Patient exposure doses from medical x-ray examinations in Korea (진단방사선검사에서 환자피폭선량에 관한 연구)

  • X-ray examinations represent the largest man-made source of radiation exposure for the population. The need for standardization of radiation exposures has been suggested and the guidance levels for various radiographic and radioisotope examinations has been proposed by the International Atomic Energy Aency(IAEA) as a safety standard. In many countries, the situation of medical radiographic exposures in each country should be researched before the appropriate guidance level is established. In this study, measurements of entrance surface dose, dose-area product(DAP), computed tomograghic dose index(CTDI) and mean glandular dose(MGD) were carried out in patients who underwent routine x-ray examinations, fluoroscopy, computed tomograghy and mamography in Korea. These measured quantities were compared with the results from the calculation method in previous study. And we suggested diagnostic reference levels in medical imaging in Korea.

An Empirical Study on the Determinants of Foreign Market Entrance of Japanese Enterprises -focusing on the Viewpoint Macro Economy- (일본기업의 해외시장 진출의 결정요인에 관한 경험적 연구 -거시경제 관점을 중심으로-)

  • This study first analyzes economical environment change (factor) of investment nation Japan and Japanese enterprise at the analysis of Japanese enterprise's over-seas expansion factor and influence. As a result of an analysis of the factor about Japanese enterprise's overseas expansion from 1990 to 1996 and from 1998 to 2006, commonly applied factors were Yen exchange rate, interest rates, wage, enterprise profit, facility investment, and consumption expenditure. Especially, as a result of regression analysis, a sudden change of "low interest rate" was main factor at Japanese enterprise's overseas expansion from 1990 to 1996, and Japan's "Yen ex-change rate" was drawn as an important factor from 1998 to 2006. That is, from 1990 to 1996, a shock by a sudden rise in Yen value could be viewed gradually accumulated and absorbed inside Japanese economy from 1998 to 2006. Whereas it could be said that Japanese enterprise's overseas expansion was accelerated under Japanese government's overseas supporting policy and "low interest rate" together with the factor in the rise of Yen value from 1998 to 2006.

  • This study is to investigate on focus of his political position change to build up his the real form about Park je sang(朴堤上) who had taken an active part within Shila nulgi king(訥祗王) period on base of previous park je sang' study results. that is, this study took attention about excellent capability by diplomat who had shown his diplomatic ability in complicated and international fields of three countries, and about process of entrance on central political fields by private capability in sap lang(?良) district in relation with building up the real form of park je sang. As result, by means of few investigations about park je sang who had taken an active part, we looked into process how local power man in sap lang(?良) district had built up his position to jin-gol(眞骨) status with strategic location of sap lang(?良) district on shila history within Shila nulgi king period. I think that we found valuable means from figures of a district society in process how Shila had grown with a country of centralizing power.

Analysis of Ambivalence Differences among Groups for Temporary Firefighting Facilities of Workers at Construction Sites (공사현장 근로자의 임시소방시설에 대한 집단 간 Ambivalence 차이분석)

  • The purpose of this study is to prevent fire accidents in advance by deriving prior knowledge among groups about temporary fire-fighting facilities of workers at construction sites and devising appropriate improvement measures. The findings are as follows.First, in the case of fire extinguishers, statistical attention was paid to the contractor, supervisor, partner worker and fire extinguisher manager between designations. 87% of fire managers and 70% of facility supervisors said fire extinguisher management managers needed to be designated for each type of construction, which requires designation of fire extinguisher management managers, frequently checking and relocating. Second, in the case of simple fire extinguishing facilities, statistical attention was paid to the application of penalties for unauthorized use of fire extinguishing facilities with construction companies, supervisors, and business partners.Third, in the case of emergency alarm measures, statistical attention was paid to the application of emergency alarm sound to temporary broadcasting facilities with construction works, supervisors, and business partners.Fourth, in the case of induction, statistical attention was paid to the application of connection between construction works, supervisors, and partner workers, such as passage guidance, emergency lighting, etc.It was found that 65% of construction workers and 55% of electrical workers had different applications such as aisle guidance and emergency lighting for each type of business partner. In order to resolve blind spots such as evacuation zone guidance due to the structure of the building, it is necessary to easily distinguish the direction of the entrance door from a long distance by applying it in conjunction with passage guidance lights and emergency lighting.

