The impacts of exercise on pediatric obesity

  • Over the last few decades, the rates of pediatric obesity have more than doubled regardless of sociodemographic categorization, and despite these rates plateauing in recent years there continues to be an increase in the severity of obesity in children and adolescents. This review will discuss the pediatric obesity mediated cardiovascular disease (CVD) risk factors such as attenuated levels of satiety and energy metabolism hormones, insulin resistance, vascular endothelial dysfunction, and arterial stiffness. Additionally, early intervention to combat pediatric obesity is critical as obesity has been suggested to track into adulthood, and these obese children and adolescents are at an increased risk of early mortality. Current suggested strategies to combat pediatric obesity are modifying diet, limiting sedentary behavior, and increasing physical activity. The effects of exercise intervention on metabolic hormones such as leptin and adiponectin, insulin sensitivity/resistance, and body fat in obese children and adolescents will be discussed along with the exercise modality, intensity, and duration. Specifically, this review will focus on the differential effects of aerobic exercise, resistance training, and combined exercise on the cardiovascular risks in pediatric obesity. This review outlines the evidence that exercise intervention is a beneficial therapeutic strategy to reduce the risk factors for CVD and the ideal exercise prescription to combat pediatric obesity should contain both muscle strengthening and aerobic components with an emphasis on fat mass reduction and long-term adherence.

Effectiveness of Simulation-Based Education for Caring Patients with COVID-19

  • Purpose: The role of medical staff gained immense significance in the context of the prolonged coronavirus disease (COVID-19) pandemic. However, few studies had explored the impact of simulation-based education on the ability of nursing students to care for the patients of COVID-19. This study provided nursing students with simulation-based education in caring for the patients of COVID-19 and confirmed its effectiveness. Methods: This study used a non-equivalent control group pretest-posttest design. The participants were recruited from the nursing departments of two universities in Korea through convenience sampling. A total of 79 participants were included: 37 in the intervention group and 42 in the control group. The intervention group received four sessions of simulation training based on the National League for Nursing Jeffries simulation theory. Results: The intervention group showed an improvement compared to the control group in terms of knowledge related to coronavirus, confidence in performing infection control skills, and perception of preparedness for caring for the patients of COVID-19, with a high-level of satisfaction and self-confidence in learning. There was no significant difference between the two groups in terms of anxiety. Conclusion: This simulation is expected to be a significant strategy for alleviating the global burden in terms of staff safety and patient outcomes by improving the competencies of prospective medical staff in responding to pandemics.

A Study on the Central Bank's Foreign Exchange Market Intervention Strategies with OTC Currency Option Market (중앙은행의 OTC 통화옵션시장을 활용한 외환시장 개입 전략에 관한 연구)

  • This paper studies the possibility of options as an instrument for central bank to intervene foreign exchange market. As opposed to spot transaction or forward transaction, which impacts spot exchange rate only once, currency options can continuously resist a directional speculative pressure on spot market due to the dynamic delta hedging of OTC currency options market maker. This research also analyzes whether and how central banks can use currency options to lower exchange rate volatility and maintain (implicit) target zones in foreign exchange markets. It argues that short position rather than long position in options will result in market makers dynamically hedging their long option exposure in a stabilizing manner, consistent with the first objective. Selling a "Strangle" allows a central bank to increase the credibility of its commitment to a target zone, and could have a lower expected cost than spot market interventions. However, this strategy also exposes the central bank to an unlimited loss potential. Therefore these kinds of intervention strategies must be used in the short run and temporarily.

Percutaneous Coronary Intervention Versus Coronary Artery Bypass Grafting for Revascularization of Left Main Coronary Artery Disease

  • Owing to a large-jeopardized myocardium, left main coronary artery disease (LMCAD) represents the substantial high-risk anatomical subset of obstructive coronary artery disease. For several decades, coronary artery bypass grafting (CABG) has been the "gold standard" treatment for LMCAD. Along with advances in CABG, percutaneous coronary intervention (PCI) has also dramatically evolved over time in conjunction with advances in the stent or device technology, adjunct pharmacotherapy, accumulated experiences, and practice changes, establishing its position as a safe, reasonable treatment option for such a complex disease. Until recently, several randomized clinical trials, meta-analyses, and observational registries comparing PCI and CABG for LMCAD have shown comparable long-term survival with tradeoffs between early and late risk-benefit of each treatment. Despite this, there are still several unmet issues for revascularization strategy and management for LMCAD. This review article summarized updated knowledge on evolution and clinical evidence on the treatment of LMCAD, with a focus on the comparison of state-of-the-art PCI with CABG.

  • This study develops an optimal control strategy for canine rabies transmission using a two-dimensional spatiotemporal model with spatial dynamics. Our objective is to minimize the number of infected and exposed individuals while reducing vaccination costs. We rigorously establish the existence of optimal control and provide a detailed characterization. Numerical simulations show that early intervention, in particular timely vaccination at the onset of an outbreak, effectively controls the disease. Our model highlights the importance of spatial factors in rabies spread and underlines the need for proactive vaccination campaigns, providing valuable insights for public health policy and intervention strategies.

  • 16th CPC National Party Congress established multi-polarization strategy as an external strategy, and 'The Great Reinvigoration of Chinese Nation' as the aim of The Great Strategy of China. It is required to maintain stabilized and peaceful surrounding circumstance in order to make it possible. The ideal surrounding circumstance that China is aiming for is to achieve multi-polarization of international society while maintaining strategic deterrence. Multi-polarization now in 'One super Multi powers' is that 'Multi powers' deter 'One super' to prevent predominance of USA and to decentralize the power. China wishes to realign international order as it accomplishes multi-polarization. China who wants 'Peace and Stabilization' emphasizes autonomy, peace and unification of Korean peninsula. Autonomy means the restraint intervention of foreign power, that is, American influence in Korean peninsula. Multi-polarization strategy of China will be used in Korean peninsula to solve nuclear issue of North Korea, to support neutral unification and to keep balance of power with USA, Japan and Russia as it renders economic advantages to South Korea and security engagement to North Korea, based on special geographical, cultural and historical relationship with two Koreas. The nuclear issue of North Korea will be a stage to test multi-polarization of China. When it deduces positive solution of nuclear issue due to successful six-party talks, and advances to security talks of Northeast Asia, it will contribute a lot to power and elevation of national stature of China in international society. Thus, the Chinese strategy will be an accommodative condition for the security of South Korea, and it requires wise decision of South Korea to make hay while the sun shine.

INTERVENTION STRATEGY FOR REDUCING ADOLESCENT SMOKING

  • This study aims to establish and analyze a mathematical model for the transmission dynamics of male adolescent smoking and to determine an optimal control strategy to reduce male adolescent smoking. We consider three groups in the population: smokers, non-smokers, and temporary nonsmokers. In our model to which optimal control theory was applied, the number of smokers decreased sharply and the number of non-smokers increased significantly. Our simulation results under various control scenarios reveal that integrated control measures(such as prevention, education, and treatment) may be necessary to reduce the growth rate of adolescent smoking. Moreover, we concluded that efforts to encourage current smokers and temporary quitters to quit should be sustained longer than efforts to reduce the rate at which nonsmokers become smokers through smoking prevention education.

  • The author introduced 12 session short term dynamic psychotherapy for the intervention of stress related disorders. An examplary case was introduced to suggest the clinal usefulness of time limitted short term dynamic psychotherapy for stress related disorders. It was suggested that the term "stress" should be limitted to certain natural and man-made catastrophies and disasters whose consequences rep quire a total reorganization of the person. Common defensive manuvers and psychological responses in the stress disorders were also reviewed.
