The purpose of this study was to explore the cultural perspectives and experiences relating to insulin therapy among the diabetes. The authors conducted four semi-structured focus groups and individual interviews with 19 adults with type 1 and 2 diabetes, focusing on the personal experiences and thoughts regarding insulin therapy. Patients' perspectives and experiences relating to taking insulin formed three categories of themes: preoccupations about insulin, barriers to taking insulin, and benefits to taking insulin. The theme for preoccupations about insulin was "vague fear," while the theme of barriers to taking insulin were "worrisome insulin-related issues", "ambivalent feelings (trust/mistrust) about healthcare providers," "dependent life," "feeling about supporters(family, friends, and religion)," "inconvenience," "regret about the past," and "embarrassment." The theme of benefits to taking insulin were "recognition" and "physical recovery and confidence in regulating blood glucose". Based on this study, patients' feelings about their insulin should be respected by healthcare providers.

Purpose: The purpose of this study was to explore decision making experiences of Korean women with breast cancer who underwent breast reconstruction with/after a mastectomy. Methods: Data were collected during 2015-2016 through individual in-depth interviews with 10 women who had both mastectomy and breast reconstruction, and analyzed using phenomenological method to identify essential themes on experiences of making a decision to have breast reconstruction. Results: Five theme clusters emerged. First, "expected loss of sexuality and discovery of autonomy" illustrates various aims of breast reconstruction. Second, "holding tight to the reputation of doctors amid uncertainty" specifies the importance of a trust relationship with their physician despite a lack of information. Third, "family members to step back in position" describes support or opposition from family members in the decision making process. Fourth, "bewilderment due to the paradox of appearance-oriented views" illustrates paradoxical environment, resulting in confusion and anger. Lastly, "decision to be made quickly with limited time to oneself" describes the crazy whirling process of decision making. Conclusion: Findings highlight aims, worries, barriers, and facilitators that women with breast cancer experience when making a decision about breast reconstruction. Deciding on breast reconstruction was not only a burden for women in a state of shock with a diagnosis of breast cancer, but also an opportunity to decide to integrate their body, femininity, and self which might be wounded from a mastectomy. These findings will help oncology professionals provide effective educational counselling before the operation to promote higher satisfaction after the operation.
Purposes: The purpose is to establish the direction of healthcare R&D through private nonprofit organization. Methodology: The data is divided into two groups: 12 physicians and pharmacists, and 16 persons including professors related to university donation, non-profit foundation executives. Each group was subjected to two Delphi surveys. To analyze the validity of the opinion, the content validity ratio and the consensus of experts were verified. Findings: Funding should be invested in 'development research' and 'application research'. The factors that hinder the donation culture are 'donation prevention system such as tax imposition system and rebate double penalty system', 'insufficient motivation of fund raising person', and 'lack of fund specializing specialist'. The fund raising strategy should be centered on a small number of large donors or a balance between large and small donors. The fund raising target should be effective to raise funds for corporate and individual donors. It is necessary to clarify the purpose of the social problem to be solved by the campaign strategy for promoting donation, to announce the validity of the trust and transparency of the institution, and to emphasize the social investment by the private sector. Practical Implications: It is necessary to present directions through private nonprofit organizations for the future development of healthcare R&D. The legal and institutional deficiencies of the domestic nonprofit organization fundraising infrastructure should be improved. In order to create a social investment climate, it is necessary to improve the awareness of donations and develop various donation programs for the private sector.

Methods : A qualitative descriptive study design was used. Data were collected through individual interviews with thirteen problem drinkers and analyzed using qualitative content analysis. Results : The experiences of the problem drinkers participating in the early intervention program were derived from four themes and nine sub-themes in terms of awareness, alcohol expectancies, perceived self-efficacy and behavioral change. They participated in programs offered in the workplace, and there were differences in motivation level among individuals, but generally accepted the necessity of the program. Participants who had trust with the program provider were more likely to change drinking behavior. Conclusion : Problem drinkers who participated in the early intervention program showed that it is a useful approach to change the problem drinking behavior by promoting awareness of alcohol-related knowledge, motivation for change through acquiring enough information and acquisition of self-efficacy as a behavior skill. In order to improve voters' reliability in electronic voting system, voter verifiable receipt technique is being actively researched. Since the receipt should be verifiable not only inside but also outside a polling place, it satisfies the requirements, individual verifiability and receipt-freeness. In the previous researches, there are some problems that special paper and printer is necessary or frequent monitoring is needed to confirm the voting machine's trustworthiness. In this paper, we propose a new receipt issuing scheme. Our scheme does not require any special equipments such as special paper and printer or optical scanner. In addition to that it does not require voters to trust any devices in the polling station and there is no need of frequent observations to the voting machines.

Purpose: This study presents the process of designing workbooks for advance care planning appropriate for the Korean cultural setting and describes actual case studies. Methods: This study focused on single inductive case studies of the utilization of an advance care planning workbook and recruited individual participants. Results: The workbook for adolescents contained six sessions and the workbook for children contained seven sessions. The workbook sessions led to four major discoveries: 1) considering the Korean cultural context, discussions on life and death must be held indirectly; 2) the role of the counselor as a supporter is crucial for the workbook to be effective; 3) the workbook must be accessible regardless of the seriousness of the illness; and 4) patients must be able to make their own choice between the workbook versions for children and adolescents. Six facilitating factors improved engagement: 1) the role of the counselor as a supporter; 2) building trust with the patient; 3) affirming freedom of expression on topics the patient wished to avoid talking about; 4) having discussions on what private information to keep secret and to whom the information can be disclosed; 5) discovering and regularly discussing relevant topics; and 6) regular communication and information-sharing with the patient's medical service providers. Conclusion: It is necessary to build on actual case studies regarding workbooks for children and adolescents in order to expand the usage of these workbooks to all relevant medical institutions in Korea.

This study explains the characteristics of micro-insurance based on the theoretical model of health insurance, such as the low demand of low-income people and the lower demand of higher risk aversion. In particular, these characteristics contradict the existing insurance theory which states that the lower the income, the higher the risk aversion, and the higher the demand for insurance. This study postulates a two-period model focusing on health insurance, contrary to a one-period model assumed in existing studies. As a result, first, we show that the decrease in income leads to a decrease in the preventive effort for illness. Second, we offer a model for micro-insurance in which the individual chooses a partial insurance under an actuarially fair insurance premium, while full insurance is optimal in existing studies. Third, we also show that the insurance demand decreases when the outlook for the future improves. Fourth, we finally show that the lack of trust and default risk of the insurer decrease the insurance demand as risk aversion increases.
