The restoration of endodontically treated teeth (ETT) with more than one cusp missing and thin remaining walls is challenging for the general practitioner. The use of posts combined with full coverage restorations is a well-established approach, yet not following the minimal invasive principles of adhesive dentistry. Endocrowns are indirect monoblock restorations that use the pulp chamber of the ETT for retention. In this study the fabrication of 4 endocrowns and their clinical performance will be discussed. Two clinical cases include computer-aided design/computer-aided manufacturing manufactured molar endocrowns (one feldspathic ceramic and one hybrid composite-ceramic restoration) and the other two are dental laboratory manufactured resin composite premolar endocrown restorations. The modified United States Public Health Service criteria were used to assess the clinical behavior of the restorations at different follow up periods. Endocrown restorations present a satisfactory clinical alternative, either by the use of resin composite or glass ceramic and hybrid materials. Specific guidelines with minimal alterations should be followed for an endocrown restoration to be successful. Due to limited evidence regarding the long term evaluation of this restorative technique, a careful selection of cases should be applied.
연구목적: 간접 복합레진 수복은 직접 수복의 여러 문제점들을 극복하는 대안으로 적용 빈도가 증가하고 있지만 간접 수복물의 특성상 치아 삭제 전 술자에 의해 결정된 색상을 기공 과정 동안 재현해야하기 때문에 기준이 되는 shade guide와 실제 간접수복용 레진간의 색상의 일치성이 요구되며, 치아의 다양한 표면의 광학적 특성을 부여하기위한 법랑질 특성의 복합레진의 광학적 특성에 대한 정보가 필요하다. 연구 재료 및 방법: 본 연구에서는 이러한 간접 복합레진 수복재료의 색상 특성을 알아보기 위해 간접수복용 레진인 Tescera ATL (Bisco, USA)의 Body composite의 색상과 동일한 색상 기호의 Vitapan shade guide 간의 색차를 알아보고, Body composite와 Incisal composite의 적층 수복 시 색상 변화 양상을 분석하고자 시행하였다. 직경 5 mm, 두께 3 mm의 원기둥형 Body composite 시편을 각 색상 당 9개씩 제조사의 지시에 따라 제작하고, 표면 연마 후 분광광도계(NF999, Nipon Denshuku, Japan)을 이용해 색상(CIE $L^*a^*b^*$)을 측정하고 Vitapan shade guide의 색상을 측정하였다. 색상 측정이 끝난 Body composite에 9 색상의 Incisal composite를 1 mm 두께로 적층하고 중합, 연마 한 후 적층면의 색상을 측정하였으며, Incisal composite를 0.5 mm 두께만큼 연마한 후 다시 색상을 동일한 측정 기기로 측정하였다. 결과: Tescera ATL과 Vitapan shade guide의 동일한 색상 간에 최소 6.88, 최대 12.80의 큰 색차(${\Delta}E^*$)를 보였으며, 모든 색상에서 시각적으로 확연한 차이를 보였다. 모든 색상의 Body composite에서 적층한 Incisal composite의 색상에 관계없이 Incisal composite의 두께가 두꺼워 질수록 Body composite 과의 색차가 증가하였다(p < 0.05). 또한 $L^*$값과 $b^*$값은 Incisal composite의 두께가 두꺼울수록 감소하는 양상을 보인 반면 $a^*$값은 큰 변화를 보이지 않았다. 결론: 이상의 결과로 Tescera ATL을 이용한 간접 복합레진 수복 시 색상 결정에 있어 기존의 shade guide의 사용은 부적절하며, 동일한 재료로 제작한 개별적인 shade guide가 필요하리라 사료되며, Incisal composite 적용 시 색상 변화에 대한 정확한 정보를 바탕으로 신중한 적용이 필요하리라 사료된다.
