End-to-End Anastomosis of an Unanticipated Vertebral Artery Injury during C2 Pedicle Screwing

  • Vertebral artery (VA) injury is a rare and serious complication of cervical spine surgery; this is due to difficulty in controlling hemorrhage, which can result in severe hypotension and cardiac arrest, and uncertain neurologic consequences. The authors report an extremely rare case of a 56-year-old woman who underwent direct surgical repair by end-to-end anatomosis of an unanticipated VA injury during C2 pedicle screwing. Postoperatively, the patient showed no neurological deterioration and computed tomography angiography of the VA demonstrated normal blood flow. Although direct occlusion of an injured VA by surgical ligation or endovascular embolization has been used for management of an unanticipated VA injury during surgery, these methods may be associated with significant morbidity and mortality. However, despite its technical demand, microvascular primary repair can restore normal blood flow and minimizes the risk of immediate or delayed ischemic complications. Here we report an iatrogenic VA injury during C2 pedicle screwing, which was successfully treated by end-to-end anastomosis.

  • The clinical features and therapeutic results of alopecia areata are variable and unpredictable. For example, genetic, psychic, immunologic factors are regarded as the reason of alopecia areata. For the relationship between alopecia and whiplash injury, Dr. Guun explained that whiplash injury by the traffic accident produces cervical muscle spasm, and it makes autonomic nerve change. The tropical changes accompanied with ischemic change of scalp vessels made by this mechanism cause alopecia areata. And Yesudian reported the case of scalp alopecia as the result of ischemic change following traffic accident. We have experienced a 25-year-old woman with Alopecia areata following whiplash injury by traffic accident, and who had no risk factor of it. The patient was treated by acupuncture and physical treatment. Her hair loss, cervical angle and pain were improved through acupuncture treatment. This case of alopecia areata following whiplash injury is uncommon, so we report the mechanism of it, but should collect more cases and observations.

  • A questionnaire survey wds conducted to assess the experience of the needle stick injury in 144 seniors of a medical school during the previous 10 months. One hundred and five of them (73%) had responded. About sixty-nine percent (72 cases) of the respondents had suffered at least one u and there had been 129 injuries in total. Seventy-eight (56.6%) of the injuries took place at the time of the venipuncture. The emergency and operating room incurred 76.7% and 23.3% of the injury, respectively. Injuries with bleeding account for 50.4%. None of the respondents was systematically educated about the prevention of injury. This survey shows that the needle-stick Injury is a great risk for the medical students' health, and that educational effort for its prevention is warranted.

Effect of gemigliptin on cardiac ischemia/reperfusion and spontaneous hypertensive rat models

  • Diabetes is associated with an increased risk of cardiovascular complications. Dipeptidyl peptidase-4 (DPP-IV) inhibitors are used clinically to reduce high blood glucose levels as an antidiabetic agent. However, the effect of the DPP-IV inhibitor gemigliptin on ischemia/reperfusion (I/R)-induced myocardial injury and hypertension is unknown. In this study, we assessed the effects and mechanisms of gemigliptin in rat models of myocardial I/R injury and spontaneous hypertension. Gemigliptin (20 and 100 mg/kg/d) or vehicle was administered intragastrically to Sprague-Dawley rats for 4 weeks before induction of I/R injury. Gemigliptin exerted a preventive effect on I/R injury by improving hemodynamic function and reducing infarct size compared to the vehicle control group. Moreover, administration of gemigliptin (0.03% and 0.15%) powder in food for 4 weeks reversed hypertrophy and improved diastolic function in spontaneously hypertensive rats. We report here a novel effect of the gemigliptin on I/R injury and hypertension.

Blunt abdominal trauma resulting in pancreatic injury in a pediatric patient in Australia: a case report

  • Pancreatic trauma from a blunt injury is fairly uncommon in the pediatric population. Furthermore, such trauma with associated disruption of the pancreatic duct (PD) is even less prevalent and is associated with high morbidity and mortality. Pancreatic injuries in the pediatric population are often missed and hence require a thorough workup in children presenting with any form of abdominal injury. This case report describes a young boy who presented with abdominal pain and did not initially inform medical staff about any injury. For this reason, his initial provisional diagnosis was appendicitis, but he was later found to have transection of the pancreas with injury to the PD on imaging. The management of such injuries in pediatric patients often poses a challenge due to a lack of pediatric physicians trained to perform interventions such as endoscopic retrograde cholangiopancreatography. Furthermore, such interventions carry a higher risk when performed on children due to the smaller size of their pancreatic ducts. As a result, our patient had to be transferred to an adult center to undergo this procedure. Thus, maintaining a high degree of suspicion, along with a detailed history and examination, is crucial for the early diagnosis and management of pancreatic injuries.

