Research on Factors Affecting Intention to Re-Use Hospitals for Patients Who Have Used Integrated Nursing and Care Services (간호·간병통합서비스를 이용한 입원환자의 병원재이용의도에 영향을 미치는 요인)

  • The purpose of this research was to investigate the effects of the communication ability, communication type and satisfaction of nursing service on the re-use intention of nursing and care integration services of the hospital. The research subjects were 291 patients who were hospitalized for more than 3 days in the integrated nursing and care service center. Data were analyzed using SPSS 21.0. As a result of the study, the variables that significantly affect the reuse of integrated nursing services in hospitals were satisfaction with nursing services and integrated services. The explanatory power of these variables was 63.4%. Therefore, to increase the intention to re-use integrated nursing and care services of the hospitals, efforts are needed to increase the satisfaction of nursing services. And research on other variables that affect the guarantee is highly recommended.

Impact of an Integrated Infection Control Education Program on Caregivers in Long-Term Care Facilities (장기요양시설 요양보호사를 위한 통합적 감염관리 교육프로그램의 효과)

  • Purpose: This study aimed to develop and assess the impact of an integrated infection control education program on the awareness, attitudes, and performance of infection control among caregivers in long-term care facilities. Methods: Participants were recruited from two long-term care facilities with 25 caregivers in both the experimental group and the control group. This study used non-equivalent control group quasi-experimental pre-post design. The effectiveness of a developed Integrated Infection Control Education Program was evaluated based on infection control awareness, attitudes, and performance. Data were analyzed using SPSS/WIN 28.0 through descriptive statistics, chi-square tests, Fisher's exact tests, and independent t-tests. Results: There was a statistically significant difference in the degree of awareness (t=-5.00, p<.001), attitude (t=-4.91, p<.001), and performance (t=-6.66, p<.001) of infection control between the two groups. Conclusion: Given these results, the integrated infection control education program significantly improved infection control awareness, attitudes, and performance among caregivers in long-term care facilities. This study is noteworthy because it provided comprehensive education on infection control practices to caregivers in environments that are particularly susceptible to infections, especially following COVID-19. This educational program is actively utilized and validated in practice, it would enhance the infection control performance of caregivers, thereby reducing infection rates within facilities, shortening the length of stay for elderly residents in long term care facilities, and contributing to the reduction of healthcare costs.

  • The purpose of this study was to develop, apply, and test the effectiveness of integrated palliative care empowerment program for the clinical nurses caring the cancer patients. Methods: The subjects were selected by a convenience sampling method from the nurses working at two university hospitals in D city. The study targeted a total of 37 nurses, 17 for the experimental group and 20 for the control group. The integrated palliative care empowerment program was implemented once a week for seven weeks to the experimental group from October 5th to November 11th in 2016, and it was analyzed by using the program IBM SPSS Statistics version 19.0 for Windows. Result and Conclusion: The study proved that the integrated palliative care empowerment program was effective on improving the integrated palliative care, empathy capacity, and ethical climate of the nurses. Based on the results of the study, it is necessary to develop an applicable program in collaboration of various health professions.

Policy Implications of the Long-term Care Insurance System Revitalization Process for Establishing Integrated Care System in Japan (일본의 지역포괄케어시스템 구축과 개호보험제도 개정과정의 정책적 시사점)

  • Recently, the Ministry of Health and Welfare has emphasized the necessity to build community care, and the interest of health care- long-term care-social welfare system continues. In order to examine the future vision of long-term care for elderly people, which can be a core system in establishing community models for elderly people in the future, we will implement aging in place as a national policy project. It is meaningful to analyze in depth the case of Japan's policy implementation through the amendment process of long-term care insurance system. The purpose of this study is to examine the concept definition, operating system, and major promotion process of the Integrated Care System in Japan and to suggest policy implications for the future Korea long-term care insurance system. As a result of reviewing the operation status and revision process of the long-term care insurance system for establishing the Integrated care system in Japan, it is necessary to clarify the basic principles and construction of community care system, diversification of residence type and upgrading work, establishment of linkage network of health care-long term care-social welfare, expansion of support for family and self-mind.

  • This study attempts to provide implications for developing more efficient and effective community-based support system with AIP perspective for the elderly in Korea. The main purpose of this study is to analyze Japan's community-based integrated care system that respond to the concept of aging in place (AIP) and its cases. In Japan, they have offered Community-Based Service since 2005, and the advanced system which include integrated support categories and consolidated community/local resources will started in 2015 by The revision of Long-term Care Insurance policy, 2012. The result of policy analysis and case studies are as follows: 1) The suggestion for ideal support system model promoted a relationship of multiple agents include private sectors even resident and senior and specified responsibility sharing, 2) the system proposed Not only health and medical care support, living care and residence are also addressed as a comprehensive support. and 3) the amount of available community resource is different by each local government, but the effort to get the understanding of community residents and to connect with a potential community resource is also essential aspect to set effective community-based support system.