4종의 레진시멘트를 통한 상아질과 간접 레진 수복물 간의 인장결합강도를 열순환 시효처리 여부에 따라 측정하여 비교하고, 주사전자현미경 관찰을 통하여 각 레진시멘트의 접착 내구성을 평가하고자 시행하였다. 48개의 건전한 제3대구치의 상아질 표면을 평탄하게 노출시키고 #320 grit Sic Paper로 연마하였다. 복합레진 블록을 제작하여 #600 grit Sic Paper로 연마한 후에 접착면을 Sandblast로 처리하였다. 각각의 레진시멘트로 제조사 지침에 따라 적용하여 복합레진 블록을 상아질 표면에 접착하였다. 이후 제작된 시편을 열순환시키지 않거나, 1,000회, 5,000회 열순환 시킨 후 ($5^{\circ}C\;-\;55^{\circ}C$) 미세인장결합강도를 측정하였다. 열순환 전 시편의 접착계면 (수직절단면)과 파절된 시편의 상아질 파단면을 전자현미경 관찰하여 다음과 같은 결론을 얻었다. 1. Variolink II의 결합강도는 다른 실험군보다 높은 결합강도를 보여주었으며, 1,000회 열순환 후 유의성 있게 결합강도가 감소되었다 (p < 0.05). 3. Multilink의 결합강도는 열순환에 가장 많은 영향을 받았으며 1,000회 열순환 이후 유의성 있게 감소되었다 (p < 0.05). 3. Panavia F 2.0과 Rely X Unicem의 결합강도는 열순환에 의하여 감소되지 않았다 (p > 0.05). 4. 접착형 레진시멘트는 복합레진형 레진시멘트에 비해서 열처리 전후 모두 낮은 결합강도를 보여주었다. 5. 결합강도가 높은 Vaviolink II와 Multilink에서는 혼합형 파괴양상을 보였고, 결합강도가 낮은 Panavia F 2.0에서는 접착성 파괴 양상을 나타내었다. 이상의 연구 결과를 토대로 적절한 전처리와 접착제를 도포한다면 복합레진형 레진시멘트는 접착형 레진시멘트보다 결합강도와 그 내구성이 우수하다고 할 수 있을 것이다. 접착성 간접 수복물의 초기 결합강도와 내구성은 레진시멘트의 접착과정과 종류, 형태에 의해 영향을 받기 때문에 이들의 적절한 선택과 올바른 사용이 성공적인 수복을 위해 중요하다.
연구목적: 본 연구는 직접수복용 레진 (Filtek Z350, Supreme XT)과 기공용 레진 (Sinfony)으로 제작한 레진 인레이를 투과하는 광중합기의 광강도를 측정하고 레진 인레이를 구성하는 색조에 따라 투과되는 광강도를 측정하였다. 연구 재료 및 방법: A3 색조의 레진 인레이를 Z350 A3 한 가지 색조로 제작한 것과 Supreme XT A3B와 A3E 두 가지 색조로 제작한 것을 이용하였으며 Sinfony는 제조사의 지시에 따라 A3, E3, T1 세 가지 색조로 제작하였고 두께는 1.5 mm로 통일하였다. 할로겐 광중합기 (Optilux 360)와 LED 광중합기 (Elipar S10)를 이용하여 레진 인레이를 투과하는 광강도를 휴대용 광강도 측정기 (Cure Rite)로 측정하였다. 각 레진의 색조가 광강도의 투과에 미치는 영향을 분석하기 위해 0.5mm 두께로 레진 시편을 제작하여 광강도를 측정하였다. 결과: Z350 A3로 제작한 레진 인레이를 투과한 광강도가 가장 낮았으며, 다음으로 Supreme XT A3B와 A3E로 제작한 레진 인레이, 그리고 Sinfony A3, E3, T1으로 제작한 레진 인레이 순으로 광강도가 유의하게 증가하였다 (p < 0.05). 0.5mm의 레진 시편을 투과한 광강도를 측정한 결과 dentin shade인 Sinfony A3, Z350 A3, Supreme XT A3B가 가장 낮았으며, enamel shade인 Supreme XT A3E, Sinfony E3, 그리고 translucent shade인 Sinfony T1 순으로 유의하게 증가하였다 (p < 0.05). 결론: 레진 인레이를 제작할 경우 단색의 직접 수복용 레진을 사용하기 보다는 기공용 레진의 dentin shade, enamel shade, translucent shade를 모두 사용하는 것이 레진 인레이 하방으로 더 많은 중합광을 투과시킬 수 있는 것으로 사료된다.
Statement of problem. Fracture of the tooth-colored superstructure material is one of the main prosthetic complications in implant-supported prostheses. Purpose. The purpose of this in vitro study was to compare the fracture strength between the cement-retained implant-supported metal-ceramic crowns and the indirect composite resinveneered metal crowns under the vertical compressive load. Material and methods. Standard implants of external type (AVANA IFR 415 Pre-mount; Osstem Co., Busan, Korea) were embedded in stainless steel blocks perpendicular to their long axis. Customized abutments were fabricated using plastic UCLA abutments (Esthetic plastic cylinder; Osstem Co., Busan, Korea). Thirty standardized copings were cast with non-precious metal (Rexillium III, Pentron, Walling ford, Conn., USA). Copings were divided into two groups of 15 specimens each (n = 15). For Group I specimens, metal-ceramic crowns were fabricated. For Group II specimens, composite resin-veneered (Sinfony, 3M-ESPE, St. Paul, MN, USA) metal crowns (Sinfony-veneered crowns) were fabricated according to manufacturer's instructions. All crowns were temporary cemented and vertically loaded with an Instron universal testing machine (Instron 3366, Instron Corp., Norwood, MA, USA). The maximum load value (N) at the moment of complete failure was recorded and all data were statistically analyzed by independent sample t-test at the significance level of 0.05. The modes of failure were also investigated with visual analysis. Results. The fracture strength of Sinfony-veneered crowns ($2292.7{\pm}576.0N$) was significantly greater than that of metal-ceramic crowns ($1150.6{\pm}268.2N$) (P < 0.05). With regard to the failure mode, Sinfony-veneered crowns exhibited adhesive failure, while metal-ceramic crowns tended to fracture in a manner that resulted in combined failure. Conclusion. Sinfony-veneered crowns demonstrated a significantly higher fracture strength than that of metal-ceramic crowns in cement-retained implant-supported prostheses.