Infection Control in Pulmonary Function Laboratories in Domestic Hospitals (국내 의료기관의 폐기능검사실에서 감염관리 실태조사)

  • The global outbreak of COVID-19 has underscored the pressing need for robust infection control practices in pulmonary function laboratories (PFL). However, the existing guidelines and regulatory frameworks provided by relevant authorities in the country have revealed certain deficiencies in effectively addressing this significant public health crisis. This study surveyed the infection control regulations, disposable item usage, ventilation facilities, spatial separation, and the configuration of entrance doors in 51 domestic hospital facilities from Oct 1, 2021, to Nov 2, 2021. The survey findings revealed that while there was a relatively satisfactory adherence to airborne, droplet, and contact precautions with adequate awareness and utilization of personal protective equipment, the environmental disinfection practices exhibited a suboptimal performance rate of 39.22% per patient. Depending on the specific survey domains, substantial variations were observed in the utilization of disposable items (81.05%), ventilation systems (45.75%), dedicated testing spaces (80.39%), separation of administrative areas (15.69%), and the installation of automated doors (19.61%). This study not only highlights the paramount importance of infection control in PFLs within domestic medical institutions but also provides foundational data for developing and enhancing standardized guidelines that align with international benchmarks for infection control in these settings.

A Study on the Educational Methods of Self-Narrative Writing for University Students (대학생 자기 서사 글쓰기의 교육 방안 연구)

  • In the purpose of this study, the college textbooks of self-narrative writing and examples of classroom practice are analyzed to find a way to educate it. The self- narrative writing subject with a learning of recognization, expression, and communication with oneself, emphasizes the necessity when they become college students through entrance exam-oriented education. The research methods are as follows. Firstly, three university textbooks which include a section on self-narrative writing were compared and analyzed. The analysis highlights the needs for a textbook covering self-narrative writing more extensively and comprehensively as what is offered by the existing textbooks is limited in facilitating students to fully develop the ability of self-reflection, which should be dealt as a long-term goal. Secondly, the current discussion on self-narrative writing and examples of real classroom practice were analyzed. It shows that a step-by-step approach is required to encourage the practice of deep self-reflection to be incorporated into writing. In addition, during the writing process, various correction and feedback activities should be carried out on a macro level and gradually while the communication and feedback should take place not only between a teacher and students, but also among students. As a result, it is expected that this study will help establish a teaching model of self-narrative writing by seeking complementary points and educational directions for self-narrative writing.

A Study for Establishment of Diagnostic Reference Level of Patient Dose in Skull Radiography (우리나라의 두부 엑스선검사에서의 환자선량 권고량)

  • Ionizing radiation is most widely used for X-Ray examination among all artificial radiation exposure, it takes up the largest proportion. Even in Korea, the medical exposure by diagnostic X-Ray examination takes up 17.4% of all radiation exposure. It takes up 92% even in artificial radiation exposure. There were 111,567 cases X-Ray radiography for skull diagnosis in 2007, which is 3% annual increase since 2004. Thus, It is need to establish the diagnostic reference level and the medical facilities as a diagnostic reference level to optimize radiation protection of the patients and to reduce the doses of X-ray. In this paper, we survey patient dose on skull radiography - collected from 114 medical facilities nationwide by using human phantom and glass dosimeter. When the patient dose for the skull radiography was measured and evaluated to establish the diagnostic reference level, 2.23 mGy was established for posterior-anterior imaging and 1.87 mGy for lateral imaging was established. The posterior-anterior skull radiography entrance surface dose of 2.23 is less than the guidance level of 5 mGy from the global organizations such as World Health Organization (WHO) and International Atomic Energy Agency (IAEA), and 1.87 mGy for the lateral skull imaging is less than the guidance level of 3 mGy, which is guided by the global organizations such as World Health Organization (WHO) and International Atomic Energy Agency (IAEA).

