Preventing the Transmission of Tuberculosis in Health Care Settings: Administrative Control

  • It is well established that health care workers (HCWs) have a considerably higher risk of occupationally acquired tuberculosis (TB). To reduce the transmission of TB to HCWs and patients, TB infection control programs should be implemented in health care settings. The first and most important level of all protection and control programs is administrative control. Its goals are to prevent HCWs, other staff, and patients from being exposed to TB, and to reduce the transmission of infection by ensuring rapid diagnosis and treatment of affected individuals. Administrative control measures recommended by the United States Centers for Disease Control and Prevention and the World Health Organization include prompt identification of people with TB symptoms, isolation of infectious patients, control of the spread of the pathogen, and minimization of time spent in health care facilities. Another key component of measures undertaken is the baseline and serial screening for latent TB infection in HCWs who are at risk of exposure to TB. Although the interferon-gamma release assay has some advantages over the tuberculin skin test, the former has serious limitations, mostly due to its high conversion rate.

A Study on the Risk of Occupational Infectious Disease of Police Scientific Investigator (The Bacterial Culture of the Putrefying Dead Body Specimen)

  • The unnatural dead body refers to a corpse which is suspicious of unusual death and the dead case has been occurred more than 20 thousands times every year during the recent five years and most of them are found decayed. Police Scientific Investigators investigate unnatural dead bodies and its surroundings in all-around way and determine whether the death is involved with a crime, and most of the Scientific Investigators are exposed to a danger to get infected with pathogenic bacteria which are generated during the decaying process of dead body and are mostly to cause serious injuries on human beings. In line with the fact, the present study conducted a bacterial analysis by collecting excretions from 60 dead bodies and culturing the bacteria to evaluate infection risk of the police agents. The study cultured bacteria from 60 bodies and classified pathogenic bacteria of 108 strains, and its main bacteria are found to be them (; Acinetobacter baumannii 20 strains (19%), Pseudomonas aeruginosa 19 strains (18%), E. coli 18 strains (17%), Klebsiella pneumoniae 11 strains (10%), Proteus mirabillis 10 strains (9%), Enterococcus faecium 10 strains (9%), Staphylococcus aureus 9 strains (8%), Bacillus spp. 5 strains (5%), Streptococcus pneumoniae 3 strains (3%), Candida albicans 2 strains (2%), Mycobacterium tuberculosis 1 strains (1%)). The study results are expected to be used as educational data for preventing the Police Scientific Investigator from infections with bacteria or as a minimum data for improving work environment of the agents.

여성근로자의 작업장 건강안전위험요인 및 관리방안 (Workplace Health and Safety Risk Factors and Management Plan for Female Workers)

  • Purpose: In Korea, the proportion of female workers among occupational injury is steadily increasing. The purpose of this study was to identify the workplace risk factors that threaten the safety and health of the female workers. Methods: Qualitative study was conducted, using focus group interview. Data were collected from five focus groups of eighteen female workers who were working in the manufacturing, health service, and educational service industries. Results: The seven themes of occupational safety and health risks of women workers are as follows: 1) Workplace with various risks; 2) Sliding, falling, burning, and cutting accidents; 3) Chronically lasting musculoskeletal symptoms; 4) Chemicals that may be harmful to workers; 5) Unprotected infectious disease; 6) Psychological safety accidents(Relationship conflict and emotional labor); 7) Loud, hot, stuffy, scary, and tired. Conclusion: The Occupational Safety & Health (OSH) Act should be rearranged gender sensitively and women's participation in the OSH decision-making process should be guaranteed. OSH education should be properly implemented in the field, and support should be provided for women workers in small businesses. A supportive organizational culture for marriage, pregnancy and childbirth should be established, and the working environment should be provided safely, taking into account gender differences.

Recurrent Prosthetic Mitral Valve Dehiscence due to Infective Endocarditis: Discussion of Possible Causes

  • Prosthetic valves are being widely used in the treatment of heart valve disease. Prosthetic valve endocarditis (PVE) is one of the most catastrophic complications seen in these patients. In particular, prosthetic valve dehiscence can lead to acute decompensation, pulmonary edema, and cardiogenic shock. Here, we discuss the medical management of late PVE in a patient with a prior history of late and redo early PVE and recurrent dehiscence. According to the present case, we can summarize the learning points as follows. A prior history of infective endocarditis increases the risk of relapse or recurrence, and these patients should be evaluated very cautiously to prevent late complications. Adequate debridement of infected material is of paramount importance to prevent relapse. A history of dehiscence is associated with increased risk of relapse and recurrent dehiscence.

