The main disadvantage of cement-retained implant restorations is their difficulty in retrievability. Advocates of cemented implant restorations frequently state that retrievability of the restoration can be maintained if a provisional cement is used. The purpose of this study was to find the optimal properties of provisional luting cements and the surface treatment of abutments in single implant abutment system. 30 prefabricated implant abutments, height 8mm, diameter 6mm, 3-degree taper per side, with light chamfer margins were obtained. Three commercially available provisional luting agents which were all zinc oxide eugenol type ; Cavitec, TempBond and TempBond NE were evaluated. No cement served as the control. TempBond along with vaseline, a kind of petrolatum (2:1 ratio) was also evaluated. Ten out of thirty abutments were randomly selected and abutment surfaces were sandblasted with $50{\mu}m$ aluminum oxide. Another ten abutments were sandblasted with $250{\mu}m$ aluminum oxide. A vertical groove, 1 mm deep and 5mm long was cut in each twenty abutments. Ten of them were sandblasted with $50{\mu}m$ aluminum oxide. The full coverage casting crowns were cemented to the abutments with the designated provisional luting agent. Specimens were stored in distilled water at $37^{\circ}C$ for 24 hours. Each specimen was attached to a universal testing machine. A crosshead speed of 0.5mm/min was used to apply a tensile force to each specimen. Within the limitations of this in vitro study, the following conclusions were drawn: 1. Tensile bond strength of provisional luting cements in no surface treatment decreased with the sequence of TempBond NE, TempBond, Cavitec, TempBond with vaseline, no cement. 2. Tensile bond strength more increased by surface treatment. Sandblasting with $250{\mu}m$ aluminum oxide exhibited the highest tensile bond strength in the abutment cemented with TempBond NE and sandblasting with $50{\mu}m$ aluminum oxide exhibited the highest tensile bond strength in cemented with TempBond. 3. In the aspect of a groove formation, tensile bond strength significantly increased in TempBond with vaseline only and the others had no significant effect on tensile bond strength.
PURPOSE. The aim of this study was to assess the effect of hemispherical dimple structures on the retention of cobalt-chromium (Co-Cr) crowns cemented to titanium abutments, with different heights and numbers of dimples on the axial walls. MATERIALS AND METHODS. 3.0-mm and 6.0-mm abutments (N = 180) and Co-Cr crowns were prepared. The experimental groups were divided into two and four dimple groups. The crowns were cemented by TempBond and PANAVIA F 2.0 cements. The retention forces were measured after thermal treatments. A two-way Analysis of Variance (ANOVA) and post-hoc Tukey HSD test were conducted to analyze change in retention forces by use of dimples between groups, as well as t test for the effect of abutment height change (α = .05). RESULTS. Results of the two-way ANOVA showed a statistically significant difference in retention force due to the use of dimples, regardless of the types of cements used (P < .001). A significantly higher mean retention forces were observed in the groups with dimples than in the control group, using the post hoc Tukey HSD test (P < .001). Results of t test displayed a statistically significant increase in the retention force with 6.0-mm abutments compared with 3.0-mm abutments (P < .001). The groups without dimples revealed adhesive failure of cements, while the groups with dimples showed mixed failure of cements. CONCLUSION. Use of hemispherical dimples was effective for increasing retention forces of cemented crowns.
Purpose: This study aimed to analyze the stress distribution and deformation in implant abutments made from titanium (Ti-6Al-4V), zirconia, and polyetheretherketone (PEEK), including their screws and fixtures, under various loading conditions using finite element analysis (FEA). Methods: Three-dimensional models of the mandible with implant abutments were created using Siemens NX software (NX10.0.0.24, Siemens). FEA was conducted using Abaqus to simulate occlusal loads and assess stress distribution and deformation. Material properties such as Young's modulus and Poisson's ratio were assigned to each component based on literature and experimental data. Results: The FEA results revealed distinct stress distribution patterns among the materials. Titanium alloy abutments exhibited the highest stress resistance and the most uniform stress distribution, making them highly suitable for long-term stability. Zirconia abutments showed strong mechanical properties with higher stress concentration, indicating potential vulnerability to fracture despite their aesthetic advantages. PEEK abutments demonstrated the least stress resistance and higher deformation compared to other abutment materials, but offered superior shock absorption, though they posed a higher risk of mechanical failure under high load conditions. Conclusion: The study emphasizes the importance of selecting appropriate materials for dental implants. Titanium offers durability and uniform stress distribution, making it highly suitable for long-term stability. Zirconia provides aesthetic benefits but has a higher risk of fracture compared to titanium. PEEK excels in shock absorption but has a higher risk of mechanical failure compared to both titanium and zirconia. These insights can guide improved implant designs and material choices for various clinical needs.