Diagnostic Reference Levels for Patient Radiation Doses in Pelvis and Lumbar spine Radiography in Korea (우리나라의 골반 및 요추 엑스선검사에서의 환자선량 권고량)

  • Purpose : Pelvis and lumbar spine radiography, among various types of diagnostic radiography, include gonads of the human body and give patients high radiation dose. Nevertheless, diagnostic reference levels for patient radiation dose in pelvis and lumbar spine radiography has not yet been established in Korea. Therefore, the radiation dose that patients receive from pelvis and lumbar radiography is measured and the diagnostic reference level on patient radiation dose for the optimization of radiation protection of patients in pelvis and lumbar spine radiography was established. Methods : The conditions and diagnostic imaging information acquired during the time of the postero-anterior view of the pelvis and the postero-anterior and lateral view of the lumbar spine at 125 medical institutions throughout Korea are collected for analysis and the entrance surface dose received by patients is measured using a glass dosimeter. The diagnostic reference levels for patient radiation dose in pelvis and lumbar spine radiography to be recommended to the medical institutes is arranged by establishing the dose from the patient radiation dose that corresponds to the 3rd quartile values as the appropriate diagnostic reference level for patient radiation dose. Results : According to the results of the assessment of diagnostic imaging information acquired from pelvis and lumbar spine radiography and the measurement of patient entrance surface dose taken at the 125 medical institutes throughout Korea, the tube voltage ranged between 60~97 kVp, with the average use being 75 kVp, and the tube current ranged between 8~123 mAs, with the average use being 30 mAs. In the posteroanterior and lateral views of lumbar spine radiography, the tube voltage of each view ranged between 65~100 kVp (average use: 78 kVp) and 70~109 kVp (average use: 87 kVp), respectively, and the tube current of each view ranged between 10~100 mAs(average use: 35 mAs) and between 8.9~300 mAs(average use: 64 mAs), respectively. The measurements of entrance surface dose that patients receive during the pelvis and lumbar spine radiography show the following results: in the posteroanterior view of pelvis radiography, the minimum value is 0.59 mGy, the maximum value is 12.69 mGy and the average value is 2.88 mGy with the 1st quartile value being 1.91 mGy, the median being 0.59 mGy, and the 3rd quartile value being 3.43 mGy. Also, in the posteroanterior view of lumbar spine radiography, the minimum value is 0.64 mGy, the maximum value is 23.84 mGy, and the average value is 3.68 mGy with the 1st quartile value being 2.41 mGy, the median being 3.40 mGy, and the 3rd quartile value being 4.08 mGy. In the lateral view of lumbar spine radiography, the minimum value is 1.90 mGy, the maximum value is 45.42 mGy, and the average value is 10.08 mGy with the 1st quartile value being 6.03 mGy, the median being 9.09 mGy and the 3rd quartile value being 12.65 mGy. Conclusions : The diagnostic reference levels for patient radiation dose to be recommended to the medical institutes in Korea is 3.42 mGy for the posteroanterior view of pelvis radiography, 4.08 mGy for the posteroanterior view of lumbar spine radiography, and 12.65 mGy for the lateral view of lumbar spine radiography. Such values are all lower than the values recommended by 6 international organizations including World Health Organization, where the recommended values are 10 mGy for the posteroanterior view of pelvis radiography, 10 mGy for the posteroanterior view of lumbar spine radiography and 30 mGy for the lateral view of lumbar spine radiography.

Dosimetric Evaluation of a Small Intraoral X-ray Tube for Dental Imaging (치과용 초소형 X-선 튜브의 선량평가)

