A Study on Improvement of Emergency Medical System for forestry accident (임업 사고 응급대응체계의 개선방안에 관한 연구)

  • The estimated on-site accident rate in Forestry is relatively high. According to statistics of the accident, in the recent 5 years, from 2014 to 2018, forestry accidents have resulted in 98% of injuries and 87% of fatalities. Especially, there are significant geographical constraints to access to the scene in case of an accident. Even though the capacity of first aid capacity is notably emphasized its importance to minimize the scale of damages, the relevant employees have been educated only basic first aid, which is not considered circumstances or geographic limitations, by Occupation Safety and Health Acts. Therefore, the purpose of this study is to derive a direction for a forest emergency service system to increase forestry workers' survival and prevent secondary injury through securing 'Golden Time.' This study conducts analyzing relevant laws and regulations in domestic and international settings as well as looking at several concerned accident cases. The outcome of analysis presents an issue regarding the implementation of onsite first aid in forestry and existing risk factors depending on the working process. Finally, we suggest two ways to improve the forest emergency service that are 1) an appropriate curriculum and kit for forest first aid; and 2) a system for emergency transfer through sharing information between National Fire Agency (NFA) and emergency medical service center, and emergency and rescue mission using helicopter from NFA and Korea Forest Service.

An Analysis of Car Seat Use from 2015 to 2019 among Children Ages under 6: Using Korea National Health and Nutrition Examination (최근 5년간 6세 미만 아동 카시트 사용 분석: 국민건강영양조사 자료(2015~2019년)를 바탕으로)

  • Purpose: Traffic accidents are the major cause of death in children. Car seat usage in children is important to reduce the risk of death or injury caused by traffic accidents. This study aims to analyze the trends of car seat use among children ages under 6. Methods: This study adopted a cross-sectional study utilizing the Korea National Health and Nutrition Examination Survey from 2015 to 2019. In total, 1,999 responses were included for the analysis. Factors associated with car seat use were age, sex, body weight, height, family type, household income, and monthly household income. Regression analysis was used to compare the car seat use before and after the revision of the Road Traffic Act (2015~2018 vs. 2019). Results: We found that car seat use was increasing by year, age, region, and household income. Car seat use was higher after fines increased in 2019. Age and body weight had negative association with car seat use, while height and household income had positive association with car seat use. Conclusion: Car seat use increased from 2015 to 2019 and is associated with different ages among children and household income. Further education and promotion that describes the safety benefits to children from car seat use are needed.

The effect of Rigid Tape in Conjunction with Elastic Tape and Elastic tape only on Shoulder Pain, Proprioceptor and Function of Chronic Stroke Patients with Scapular Dyskinesis (탄력 및 비탄력과 탄력을 결합한 테이프 적용이 견갑골 운동이상증을 가진 만성 뇌졸중 환자의 어깨 통증과 고유수용감각, 상지 기능 수준에 미치는 영향)

  • Background: Scapular dyskinesis is one of the risk factors for upper extremity injury in patients with chronic stroke. Taping can used as an adjunctive treatment for this. The aim of this study was to investigate the effects of rigid tape in conjunction with elastic tape and elastic tape only on shoulder pain, proprioceptor, and function of chronic stroke patients with scapular dyskinesis. Methods: Participants were randomly assigned to elastic tape (ET) group (n=10) and both tape (BT) group (n=10). Both ET and BT groups performed therapeutic exercise on the upper extremity with taping applied for 4 weeks. Therapeutic exercise was performed 30min/day, 5 times/week. Clinical outcome measures used Numerical pain rating scale with a faces pain scale for pain, angle at shoulder join position sense (JPS) for proprioceptor, and Fugl-meyer assessment upper extremity for function. Results: Clinical outcomes were measured at initial baseline, after 4weeks. There were statistically significant differences in pain, JPS, and function after 4 weeks in both groups (p<.05). There was no significant difference in pain and JPS between the groups, and There was a statistically significant difference in function in the BT group compared to the ET group (p<.05). Conclusions: This results suggest that accurate clinical assessment and appropriate taping can improve proprioceptor and function of the upper extremity in patients with chronic stroke.