The composite resin, due to its esthetic qualities, is considered the material of choice for restoration of anterior teeth. With respect to shade control, the direct-placement resin composites offer some distinct advantages over indirect restorative procedures. Visible-light-cured (VLC) composites allow dentists to match existing tooth shades or to create new shades and to evaluate them immediately at the time of restoration placement. Optimal intraoral color control can be achieved if optical changes occurring during application are minimized. An ideal VLC composite, then, would be one which is optically stable throughout the polymerization process. The shade guides of the resin composites are generally made of plastic, rather than the actual composite material, and do not accurately depict the true shade, translucency, or opacity of the resin composite after polymerization. So the numerous problems associated with these shade guides lead to varied and sometimes unpredictable results. The aim of this study was to assess the color changes of current resin composite restorative materials which occur as a result of the polymerization process and to compare the color differences between the shade guides provided with the products and the actual resin composites before- and after-polymerization. The results obtained from this investigation should provide the clinician with information which may aid in improved color match of esthetic restoration. Five light activated, resin-based materials (${\AE}$litefil, Amelogen Universal, Spectrum TPH VeridonFil-Photo, and Z100) and shade guides were used in this study. Three specimens of each material and shade combination were made. Each material was condensed inside a 1.5mm thick metal mold with 10mm diameter and pressed between glass plates. Each material was measured immediately before polymerization, and polymerized with Curing Light XL 3000 (3M Dental products, USA) visible light-activation unit for 60 seconds at each side. The specimens were then polished sequentially on wet sandpaper. Shade guides were ground with polishing stones and rubber points (Shofu) to a thickness of approximately 1.5mm. Color characteristics were performed with a spectrophotometer (CM-3500d, Minolta Co., LTD). A computer-controlled spectrophotometer was used to determine CIELAB coordinates ($L^*$, $a^*$ and $b^*$) of each specimen and shade guide. The CIELAB measurements made it possible to evaluate the amount of the color difference values (${\Delta}E{^*}ab$) of resin composites before the polymerization process and shade guides using the post-polishing color of the composite as a control, CIE standard D65 was used as the light source. The results were as follows. 1. Each of the resin composites evaluated showed significant color changes during light-curing process. All the resin composites evaluated except all the tested shades of 2100 showed unacceptable level of color changes (${\Delta}E{^*}ab$ greater than 3.3) between pre-polymerization and post-polishing state. 2. Color differences between most of the resin composites tested and their corresponding shade guides were acceptable but those between C2 shade of ${\AE}$litefil and IE shade of Amelogen Universal and their respective shade guides exceeded what is acceptable. 3. Comparison of the mean ${\Delta}E{^*}ab$ values of materials revealed that Z100 showed the least overall color change between pre-polymerization and post-polishing state followed by ${\AE}$litefil, VeridonFil-Photo, Spectrum TPH, and Amelogen Universal in the order of increasing change and Amelogen Universal. Spectrum TPH, 2100, VeridonFil-Photo and ${\AE}$litefil for the color differences between actual resin and shade guide. 4. In the clinical environment, the shade guide is the better choice than the shade of the actual resin before polymerization when matching colors. But, it is recommended that custom shade guides be made from resin material itself for better color matching.

