한의학 정통요법과 보조요법의 임상적 운용방안 (Study on Clinical Operating Measures of Authentic Therapy and Supplementary Therapy in Oriental Medicine)

  • This research surveyed oriental medicine therapies currently used in oriental medicine circles by referring to oriental medicine books-based existing traditional authentic therapies, as well as market-sold supplementary therapy-related ordinary health books and Internet information, and supplementary therapy-related dissertations, reviewed the fundamental operating significance of these therapies, and pursued their clinical operating measures based on their clinical information as follows Various therapies currently used in oriental medicine circles are categorized into authentic therapies and supplementary therapies in oriental medicine. Authentic therapies in Oriental medicine are based on bodily self-vitality abilities, pursue positive methods of treating diseases, and include medication therapies, acupuncture therapies, and external therapies. Supplementary therapies in oriental medicine are based on bodily self-vitality abilities, pursue indirect methods of treating diseases, and consist of lifestyle therapy and body management therapy. Authentic and supplementary therapies in Oriental medicine are used either separately or together according to clinical treatment methods. In treating diseases, clinical operating methods exclusively based on traditional authentic therapies and supplementary therapies include Donguibogam (anthology of ancient oriental medicine), Uihakipmun (introduction to medicine), Somun school, and bodily acupuncture according to diagnoses of symptoms, as well as sasangbang (prescription based on four types of bodily constitution), myeongribang prescription, hyeongsangbang prescription, and bodily constitution-based acupuncture according to diagnoses of bodily constitutions. In clinical operation of Oriental medicine, supplementary therapies are parity duplicated dy alternative medicine. However, given patients' needs and clinical practicality, Oriental medicine circles should establish a system based on the fundamental viewpoint of Oriental medicine theories, and corresponding policies should also be researched.

  • Objectives : The study aimed to investigate the satisfaction, reasons for the progress, and the advantages of the school doctor of Korean Medicine (SDKM) program in the applied Korean Medicine doctors. Methods : The school doctor of Korean Medicine (SDKM) program has been conducted in 21 schools in Seoul since March 2018. The program was varied by school, included the introduction of Korean Medicine, sex education, and anti-smoking/drinking session. This survey was conducted as a paper-based questionnaire after the SDKM. Results : The SDKM results evaluation was better in the Korean doctors who applied for SDKM. The factors for negative progression of SDKM were 'Bad cooperation of the schools', 'Lack of personal capabilities', and 'Lack of motivation'. The factors for the positive progression of SDKM were 'Good cooperation of the schools', 'Support of Seoul Korean Medicine Association'. The question of prerequisites for activation SDKM Program included 'Active cooperation of school', 'Expand teaching materials and programs', 'Support of Seoul Korean Medicine Association'. Conclusions : In order to activate the SDKM, it is necessary to cooperate with the schools, to support of Seoul Korean Medicine Association, and to expand the education program.

Changes in the Prosthesis Types Used for Aortic Valve Replacement after the Introduction of Sutureless and Rapid Deployment Valves in Korea: A Nationwide Population-Based Cohort Study

  • Background: Sutureless and rapid deployment valves for aortic valve replacement (AVR) were introduced in Korea in December 2016. This study evaluated changing trends in the prosthetic valves used for AVR in Korea after the introduction of sutureless and rapid deployment valves. Methods: From December 2016 to December 2018, 4,899 patients underwent AVR in Korea. After applying the exclusion criteria, 4,872 patients were analyzed to determine changes in the type of prosthetic valve used for AVR. The study period was divided into 5 groups corresponding to 5-month intervals. Results: The total number of AVR cases was 194.88±28.78 per month during the study period. Mechanical valves were used in approximately 27% to 33% of cases, and the proportion of mechanical valve use showed a tendency to decrease, with marginal significance overall (p=0.078) and significant decreases in patients less than 60 years of age and in men (p=0.013 and p=0.023, respectively). The use of sutureless valves increased from 13.4% to 25.8% of cases (p<0.001), especially in elderly patients (>70 years) and those requiring concomitant surgery. In a comparison between sutureless and rapid deployment valves, the use of Perceval S valves (a type of sutureless valve), gradually increased (p<0.001). Conclusion: After the introduction of sutureless and rapid deployment valves in Korea, the rate of use of these new valves remarkably increased, especially in elderly patients and those requiring concomitant surgery. Further studies should investigate the clinical outcomes of these new prostheses.

