Effects of a School-based Intervention Program for Middle School Adolescent Girls with Depression: As Part of the School Health Services

  • Purpose: The purpose of this study was to evaluate the effects of a school-based intervention program for middle school adolescent girls with depression. Methods: The study was a pretest-posttest repeated-measure design with a nonequivalent control group. Fifty eight students with depressive symptoms were recruited from two middle schools in Seoul, Korea. The data were collected from the intervention (n=30) and the comparison group (n=28). The research instrument was Reynolds Adolescent Depression Scale. Results: The intervention group greatly improved from baseline to 10 weeks and then saw a slight positive change between 10 and 13 weeks. Conclusion: The results of this research show that depression intervention programs are effective for young female adolescents. Thus the investigation has important school-based treatment implications, and should be integrated into school curriculums by school health nurses for early intervention of depressive symptoms in middle school adolescent girls.

국내 자폐스펙트럼장애 아동의 병원 기반 조기 집중 중재 실태 조사 (A Survey on the Status of Hospital-Based Early Intensive Intervention for Autism Spectrum Disorder in South Korea)

  • Objectives: Early intensive interventions are very important for children with autism spectrum disorder. We examined the actual conditions of hospital-based early intensive interventions for autism spectrum disorder in Seoul, in order to help develop and implement an evidence-based early intensive intervention model for use in Korea. Methods: Nine hospital-based institutes running an early intensive intervention program for children with autism spectrum disorder responded to a questionnaire in September 2014. They provided a brief introduction to their program, explained its theoretical bases, and reported the number of children, their age, intervention time, duration and so on. Results: In the majority of the institutions, the intervention was provided for over 20 hours every week, and the theoretical bases included various applied behavioral analysis (ABA) methods and other therapies (language and occupational therapy). The therapist-child ratio ranged from 1:1 to 5:3. Various types of therapists were involved, including behavioral analysts, special education teachers and (or) language pathologists. There was only one clinic where the behavioral analyst was the main therapist. Usually, the intervention was terminated just before the child entered elementary school. The main merit of the hospital-based intervention in our survey was the effectiveness of the multi-disciplinary intervention plan and its other merits were the accuracy of the diagnosis, its ability to be combined with medicine, and so on. Conclusion: The current hospital-based early intensive intervention programs provide interventions for over 20 hours per week and employ multidisciplinary approaches. However, there are very few institutes for children with autism and very few intervention specialists and specialist education courses in the country. We need more educational programs for intervention therapists and have to try to develop policies which encourage the implementation of an evidence-based early intensive intervention program nationwide.

건강증진사업을 위한 보건교육프로그램 개발 : 금연, 절주교육을 중심으로 (Developing Health Education Programs for Health Promotion: Development of a Training Guide on Prevention of Smoking and Excessive Drinking)

  • Background: Alcohol and tobacco use are two major behavioral risk factors implicated in increased morbidity and mortality. Since both substances are widely used in Korea, a concerted effort is currently underway to reduce the use of tobacco and alcohol in Korea. Objectives: Efforts directed toward educating health promotion planners and health educators from local health departments to organize and implement health education programs to reduce the proportion of people smoking and excessive drinking in the community. Methods: A training guide on prevention of smoking and excessive drinking has been developed. Comprehensive multi-media health promotion materials were developed based on health behavior theories and strategies for effective health behavioral interventions. To better control of behavioral risk and promote health enhancement, the materials were developed to introduce a user-oriented developmental approach by making messages more persuasive and organizing content in a user-friendly manner. Sections of the report explain theoretical background of the intervention, choosing goals and a target population, specific program activities to include in an intervention, development, organization, methods of making program content vivid and persuasive, and evaluation,. Implications for Practitioners: The process followed in developing the health intervention materials is described in detail to assist practitioners who need to develop effective programs to reduce the use of tobacco and excessive alcohol. Health educators from all local health departments in the country were trained in the use of the materials, to enable them to develop community interventions to reduce smoking and excessive drinking.

Mindfulness Based Stress Reduction (MBSR) 프로그램이 스트레스 및 삶의 질에 미치는 효과에 대한 분석 (Critical Review of the Effects of Mindfulness-Based Stress Reduction (MBSR) on Stress and Health-related Quality of Life (QOL))

  • Purpose: In numerous studies, MBSR programs have been shown to reduce stress and improve health-related QOL. Although MBSR programs have been shown to be beneficial, its effects have not been well-determined among Korean populations in clinical nursing practice. The purpose of this review was to describe the effects of MBSR on stress and health-related QOL so that the potential benefits of this intervention may be evaluated for wider application in Korea. Methods: Articles published in peer-reviewed journals in the English or Korean language up to 2009 were reviewed; 14 articles met the inclusion criteria for this review and there were no Korean papers. Results: MBSR groups were found to have significantly reduced stress outcomes relative to non-MBSR groups in 6 of 11 studies. In regards to health-related QOL, participants of the MBSR programs had increased scores after MBSR intervention relative to control groups in 5 of 7 studies. Conclusion: MBSR programs was effective in decreasing stress and increased the health-related QOL in the majority of studies reviewed. MBSR merits study in Korean patients to determine whether similar salutary patient outcomes may be obtained.

