소아에서 자전거 핸들에 의한 총담관 절단 증례보고 (Transection of Distal Common Bile Duct by Bike Handlebar in a Child)

  • A 10 year old boy was admitted with blunt abdominal trauma by bike handle injury. The patient was operated upon for a generalized peritonitis due to pancreaticoduodenal injury. On opening the peritoneal cavity. complete transection of distal end of common bile duct and. partial separation between pancreas head and second portion of duodenum were found. Ligation of the transected end of the common bile duct. T-tube choledochostomy, and external drainage were performed. A pseudocyst was found around the head portion of the pancreas on the 7th postoperative day with CT. An internal fistula had developed between the pseudocyst and ligated common bile duct. The pseudocyst was subsided after percutaneous drainage. In the case of the undetermined pancreatic injury, percutaneous external drainage can be effective in treating the traumatic pancreatic pseudocyst in a pediatric patient.

  • Extensive surgical resection of the aerodigestive track can result in a large and complex defect of the oropharynx, which represents a significant reconstructive challenge for the plastic surgery. Development of microsurgical techniques has allowed for free flap reconstruction of oropharyngeal defects, with superior outcomes as well as decreases in postoperative complications. The reconstructive goals for oral and oropharyngeal defects are to restore the anatomy, to maintain continuity of the intraoral surface and oropharynx, to protect vital structures such as carotid arteries, to cover exposed portions of internal organs in preparation for adjuvant radiation, and to preserve complex functions of the oral cavity and oropharynx. Oral and oropharyngeal cancers should be treated with consideration of functional recovery. Multidisciplinary treatment strategies are necessary for maximizing disease control and preserving the natural form and function of the oropharynx.

초음파 전파속도법을 이용한 목조 문화유산 흰개미 피해의 정량 평가 (Quantitative Analysis for Termites Damage of Wooden Heritage using Ultrasonic Pulse Velocity)

  • Quantitative analysis of termites damage is important in terms of conservation and maintenance of wooden cultural heritage buildings, because termites makes cavities and decreases the section area of wooden structural members. The purpose of this study is to forecast the range and spread of termites damage in the wooden structural members by using ultrasonic pulse velocity method. Ultrasonic pulse velocity has been used as one of non-destructive test to analysis the internal defect by using difference velocity between medium material and cavity. This method would be effective to analysis termites damages. From the result of the ultrasonic velocity test, the loss rate of area effected by termites damage had a strong correlation with ultrasonic velocity. And it is possible to predict the loss rate of area from by termites damage by using regression equation in the case of structural member of fine tree.

  • The aluminum die-casting technique has been widely used to manufacture a rotor in motor industry because of its highly productivity, even though it sometime causes the various type of defects in the rotor, such as shrinkage, cavity, blow holes etc. which results in the decrease of the efficiency of system. Therefore the development of reliable technique to detect the flaws in the rotor is strongly needed not only to control quality assurance but to improve its efficiency. In this study, the wide variety of ultrasonic techniques have been applied to detect the flaws in the rotor and then to discuss the detectibility of the flaw and the applicability for NDE tool in the aluminum die-casting rotor.

  • The volume of an acoustic source is important in determining various acoustic parameters. One of the suggested techniques is the internal pressure method incorporating a loudspeaker attached to a chamber wall and a microphone inserted into the cavity. Although the method is easy to handle with a very simple measurement setup, the coupling effects between the dynamic system of the loudspeaker and acoustic field, and the effects of higher order modes introduced by the discontinuities in the acoustic field, and the effects of higher order modes introduced by the discontinuities in the acoustic field should be considered for precise result. In this study, higher order modes due to the discontinuities of loudspeaker and microphone boundaries are included and the electro-acoustic coupling effects are compensated for by using the results of two cylinders with different lengths. The volume velocity of a loudspeaker thus obtained agrees very with that measured by laser sensor.

Lateral Far-field Characteristics of Narrow-width 850 nm High Power GaAs/AlGaAs Laser Diodes

  • We investigate the lateral far-field pattern characteristics, including divergence angle change and far-field pattern analysis as output power increases, of narrow-emitter-width 850 nm GaAs/AlGaAs laser diodes (LDs). Each LD has a cavity of 1200 and 1500 ㎛ and narrow emitter width of 2.4 ㎛ for the top and 4.6 ㎛ for the bottom. The threshold currents are 35 and 40 mA, and L-I kinks appear at power levels of 326 and 403 mW, respectively. The divergence angle tends to increase due to the occurrence of first-order lateral mode and the thermal lensing effect. But with the L-I kink, the divergence angle decreases and the far-field pattern becomes asymmetric. This is due to coherent superposition between the fundamental and the first-order lateral mode. We provide detailed explanations for these observations based on high-power laser diode simulation results.

