회복지향질병관리(Illness Management and Recovery: IMR) 프로그램은 정신과 환자의 질병에 대한 자가 관리 및 회복을 촉진하기 위해 개발되었다. 본 연구에서는 한국판 IMR 프로그램이 입원한 만성 중증정신과 환자의 회복, 정신질환 지식, 희망에 미치는 영향을 살펴보았다. 연구 대상자는 조현병, 양극성장애, 조현형정동장애로 진단 받은 사람들로 실험군(51명)은 IMR 프로그램에 참여하였으며, 대조군(41명)은 기존의 치료를 유지하였다. 자료 수집은 2015년 5월부터 2016년 12월까지 이루어졌다. 정신건강회복척도, 정신장애 지식척도, 한국판 희망척도를 사전과 중재 이후에 실시하였다. 연구결과 IMR 프로그램에 참여한 사람들이 기존 치료를 유지한 대조군에 비해 자신의 회복 추구와 정신장애에 대한 지식이 향상하였다. 그러나 두 그룹 모두 희망에 대한 변화는 유의하지 않았다. 본 연구는 한국에서 처음 시행된 IMR 프로그램의 효과성 연구이다. 본 연구 결과는 한국판 IMR 프로그램이 우리나라 정신보건 임상에서 만성 정신과 환자의 회복을 촉진시키는 효과적인 중재로 활용될 가능성을 지지한다.
본 연구는 회복지향실천이 정신장애인의 정신과적 증상에 미치는 영향을 살펴보고 나아가 회복지향실천의 각 하위영역 중 어떠한 요인이 정신과적 증상에 영향을 미치는지 탐색함으로써 지역사회 정신보건기관의 회복지향실천 적용을 위한 실천적 의의를 제시하는데 그 목적이 있다. 이를 위해 전국 정신장애인 사회복귀시설을 이용하는 정신장애인 378명을 대상으로 설문조사를 실시하였으며, 상관관계분석 및 위계적 회귀분석을 실시하였다. 연구결과, 연령과 유병기간을 포함한 회복지향실천의 5개 하위요인 중 '삶의 목표', '관여', '치료대안의 다양성'이 정신장애인들의 증상을 완화시키는 것으로 확인되었다. 이러한 연구결과를 바탕으로 국내 정신보건기관이 회복지향실천의 가치와 요소를 반영하여 정신장애인의 회복을 촉진시킬 수 있도록 정신의학 및 사회복지학의 융복합적 의의를 제시하였다.
본 연구의 목적은 국립법무병원 간호사의 정신건강 회복에 대한 태도와 지각된 회복의 의미, 장애요인 및 촉진 요인을 파악하는 것으로 서술적 연구 설계가 사용되었다. 대상자는 C 지역 국립법무병원에서 근무하는 간호사 100명이다. 수집된 자료는 SPSS 26.0 WIN 프로그램을 이용하여 서술통계, t-test, ANOVA, scheffe-test로 분석하였으며 대상자가 서술한 응답은 내용분석을 이용하였다. 대상자의 정신건강 회복에 대한 태도는 긍정적이었으며(3.90±0.41), 정신 전문간호사 자격증 취득 여부(t=-2.06, p= 042), 중독정신간호사(t=-2.36, p= 020) 자격증 취득 여부에 따라 차이가 있었다. 간호사의 지각된 회복의 의미는 증상이 완화된 상태(42.0%), 장애요인은 가족의 무관심과 편견(31.0%), 촉진 요인은 약물복용, 정신 재활 교육 및 치료적 면담(33.0%)이었다. 국립법무병원 간호사의 정신건강 회복에 대한 이해를 증진시키기 위한 교육이 필요하며 정신장애 범죄자의 회복을 위해 가족 지원 체계와 치료 환경 구축을 위한 조직적 노력이 필요하다.