경기도 정신보건센터 서비스에 대한 가족 만족도 조사 - 경기도 31개 정신보건센터를 중심으로 - (A Study on Family Satisfaction with Community Mental Health Center Services in Gyeonggi Province by Families of People with Mental Illness)

  • Purpose: The purpose of this study was to evaluate the service satisfaction for families who have someone with mental illness, and are using community mental health centers in Gyeonggi Province. Methods: The participants in this study were 796 family members. Data were collected using Family satisfaction survey questionnaire developed by the author (23 items on family service and 15 items on client service). Results: The total satisfaction level for the service with family and client resulted in above average scores. Of the 23 service items, data showed the highest level of satisfaction was with professional skills and attitude, and day rehabilitation programs, and the lowest for professional activities for advocacy and social welfare benefits, emergency & crisis intervention, medical expense subsidies. Of the 15 service items, job and housing related service had the lowest level of satisfaction. Conclusion: It is recommended that psychiatric emergency & crisis intervention programs and system development be accelerated. Also, there is a need to develop medical expense subsidy programs for older family caregivers, job and housing focused rehabilitation programs and community facilities for the client, as well as more active and powerful professional advocate activities for persons who have mental disabilities and their families.

경두개 직류전류자극과 전산화 인지재활 프로그램이 뇌졸중 환자의 인지기능 및 우울감에 미치는 영향 (The Effect of Transcranial Direct-Current Stimulation on Cognitive Function and Depression in Stroke Patient's through a Computerized Cognitive Rehabilitation Program)

  • Purpose : The purpose of this randomized controlled trial study was to examine the effect of transcranial direct current stimulation (tDCS) on cognitive function and depression in stroke patients. Methods : Thirty stroke patients were randomly divided into an experimental group (n = 15) and a control group (n = 15). The experimental group received tDCS while performing computerized cognitive rehabilitation programs, and the control group was provided with sham tDCS while operating the same programs. The 30-minute intervention was implemented five times per week for six weeks. To assess cognitive function before and after the intervention, the Neurobehavioral Cognitive Status Examination was conducted; the Beck Depression Inventory BDI was employed to assess depression. Results : The experimental group showed statistically significant increases in cognitive function and decreases in depression (p < .05 ). Comparing the amount of variation between the groups after arbitration also showed significant differences in cognitive function and depression between the two groups (p > .05). Conclusion : The application of tDCS and computerized cognitive rehabilitation programs for stroke patients may positively affect their cognitive function and depression. Therefore, tDCS used with computerized cognitive rehabilitation programs is positively applicable to the enhancement of cognitive function in stroke patients and reduction of depression.

노인영양교육프로그램 참여자의 특성 분석-건강증진모델의 요인을 중심으로- (Analysis of the Characteristics of an Attendee in an Elderly Nutrition Education Program -Using the Factors of Health Promotion Model-)

  • Although many people initially enroll in health education programs, there are many instances of erratic participation and dropouts. Inconsistent participation in intervention programs minimizes their impact on health promotion. Therefore, a theoretical understanding of factors influencing participation in these programs can potentially enhance the effectiveness of its educational strategy. This study used the Pender's Health Promotion Model to examine specific factors influencing incentives to participate in an elderly nutrition education program. The Elderly Nutrition Counseling and Education Program was conducted with 147 volunteers (76 males, 71 females), aged 60 to 87, at 5 separate community elderly centers, by public health dietitians from February to April 1997. Some participants dropped out during the program. Overall, 61 people(18 males, 43 females) finished all 7 steps over 2 months. Pre-intervention data were collected by trained dietitians. This data included individual cognitive-perceptual factors(perceived benefits of nutrition improvement, importance of health, perceived control over health by multidimensional health locus of control, self esteem, perceived health status, concern about health, depression scale and social health scale), which were known to influence the likelihood of health behavior, and modifying factors(socioeconomic variables, biological characteristics, behavioral factors, such as smoking, alcohol drinking and exercise). Male finalists had a significantly lower chance for health locus of control, and better social health status with their children and grandchildren, compared to males who dropped out. Female finalists had a significantly higher locus of control regarding food behavior, higher self-esteem, better recognized nutritional status, worse self-recognized health status and lower concern about health than those who dropped out. There was no significant difference between the attendees and dropouts in age, BMI$(kg/m^2)$, Nutritional Risk Index, depression scale and daily nutrient intake. These results suggest that elderly nutrition intervention plans should focus on the individual cognitive and perceptual factors, with interpersonal influences, to increase participation in nutrition in nutrition improvement programs.