  • Soccer is type of sport that carries a high risk of injury. Injury is not only cause in the unlucky soccer carrier and also team performance as well as financial effects can be worse since soccer is a team-based game. The duration of recovery from a soccer injury typically relies on its type and severity. Therefore, we conduct this research in order to predict the probability of players injury type using machine learning technologies in this paper. Furthermore, we compare different machine learning models to find the best fit model. This paper utilizes various supervised classification machine learning models, including Decision Tree, Random Forest, K-Nearest Neighbors (KNN), and Naive Bayes. Moreover, based on our finding the KNN and Decision models achieved the highest accuracy rates at 70%, surpassing other models. The Random Forest model followed closely with an accuracy score of 62%. Among the evaluated models, the Naive Bayes model demonstrated the lowest accuracy at 56%. We gathered information about 54 professional soccer players who are playing in the top five European leagues based on their career history.   • The objective of this study was to find the factors affecting the results of full frontal barrier impact test for the NCAP (New Car Assessment Program). To find the factors, the frontal NCAP test results of the NHTSA (National Highway Traffic Safety Administration) were utilized. The three tested vehicle were same model year. It was observed the second peak value of barrier force affected the occupant injury risk. As the second peak value of the barrier force increases, the injury risk of the driver side occupant increases as well.

  • 많은 고령의 선수들이 골프 운동에 참여하고 있으며, 견관절, 특히 비우세 상지나 Swing을 선도하는 상지, 이 과격한 골프 동작으로 인하여 상대적으로 손상을 받을 위험이 매우 크다. 골프 손상의 위험을 감소시키고 방지하며 경기력 향상을 위해서는 골프 swing에 따른 생역학을 이해하고, 정기적으로 유연성을 기르고, 신장운동 및 근육 강화운동을 시행하여야 한다.

석면 해체제거 보양작업에서 JSA 및 Checklist 적용 (Application of JSA and Checklist in Asbestos Sealing)

  • As asbestos-containing buildings are getting older, asbestos deconstruction works are increasing. As a result, accident risks such as falls, cuts, electric shocks, and suffocation are increasing. Existing studies are mostly about health management and institutional policy research and there is little research on work risk. So workplace risk assessments that are easily applicable in the field are required to be applied. Sealing is the first process of asbestos deconstruction and is the first step to ensure worker's safety. Job Safety Analysis(JSA) and Checklist were used to identify the risk factors and to calculate the level of the risk. By comparing the two risk assessment tools, it was figured out that the JSA is appropriate for the initial process and change of work procedure while Checklist is appropriate for repetitive work. Because the sealing process is sort and simple, it is unlikely to cause serious injury. But since the risk of falling and cuts are exist, safety education and supervision are necessary to maintain a safe working environment.

Analysis of the risk factors of acute kidney injury after total hip or knee replacement surgery

  • Background: Postoperative acute kidney injury (AKI), which increases the risk of postoperative morbidity and mortality, poses a major concern to surgeons. We conducted this study to analyze the risk factors associated with the occurrence of AKI after orthopedic surgery. Methods: This was a retrospective study that included 351 patients who underwent total hip or knee replacement surgery at Inje University Haeundae Paik Hospital between January 2012 and December 2016. Results: AKI occurred in 13 (3.7%) of the 351 patients. The patients' preoperative estimated glomerular filtration rate (eGFR) was 66.66 ±34.02 mL/min/1.73 m2 in the AKI group and 78.07±21.23 mL/min/1.73 m2 in the non-AKI group. The hemoglobin levels were 11.21±1.65 g/dL in the AKI group and 12.39±1.52 g/dL in the non-AKI group. Hemoglobin level was related to increased risk of AKI (odds ratio [OR], 0.13; 95% confidence interval [CI], 0.02-0.68; p=0.016). Administration of crystalloid or colloid fluid alone and the perioperative amount of fluid did not show any significant relationship with AKI. Further analysis of the changes in eGFR was performed using a cutoff value of 7.54. The changes in eGFR were significantly related to decreased risk of AKI (OR, 0.74; 95% CI, 0.61-0.89; p=0.002). Conclusion: Renal function should be monitored closely after orthopedic surgery if patients have chronic kidney disease and low hemoglobin level. Predicting the likelihood of AKI occurrence, early treatment of high-risk patients, and monitoring perioperative laboratory test results, including eGFR, will help improve patient prognosis.
