가정 호스피스케어환자 방문간호 조사분석 (A Study on Home Visiting Hospice Care of the Terminally Ill Patients)

  • 목적 : 오늘날 호스피스 운동은 만성질환자가 급격히 증가함에 따라 인간의 삶을 추구하여 이루어낸 가장 완벽한 대답의 하나가 되고 있다. 우리의 1996년, 1997년의 연구에서 말기환자와 그 가족들에 대한 조사를 통해 한국 호스피스의 현재 상황을 파악하였다. 또한 호스피스케어 팀과 자원봉사자와 호스피스환자 관리를 위한 정보서비스 시스템을 개발하였다. 본 연구는 호스피스 정보서비스 시스템를 통한 간호사의 가정방문 호스피스케어와 그 문제점을 분석하였다. 방법 : 1997년 10월 1일부터 1998년 3월 31일까지 서울대학교병원 등에서 의뢰받은 26명의 말기암환자를 대상으로 하였다. 데이터 베이스와 홈페이지를 통해 호스피스케어에 필요한 정보를 갖춘 호스피스 정보서비스 시스템을 인터넷을 통해 방문간호사에게 제공하였고 방문간호사들은 이 시스템을 교육받고 환자 방문시 노트북 컴퓨터를 소지하여 호스피스 정보서비스 시스템을 통해 호스피스케어를 제공하였으며 첫 방문시와 호스피스케어동안에 환자들의 신체적 심리적 사회적 자료를 수집하였다. 결과 : 연구기간동안 26명중 16명이 사망하였으며 사망자의 평균 생존기간은 20.7일이었다. 첫 방문시 식욕부진(96.2%), 거동장애(88.5%), 통증(84.6%)이 주요한 증상이었으며 226개의 간호진단 중 영양부족과 통증이 가장 흔한 진단이었다. 가족은 환자보다 호스피스케어를 더 잘 이해하고 더 요구하였다. 대부분의 환자와 가족들은 간호사에게 영적 사회적 간호를 요구하지 않았다. 결론 : 시범사업을 통해 환자와 방문 호스피스 간호사에게 통증과 영양 관리에 대한 보다 많은 정보를 제공하여야 하여야 함을 알 수 있었다. 호스피스 정보서비스 시스템은 영적 사회적 케어에 대한 정보와 인력을 보안되어질 필요가 있다.

소비자의 의약품안전성 인식정도 및 관련정보 획득경로 (Public Perception and Routes of Acquiring Information on Drug Safety)

  • Misinformation and inappropriate use of medication has become one of the most pressing concerns in drug safety. The purpose of this study was to survey public perception on drug safety as well as the channels most relied upon providing such information. The survey was performed for patients or their families visiting pharmacies in a local city in Korea. Analysis was performed from 367 respondents to the survey. The contents of this survey revealed that consumers were aware of the fact that medications should not be taken at any higher dosage or more often than directed by their prescriptions. The survey revealed a general awareness that symptoms might not be relieved immediately by their medications. However, the perception that there could be adverse drug reaction (ADR) at therapeutic dose was low except among the young or highly educated members. Respondents recognized that skin rashes were the most whereas drowsiness was the least common ADR symptom. There was a high awareness of drug-food or drug-drug interactions except in the case of certain nutraceuticals. Doctors and pharmacists were ranked as the most reliable resources to the consumer for providing drug related information. However, public relations or education programs were in need since there were still not negligible numbers of consumers depending on personal experience rather than health professionals.

집단지성의 품질, 그 결정요인, 유용성의 관계: 수용자 관점에서 한국의 위키서비스와 Q&A 서비스의 비교 (Relationships between Collective Intelligence Quality, Its Determinants, and Usefulness: A Comparative Study between Wiki Service and Q&A Service in Perspective of Korean Users)