영양위험 농촌노인집단에 적용한 영양중재 프로그램의 추후관리 효과( I ) (The Effect of Follow-up Nutrition Intervention Programs Applied Aged Group of High Risk Undernutrition in Rural Area( I ))

  • This research was peformed to investigate the anthropometric data, blood profiles, and nutrient intakes of elderly persons living in a rural area. The subjects were 67 undernourished people who participated in follow-up nutrition intervention programs for9 weeks. Anthropometric data showed that the mean heights and weights in the management group were 157.6 cm and 59.1 kg, respectively, for the males and 152.6 cm and 51.0 kg, respectively, for the females. The mean BMIs of the management group were 23.8 kg/m$^2$ in the males and 22.4 kg/m$^2$ in the females. The total cholesterol, HDL-cholesterol, and albumin levels of the subjects were 181.7-191.4mg/dL, 48.3-53.0mg/dL, and 3.85-4.00g/dL, respectively. Energy, ash, P, Na, vitamin B$_1$, vitamin B$_2$, vitamin B$_6$, and niacin increased significantly after intervention for the management group. The mINQ, however, did not significantly increase after intervention. Also after intervention, there was no significant difference in mINQ between the management group and the comparison group. MAR (14) in the management group was significantly increased from 0.62${\pm}$0.2 before intervention to 0.68${\pm}$0.2 after intervention (p=0.022), and it was significantly different between the management group and the comparison group (p=0.017). MAR (8) in the management group was not significantly different (p=0.915) before and after intervention. However, MAR (8) between the management group and the comparison group did show a significant difference (p=0.031). MAR (3) in the management group was significantly increased from 0.48${\pm}$0.2 before intervention to 0.55${\pm}$0.2 after intervention (p=0.045), however, MAR (3) was not significantly different between the management group and the comparison group (p=0.093). For the probability of nutrient insufficiency, in the management group the probability of nutrient values below the EAR (except for Fe) decreased after intervention compared to before intervention. 비대면 중재 방법에 따른 노인성 근감소증의 개선에 대한 연구 (A Study on the Improvement of Geriatric Sarcopenia by Non-face-to-face Intervention Method)

  • Purpose : This study was conducted to compare two non-face-to-face exercise interventions depending on whether mobile applications and wearable exercise aids are used to find out which interventions are more effective in improving senile sarcopenia. Ultimately, it was conducted to provide basic data for developing non-face-to-face intervention methods to improve sarcopenia. Method : In this study, 18 elderly sarcopenia and possible sarcopenia aged 65 or older were randomly assigned to the digital and self-exercise intervention groups. The digital exercise intervention group performed eight exercise programs with mobile applications and wearable exercise aids to record and manage the elderly performing the programs in real time. And the self-exercise intervention group performed the same program on its own as implemented in the digital exercise group. The intervention was applied for 8 weeks, and before and after the intervention, sarcopenia evaluation and physical function evaluation were performed. Results : In the digital exercise intervention group, arm muscle mass, skeletal muscle index, SPPB, 5TSTS, and BBS were improved, and in the self-exercise intervention group, grip strength, SPPB, 5TSTS, and BBS were improved. Conclusion : It was confirmed that both groups are effective in improving physical performance and physical function, the digital exercise intervention is effective in improving muscle mass and self-exercise intervention is effective in improving muscle strength. Therefore, this study proposes to apply intervention methods separately according to the indicators to improve and prevent sarcopenia, and also simplify the instructions of applications used to improve sarcopenia and to create an environment where users can be trained regularly on how to use it. And, In the future, studies for the development of devices to be designed to help non-face-to-face exercise interventions or studies on the differences between face-to-face and non-face-to-face exercise interventions should be conducted in terms of the effect of improving sarcopenia.

건강증진병원에서 실시된 당뇨병 및 심혈관질환 위험군 대상 생활습관개선 프로그램의 질병예방 효과 (Effectiveness of lifestyle interventions to prevent diabetes and cardiovascular diseases in a health promoting hospital)

  • Objectives: This study was conducted to expand evidence-base for the effectiveness of lifestyle intervention programs to prevent the onset of diabetes and cardiovascular diseases. Methods: Seven intervention studies between 2006 and 2014 at Korea Association of Health Promotion were analyzed. All the studies were randomized controlled trials(RCTs) and included multi-component behavioral interventions. The participants of the programs were 2,172 adults with risk factors regarding metabolic syndrome criteria. The proportions of normalized participants were compared within and across the studies using odds ratio effect sizes. Results: The reductions in the prevalence of metabolic syndrome were from 49.6% to 65.1% in intervention groups, and from 38.7% to 52.3% in comparison groups. Significant differences in effectiveness between groups were found in two studies, one in 2006 with odds ratio of 1.69(p<0.01) and another in 2009 with odds ratio of 2.36(p<0.001). Proportions of normalized participants were higher in blood pressure(31.9% to 52.5% in the intervention groups and 23.0% to 43.3% in comparison groups) than other risk factors. Abdominal obesity showed weakest improvement after the intervention in both groups. Conclusions: Lifestyle modification program is an effective method to reduce diabetes and cardiovascular risks in adults by decreasing the prevalence of metabolic syndrome and its components.
