Statement of problem: A difficulty in achieving a passive-fitting prosthesis can be overcome by individual crown restoation of multiple implants. But individualized crown has another difficulty in control of contact tightness and stress distribution. Purpose: This in vitro study is to evaluate the stress distribution and the magnitude in the supporting tissues around Endopore implants with different crown lengths, interproximal contact tightness, and the splinting effects. Material & methods: Three Endopore implants($4.1{\times}9mm$) were placed in the mandibular posterior edentulous area distal to the canine and photoelastic model was made with PL-2 resin(Measurements Group, Raleigh, USA). Restorations were fabricated in two crown lengths: 9, 13 mm. For non-splinted restorations, individual crowns were fabricated on three custom-milled titanium abutments. After the units were cemented, 4 levels of interproximal contact tightness were evaluated: open, ideal($8{\mu}m$ shim stock drags without tearing), medium($40{\mu}m$), and heavy($80{\mu}m$). For splinted restorations, 3-unit fixed partial dentures were fabricated. This study was examined under simulated non-loaded and loaded conditions(6.8 kg). Photoelastic stress analysis was carried out to measure the fringe order around the implant supporting structure. Results: 1. When restorations were not splinted, the more interproximal contact tightness was increased among the three implants, the more stress was shown in the cervical region of each implant. When crown length was increased, stresses tended to increase in the apex of implants but there were little differences in stress fringes. 2. When nonsplinted restorations were loaded on the first or third implant, stresses were increased in the apex and cervical region of loaded implant. Regardless of interproximal contact tightness level, stresses were not distributed among the three implants. But with tighter interproximal contact, stresses were increased in the cervical region of loaded first or third implant. 3. When the nonsplinted restorations were not loaded, there were little stresses on the supporting structure of implants, but low level stresses were shown in the splinted restorations even after sectioning and soldering. 4. With splinted restorations, there were little differences in stresses between different crown lengths. When splinted restorations were loaded, stresses were increased slightly on the loaded implant, but relatively even stress distribution occurred among the three implants. Conclusions: Splinting the crowns of adjacent implants is recommended for Endopore implants under the overloading situation.

Statement of the problem: In cases of low bone level in maxilla followed by extraction due to severe periodontitis or enlarged maxillary sinus, crown-root ratio of implant prosthesis will increase. The prognosis of these cases is not good as expected. Purpose : The purpose is to compare stress distribution due to crown-root ratio and effect of splinting between two implants in maxillary molar area under different loads Material and methods: Using ITI($4.1{\times}10$ mm) implant. two finite element models were made(model S: two parallel implants, model A: one of two is 20 degree inclined). Each model was designed in different crown-root ratio(0.7:1, 1:1, 1.25:1) and set cement type gold crown to make it splinted or non-splinted clinical situations. After that, 300 N force was loaded to each model in four ways.(load 1 : middle of occlusal table, load 2 : middle of buccal cusp, load 3 : middle of lingual cusp, load 4 : horizontal load to middle of buccal cusp), and stress distribution was analyzed. Results: On all occasions, stress was concentrated on neck of implant near cortical bone. In the case of inclined implant, stress was increased compared with parallel implants. Under load 1, 2, 3, stress was not increased even when crown-root ratio increases, but under load 4, when crown-root ratio increases, stress also increased. And more stress was concentrated under load 1 than load 2, 3. When crown-root ratio was same, stress under load 1, 2, 3 decreased when splinting, but under load 4, stress did not really decrease. Conclusion: Under vertical load, stress distribution related to crown-root ratio did not change. But under horizontal load, stress increased as crown-root ratio increases. Under vertical load, splinting decreased stress but under horizontal load, effect of splinting was decreased as condition of implant changes for the worse such as increase of crown-root ratio, inclined implant.
Implant success is achieved by the synergistic combination of numerous biomechanical factors. This report examines the mechanical aspect of implants. In particular, it is focused on macrodesign such as thread shape, pitch, width and depth, and crestal module of implants. This study reviews the literature regarding the effect of implant thread geometry on primary stability and osseointegration under immediate loading. The search strategy included both in vitro and in vivo studies published in the MEDLINE database from January 2000 to June 2014. Various geometrical parameters are analyzed to evaluate their significance for optimal stress distribution, implant surface area, and bone remodeling responses during the process of osseointegration.

