study, we synthesized a new benzamido tracer (SnBz-PEG36) with the introduction of a low molecular weight polyethylene glycol unit (PEG36, ~2,100 Da). The tumor targeting efficiency and biodistribution of [131I]-Bz-PEG36 or radiotracer-loaded liposomes were evaluated after their administration to normal mice or mouse tumor models including CT26 (xenograft) and 4T1 (xenograft and orthotopic). Most of the radiotracer was cleared out rapidly (1-24 h post-administration) through the kidney and there was little tumor uptake.

Lack of Prognostic Significance of SOCS-1 Expression in Colorectal Adenocarcinomas

  • Introduction: Recent studies have indicated that down-regulation of the suppressor of cytokine signaling-1 (SOCS-1) gene results in tumor formation and that SOCS-1 acts as a tumor suppressor gene. SOCS-1 has been also suggested to function as a tumor suppressor with colorectal cancer. Objectives: In the present study, we aimed to determine the association of SOCS-1 expression in colorectal cancer tissues with clinicopathologic characteristics immunohistochemically and also to identify its prognostic significance. Materials and Methods: SOCS-1 expression was studied immunohistochemically in 67 patients diagnosed with resected colorectal carcinomas and 30 control subjects. Results: SOCS-1 expression was found in 46.3% of tumor tissues and 46.7% of the control group. Statistical analyses did not establish any significant association between SOCS-1 expression and clinicopathologic characteristics. Also, no significant association with SOCS-1 expression was found using progression-free survival and overall survival analyses (p=0.326 and p=0.360, respectively). Conclusions: Our results show that SOCS-1 has no prognostic significance in colorectal cancer.

Hypoxia Upregulates Mitotic Cyclins Which Contribute to the Multipotency of Human Mesenchymal Stem Cells by Expanding Proliferation Lifespan

  • Hypoxic culture is widely recognized as a method to efficiently expand human mesenchymal stem cells (MSCs) without loss of stem cell properties. However, the molecular basis of how hypoxia priming benefits MSC expansion remains unclear. In this report, our systemic quantitative proteomic and RT-PCR analyses revealed the involvement of hypoxic conditioning activated genes in the signaling process of the mitotic cell cycle. Introduction of screened two mitotic cyclins, CCNA2 and CCNB1, significantly extended the proliferation lifespan of MSCs in normoxic condition. Our results provide important molecular evidence that multipotency of human MSCs by hypoxic conditioning is determined by the mitotic cell cycle duration. Thus, the activation of mitotic cyclins could be a potential strategy to the application of stem cell therapy.

  • Recently, sedation in dentistry is gaining more and more interests of dental practitioners. Moreover. 'dental sedation', 'sleep dentistry', or 'pain-free dental treatment' become everyday words familiar to ordinary persons. Sedation clinic in Hanyang University Medical Center was open on Mar 2006 as specialty clinic at department of dentistry. Our aim is performing comfortable and safe dental treatments for patients who have problems for routine dental treatments because of extreme fear and anxiety or for patients who are supposed to have stressful and painful time-consuming treatment, such as sinus elevation and bone graft for implant placement. Our sedation was focused on conscious sedation, especially intravenous sedation using midazolam alone in technique. In two series, the sedation protocol at our Sedation Clinic will be described in details and then, analysis of our cases will be presented to help beginners for sedation in dentistry.

Brief Introduction of Traditional Chinese Medicine(TCM) and Possible Measures to Protect the Endangered Species of TCM

  • Recently the endangered species of wild fauna and flora used in TCM have been held under a spotlight by CITES-implementing countries. USA and other western countries have suspected Oriental countries might not keep the regulation of CITES effectively for the continuous use of endangered species such as tiger bone, rhino horn, bear gallbladder and others. However, most of Oriental countries have tried to keep CITES truly. The misunderstanding of culture and thought in Oriental countries can be one of important factors. Thus, for effective implementation of CITES, we have to keep communication with western countries especially in the principles of TCM or others and vice versa. In addition, TCM using countries and counterpart countries should prevent illegal trade of endangered species in TCM and also fry to develop natural or synthesized substitutes for endangered species. We have to continue to develop methods to artificially propagate endangered species, while trying to alleviate the ethical problems entailed by breeding in captivity. In south Korea we also have to make known our implementation of CITES such as reinforced law for prohibition of using CITES items, public awareness by training program, brochure and surveys to other countries.

혈액응고 기전에 대한 고찰 (Introduction to Coagulation System)

  • Coagulation involves the regulated sequence of proteolytic activation of a series of proteins to achieve appropriate and timely hemostasis in an injured vessel. In the non-pathological state, the inciting event involves exposure of circulating factor VIIa to extravascularly expressed tissue factor, which brings into motion the series of steps which results in cell based model of coagulation. In the new concepts of coagulation system, initiation, amplification and propagation steps are involved to converse of fibrinogen to fibrin. The precisely synchronized cascade of events is counter-balanced by a system of anticoagulant mechanisms. Developmental hemostasis refers to the age-related changes in the coagulation system that are most marked during neonate and childhood. An understanding of these changes in crucial to the accurate diagnosis of hemostatic abnormalities in neonate and children. This review aims to elucidate the main events within the coagulation cascade as it is currently understood to operate in vivo, and also a short review of the anticoagulants as they relate to this model. Also this paper describes the common pitfalls observed in the clinical data related to the coagulation system in neonate to children.