  • Innovation can come from inside or outside organizations. Recently, organizations have begun turning to external knowledge more often, through various forms of collective intelligence (CI) as collaborative platform to solve complex problems. Several factors facilitate this CI utilization phenomenon. First, with the rapid development of Internet and social media, numerous web applications have become available to millions of the Internet users over the past few decades. Web 2.0 and social media have become innovative web applications that provide an environment for human social interaction and collaboration. Second, the diffusion of simple and easy-to-use technologies that enable users to interact and design web applications without programming skills have led to vast, previously unknown amounts of user-generated content. Finally, the Internet has enabled communities to connect and collaborate, creating a virtual world of CI. In this study, web enabled CI is defined as a composed ability of individuals who are acting as a single cognitive unit to achieve common goals, think reasonably, solve problems, make decisions, carry out complex tasks, and develop creative ideas collectively through participation and collaboration on the web. Although CI plays a critical role in organizational innovation and collaboration, the dubious quality of CI is still problem that is difficult to solve. In general, the quality level of content collected from the crowd is lower than that from professionals. Thus, it is important to identify determinants of CI quality and to analyze the relationship between CI quality and its usefulness. However, there is a lack of empirical study on the quality factors of web-enabled CI. There exist a variety of web enabled CI sites such as Threadless, iStockphoto or InnoCentive, Wikipedia, and Youtube. One of the most successful forms of web-enabled CI is the Wikipedia online encyclopedia, accessible all over the world. Another one example is Naver KnowledgeiN, a typical and popular CI site offering question and answer (Q&A) services. It is necessary to study whether or not different types of CI have a different effect on CI quality and its usefulness. Thus, the purpose of this paper is to answer to following research questions: ${\bullet}$ What determinants are important to CI quality? ${\bullet}$ What is the relationship between CI quality factors and the usefulness of web-enabled CI? ${\bullet}$ Does CI type have a moderating effect on the relationship between CI quality, its determinants, and CI usefulness? Online survey using Google Docs with email and Kakao Talk was conducted for collecting data from Wikipedia and Naver KnowledgeiN users. A totoal of 490 valid responses were collected, where users of Wikipedia were 220 while users of Naver KnowledgeiN were 270. Expertise of contributors, community size, and diversity of contributors were identified as core determinants of perceived CI quality. Perceived CI quality has significantly influenced perceived CI usefulness from a user's perspective. For improving CI quality, it is believed that organizations should ensure proper crowd size, facilitate CI contributors' diversity and attract as many expert contributors as possible. Hypotheses that CI type plays a role of moderator were partially supported. First, the relationship between expertise of contributors and perceived CI quality was different according to CI type. The expertise of contributors played a more important role in CI quality in the case of Q&A services such as Knowledge iN compared to wiki services such as Wikipedia. This implies that Q&A service requires more expertise and experiences in particular areas rather than the case of Wiki service to improve service quality. Second, the relationship between community size and perceived CI quality was different according to CI type. The community size has a greater effect on CI quality in case of Wiki service than that of Q&A service. The number of contributors in Wikipeda is important because Wiki is an encyclopedia service which is edited and revised repeatedly from many contributors while the answer given in Naver Knowledge iN can not be corrected by others. Finally, CI quality has a greater effect on its usefulness in case of Wiki service rather than Q&A service. In this paper, we suggested implications for practitioners and theorists. Organizations offering services based on collective intelligence try to improve expertise of contributeros, to increase the number of contributors, and to facilitate participation of various contributors.

GWP 조직문화가 성취욕구, 직무만족, 직무성과에 미치는 영향 - 제조, 서비스, 금융업 종사자 간 비교 분석 - (The Effects of GWP Organization Culture on Need for Achievement, Job Satisfaction, and Job Performance - Comparison Analysis of Workers in Manufacturing, Service, and Finance Industries -)

  • 본 연구는 GWP 조직문화가 조직구성원들의 성취욕구(Need for Achievement)와 직무만족(Job Satisfaction), 직무성과(Job Performance)에 미치는 영향을 분석한 실증연구이다. 이를 위해, 본 연구는 2010년 이후 국내에서 일하기 좋은 기업(GWP)으로 선정된 대기업 중, 제조/서비스/금융업 종사자들을 대상으로 설문조사를 실시하여, GWP 조직문화가 산업별 종사자들에게 미치는 영향과 변수 간의 관계를 비교분석하였다. 본 연구 결과 첫째, 제조업에서는 GWP 조직문화가 성취욕구와 직무만족에 유의한(+) 영향을 미치는 것으로 나타났고, 직무성과에는 유의한 영향을 미치지 못하는 것으로 분석되었다. 또한 성취욕구는 직무만족과 직무성과에 유의한(+) 영향을 미치는 것으로 나타났고, 직무만족도 직무성과에 유의한(+) 영향을 미치는 것으로 분석되었다. 둘째, 서비스업에서는 GWP 조직문화가 성취욕구와 직무만족에 유의한(+) 영향을 미치는 것으로 나타났으나, 직무성과에는 유의한 영향을 미치지 못하는 것으로 분석되었다. 그리고 성취욕구는 직무만족에 유의한(+) 영향을 미치는 것으로 나타났고, 직무성과에는 유의한 영향을 미치지 못하였으며, 직무만족도 직무성과에 유의한 영향을 미치지 못하는 것으로 분석되었다. 마지막으로, 금융업에서는 GWP 조직문화가 성취욕구과 직무만족(+), 직무성과(-)에 유의한 영향을 미치는 것으로 나타났고, 성취욕구는 직무만족과 직무성과에 유의한(+) 영향을 미쳤으며, 직무만족도 직무성과에 유의한(+) 영향을 미치는 것으로 분석되었다. 본 연구는 이와 같은 분석결과를 토대로, GWP 조직문화가 산업별 종사자들에게 미치는 공통점과 차이점을 중심으로 그 의미와 시사점을 제시하였다.