Nowadays, Cone-Beam CT is widely supplied in dental clinics, the distribution rate in south korea is highly ranked worldwidely. Recently, The number of Cone-Beam CTs reached 10 thousands according to national healthcare system report. Also, dental manufacturers released many kinds of In-house 3D digital printers, the distribution rate of which rises rapidly in dental clinics. Accordingly, using Cone-Beam CT data and Intraloral scan data, the application of implant guide surgery is widespread in a unit of private clinic. Through the previous articles, the latest methods of computer-guided implant surgery are reviewed, and also the considerations for precise and reliable guide surgery are summarized.
Purpose : This study was to assess the loading distributing characteristics of implant systems with internal connection or external connection under vertical and inclined loading using finite element analysis. Materials and methods : Two finite element models were designed according to type of internal connection or external connection The crown for mandibular first molar was made using cemented abutment. Each three-dimensional finite element model was created with the physical properties of the implant and surrounding bone This study simulated loads of 200N at the central fossa in a vertical direction (loading condition A), 200N at the centric cusp tip in a 15$^{\circ}$ inward inclined direction (loading condition B), or 200N at the centric cusp tip in a 30$^{\circ}$ outward inclined direction (loading condition C) respectively. Von Mises stresses were recorded and compared in the supporting bone, fixture, abutment and abutment screw. Results : 1. In comparison with the whole stress or the model 1 and model 2, the stress pattern was shown through th contact of the abutment and the implant fixture in the model 1, while the stress pattern was shown through the abutment screw mainly in the model 2. 2. Without regard to the loading condition, greater stress was taken at the cortical bone, and lower stress was taken at the cancellous bone. The stress taken at the cortical bone was greater at the model 1 than at the model 2, but the stress taken at the cortical bone was much less than the stress taken at the abutment, the implant fixture, and the abutment screw in case of both model 1 and model 2. 3. Without regard to the loading condition, the stress pattern of the abutment was greater at the model 1 than at the model 2. 4. In comparison with the stress distribution of model 1 and model 2, the maximum stress was taken at the abutment in the model 1. while the maximum stress was taken at the abutment screw in the model 2. 5. The magnitude of the maximum stress taken at the supporting bone, the implant fixture, the abutment, and the abutment screw was greater in the order of loading condition A, B and C. Conclusion : The stress distribution pattern of the internal connection system was mostly distributed widely to the lower part along the inner surface of the implant fixture contacting the abutment core through its contact portion because of the intimate contact of the abutment and the implant fixture and so the less stress was taken at the abutment screw, while the abutment screw can be the weakest portion clinically because the greater stress was taken at the abutment screw in case of the external connection system, and therefore the further clinical study about this problem is needed.
The purpose of this study is to examine the effect of partial osseointegration situation on bone loading patterns around two different free-standing screw shaped implants (Nobel Biocare, Gothenburg, Sweden and Degussa-Huls, Hanau, German). Two dimensional axisymmetric Finite element models of two implants(10mm length and 4mm diameter) were created according to different bone quantity, quality and osseointegration ratio in maxilla and mandible bone. At the same time uni-cortical and hi-cortical fixation were analyzed. Generally, full bond case showed less stress than partial bond case in overall area and mandibular model showed less amount of stress than that of maxilla model. Maximum stress of the Branemark implant is higher than that of ANKYLOS regardless of bonding ratio at crestal and apex region. However, more stress concentration was noted in ANKYLOS implant at screw body area especially in mandible. The effect of bicortical fixation on crestal bone stress reduction is dramatical in mandible however, there was no significant effect in maxillary case. The effect of partial bond on stress distribution was more significant at screw body and apex region than in crestal region. Partial bond cases demonstrated greater stress accumulation in trabecular bone than cortical bone. It is concluded that the more accurate model of implant and bone which affects stress and strain distribution is needed to mimic in vivo behavior of implants.
The osseointegrated implant conducts the stress directly to the bone due to lack of cushoning effect of periodontal ligament. So, the design and material quality of superstructure plays an important role in resolution and diffusion of stress. Recently, the various superstructures have been developed to improve esthetics and resolve various complicated conditions. The purpose of this study was to evaluate the stress induced by various system on the osseointegrated implant using UCLA abutment, EsthetiCone abutment, Anatomic abutment as well as Branemark conventional abutment. The stress distribution was evaluated by the photoelastic method which can simultaneously observe all around stress distribution. The superstructures embedded in epoxy resin specimen were loaded at various angle with a force of 15Kg to analyse the stress distribution of the fixture. The results of this study were obtained as follows : 1. Under vertical loading, the large and broad stress was distributed below the fixture in all systems. 2. The fringe order of the stress was increased in proportion to tillting the specimen. The largest stress was shown in 25 angled degree tilting case. 3. The Branemark conventional abutment showed the lowest value, and EsthetiCone abutment, Anatomic abutment and UCLA abutment showed the stress value in accending order.