복강내침술과 관련 혈위의 자침 깊이에 대한 고찰 (Review on Intraperitoneal Acupuncture and Needling Depth)

  • Objectives : To review the needling depth, therapeutic and adverse effects of intraperitoneal acupuncture and related acupoints. Methods : Studies and reports from the literature, Korean medical health insurance data and databases were searched and selected. Results : The depth of needle insertion for acupoints related to intraperitoneal acupuncture has been documented to range from 0.9 to 10.5 cm depending on the acupoints and literature. In studies of safe needling depth, the justice of safe needling depth varied from study to study, but was generally based on the peritoneum, and the depth varies by patient age, sex, body type, and weight. Clinical studies in China have shown good efficacy and no side effects of abdominal long needle acupuncture, but there have been case reports of adverse events following abdominal acupuncture. Conclusions : To ensure both therapeutic effectiveness and safety of intraperitoneal acupuncture, It is necessary to utilize the ultrasound in clinical practice and conduct related research.

형석단결정의 내부결함 및 그 영향에 관한 연구 (Astudy of internal defects and their effects in $CaF_2$ single crystals)

  • 형석$(CaF_2)$ 단결정 내부에 형성되는 다양한 결함에 대하여 현미경관찰, 성분분석 및 투과율분석을 하고 광투과에 미치는 영향을 고찰하였다. 내부 cavity의 종류로는 bubble과 결정면을 포함하는 공공형태의 negative crystals이 있으며, 특히 negative crystals이 결정에 고루 퍼져있는 경우, UV-Visible 파장영역에서 약 3%의 투과율 저하현상이 나타났다. 또한, 결정내부의 석출물은 주로 내부 cavite에 형성됨을 WDX 분석을 통하여 확인할 수 있었다. Violet colored 결정은 non-colored 결정에 비하여 전위밀도가 3배 정도 높았고, 성분분석 F이온이 적은 Ca와 F의 원자비가 나타내었다. 이러한 결과로 F이온의 부족으로 인하여 형성된 violet 색상이 발생되었음을 간접적으로 알 수 있었다.

전반사 미러와 방향성 결합기를 이용한 직사각형 링 레이저 (Rectangular ring laser based on total internal reflection mirror and directional coupler)

  • 본 연구에서는 InP 물질의 능동과 수동 광도파로를 포함하는 신개념의 직사각형 링 레이저를 제작하여 그 특성을 측정하고 분석하였다. 직사각형 링 레이저의 구성은 작은 광 손실을 갖는 네 개의 전반사 미러와 세 개의 수동 광도파로로 구성된 방향성 결합기로 되어 있다. 제작된 두 개의 서로 다른 직사각형 링 공진기의 능동 영역의 길이는 250과 $350{\mu}m$이고, 전체 공진기 길이는 각각 580과 $780{\mu}m$이다. 측정된 링 레이저의 문턱 전류는 상온에서 연속 전류를 인가했을 때 38 mA를 얻을 수 있었다. 이때 20 dB 이상의 인접모드 억압비를 갖는 단일 모드 발진 특성을 확인하였다.

  • 최근 각 분야에서 연구 및 개발하고 있는 IT 및 자동화 경향이 건설 유지관리에도 전파됨에 따라 유지관리 로봇의 개발과 함께 개발될 비파괴 시스템의 장착이 각광 받을 것으로 예상되고 있다. 토목 구조물의 경우, 이러한 첨단기술의 발전과 더불어 유지관리 측면에서 이러한 첨단기술을 사용하는 것이 적극적으로 고려되고 있다. 그 중 NDT 비파괴 검사방법이 토목구조물의 효율적인 유지관리를 해 적합한 방법으로 사료되고 있다. 본 연구에서는 콘크리트 구조물의 내부결함 탐상을 위해서 초음파 및 레이더 장비가 적용되었으며, 이를 활용하여 실험실 및 현장실험을 통하여 그 적용성을 실험적으로 분석하였다. 실험결과, 초음파 및 레이더 장비는 콘크리트 구조물의 내부결함을 탐상하는데 효과적임으로 분석되었다. 또한 본 연구에서는 실험결과를 바탕으로, 초음파 및 레이더 장비를 적용한 콘크리트 구조물의 유지관리를 위한 알고리즘을 개발 및 제안하였다.