The issue of vaccine refusal: the study of a risky behavior

  • Purpose: Influenza is the most common seasonal infectious disease that causes permanent social, economic, and medical problems worldwide. Therefore, the most effective way to prevent influenza is through vaccines. The aim of this study is to identify the influence of factors that determine the refusal of influenza vaccine among three subjects groups. Materials and Methods: A survey was conducted amongst the three high-risk groups in 2018-2019 (Moscow, Russia). The survey involved 1,620 parents and pregnant women (group 1), 324 doctors (group 2), and 433 students (group 3). Poor vaccine uptake was observed among respondents in all three groups. Results: According to the survey results, only 22.2% of children and 13.8% of adults were vaccinated against influenza. Group 2 showed increased rates with 36.7% of vaccinated adults and 58.7% of children. The lowest adherence to annual vaccinations was recorded in group 3 (only 17.3%). There is also a negative correlation between adherence to vaccination and smoking (-0.66), unhealthy diet (-0.73), poor oral hygiene (-0.61), and insufficient awareness of the need for influenza vaccine as well (-0.81). Conclusion: Thus, a general lack of vaccination awareness has a fundamental role in forming a negative attitude toward influenza vaccine. It is necessary to conduct research to promote vaccination against influenza to improve vaccine uptake among high-risk groups, particularly students.

Association of the PTEN IVS4 (rs3830675) Gene Polymorphism with Reduced Risk of Cancer: Evidence from a Meta-analysis

  • PTEN (phosphatase and tensin homologue), as a tumor suppressor gene, plays a significant role in regulating cell growth, proliferation, and apoptosis. Results from published studies for association between the PTEN IVS4 I/D (rs3830675) polymorphism and cancer risk are inconsistent and inconclusive. We therefore conducted a meta-analysis to evaluate the potential association between PTEN IVS4 I/D polymorphism and risk of cancer in detail. We searched PubMed (Medline) and EMBASE web databases to cover all relevant studies published until December 2013. The meta-analysis was carried out and pooled odds ratios (ORs) and 95% confidence intervals (95%CIs) were used to appraise the strength of association. A total of 1,993 confirmed cancer cases and 3,200 controls were included from six eligible case-control studies. Results from overall pooled analysis suggested a significant effect of the PTEN IVS4 I/D polymorphism and cancer risk in all genetic models, i.e., allele (I vs D: OR=0.743, 95%CI=0.648 to 0.852, p=0.001), homozygous (II vs DD: OR=0.673, 95%CI=0.555 to 0.816, p=0.001), heterozygous (ID vs DD: OR=0.641, 95%CI=0.489 to 0.840, p=0.001), dominant (II+ID vs DD: OR=0.626, 95%CI=0.489 to 0.802, p=0.001) and recessive (II vs DD+ID: OR=0.749, 95%CI=0.631 to 0.889, p=0.001). Significant publication bias was detected during the analysis. The present meta-analysis suggests that the PTEN IVS4 I/D polymorphism is significantly associated with reduced risk of cancer. However, future larger studies with other groups of populations are warranted to clarify this association.

어린이집 아동과 요보호 아동의 건강문제와 건강행위 실태 (Health Problems and Health Behaviors of Preschoolers in Day Care Centers and Living With Guardians)

  • Purpose : This study was conducted to understand the health status and health behaviors of preschoolers to provide baseline data for developing health promotion programs. Method 'Parents of children attending day-care centers and guardians of preschoolers were recruited for the study. Data were collected from June to August2002 using a structured questionnaire. Result . Among 754 preschoolers, 17,3% were overweight, while 18.2% were underweight, while 15.9% and 23.2% were overweight and underweight respectively, among 85 children living with guardians. The most frequent infectious diseases that children have had previously were hand-foot-mouth disease, chicken pox, and measles. Current health conditions that children have frequently are respiratory disease and atopic dermatitis for bother groups. Only 54%-61% brush their teeth everyday at bedtime, 49%-54% wash their hands every time after returning home, 4.5%-8.8%wear bicycle helmets, 9,3%-11.1% use a child car seat, and 3.5%-8.1% eat fruits and vegetables five times a day, Children living with guardians were less likely toper form positive health behaviors compared with children in day-care centers. Conclusion 'Based on the study results, health professionals could plan and develop health promotion programs to change unhealthy behaviors of preschoolers targeting high-risk groups.