A RURAL HEALTH SERVICE MODEL FOR KOREA BASED OH A PRIMARY CARE NURSING SERVICE SYSTEM

  • This study concerns itself with the development of a new model of comprehensive health service for rural communities of Korea. The study was conceived to resolve the problems of both underservice in rural communities and underutilization of valuable health manpower, namely the nurses, the disenchanted elite health personnel in Korea. On review of the current situation, the greatest deficiencies in the Korean health care system were found in the availability of primary care at the peripheries of md communities, in the dissemination of knowledge of disease prevention and health care, and in the induction of and guidance for active participation by the clientele in health maintenance at the personal, family and community level Abundant untapped health resources were identified that could be brough to bear upon the national effort to extend health services to every member of the Korean Population. Therefore, it was Postulated that the problem of underservice in rural communities of Korea can be structurcturally resolved by the effective mobilization and organization of untapped health resources, and that. a primary care Nursing Service System offers the best possibility for fulfillment of rural health service goals within the current health man-power situation. In order to identify appropriate strategies to combat the present difficulties in Korean rural health services and to utilize nurses and other health personnel in community-centered health programs, a search was made for examples of innovative service models throughout the world. An extensive literature survey and field visits to project sites both in Korea and in the United States were made. Experts in the field of world health, health service, planners, administrators, and medical and nursing practitioners in Korea, in the United States as well as visitors from other Asian countries were widely consulted. On the basis of information and inputs from these experts a new rural health service model has been constructed within the conceptual framework of community development, especially of the innovation diffusion Model. It is considered especially important that citizens in each community develop capacities for self-care with assistance and supports from available health professionals and participate in health service-related decisions that affect their own well-being. The proposed model is based upon the regionalization of health care planning utilizing a comprehensive Nursing Service System at the immediate delivery level The model features: (1) a health administration unit at each administrative level; (2) mechanisms for community participation; (3) a continuous source of primary health care at the local community level; (4) relative centralization of specialty care and provision of tertiary or super-specialty care only at major national metropolitan centers; and (5) a system for patient referral to the appropriate level of care. This model has been built around professional nurses as the key community health workers because their training is particularly suited and because large numbers of well-trained nurses are currently available and being trained. The special element in this model is a professional nurse-guided, self-care facilitating primary care Community Nursing Service System. This is supported by a Nursing Extension Service as a new training and support structure. (See attached diagrams). A broad spectrum of programs was proposed for the Community Nursing Service System. These were designed to establish a balance of activities between the clinic-centered individual care component and the field activity-centered educational and supportive component of health care services. Examples of possible program alternatives and proposed guidelines for health care in specific situations were presented, as well as the roles and functions of the key health personnel within the Community Nursing Service System. This Rural Health Service Model was proposed as a real alternative to the maldistributed, inequitable, uncoordinated solo-practice, physician-centered fee-for-service health care available to Koreans today.

  • 인터넷의 발달로 새로운 비즈니스 환경으로 인식되어 급속하게 성장한 전자상거래가 최근 WiFi의 보급과 모바일 폰 등으로 결제 수단이 다양해지면서 더욱 가파르게 진화하고 있지만 사기, 개인 및 신용정보 노출, 개인의 사회적 신뢰도 저하 등 전자상거래 시장의 활성화를 위협하는 보안 문제는 여전한 상황이다. 본 논문에서는 웹 기반의 전자상거래 발전에 있어 보안 문제의 해결이 가장 중요함을 인식하고 전자상거래 활성화를 위한 보안 요구사항을 도출하고 분석하고자 한다. 이를 위해 전자상거래 보안과 관련한 다수의 보안 항목을 선정하고, 연구자그룹과 실무자그룹을 대상으로 AHP(Analytic Hierarchy Process)를 사용하여 영역별 가중치를 산정한 모델을 설계한다. 또한 향후 안전한 전자상거래 시스템 설계, 구축 및 운영에 필요한 보안 가이드라인으로 활용될 수 있도록 두 그룹을 통해 나타난 상대적인 우선순위를 비교하여 분석한다.