A Comparative Study on the Characteristics of Performance-related Musculoskeletal Symptoms between Orchestra Performers and University Student Performers (관현악단 전문 연주자와 대학생 연주자의 연주 관련 근골격계 증상의 발생 특성에 관한 비교)

  • Background: Instrument players are exposed to occupational musculoskeletal symptoms due to constant movement and strain to meet the needs of their audience. This can cause dissonance between the player and the instrument and can cause medical problems in the musculoskeletal system such as pain, tendinitis, muscle spasms, and joint hyperextension from overuse of body parts. Objective: The purpose of this study is to comparatively analyze the characteristics and occurrence of musculoskeletal symptoms in professional and student players, and to find out the awareness of the need for health care professionals and special exercise programs for orchestra players. Methods: The subjects were professional orchestral musicians 191 from the three symphony orchestras and music students 209 from the four universities volunteered to participate in this study; 393 subjects (98.25%) completed the questionnaire. symptom prevalence and related factors of playing-related musculoskeletal disorders (PRMDs) were surveyed with a self-administered questionnaire. Standardized Nordic Questionnaire was complemented by adding a visual-among scale and used to diagnose PRMD. Results: High prevalence of musculoskeletal complaints in both professional musicians (85.30%) and student musicians (82.30%). Professional musicians and student musicians also suffered the highest problem in shoulder and neck complaints. Significant differences were found between groups that recognized of necessity for specialized health professionals, professional organizations, and special programs for musicians. Conclusions: Professional musicians and student musicians, like orchestra players, are already exposed to overuse syndrome. However, student musicians lack awareness of the risk of injury compared to orchestra players. We suggest that an appropriate treatment management program for the prevention and early treatment of musculoskeletal system damage should be provided to performers at an early stage.

Effects of Inhalation versus Total Intravenous Anesthesia on Postoperative Pulmonary Complications after Anatomic Pulmonary Resection

  • Background: No consensus exists regarding whether volatile anesthetics are superior to intravenous anesthetics for reducing postoperative pulmonary complications (PPCs) in patients undergoing general anesthesia for surgery. Studies of this issue focused on anatomic pulmonary resection are lacking. This study compared the effects of total intravenous anesthesia (TIVA) versus volatile anesthesia on PPCs after anatomic pulmonary resection in patients with lung cancer. Methods: This retrospective study examined the medical records of patients with lung cancer who underwent lung resection at our center between January 2018 and October 2020. The primary outcome was the incidence of PPCs, which included prolonged air leak, pneumonia, acute respiratory distress syndrome, empyema, atelectasis requiring bronchofiberscopy (BFS), acute lung injury (ALI), bronchopleural fistula (BPF), pulmonary embolism, and pulmonary edema. Propensity score matching (PSM) was used to balance the 2 groups. In total, 579 anatomic pulmonary resection cases were included in the final analysis. Results: The analysis showed no statistically significant difference between the volatile anesthesia and TIVA groups in terms of PPCs, except for prolonged air leak. Neither of the groups showed atelectasis requiring BFS, ALI, BPF, pulmonary embolism, or pulmonary edema after PSM. However, the length of hospitalization, intensive care unit stay, and duration of chest tube indwelling were shorter in the TIVA group. Conclusion: Volatile anesthetics showed no superiority compared to TIVA in terms of PPCs after anatomical pulmonary resection in patients with lung cancer. Considering the advantages of each anesthetic modality, appropriate anesthetic modalities should be used in patients with different risk factors and situations.