본 연구의 목적은 국립법무병원에 입원 중인 정신장애 범죄자의 정신 건강 회복에 대한 인식을 파악하는 것이다. 연구 대상자는 C 지역에 있는 국립법무병원에 입원 중인 정신장애 범죄자 91명이다. 자료수집은 정신 건강 회복에 대한 서면 질문지를 제공하여 자유롭게 기술하도록 하였으며 수집된 자료는 연구자 2인이 Krippendorff의 내용 분석 방법에 따라 분석하였다. 연구 결과, 정신 건강 회복에 대한 의미는 4개의 범주, 17개의 주제가 도출되었으며 4개의 범주는 '정신과적 증상으로부터 해방', '안정적으로 일상생활과 사회생활이 이루어짐', '희망적이고 의미 있는 삶을 살아감' 그리고 '퇴원 후 범죄 없이 살아가는 상태'이었다. 정신 건강 회복에 대한 촉진 요인 및 장애요인은 각각 4개의 범주, 14개의 주제가 도출되었으며 '치료적 요인', '개인적 요인', '대인관계 요인' 그리고 '환경요인'이었다. 본 연구 결과는 국립법무병원에 입원 중인 정신장애 범죄자의 정신 건강 회복을 위한 교육 및 중재 개발을 위한 기초자료로 사용될 수 있다.
This study analyzed how mental health affects stress in adolescents. The subjects of the study were data of about 60,000 middle and high school students living in the country, using cross-tabulation data and chi - square test to analyze the relationship between stress and mental health of adolescents. As a result of the research, it was found that the adolescents exposed to the excessive stress showed a poor correlation with the recovery of the fatigue, while the adolescents with relatively low stress showed the good recovery of fatigue. In the case of mental illness, depression or despair, the students who answered that they were stressed showed a lot of experience. In the case of suicide ideation, suicide attempt, and suicide attempt, the number of attempts to commit suicide was high when the level of perceived stress was high similar to the result of previous analysis. This finding confirms that stress is an important factor in the quality of life in adolescent mental health. It can be used as a strategy and business to prevent the problem of youth suicide in modern society in advance by stress management.
In mass casualty situation due to radiological accidents, it is important to start aggressive management with rapid triage decisions. External contamination needs immediate decontamination and internal contamination should be treated with special expertise and equipment to prevent the rapid uptake of radionuclides by target organs. Acute radiation syndrome shows a sequence of events that varies with the severity of the exposure. More severe exposures generally lead to more rapid onset of symptoms and severe clinical findings. After the massive exposure, various systems of the body reflect their severe damages that can lead to death within hours or up to several months. The disease progression has classically been divided into four stages: prodromal, latent, manifest illness, and recovery or death. Three characteristic clusters of symptoms including the hematopoietic syndrome, the gastrointestinal syndrome and the cerebrovascular syndrome are all associated with the acute radiation syndrome. The standard medical management of the patients with a potentially survivable radiation exposure includes good medical, surgical and supportive measures. Specific treatment with cytokines and bone marrow transplantation should be considered. The management of internal contamination is much the same as the treatment of poisoning. The standard decontamination should be applied to reduce uptake, and the chelating agents can be administered to enhance the clearance of radioisotopes. Radioactive iodine ($^{131}I$) as one of the nuclear fission products can increase the incidence of thyroid cancer in children. Potential benefit of potassium iodide prophylaxis is greater especially in neonates, infants and small children.
Purpose: As the daily life experiences of community-dwelling people with mental disabilities are closely correlated to mental health recovery, this study aimed to analyze their daily life experiences in depth to determine their significance. Method: Participants included eight community-dwelling people with mental disabilities. A total of 12 in-depth interviews were conducted once or twice per participant. The collected data were analyzed using the phenomenological analysis method described by Giorgi. Results: The daily life experiences of community-dwelling people with mental disabilities were categorized into the following four components: "encountering shocking reality in the outside world", "life tied down by the mental illness", "happiness and gratefulness encountered in daily life", and "wishing for a change and self-sustaining life". Conclusion: This study found that the recovery and welfare of community-dwelling people with mental disabilities were promoted and that these patients made necessary efforts to become a member of the local community. These findings could be used as a reference for establishing social welfare policies which enable people with mental disabilities to participate as functioning members of the community that facilitate the prevention of relapse, to promote their integration into the community. Furthermore, the present results will contribute to the development of nursing intervention programs to promote recovery and prevent relapse, to ultimately establish a mental health management system.