공공도서관 어린이서비스 마케팅 전략 - 미국 공공도서관의 '가족장소도서관'을 중심으로 - (A Study on Children Service Marketing Strategies of Public Libraries: With Special Reference to 'Family Place Library' at the Public Libraries in USA)

  • 21 세기 지식기반사회에서 공공도서관은 다양한 변화와 도전에 직면하고 있다. 이에 따라 공공도서관 경영에서 마케팅 개념을 도입할 필요성은 점차 커지고 있다. 이 연구는 미국 공공도서관이 지역사회에 제공하는 어린이서비스 중 '가족장소도서관' 프로그램에 초점을 맞추었다. '가족장소도서관'은 영 유아와 그 가족을 위한 프로그램으로 2011년 2월 현재 미국 공공도서관 중 300여개 관이 참여하고 있다. 이 연구는 미국 뉴욕주를 중심으로 이러한 프로그램을 시행하는 주요 사례 도서관을 대상으로 참여관찰 및 인터뷰 조사를 수행하여 이 프로그램의 내용을 분석하고 미국 여러 주의 도서관 내외 전문가들과의 심층인터뷰를 진행하였다. 이러한 조사결과를 바탕으로 공공도서관 경영을 위한 마케팅 전략을 제시하였다.

트리클로로에틸렌의 유통·사용 실태조사 결과 (A Survey on the status of using trichloroethylene(TCE) in Korea)

  • Trichloroethylene(TCE) is widely used as a degreasing solvent in workplaces. TCE is primarily toxic to the nervous system, however, systemic disorder like Stevens-Johnson syndrome has been recently reported in small-scale factories, where the government has had limited information of chemical use. A survey was performed to investigate the actual condition of using TCE and to provide practical information to occupational health service agencies and professionals. This survey was carried out on 103 factories out of 430 factories which were conducted periodic work environment measurement for TCE. Degreasing was the most popular reason for using TCE in Korea, which reached to 94%. TCE was also used as a solvent for rubber in the coating or molding process, and adhesives in the bonding process. Metal fabrication was the most common as 23%, followed by assembling automobile parts (17%), and machinery (12%). Workers exposed to TCE during full-shift were 52% while 48% were exposed during short period of the shift or intermittently. Manual or semi-automatic work occupied 87% while automatic work was just 13%. Though automatic work by a closed system was generally lower exposed to TCE, compared to manual work, it can cause a high exposure when the maintenance system is improper. Semi-automatic work especially like open-top degreasing process can cause a high exposure when local exhaust system with condensing and refrigerating coils in the degreaser does not work well. In conclusion, the survey showed nationwide status of TCE exposure in various aspects. It can be used to monitor workplaces and workers exposed to TCE to prevent occupational diseases.

빅데이터를 활용한 맞춤형 교육 서비스 활성화 방안연구 (Data Analytics in Education : Current and Future Directions)

  • 데이터의 급속한 증가로 데이터를 활용한 새로운 가치 창출은 기업뿐 아니라 국가 경쟁력의 중요한 요소로 대두대고 있다. 이에 따라 데이터를 분석하여 통찰력을 제시할 수 있는 데이터 과학자라 불리는 분석 전문가의 수요가 늘고 있는데, 이들 전문 인력 양성을 위해서는 정부, 학계, 산업의 공동 노력이 필요하다. 본 연구는 특히 교육 분야에서의 빅데이터 활용현황을 조사하고, 새로운 데이터 기반의 맞춤형 서비스 및 교육 과정을 제안한다. 또한, 데이터 과학자 양성을 위한 국내외 대학 및 기업의 대표적인 프로그램들을 살펴보고, 장기적인 관점에서 분석능력을 배양할 수 있는 새로운 교과과정도 제시한다. 본 연구는 다양한 사례를 바탕으로 대학에서 데이터를 활용한 교육환경 개선을 위한 방안을 모색하는데 도움을 주고자 한다.

학교도서관 순회사서 직무만족도 연구 (A Study on the Job Satisfaction of School Library Itinerant Librarians)

  • 본 연구는 학교도서관 인적자원의 현황을 파악하고, 인적자원이 부재한 학교도서관을 지원하는 순회사서의 직무만족도를 통해 학교도서관의 발전방향을 제시하고자 한다. 순회사서제도가 운영되고 있는 전남, 경북, 경남의 직원을 대상으로 일반적인 현황과 근무체계, 업무 특성을 파악하고 직무만족도를 조사·분석하였다. 분석결과 사서 1명이 10여개에서 200여개의 학교를 담당하고 있었으며, 30%의 사서는 공공도서관 소속으로 나타났다. 직무만족도는 3.45점으로 나타났으며, 세부항목을 보면 직무(4.15), 보람(3.89), 의사소통(3.70)은 평균 이상인 반면, 승진공정(2.08), 교육(1.96)은 낮게 나타났다. 직무만족을 높이기 위해 본 연구에서는 순회사서 인력 증원, 계속교육 기회제공, 순회사서 소속 변경 등을 제안하였다.