Postinfectious bronchiolitis obliterans in children: lessons from bronchiolitis obliterans after lung transplantation and hematopoietic stem cell transplantation

  • Postinfectious bronchiolitis obliterans (PIBO) is an irreversible obstructive lung disease characterized by subepithelial inflammation and fibrotic narrowing of the bronchioles after lower respiratory tract infection during childhood, especially early childhood. Although diagnosis of PIBO should be confirmed by histopathology, it is generally based on history and clinical findings. Irreversible airway obstruction is demonstrated by decreased forced expiratory volume in 1 second with an absent bronchodilator response, and by mosaic perfusion, air trapping, and/or bronchiectasis on computed tomography images. However, lung function tests using spirometry are not feasible in young children, and most cases of PIBO develop during early childhood. Further studies focused on obtaining serial measurements of lung function in infants and toddlers with a risk of bronchiolitis obliterans (BO) after lower respiratory tract infection are therefore needed. Although an optimal treatment for PIBO has not been established, corticosteroids have been used to target the inflammatory component. Other treatment modalities for BO after lung transplantation or hematopoietic stem cell transplantation have been studied in clinical trials, and the results can be extrapolated for the treatment of PIBO. Lung transplantation remains the final option for children with PIBO who have progressed to end-stage lung disease.

Prognostic Analysis of Schistosomal Rectal Cancer

  • Background: Schistosomiasis is an infectious disease that affects more than 230 million people worldwide, according to conservative estimates. Some studies published from China and Japan reported that schistosomiasis is a risk factor for colorectal cancer in Asia where the infective species is S. japonicum. Hoqwever, there have been only few reports of prognosis of patients with schistosomal rectal cancer SRC. Objectives: This study aimed to analyze differences in prognosis between SRC and non-schistosomal rectal cancer(NSRC) with current treatments. Materials and Methods: A retrospective review of 30 patients with schistosomal rectal cancer who underwent laparoscopic total mesorectal excision operation (TME) was performed. For each patient with schistosomal rectal cancer, a control group who underwent laparoscopic TME with non-schistosomal rectal cancer was matched for age, gender and tumor stage, resulting in 60 cases and controls. Results: Univariate analysis showed pathologic N stage (P=0.006) and pathologic TNM stage (P=0.047) statistically significantly correlated with disease-free survival (DFS). Pathologic N stage (P=0.014), pathologic TNM stage (P=0.002), and with/without schistosomiasis (P=0.026) were statistically significantly correlated with overall survival (OS). Schistosomiasis was the only independent prognostic factor for DFS and OS in multivariate analysis. Conclusions: The prognosis of patients with schistosomal rectal cancer is poorer than with non-schistosomal rectal cancer.

COVID-19와 두경부암 (Head and Neck Oncologic Care During the COVID-19 Pandemic)

  • It is well established that cancer patients are more susceptible to infection because of the immunosuppressive status caused by both disease itself and anticancer treatment, including surgery and chemoradiation. Head and neck cancer (HNC) patients are generally at high-risk for coronavirus disease 2019 (COVID-19) infection and serious adverse outcomes. Although there is an urgent need for guidance in the delivery of safe, quality oncologic care, no international consented recommendation addressed the management of HNC patients in COVID-19 due to limited data. In this review, we summarized the consideration for head and neck oncologic care in the context of the COVID-19 pandemic, based on the data and the very recent recommendations from the Korean Cancer Association and National Cancer Center. COVID-19 should be taken into consideration in the comprehensive management of HNC patients, and multidisciplinary evaluation of multilevel surgical-risks, discussion of optimized strategy, and shared-decision-making with the patient are needed to maximize both the safety from infectious pandemic and outcome of surgical and oncologic care.