본 논문에서는 EPC Network의 응용 기술 분야에서 가장 중요한 기술로 연구되고 주목받고 있는 RFID를 활용하여 특수질병 감염환자를 효율적으로 관리할 수 있는 U-MAS (U-Medical Administrative Services)시스템을 설계하였다. U-MAS시스템은 질병관리본부에서 질병의 유형에 따라 감염환자들을 격리 수용과 치료, 회복, 퇴원, 격리병동 이탈 등을 집중 관리할 수 있도록 효율성을 증대시킬 수 있으며, 수작업과 단순한 컴퓨터 프로그램을 이용하는 현 수준의 질적 향상을 높이기 위해 RFID 태그를 이용하여 환자의 관리를 좀 더 면밀하고, 격리병동을 벗어나게 될 경우 관할 질병관리지역구에서 보다 빠르게 위치추적 등의 대응을 원활히 할 수 있도록 기여하는데 목적이 있다. 먼저, EPC 네트워크의 관련 기술과 모바일 RFID 시스템에 대한 관련 연구를 기술하고, U-MAS 시스템 설계를 제안한다. 제안된 U-MAS 시스템을 활용할 경우 전염성 질병환자에 대한 모니터링과 격리병동에 있는 환자가 무단으로 이탈하였을 때 위치추적에 소요되는 시간을 단축시키는 질병관리업무의 효율성을 개선할 수 있는 기대 효과를 제시하였다.

Many neurologic disorders manifest as psychiatric symptoms. Anti-N-Methyl-D-Aspartate (NMDA) receptor encephalitis is an autoimmune disease of the brain characterized by numerous neurological and psychiatric features. Despite being rare, its prevalence is rapidly increasing and early management is critical in ensuring successful and sustainable recovery. Therefore, the illness should be considered as a differential diagnosis when clinically assessing patients. This report presents a case of a female child who was hospitalized for acute psychiatric manifestations, which was later confirmed as anti-NMDA receptor encephalitis. She recovered relatively successfully after combined neurological and psychiatric treatment. This report provides information on the clinical course of early onset anti-NMDA receptor encephalitis, including treatment strategy and prognosis.
The purpose of this study was to produce proper nutrition and foodservice through positive intervention for diet-refusal patients who has difficulties in adaptation to hospital diet, and to contribute to hospital management with the focus of fast recovery. The survey was conducted using questionnaires between July 18 and August 17, 2000. The subjects were consisted of 24 diet-refusal patients who were hospitalized in St. Mary's Hospital except the patients hospitalized for the purpose of operation or pre-therapy tests. Also chart reviews were conducted. The statistical analysis of data was done by SAS/win 6.11 package to determine descriptive analysis and paired t-test and Pearson's correlation. The summary of study results is as follows : 1. Most of the subjects was receiving chemotherapy. The leading place for offering private food was from patient's home. Subjects worried about delayed recovery from illness due to nutritional problems caused by extreme fasting. The problems were in order of anorexia, nausea and vomiting during therapy process. 52.4% of subjects ate hospital diet after positive intervention. 2. Major reasons of hospital diet-refusal patients' dissatisfaction were in order of expenses, dissatisfaction, serum total protein levels were significantly lower(p<.05). 3. Serum Alb(p<.05), Hb(p<.05), Hct(p<.05) and TLC(p<.001) levels showed significantly negative correlations with fasting duration. With these results, it is concluded that a major cause of malnutrition among patients is to last fasting.