PURPOSE. Limited data is available regarding the differences for possible microleakage problems and fitting accuracy of zirconia versus titanium abutments with various connection designs. The purpose of this in vitro study was to investigate the effect of connection design and abutment material on the sealing capability and fitting accuracy of abutments. MATERIALS AND METHODS. A total of 42 abutments with different connection designs [internal conical (IC), internal tri-channel (IT), and external hexagonal (EH)] and abutment materials [titanium (Ti) and zirconia (Zr)] were evaluated. The inner parts of implants were inoculated with $0.7{\mu}L$ of polymicrobial culture (P. gingivalis, T. forsythia, T. denticola and F. nucleatum) and connected with their respective abutments under sterile conditions. The penetration of bacteria into the surrounding media was assessed by the visual evaluation of turbidity at each time point and the number of colony forming units (CFUs) was counted. The marginal gap at the implant- abutment interface (IAI) was measured by scanning electron microscope. The data sets were statistically analyzed using Kruskal-Wallis followed by Mann-Whitney U tests with the Bonferroni-Holm correction (${\alpha}=.05$). RESULTS. Statistically significant difference was found among the groups based on the results of leaked colonies (P<.05). The EH-Ti group characterized by an external hexagonal connection were less resistant to bacterial leakage than the groups EH-Zr, IT-Zr, IT-Ti, IC-Zr, and IC-Ti (P<.05). The marginal misfit (in ${\mu}m$) of the groups were in the range of 2.7-4.0 (IC-Zr), 1.8-5.3 (IC-Ti), 6.5-17.1 (IT-Zr), 5.4-12.0 (IT-Ti), 16.8-22.7 (EH-Zr), and 10.3-15.4 (EH-Ti). CONCLUSION. The sealing capability and marginal fit of abutments were affected by the type of abutment material and connection design.

Purpose. The aim of this study was to investigate to what extent cyclic load affects the screwless implant-abutment connection for Morse taper dental implants. Materials and Methods. 16 implants (SICvantage max) and 16 abutments (Swiss Cross) were used. The screwless implant-abutment connection was subjected to 10,000 cycles of axial loading with a maximum force of 120 N. For the pull-off testing, before and after the same cyclic loading, the required force for disconnecting the remaining 6 implant-abutment connections was measured. The surface of 10 abutments was examined using a scanning electron microscope 120× before and after loading. Results. The pull-off test showed a significant decrease in the vertical force required to pull the abutment from the implant with mean 229.39 N ± 18.23 before loading, and 204.30 N ± 13.51 after loading (P<.01). Apart from the appearance of polished surface areas and slight signs of wear, no visible damages were found on the abutments. Conclusion. The deformation on the polished abutment surface might represent the result of micro movements within the implant-abutment connection during loading. Although there was a decrease of the pull-off force values after cyclic loading, this might not have a notable effect on the clinical performance.
Statement of problem : Stress concentration on the neck bone affects the bone resorption, and finally the implant survival. Purpose: In order to examine the stress distribution on the neck bone and prosthesis abutment for implants, decreasing abutment sizes were used. Material and methods : Axisymmetric models were used to obtain the data required. These models were composed of 4mm implants with 3.4mm and 4mm abutments, 5mm implants with 3.4mm and 5mm abutments and 6mm implants with 3.4mm and 6mm abutments. All abutments were designed to received a 10mm high by 10mm diameter gold crown. Functional element analysis was used to obtain these results using data that consisted of 50 N vertical and 45 degree inclination forces. Results : 1. Changing the diameter of the abutment on the implant affects the effect of the inclination forces more than the effect of the vortical forces. 2. Changing the diameter of the abutment on the implant affect the effect of the inclination forces more than the effect of the vertical forces. 3. Experimentation showed that the larger diameter implants provided a decreased neck bone stress, whereas a larger diameter abutment provided a decrease marginal abutment stress. 4. Experimentation showed that the neck bone and abutment received more stress from inclination forces than vertical forces, Conclusions: By decreasing the size of the abutment on the implant we were able to diminishneck bone stress.
Purpose: This study aimed to comparatively evaluate the stress distribution of bones surrounding the implant system to which both titanium and polyetheretherketone (PEEK) abutments are applied using a three-dimensional finite element analysis. Methods: The three-dimensional implant system was designed by the computer-aided design program (CATIA; Dassault Systemes). The discretization process for setting nodes and elements was conducted using the HyperMesh program (Altair), after finishing the design of each structure for the customized abutment implant system. The results of the stress analysis were drawn from the Abaqus program (Dassault Systèmes). This study applied 200 N of vertical load and 100 N of oblique load to the occlusal surface of a mandibular first molar. Results: Under external load application, the PEEK-modeled dental implant showed the highest von Mises stress (VMS). The lowest VMS was observed in the Ti-modeled abutment screws. In all groups, the VMS was observed in the crestal regions or necks of implants. Conclusion: The bones surrounding the implant system to which the PEEK abutment was applied, such as the cortical and trabecular bones, showed stress distribution similar to that of the titanium implant system. This finding suggests that the difference in the abutment materials had no effect on the stress distribution of the bones surrounding implants. However, the PEEK abutments require mechanical and physical properties improved for clinical application, and the clinical application is thought to be limited.