PURPOSE. A novel retentive type of implant prosthesis that does not require the use of cement or screw holes has been introduced; however, there are few reports examining the biomechanical aspects of this novel implant. This study aimed to evaluate the biomechanical features of cementless fixation (CLF) implant prostheses. MATERIALS AND METHODS. The test groups of three variations of CLF implant prostheses and a control group of conventional cement-retained (CR) prosthesis were designed three-dimensionally for finite element analysis. The test groups were divided according to the abutment shape and the relining strategy on the inner surface of the implant crown as follows; resin-air hole-full (RAF), resin-air hole (RA), and resin-no air hole (RNA). The von Mises stress and principal stress were used to evaluate the stress values and distributions of the implant components. Contact open values were calculated to analyze the gap formation of the contact surfaces at the abutment-resin and abutment-implant interfaces. The micro-strain values were evaluated for the surrounding bone. RESULTS. Values reflecting the maximum stress on the abutment were as follows (in MPa): RAF, 25.6; RA, 23.4; RNA, 20.0; and CR, 15.8. The value of gap formation was measured from 0.88 to 1.19 ㎛ at the abutment-resin interface and 24.4 to 24.7 ㎛ at the abutment-implant interface. The strain distribution was similar in all cases. CONCLUSION. CLF had no disadvantages in terms of the biomechanical features compared with conventional CR implant prosthesis and could be successfully applied for implant prosthesis.
This study investigated the effects of cantilever length, location and load condition on stress distribution developed in the implants, prostheses and supporting tissues. The osseointegrated prostheses with two 10mm Branemark implants at 2nd premolar and 1st molar sites with cantilever extensions at 1st premolar, 2nd and 3rd molar sites were constructed. Under 100N, 200N of vertical and $45^{\circ}$ oblique loads at the cantilever pontics, stress distribution patterns and displacement were analyzed with three dimensional finite element method. The results were as follows : 1. The stress was concentrated at the joint of the cantilever pontic and implant superstructure, the neck of implant and the ridge crest near the cantilever But there was little load transfer to the lower supporting tissues of implants. 2. The implant near the cantilever was displaced inferiorly while the implant far from the cantilever was displaced superiorly. In horizontal direction the implants were displaced to the direction where the loads were applied, except the apexes of the implants. 3. In case of anterior cantilever, the stress and displacement were higher than the prosthesis connected with natural tooth. 4. The stress developed in the posterior cantilevered type was higher than in the anterior cantilevered type. The greastest stress was concentrated at the ridge crest near the posterior cantilever. 5. The longer the cantilever, the more the stress was developed and was concentrated at the joint of the cantilever pontic and implant superstructure. 6. Under oblique load, the stress was concentrated at the necks of implants and the ridge crests, but decreased at the joint of the cantilever pontic and implant superstructure than under vertical load.

Objectives: In mandibular edentulism, the treatment option with a two-interforaminal implant-retained bridge and a removable partial denture is rarely evaluated in literature. The aim of this in vitro study was to evaluate the stress distribution of this treatment option by comparing it with traditional treatment options with interforaminal implants in the edentulous mandible. Materials and Methods: Two interforaminal implants were placed in a formalin-fixed cadaver mandible, and overdentures with three different types of attachments were fabricated: (1) two ball attachments and an overdenture, (2) a Dolder bar and an overdenture, and (3) screw-retained two-implant inter-canine porcelain fused to a metal bridge and an implant-assisted removable denture (IARD) with precision attachments. Three biting conditions were generated for each denture type, and the strains were documented under vertical loading of 100 N. Results: The calculated strain values from measured strains in all measurement sites and loading conditions for the screw-retained two-implant intercanine porcelain fused to a metal bridge and a cast framework partial denture with precision attachments situation were lower than in the other scenarios (P<0.05). Conclusion: Within the limitations of the present study, it can be concluded that an IARD may be a reasonable and valuable alternative to ball attachments or a bar in two interforaminal implants, especially when the patients prefer to be able to show their teeth even when they take out their removable dentures.