Novel Three-Dimensional Knitted Fabric for Pressure Ulcer Prevention: Preliminary Clinical Application and Testing in a Diabetic Mouse Model of Pressure Ulcers

  • Background Population aging has led to an increased incidence of pressure ulcers, resulting in a social burden and economic costs. We developed a three-dimensional knitted fabric (3-DKF) with a pressure-reducing function that can be applied topically in the early stages of pressure ulcers to prevent progression. Methods We evaluated the effects of the 3-DKF in a streptozotocin-induced diabetes mellitus pressure ulcer mouse model, and the fabric was preliminarily applied to patients. Twelve-week-old male C57BL/6 mice were used for the animal experiments. In the pressure ulcer mouse model, an ischemia-reperfusion injury was created using a magnet on the dorsa of the mice. Pressure was measured with BodiTrak before and after applying the 3-DKF to 14 patients at risk of sacral pressure ulcers. Results In the 3-DKF-applied mice group, the ulcers were shallower and smaller than those in the control group. Compared with the mice in the control group, the 3-DKF group had lower platelet-derived growth factor-α and neutrophil elastase expression, as parameters related to inflammation, and increased levels of transforming growth factor (TGF)-β1, TGF-β3, proliferating cell nuclear antigen, and α-smooth muscle actin, which are related to growth factors and proliferation. Additionally, typical normal tissue staining patterns were observed in the 3-DKF group. In the preliminary clinical analysis, the average skin pressure was 26.2 mm Hg before applying the 3-DKF, but it decreased to an average of 23.4 mm Hg after 3-DKF application. Conclusion This study demonstrated that the newly developed 3-DKF was effective in preventing pressure ulcers through testing in a pressure ulcer animal model and preliminary clinical application.

Bone Health Evaluations and Secondary Fragility Fractures in Hip Fracture Patients

  • Purpose: This study sought to examine the utilization of bone health evaluations in geriatric hip fracture patients and identify risk factors for the development of future fragility fractures. Materials and Methods: A consecutive series of patients ≥55 years who underwent surgical management of a hip fracture between September 2015 and July 2019 were identified. Chart review was performed to evaluate post-injury follow-up, performance of a bone health evaluation, and use of osteoporosis-related diagnostic and pharmacologic treatment. Results: A total of 832 patients were included. The mean age of the patients was 81.2±9.9 years. Approximately 21% of patients underwent a comprehensive bone health evaluation. Of this cohort, 64.7% were started on pharmacologic therapy, and 73 patients underwent bone mineral density testing. Following discharge from the hospital, 70.3% of the patients followed-up on an outpatient basis with 95.7% seeing orthopedic surgery for post-fracture care. Overall, 102 patients (12.3%) sustained additional fragility fractures within two years, and 31 of these patients (3.7%) sustained a second hip fracture. There was no difference in the rate of second hip fractures or other additional fragility fractures based on the use of osteoporosis medications. Conclusion: Management of osteoporosis in geriatric hip fracture patients could be improved. Outpatient follow-up post-hip fracture is almost 70%, yet a minority of patients were started on osteoporosis medications and many sustained additional fragility fractures. The findings of this study indicate that orthopedic surgeons have an opportunity to lead the charge in treatment of osteoporosis in the post-fracture setting.

The Clinical Nurses' Knowledge, Attitudes, and Performance on Incontinence Associated Dermatitis (임상간호사의 실금 관련피부염에 대한 지식, 태도 및 수행 정도)