  • Radiation exposure from medical diagnostic imaging procedures to patients is one of the most significant interests in diagnostic x-ray system. A miniature x-ray intraoral tube was developed for the first time in the world which can be inserted into the mouth for imaging. Dose evaluation should be carried out in order to utilize such an imaging device for clinical use. In this study, dose evaluation of the new x-ray unit was performed by 1) using a custom made in vivo Pig phantom, 2) determining exposure condition for the clinical use, and 3) measuring patient dose of the new system. On the basis of DRLs (Diagnostic Reference Level) recommended by KDFA (Korea Food & Drug Administration), the ESD (Entrance Skin Dose) and DAP (Dose Area Product) measurements for the new x-ray imaging device were designed and measured. The maximum voltage and current of the x-ray tubes used in this study were 55 kVp, and 300 mA. The active area of the detector was $72{\times}72mm$ with pixel size of $48{\mu}m$. To obtain the operating condition of the new system, pig jaw phantom images showing major tooth-associated tissues, such as clown, pulp cavity were acquired at 1 frame/sec. Changing the beam currents 20 to $80{\mu}A$, x-ray images of 50 frames were obtained for one beam current with optimum x-ray exposure setting. Pig jaw phantom images were acquired from two commercial x-ray imaging units and compared to the new x-ray device: CS 2100, Carestream Dental LLC and EXARO, HIOSSEN, Inc. Their exposure conditions were 60 kV, 7 mA, and 60 kV, 2 mA, respectively. Comparing the new x-ray device and conventional x-ray imaging units, images of the new x-ray device around teeth and their neighboring tissues turn out to be better in spite of its small x-ray field size. ESD of the new x-ray device was measured 1.369 mGy on the beam condition for the best image quality, 0.051 mAs, which is much less than DRLs recommended by IAEA (International Atomic Energy Agency) and KDFA, both. Its dose distribution in the x-ray field size was observed to be uniform with standard deviation of 5~10 %. DAP of the new x-ray device was $82.4mGy*cm^2$ less than DRL established by KDFA even though its x-ray field size was small. This study shows that the new x-ray imaging device offers better in image quality and lower radiation dose compared to the conventional intraoral units. In additions, methods and know-how for studies in x-ray features could be accumulated from this work.

A study on Operation Rules of Korean Air Defence Identification Zone (한국 방공식별구역 운영규칙에 관한 고찰)

  • Declaration of Air Defense and Identification Zones started with the United States in 1950, which was followed by declaration of KADIZ by the Republic of Korea in 1951. Initial ADIZ were solely linked with air defense missions, but their roles have changed as nations around the globe manifested a tendency to expand their influence over maritime resources and rights. In particular, China declared ADIZ over the East China Sea in October 2013 and forced all passing aircraft to submit flight plan to ATC or military authority, saying failure of submission will be followed by armed engagement. China announced it would declare another zone over the South China Sea despite the ongoing conflict in the area, clearly showing ADIZ's direct connection with territorial claim and EEZ and that it serves as a zone within which a nation can execute its rights. The expanded KADIZ, which was expanded in Dec 15, 2013 in response to Chinese actions, overlaps with the Chinese ADIZ over the East China Sea and the Japanese ADIZ. The overlapping zone is an airspace over waters where not only the Republic of Korea but also of China and Japan argue to be covering their continental shelf and EEZ. Military conventions were signed to prevent contingencies among the neighboring nations while conducting identifications in KADIZ, including the overlapping zone. If such military conventions and practice of air defense identification continue to be respected among states, it is under the process of turning into a regional customary law, although ADIZ is not yet recognized by international law or customary law. Moreover, identification within ADIZ is carried out by military authorities of states, and misguided customary procedures may cause serious negative consequences for national security since it may negatively impact neighboring countries in marking the maritime border, which calls for formulation of operation rules that account for other state activities and military talks among regional stake holders. Legal frameworks need to be in place to guarantee freedom of flights over international seas which UN Maritime Law protects, and laws regarding military aircraft operation need to be supplemented to not make it a requirement to submit flight plan if the aircraft does not invade sovereign airspace. Organizational instructions that require approval of Chairman of Joint Chiefs of Staff for entrance and exit of ADIZ for military aircraft need to be amended to change the authority to Minister of National Defense or be promoted to a law to be applicable for commercial aircraft. Moreover, in regards to operation and management of ADIZ, transfer of authority should be prohibited to account for its evolution into a regional customary law in South East Asia. In particular, since ADIZ is set over EEZ, military conventions that yield authority related to national security should never be condoned. Among Korea, China, Japan and Russia, there are military conventions that discuss operation and management of ADIZ in place or under negotiation, meaning that ADIZ is becoming a regional customary law in North East Asia region.
