Qualitative Research on Integrated Support Through Health, Medical and Welfare Network - Based on the Experience of 301 Network Service Users in Seoul Northern Municipal Hospital - (보건의료복지 네트워크를 통한 통합적 지원에 관한 질적 연구 - 서울특별시 북부병원 301네트워크 사업 이용자 경험을 중심으로 -)

  • The purpose of this study was to explore the implications of the integrated support through the 'health - medical - welfare' network and the specific context of the operation through the experiences of the users who participated in the 301 network project of Seoul northern municipal hospital. To do this, In-depth interviews were carried out with 10 research participants. The data was analyzed by general qualitative research methods. As a result of analysis, users lost their willingness because of living in a vicious cycle of poverty and illness before participating in 301 network services, but through the participation of the service, they were able to receive the integrated support of 'cure-care-life stability'. These experiences ultimately led to the regeneration of the will of their lives. At the basis of this experience were operating strategies and conditions such as the formation of a diverse professional team, the establishment of a linkage system within and outside the hospital, the establishment of a treatment linkage system through the acquisition of treatment subsidies, and the linkage of resources at mediation level. As the attempt to integrate 'health, medical and welfare' with well-coordinated strategies and conditions showed the possibility of complementing the limitation of the health welfare support system in Korea, the extension of the related business was suggested. For this, it suggested the more stable stabilization of the linkage system and the improvement of the institutional aspect.

  • Using a Consensus Qualitative Research approach, this study aimed to identify the ageism and explore age-integration as a solution of age discrimination that occurs during the delivery of medical services by nine healthcare professionals who have experience in treating elderly patients. There were two-sided confession has shown by health care professionals about the Ageism. They reported that they don't discriminate by age. However, They also appealed an inconvenience due to the elderly. There were real Ageism in the Healthcare Professional site as a way of Unsuitable care, elderly alienation and dependence on caregivers, polarization of medical service and double discrimination against poor elderly. They found it difficult to offer age integrated health care as a means to mitigate or solve. However they have sought to break barriers to communication, provided a comfortable environment not only for senior citizens but also for all others, and have diversified institutional and service standards. To ensure the healthy life and proper medical service of the rapidly increasing elderly patients, we proposed to do critical review of the factors in the Korean medical system that accelerate the Ageism, reorganization of the health care system for the poor elderly, including the curriculum associated with age-integrating within the health care professional education system, raising the Geriatric Medical Service and the relating professionals and improvements in perception of the health care domains for the elderly and older adults.

Case Study on the Application and Evaluation of an Integrated Medical Service Model to Improve the Quality of Life for Breast Cancer Patients and Caregivers (유방암 환자와 보호자의 삶의 질 증진을 위한 통합의료서비스모델 적용평가 사례 연구)

  • Purpose : This study aimed to evaluate the effectiveness of an integrative medical service model applied to breast cancer patients and their caregivers, exploring its feasibility and challenges within the context of South Korean healthcare system. Methods : A case study approach was chosen to assess the integrative medical service model's efficacy, involving one breast cancer patient and her primary caregiver from W University Hospital. The patient had completed reconstructive surgery and chemotherapy and was undergoing radiotherapy. The model included standard treatments alongside psychological counseling, aromatherapy, axillary rehabilitation exercise, make-up program, art therapy, laughter therapy, horticultural therapy, and yoga programs, and meditation programs delivered over eight weeks. Quantitative and qualitative data were collected through surveys, psychological tests, and feedback assessments. Results : The integrative medical service model demonstrated notable improvements in the quality of life for both breast cancer patients and their caregivers. Participants reported enhanced emotional well-being, reduced stress levels, and improved coping mechanisms throughout the treatment journey. Qualitative feedback highlighted the positive impact of holistic interventions in alleviating psychological distress and fostering resilience. Quantitative data corroborated these findings, showing statistically significant improvements in various psychosocial parameters assessed. Conclusions : Our findings underscore the benefits of integrative medical service model with standard medical treatments in the care of breast cancer patients and their caregivers. The holistic approach not only addresses physical symptoms but also enhances overall well-being and quality of life. However, the implementation of such models faces challenges within the South Korean healthcare system, including fragmented service networks and financial constraints. Addressing these structural barriers is crucial for the widespread adoption and sustainability of integrative care models in oncology practice. Future research should focus on larger-scale studies to further validate these findings and inform policy decisions aimed at optimizing cancer care delivery.

  • In recent years, two major streams in medical information systems are:1) system integration among OCS(Order Communication System), EMR(Electronic Medical Record), PACS(Picture Archiving and Communication System), and ERP(Enterprise Resource Planning) and 2) system integration through medical collaboration between East and West medical service providers. One of the characteristics which differentiate the Korean medical industry from the western medical industry is the East-West medical collaboration. In many respects there are many differences between East and West medical treatment. Although East and West medical treatment have developed from different medical philosophies and standards, we assume that the better medical care can be provided by integrating their medical procedures effectively. The two possible approaches to the integration of East and West medical information systems are suggested in this paper:One is loosely coupled model and the other is tightly coupled model. EMR improves the quality of medical record which reflects the quality of clinical practice. It provides more efficient and convenient way of input, retrieval, storage, communication and management of medical data. We abstracted the standard medical procedures from the two medical procedures performed in Daejeon Oriental Hospital and Hehwa Clinic at Daejeon University and also abstracted database schema by analyzing the characteristics of information needed in East-West medical collaboration. Our EMR is composed of two types of data:one is structured data and the other is unstructured data, which are formalized by SOAP(Subjective, Objective, Assessment, Plan) format. Currently the integrated system is implemented and operated successfully for six months.