수족냉증을 호소하는 소양인 비수한표한병 환자의 증례보고 7례 (A Case Series of Cold Hypersensitivity in Hands and Feet in Soyangin Cold Pattern Patients)

  • Introduction This study reports case series of Soyangin patients with coldness in hands and feet using Sasang constitutional medicine approach. This study suggests a value of using constitutional medicine indicators as both prognostic and diagnostic tools. Methods Among patients who visited an outpatient clinic, 7 patients were selected based on the criteria. These selected patients were exclusively treated with herbal medicine and monitored every 2 to 4 weeks retrospectively. Results The average treatment period was 8 weeks. Patients with a favorable pattern showed the improvement within 8 weeks, while those with an unfavorable pattern required more time. Discussion This study confirms the significance of Sasang constitutional medicine approach in Soyangin exterior cold syndrome with coldness in hands and feet. It also validates the current coldness in hands and feet as meaningful diagnostic indicator of Soyangin exterior cold syndrome. Furthermore, it highlights the value of original symptoms as prognostic tools.

정액수가제 도입이 의료급여 혈액투석환자의 투석횟수 및 진료비에 미치는 영향 (Impacts of Implementing Case Payment System to Medical Aid Hemodialysis Patients on Dialysis Frequencies and Expenditure)

  • Objectives : To assess the impacts of implementing case payment system (CPS) to Medical Aid (MA) hemodialysis patients on the frequencies and expenditure of dialysis. Methods : Fifty-eight clinics and 35 tertiary care hospitals were identified as having a minimum of 10 hemodialysis patients for each of the MA and Medical Insurance (MI) programs, who received hemodialysis from the same dialysis facilities for both periods of July 2001 and July 2002. From these facilities, a total of 2,167 MA and 2,928 MI patients were identified as the study subjects. Using electronic claims data, the changes in the total number of monthly treatments and charges for outpatient hemodialysis treatments for each patient after the introduction of the CPS were compared between the MA and MI patients. Multiple regression analyses were performed to examine the independent impact of the CPS on the utilization and expenditure of dialysis treatments among the MA patients. Results : There was a significant decrease in the total charges for the hemodialysis treatments of the MA patients, 3.4% (p<0.05), whereas a significant increase was observed for the MI patients, 2.5% (p<0.05). For both the MA and MI patients, the frequency of the monthly hemodialysis treatments were significantly increased, 5.5 (from 12.1 to 12.7) and 7.8% (from 11.6 to 12.5), for the MA and MI patients, respectively. However, a multivariate regression analysis showed no significant difference in the changes in the total number of monthly hemodialysis treatments between the MA and MI patients after implementation of the CPS. Another regression model, regressing on the changes in the monthly claims of dialysis treatments, showed a significant negative coefficient for the MA ((=-70725, p<0.05). Conclusion : The significant decrease in the total charges for hemodialysis treatments among MA as compared to MI patients suggests that there was a cost reduction in the MA program following the introduction of the CPS.

  • Laser cleaning has begun to attract a considerable amount of interest recently as a new cleaning technique among scientists and engineers. The unique characteristics of laser cleaning are currently finding successful applications in industry, in medicine as well as in the world of art conservation. This paper takes an overview of the laser cleaning technique itself including basic principles and characteristics, and provides an account of current trends especially with regard to practical applications. Experience with its successful applications in various fields shows that laser cleaning may be about to emerge as a real alternative to conventional cleaning methods.

한의학 분야 연구윤리 교육에 대한 전국 한의과대학 및 한의학전문대학원 교수들의 인식 (Awareness and attitudes of professors regarding research ethics education in the colleges and the school of Korean Medicine)