  • Purpose: Incontinence-associated dermatitis (IAD) is widespread and a common problem in healthcare settings, It is defined as irritation of the perineal skin due to contact with urine, feces, or both. It is one of the major risk factors for the development of pressure injuries are regarded as a quality indicator of nursing care. It is important to understand the knowledge and attitudes of nurses who are primarily responsible for providing appropriate nursing care that includes differentiation of IAD from other skin breakdowns, and appropriate management. Therefore, the purpose of this study was to determine the knowledge, attitudes, and performance of IAD among clinical nurses in Korea. Methods: Data about IAD knowledge, attitudes, and performance were collected from August to November 2022. 656 nurses from six hospitals participated in the study. Results: The mean scores of nurses' knowledge, attitudes, and performance were 17.98±2.91, 76.34±7.85, and 91.53±16.18, respectively. Nurses' knowledge showed a positive correlation in both attitudes (r=0.27, p<.001) and performance (r=0.09, p=.02), and attitudes and performance (r=0.34, p<.001) also showed a positive correlation. Conclusion: To improve the knowledge and performance of nurses, an educational program consisting only of incontinence-related dermatitis is needed, and case-based education and clinical skills education are needed to enhance the ability to differentiate from pressure injury.

Correlation Between the Soleus and Quadriceps Femoris Muscles During Squat Exercises on Various Support Surface in Healthy Adult Males (다양한 지지면에서 스쿼트 운동 중 건강한 성인 남성의 가자미근과 넙다리네갈래근의 상관관계)

  • Background: Squats are well-known for their effectiveness in strengthening the quadriceps femoris muscle. Flexibility and stability of the ankle joint are critical factors in performing squats correctly. Ankle instability can lead to injuries owing to compensatory actions. The role of the soleus muscle in maintaining ankle balance is crucial to minimize the risk of injury. Moreover, squats on unstable surfaces have gained attention for their potential to enhance balance and stability, which in turn helps prevent injuries. Therefore, this study aims to investigate the correlation between the soleus and quadriceps femoris muscles during squat exercises on various support surfaces. Design: Simple regression analysis Methods: Participants performed three different types of squats. Prior to commencing the exercises, we conducted electromyography (EMG) measurements to assess the activity of both the soleus and quadriceps femoris muscles. The order of the exercise execution was randomized. The squats were performed in a Full Squat format, and a one-minute rest was provided between each exercise type. Results: Our analysis of the results revealed significant differences in the activation levels of the soleus and quadriceps femoris across different squat types (p<.05). The regression analysis revealed an inverse relationship between the activity levels of the soleus and quadriceps femoris. Specifically, the quadriceps femoris exhibited the highest activation during squats using a wedge, while its activation was lowest during squats using the TOGU. In contrast, the soleus muscle demonstrated its highest activation during squats using the TOGU. Conclusion: Compared to traditional floor squats, the disparities in soleus and quadriceps femoris muscle activation observed during Wedge or TOGU squats can be attributed to the influence of ankle stability and balance. As the surface instability increased during squats, the soleus muscle's activity increased while the activation of the quadriceps femoris muscle decreased. These findings suggest that heightened soleus activation on unstable surfaces can significantly impact the activation of the quadriceps femoris muscle during squat exercises.

Risk Factors Affecting the Injury Severity of Rental Car Accidents in South Korea : an Application of Ordered Probit Model (순서형 프로빗 모형을 이용한 렌터카 사고 심각도 영향요인 분석)

  • Over the past five years (2010-2014), the total number of traffic accidents has decreased from 226,878 to 223,552 with decrease of 0.37 percent each year. The death toll has also decreased from 5,505 to 4,762. However, the number of rental car accidents and fatalities has been steadily increased. Despite of its growth, no previous study has been conducted on rental car accident severity. This study analyzed data of 18,050 rental car accidents in South Korea collected from 2010 to 2014 and then processed in order to identify which factors could affect the accident severity. Seventeen factors related to rental car accident severity were grouped into four categories: driver, vehicle, roadways and environment. As a result of the ordered probit model analysis, fourteen variables excluding age, intersection, and day of week were found to affect the severity of rental car accidents. The results of the study summarized as follows. First of all, violation of traffic regulations such as speeding increase the severity of rental car accidents. Secondly, rental accident severity is higher at curved sections of complicated roadway, which the driver's field of view is impaired. The results of this study can be used to reduce the severity of rental car accidents in transportation safety.