Comparative Study on the Expertise, Convenience, and Interaction of Medical Treatment Systems Applied by Geriatric Hospital (요양병원 진료시스템에 따른 진료전문성, 편의성, 상호작용성의 역할 비교연구)

  • Medical treatment for the aged has become an increasingly important concern as the aged population continues to grow. However, little research has been done on the issue. This study examines the roles of the expertise, convenience, and interaction of medical treatment systems applied by geriatric hospital. Medical treatment systems include oriental medicine, western medicine and integrated(oriental and western) medicine practices. The empirical results of the study are as follows. First, the relative impacts of the expertise, convenience and interaction on customer satisfaction are different depending upon the medical treatment systems. Second, convenience and interaction are shown to be more important factors in western medicine, while interaction is the case in oriental medicine. As for the integrated medicine practices, expertise is considered more important. Thus, it is necessary for the western medicine to make more efforts on interaction and convenience, while it is critical to enhance medical personnels' service quality in the oriental medicine. In the integrated medical treatment, actual and effective cooperation should be achieved to secure expertise. Third, the data demonstrate that customer satisfaction has a positive influence on relationship quality which, in turn, impacts on repurchase intention. Finally, relationship quality has a negative influence on intention to switch in the western medicine, whereas the impact of relationship quality is not significant in the oriental and integrated medicine practices.

Review of the Domestic Trend of Pulse Diagnosis Studies in Korea (맥진 연구의 국내 동향에 대한 고찰)

  • Objectives: The aim of this study was to present several proposals for future pulse diagnosis practice and research by investigating the trend of pulse diagnosis studies in Korea. Methods: We searched online medical databases, including National Discovery for Science Leaders (NDSL), the Oriental Medicine Advanced Searching Integrated System (OASIS), the Research Information Sharing Service (RISS), and the Korean Traditional Knowledge Portal (KTKP), for pulse diagnosis articles in Korea. We selected articles on pulse diagnosis but excluded duplicate articles, articles irrelevant to pulse diagnosis, and articles published in foreign countries. Results: In the first screening, 801 articles were selected. We found 251 articles and classified them according to category. The medical engineering field had 148 articles. A total of 24 articles were related to algorithms for pulse wave detection, 34 to sensors, five to pressurization technology, 16 to systems, 11 to remote medical service, five to mobiles, nine to trends, and 44 to basic research. The Korean medicine field had 103 articles. A total of 41 articles were devoted to literature reviews, 20 to case reports, 11 to constitutional medicine, six to experimental studies, and 25 to relevant research. Conclusion: More efforts to practice pulse diagnosis for various diseases should be made and the results actively published.

  • High-quality customized services demand was growing due to the increase of aging, extremely nuclear family, disaster vulnerable of society. Ambulance service is required of fast and professional rescue and emergency service because of preventable death rate such as acute diseases, cerebral and cardiovascular diseases, suicides, etc. was higher than in developed countries. First aid will be available using patients information when emergency occurs, before arriving at the hospital. And emergency department is equipped with that patient care can be prepared in advance, increase the efficiency of emergency care. We received a variety of complex emergency call using high social awareness of 119 number and propose an efficient emergency medical service advancement strategy building an integrated response system with relevant organization.

What Else Is Needed in the Korean Government's Master Plan for People With Developmental Disabilities?

  • On September 12, 2018, President Jae-In Moon announced the Comprehensive Plan for Lifelong Care for People with Developmental Disabilities, with representatives from the associated government branches (Ministry of Health and Welfare, Ministry of Education, and Ministry of Employment and Labor) in attendance. The goals of this plan are to provide health, medical, rehabilitative, special education, and social welfare services according to the life-stages of the affected individuals; to reduce parental pressure; to promote social interventions; and to enhance community-level participation in order to create a 'welfare society in harmony.' However, in order for the plan to succeed, additional efforts must be made in the following areas. First, an epidemiological survey is needed to understand the scale, prevalence, and incidence of developmental disabilities and to establish an evidence base to support policy development. Second, accurate definitions of developmental disabilities must be established in order to avoid policy discrimination based on impairment type and age. Third, personal evaluations to assess disabled individuals' unmet needs and customized service designs to deliver those needs are required. Fourth, the plan must fulfill the goals of accessibility and fairness that the government intends to provide. Fifth, the government should consider an integrated financial support system and to propose a detailed plan for monetary distributions. Finally, an integrated system that links health, medical, employment, educational, and welfare services must be constructed.

  • An integrated medical information system that integrates systems consisting of different environments centered on hospital information systems should be provided as a system that prioritizes the improvement of the quality of medical services, customer satisfaction, and patient safety. The RBAC-based medical information system is granted the access right according to task type, role, and rules. Through this, it is possible to use SMS channel, medical reservation and cancellation, customized statistics, and CRM / EMR interworking service using multi-channel to enable communication service without help of counselor and reduce the default rate of reservation patient, Operational improvement services can be extended to medical staff, patients and their families, as well as expanding to important decisions for patients.

Digital Signature Considering the Medical Information Property on Mobile Environment (모바일 환경에서 의료 정보 특성을 고려한 디지털서명)

  • In the most of medical institution medical information is totally stored in a database and many number of researchers and staffs of the hospital access these information anytime. This can be caused patient's privacy to be violated. Introducing a tool for security should be considered as one of the most important requirement especially in the case that today's medical information service expands into an integrated one. In this paper we review the matters of security threat on a medical information system and propose a secure medical information service model equipped on mobile device such as PDA. Also we propose a security architecture employing a digital signature mechanism to protect the personal information on the model. Proposed architecture can lead the doctor to diagnose with high responsibility, help to build a reliable medical information system. and through the signed data, we can get some useful information against medical strife.