The Effects of Integrated Intervention Program for Community Dwelling Cancer Patients' Quality of Life, Depression and Self Care Agency (통합중재프로그램이 지역사회 재가 암환자들의 삶의 질, 우울, 자가간호역량에 미치는 효과)

  • Purpose: This study was performed to identify the effects of an integrated nursing intervention program on the quality of life, depression and self care agency of community dwelling cancer patients. Methods: The design of this study was a quasi-experimental, one group pre-post test. A total of 25 community dwelling cancer patients in J city were included in this study. An integration intervention was 30 times, 10 weeks program. The data were collected between May and November of 2014. The collected data were analyzed using SPSS 18.0 through paired t-test. Results: There were significant differences in the quality of life (p=.007) and self-care agency (p=.042) after 10 week's intervention. Conclusion: The results indicate that an integrated intervention program including educational, cognitive, emotional and physical intervention is effective for community dwelling cancer patients. Further attention should be paid to the intensity and period of an integrated intervention program.

Analysis of the Effect of a Resident-tailored Integrated health Promotion Program based on the Primary Health Care Post (보건진료소 중심의 주민 맞춤형 통합건강증진프로그램의 효과분석)

  • Purpose: The purpose of this study was to examine the effects of an integrated health promotion program provided by one community health post by comparing this program with more traditional health promotion programs provided by other institutions. Methods: The participants in this study were 110 selected local residents from I city. Of them, 55 residents participated in the integrated health promotion program in the community health post, and the other 55 participated in a program at another institution. SPSS 21.0 was used for descriptive analysis. Result: Scores for program effects, satisfaction level, quality of life related to health and depression were higher for the integrated health promotion program offered by the community health post compared to health promotion programs of the other institution. The differences were statistically significant. Conclusion: The findings of the study indicate that the integrated health promotion program can be helpful for community health posts in being successful in future plans to meet the needs of residents. Use of this program will also contribute to the development of community health posts but sustained research efforts need to be channeled into these programs.

Development and Effects of Integrated Simulation Program (Maternal-Child) for Nursing Students (간호학생을 대상으로 한 통합(모성-아동) 시뮬레이션 프로그램 개발 및 효과)

  • Purpose: This study was done to analyze the effectiveness of simulation-based integrated practice program (maternal-child) on nursing knowledge, self-confidence, and clinical competence of nursing students. Methods: A nonequivalent control group pre-post experimental design was used to compare experimental and control group. The experimental group received the integrated simulation practice and the control group received a separate simulation for maternal care and for newborn care. Results: The experimental group who had the integrated simulation had significantly higher scores for self-efficacy on nursing handover (F=0.480 p=.012) and oxygen therapy in newborn care (F=3.262 p=.037), and for clinical competence (F=2.639, p<.001) and personal satisfaction with debriefing compared to the control group (F=2.179, p=.044). But the experimental group did not have significantly higher scores in nursing knowledge. Conclusions The results indicate that an integrated simulation practice is an effective practice method to improve self-confidence, clinical competence and satisfaction. Also this study had significance in providing a setting similar to the clinical situation.

Cardiopulmonary Resuscitation: New Concept

  • Cardiopulmonary resuscitation (CPR) is a series of life-saving actions that improve the chances of survival, following cardiac arrest. Successful resuscitation, following cardiac arrest, requires an integrated set of coordinated actions represented by the links in the Chain of Survival. The links include the following: immediate recognition of cardiac arrest and activation of the emergency response system, early CPR with an emphasis on chest compressions, rapid defibrillation, effective advanced life support, and integrated post-cardiac arrest care. The newest development in the CPR guideline is a change in the basic life support sequence of steps from "A-B-C" (Airway, Breathing, Chest compressions) to "C-A-B" (Chest compressions, Airway, Breathing) for adults. Also, "Hands-Only (compression only) CPR" is emphasized for the untrained lay rescuer. On the basis of the strength of the available evidence, there was unanimous support for continuous emphasis on high-quality CPR with compressions of adequate rate and depth, which allows for complete chest recoil, minimizing interruptions in chest compressions and avoiding excessive ventilation. High-quality CPR is the cornerstone of a system of care that can optimize outcomes beyond return of spontaneous circulation (ROSC). There is an increased emphasis on physiologic monitoring to optimize CPR quality, and to detect ROSC. A comprehensive, structured, integrated, multidisciplinary system of care should be implemented in a consistent manner for the treatment of post-cardiac arrest care patients. The return to a prior quality and functional state of health is the ultimate goal of a resuscitation system of care.

Influences of Nurses' Empathy and Self-efficacy on Nursing Care of Older Adults in an Integrated Nursing Care Services (INCS) Unit (간호·간병통합서비스 병동 간호사의 공감력과 자기효능감이 노인간호수행에 미치는 영향)

  • Purpose: The aim of this study was to investigate the influences of nurses' empathy and self-efficacy on nursing care of older adults in an integrated nursing care services (INCS) unit. Methods: The participants were 210 nurses caring for elderly patients in the INCS unit in Korea. Data were collected using self-report questionnaires from February 9 to February 23, 2017. The questionnaires are composed of empathy construct rating scale, self-efficacy scale and nursing care of older adults scale. Results: The mean age of the participants was 29.9 years old. There were significant positive relationships between age and clinical career (r=.78; p<.001), self-efficacy and empathy (r=.33; p<.001) and empathy and nursing care of older adults (r=.25; p<.001). The quality of nursing care of older adults were significantly different according to working experience in psychiatric unit (p=.021). Influencing factor of nursing care of older adults was empathy (${\ss}=.29$; p<.001), which explained 30% of the variance. Conclusion: The findings of this study indicated that attention should be given to empathy of nursing care of older adults and numerous efforts should be made to improve nurses' empathy for quality elderly care.