Composite inlays are indicated for large cavities and different approaches have been proposed to improve the adaptationof Class II restorations, including applying base. The purpose of this study was to compare in vitro the marginal adaptation of class II(MOD) composite inlays(Tescera ATL system, Bisco) made with or without bases, having different physical properties. Extracted human lower molars were used for this study. The base was made from Aeliteflo(Bisco), Dyract AP(Bisco) or Fuji II LC improved(GC) respectively and the comtrol group has no base.(omitted)
연구목적: 본 연구는 간접 레진수복시 상아질 접착제의 중합 여부가 레진인레이와 상아질간의 미세인장결합강도와 결합계면의 형태에 미치는 영향을 공초점 현미경 관찰을 통하여 평가하고자 하였다. 연구 재료 및 방법: 24개의 발거된 대구치를 절단하여 교합면 상아질을 노출시켰다. 수복 방법에 따라 크게 간접 레진수복과 직접 레진수복의 두 군으로 나누었다. 간접 레진수복군은 임시 수복하여 1주일 후, 상아질 접착 과정에 따라 4군(OB-C와 OB-NC군, OS-C와 OS-NC군)으로 나누었다. 간접 레진수복군은 산부식 처리된 상아질면에 OptiBond FL (Kerr) 또는 One-Step (Bisco) 상아질 접착제를 중합 여부에 따라 나누어 적용하였다. 처리된 상아질면에 제작된 3 mm 높이의 레진 인레이(Tescera, Bisco)를 Variolink II (Ivoclar Vivadent)를 사용하여 제조사의 지시에 따라 합착하였다. 직접 레진수복군은 산부식 처리된 상아질면에 상아질 접착 후, 복합레진 (Estelite${\Sigma}$, Tokuyama)으로 즉시 수복하였다(OB-D군, OS-D군). 수복을 마친 모든 치아는 24시간 후, 미세인장 결합 강도 평가와 공초점 레이저 주사 현미경 관찰(CLSM)을 시행하였다. 결과: 간접 레진수복군은 직접 레진수복군에 비하여 낮은 미세인장결합강도를 나타내었으며, 상아질 접착제를 광중합한 군(OB-C군, OS-C군)이 광중합을 시행하지 않은 군(OB-NC군, OS-NC군)에 비하여 높은 미세인장결합강도를 나타내었다. 접착 계면에 대한 CLSM 관찰에서 상아질 접착제를 광중합한 군에서 더 분명하고 균일한 혼성층이 관찰되었다. 상아질 접착제를 광중합하지 않은 군에서는 혼성층이 불분명하거나 거의 관찰되지 않았다. 결론: 간접 레진수복물의 합착시 레진시멘트의 적용 전에 상아질 접착제의 광중합은 분명하고 균일한 혼성층의 형성하며, 이는 결합강도의 향상에 기여한다.

PURPOSE. To measure the surface loss of dental restorative zirconia and the short-term bond strength between an indirect composite resin (ICR) and zirconia ceramic after various sandblasting processes. MATERIALS AND METHODS. Three hundred zirconia bars were randomly divided into 25 groups according to the type of sandblasting performed with pressures of 0.1, 0.2, 0.4 and 0.6 MPa, sandblasting times of 7, 14 and 21 seconds, and alumina powder sizes of 50 and $110{\mu}m$. The control group did not receive sandblasting. The volume loss and height loss on zirconia surface after sandblasting and the shear bond strength (SBS) between the sandblasted zirconia and ICR after 24-h immersion were measured for each group using multivariate analysis of variance (ANOVA) and Least Significance Difference (LSD) test (${\alpha}$=.05). After sandblasting, the failure modes of the ICR/zirconia surfaces were observed using scanning electron microscopy. RESULTS. The volume loss and height loss were increased with higher sandblasting pressure and longer sandblasting treatment, but they decreased with larger powder size. SBS was significantly increased by increasing the sandblasting time from 7 seconds to 14 seconds and from 14 seconds to 21 seconds, as well as increasing the size of alumina powder from $50{\mu}m$ to $110{\mu}m$. SBS was significantly increased from 0.1 MPa to 0.2 MPa according to the size of alumina powder. However, the SBSs were not significantly different with the sandblasting pressure of 0.2, 0.4 and 0.6 MPa. The possibilities of the combination of both adhesive failure and cohesive failure within the ICR were higher with the increases in bonding strength. CONCLUSION. Based on the findings of this study, sandblasting with alumina particles at 0.2 MPa, 21 seconds and the powder size of $110{\mu}m$ is recommended for dental applications to improve the bonding between zirconia core and ICR.
This study evaluated the effect of time before addition and surface treatment on the shear bond strength between new and old(direct and indirect) composites. One hundred specimens of Charisma(Heraeus Kulzer, Germany) and thirty-five of Artglass(Heraeus Kulzer, Germany) were prepared in cavities($10mm{\times}5mm$) in acrylic resin molds. Charisma specimens were stored for 5min., 1hr., 24hrs., and 1 week in $37^{\circ}C$ distilled water and Artglass specimens were stored for 1 week in $37^{\circ}C$ distilled water.(omitted)

제 14 조 (유료서비스)
① 당 사이트 및 협력기관이 정한 유료서비스(원문복사 등)는 별도로 정해진 바에 따르며, 변경사항은 시행 전에

② 유료서비스를 이용하려는 회원은 정해진 요금체계에 따라 요금을 납부해야 합니다.
제 5 장 계약 해지 및 이용 제한
제 15 조 (계약 해지)

제 16 조 (서비스 이용제한)
① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는

- 2년 이상 서비스를 이용한 적이 없는 경우
- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