  • Objectives: This study aimed to investigate the experiences and awareness of Korean Medicine professors towards research ethics issues and curriculum development. Methods: Electronic mails for requesting the survey participation were sent to 531 professors who were listed in the DB of Association of Korean Medicine. Professors who agreed to participate in the survey were connected to the self-administered questionnaire site, and the responses were stored in real time. Professors were asked the experience of research misconduct cases, the necessity, contents and evaluation method of research ethics education, etc. ANOVA was performed to test the differences of ethics education effects among sub-groups of respondents. Results: Of 137 professors who opened the e-mail, 127 completed the questionnaire. Most professors (87.4%) reported to witness the research misconduct cases, and felt the necessity of research ethics education for professors as well as students. 31.4% of respondents preferred the practice-focused course, and 43.3% preferred the PASS/FAIL basis evaluation system. Professors with shorter education career and professors majoring basic Korean Medicine expected higher effect of ethics education than others. For detailed items of research ethics education, 'ethical writing' and 'ensuring research integrity' were prioritized in the aspects of both personal ethics and social demand. Conclusions: In this study, detailed matters of research ethics education in Korean Medicine area were drawn. Further study and policy development are needed to facilitate the introduction of formal education of research ethics.

티베트 의학과 한의학의 요진법에 대한 비교 고찰 (A Comparative Study on the Urinalysis of the Tibetan Medicine and the Traditional Korean Medicine)

  • This study focuses on the contents about the Urinalysis which is the most representative one of the Tibetan medicine. Also, this study compares it with the Urinalysis of Traditional Korean medicine. This study focuses on the simple history, the theoretical system, the diagnosis and the urinalysis mainly based on the documents related to the Tibetan medicine. Also, through the comparison with the contents of textbook of Traditional Korean Medicine and the urinalysis shown in the Dongeuibogam(東醫寶鑑) which is the representative Korean medical book, the difference between Tibetan Medicine and Traditional Korean Medicine will be observed. The contents related about the urinalysis of the Tibetan medicine are more specific than those of Traditional Korean Medicine. By observing the color, scent, vapor and sediment of one's urine together with periodic changes more thoroughly, it is possible to find out the cause of a certain disease. However, Traditional Korean Medicine examines the medical conditions mainly based on the state of discharge and fever, showing a clear difference. The Tibetan medicine has an extremely specific type of urinalysis in comparison with that of Traditional Korean Medicine. It is the only characteristic of the Tibet medicine, which cannot be found in any other traditional medicine. By applying the viewpoint of the doctor who diagnoses the patient by measuring his or her pulse, it is possible to make the diagnosing process more specific and accurate. It is expected that the follow-up study will be continuously executed with the introduction of the system for the urinalysis of the Tibetan medicine to Traditional Korean medicine.

Non-Operatively Treated Thoracolumbar Burst Fracture with Posterior Ligamentous Complex Injury: Case Report and Consideration on the Limitation of Thoracolumbar Injury Classification and Severity (TLICS) Score

  • Fractures at the thoracolumbar region are commonly followed after major traumatic injuries, and up to 20% of these fractures are known to be burst fractures. Making surgical decisions for these patients are of great interest however there is no golden standard so far. Since the introduction of Thoracolumbar Injury Classification and Severity (TLICS) score in 2007, it has been widely used as a referential guideline for making surgical decisions in thoracolumbar fractures. However, there is still limitations in this system. In this clinical case report, we introduce a L1 burst fracture after motor vehicle injury, who was successfully treated conservatively even while she was graded as a TLICS 5 injury. A case report is presented as well as discussion on the limitations of this grading system.

사상의학의 임상 응용과 저변 확대 - 원지상의 『동의사상신편』을 중심으로 - (Clinical Application of Sasang Constitutional Medicine and the Spread of Its Use : Focusing on Dongui sasang sinpyeon (東醫四象新編))

  • Introduction : Sasang Constitutional Medicine (SCM) is one of the most popular specialty in Korean traditional medicine. After Donguisusebowon (東醫壽世保元), the book initiated SCM at first, there have been several books related with SCM. Through this study on Donguisasangsinpyeon (東醫四象新編), we intend to present clinical application of SCM and the spread of its use. Methods and Discussion : Firstly, we will examine the author's life and philosophy, and consider various editions of Donguisasangsinpyeon. And we will attempt content analysis to the preface, foreword, afterword, and main text. Using these methods, we will show that Donguisasangsinpyeon not only succeed the theory of Donguisusebowon faithfully, but also expand the clinical use of SCM. Conclusions : Donguisasangsinpyeon is the first SCM-based clinical book after Donguisusebowon. And the book can also be valued for its adoption of explanatory diagrams that help people select customized prescriptions conveniently and easily.