PURPOSE. The purpose of this study was to compare computer-aided design/computer-aided manufacturing (CAD/CAM) abutment and prefabricated abutment in Morse taper internal connection type implants after cyclic loading. MATERIALS AND METHODS. The study was conducted with internal type implants of two different manufacturers (Group Os, De). Fourteen assemblies were prepared for each manufacturer group and divided into 2 groups (n=7): prefabricated abutments (Os-P, De-P) and CAD/CAM abutments (Os-C, De-C). The amount of axial displacement and the removal torque values (RTVs) were measured before and after cyclic loading (106 cycles, 3 Hz with 150 N), and the tensile removal force to dislodge the abutments was measured after cyclic loading. A repeated measures ANOVA and a pattern analysis based on the logarithmic regression model were conducted to evaluate the effect of cyclic loading on the axial displacement. The Wilcoxon signed-rank test and the Mann-Whitney test was conducted for comparison of RTV reduction% and tensile removal forces. RESULTS. There was no significant difference between CAD/CAM abutments and prefabricated abutments in axial displacement and tensile removal force; however, significantly greater RTV reduction% after cyclic loading was observed in CAD/CAM abutments. The correlation among the axial displacement, the RTV, and the tensile removal force was not significant. CONCLUSION. The use of CAD/CAM abutment did not significantly affect the amount of axial displacement and tensile removal force, but presented a significantly greater removal torque reduction% than prefabricated abutments. The connection stability due to the friction at the abutment-implant interface of CAD/CAM abutments may not be different from prefabricated abutment.
PURPOSE. The purpose of this study is to evaluate the stability of interface between Co-Cr-Mo (CCM) UCLA abutment and external hex implant. MATERIALS AND METHODS. Sixteen external hex implant fixtures were assigned to two groups (CCM and Gold group) and were embedded in molds using clear acrylic resin. Screw-retained prostheses were constructed using CCM UCLA abutment and Gold UCLA abutment. The external implant fixture and screw-retained prostheses were connected using abutment screws. After the abutments were tightened to 30 Ncm torque, 5 kg thermocyclic functional loading was applied by chewing simulator. A target of $1.0{\times}10^6$cycles was applied. After cyclic loading, removal torque values were recorded using a driving torque tester, and the interface between implant fixture and abutment was evaluated by scanning electronic microscope (SEM). The means and standard deviations (SD) between the CCM and Gold groups were analyzed with independent t-test at the significance level of 0.05. RESULTS. Fractures of crowns, abutments, abutment screws, and fixtures and loosening of abutment screws were not observed after thermocyclic loading. There were no statistically significant differences at the recorded removal torque values between CCM and Gold groups (P>.05). SEM analysis revealed that remarkable wear patterns were observed at the abutment interface only for Gold UCLA abutments. Those patterns were not observed for other specimens. CONCLUSION. Within the limit of this study, CCM UCLA abutment has no statistically significant difference in the stability of interface with external hex implant, compared with Gold UCLA abutment.
이 연구는 다수의 임플란트에 의해 지지되는 보철물과 단일 임플란트 보철물에서 호환 가능한 세 가지 종류의 나사의 풀림토크값을 측정하여 나사 결합부 안정성에 차이가 있는지를 알아보고자 하였다. 이를 위해, 네 개의 외부 육각 임플란트에 직접 연결되는 임플란트 상부구조물을, 아크릴릭 레진을 이용한 연결인상법으로 얻은 총 6개의 실험모형에 20 Ncm의 힘으로 조인 후, 각 나사의 풀림토크값을 총 2회 측정하였다. 사용한 지대주 나사는 토크타이트(TorqTite), 골드타이트(Gold-Tite), 그리고, 티타늄(Titanium) 나사였다. 또한, 단일 임플란트 수복의 경우 를 가정하여 총 5개의 실험모형 상의 2개의 임플란트에, 한 개의 지대주(GoldAdapt Engaging)를 다시 세 종류의 나사로 연결한 후, 각 나사의 풀림토크값을 총 2회 측정하였다. 나사의 풀림토크값의 비교를 위한 통계적 분석을 혼합모형(mixed model)을 이용하여 유의수준 .05에서 실시한 결과, 다수 임플란트 지지 상부구조물의 경우, 나사의 종류에 따른 풀림토크값은 통계적으로 유의성 있는 차이를 보이지 않았다(p>0.05). 그러나 단일 임플란트 지대주의 경우에는 통계적으로 유의성 있는 차이를 보였으며(p=0.0175), 토크타이트 나사(p=0.0462)와 티타늄 나사(p=0.0348)는 각각, 골드타이트 나사보다 유의성 있게 큰 풀림토크값을 보였으나, 두 나사 간에는 유의성 있는 차이가 없었다(p>0.05). 이상의 연구 결과로 보아, 서로 다른 종류의 나사가 나사 결합부의 초기 안정성에 미치는 영향은, 단일 임플란트 보철물의 경우에서와는 달리, 다수의 임플란트에 의해 지지되는 보철물의 경우에는 미미하다고 할 수 있다.
