Adventitial Cyst of the Radial Artery in the Wrist - A Case Report - (손목에서 발생한 요골 동맥의 외막 낭포 - 1예 보고 -)

  • Simple ganglions are most common benign tumor of the hand and wrist. However, cystic adventitial disease is an uncommon vascular anomaly first described in 1947 in the external iliac artery. It usually involves the popliteal artery, although other arteries and veins may also be involved. Radial artery adventitial cysts are found directly within the adventitia, whereas the more common wrist ganglions may extrinsically compress or adhere to the artery walls. The diagnosis is rarely made before surgery because of their similar appearance and location. The authors report a rare case of a 46-year old woman with mucoid adventitial cyst of the radial artery in the wrist.

  • This is to report a case of secondary reconstruction after partial mandibulectomy by using of marrow-cancellous bone and cortical bone harvested from the iliac crest in the case of an ameloblastoma on the mandible. According to the past experimental studies, the marrow and cancellous bone have the marked osteogenic potential of hematopoietic. And the cortical bone has the highest activity of bone induction, which is mediated through the action of bone morphogenic protein(BMP). This grafting technique, the combination of PMCB and cortical bone, has advantage in restoring lange defect of the mandible.

  • There are many residual nasal deformities following midface injury. The treatment of primary nasal deformities is delayed frequently due to several factors. And then, we usually perform the secondary nasal reconstruction. Autogenous bone is the material of choice for major nasal augmentation. It can be contoured appropriately and securely immobilized. It is rapidly incorporated as living tissue by the recipient bed. We used with iliac bone, contochondral graft and ear cartilage for secondary nasal augmentation and obtained the relatively fair results.

  • Background: To determine effects of mobilization with movements (MWM) application on stroke patient with hypomobility on sacroiliac joint. Methods: The subject was 47years old male who have left hemiplegia because of right intra-cerebral hemorrhage. The subject was have hypomobility on sacroiliac joint. MWM using during 4weeks and using Berg balance scale (BBS), Timed up and go (TUG), 10meter walking (10MW) test for evaluation. Results: The results of this study were summarized below; BBS score for evaluating balance ability was increased, and TUG time, 10MW test time was decrease. Conclusion: We consider that MWM application on stroke patient with hypomobility on sacroiliac joint is not only regain mobility on sacroiliac joint but also increase in balance ability and walking speed.

The Anatomical Study on the Distribution of Oviductal Vessels in Korean Native Pheasants (한국산 꿩 난관의 혈관분포에 관한 해부학적 연구)

  • The distribution of oviductal vessels of eight Korean native pheasants was observed after Latex casts of vessels. Latex was injected into thoracic aorta and posterior vena cava of these birds for cast preparation. The results were as follows: 1. The arteries suppling oviduct of the Korean native pheasants contained A. oviductalis cranialis, media, caudalis and vaginalis. The veins drainaging oviduct were stretched along the following ateries and all of the oviductal vessels were situated on the left side of the body. 2. The anterior oviductal artery arose from the pubic artery of the left external iliac artery and distributed to the infundibulum and the magnum of the oviduct. 3. The middle oviductal artery arose from the left sciatic artery and distributed to the isthmus and shell gland of the oviduct. 4. The caudal artery arose from the left internal pudendal artery of the median sacral artery and distributed to the caudal part of uterus and the cranial part of vagina. 5. The vaginal artery arose from the left internal pudendal artery of median sacral artery and distributed to the caudal part of vagina.

Surgical Management of Patent Ductus Arteriosus (동맥관개존증의 외과적 요법)

  • Seventeen cases of vascular injuries treated in Chungnam National University Hospital during the period from Apr. 1980 to Sep. 1988 were reviewed. Common causes of injuries were stab wound, automobile accidents and iatrogenic injuries. Of the 11 arterial injuries, 3 were femoral artery which was the commonest in frequency, the next was 2 cases of subclavian, common carotid, iliac artery. Of the 10 venous injuries, the subclavian vein and internal jugular vein were common in frequency. The most frequent type of injuries was laceration[13 cases in 21]. Vascular reconstruction was done by lateral suture repair in 8 cases, autogenous vein graft in 2, prosthetic vascular graft in 5, direct anastomosis in 1 case. Simple ligation was done in 5 cases. There was 1 case of visual field defect as a sequelae in right common carotid artery transaction and 1 case of mortality.

  • Chyloascities is an extravasation of milky chyle into the peritoneal cavity due to tumor , inflammation or rarely postoperative lymphatic trauma.It is an unusual complication that can lead to significant immunologic and nutritional consequences.We experienced one case of chyloascites after aorto-bifemoral bypass graft in a patient with aorto-iliac occlusive disease.The patient was a 62-year-old male, who suffered from severe progressive claudication for 5 months. A 16$\times$ 8mm gelsealed Dacron-Y shaped graft was used in arterial reconstruction. A bloody-milky fluid was drained through the operative wound from 3days after operation and evaluated biochemically.Diagnosis of chyloascites was made with repeated paracentesis and examination of the fluid.After Total parenteral nutrition[T.P.N] for 3 weeks from 6days after operation, chyloascites was controlled sufficiently and maintained a good graft-patency in abdominal sonogram.

  • Complex regional pain syndrome Type II(CRPS) can be diagnosed by new IASP criteria in 1994. Sympathetically maintained pain may or may not be present in a patient with complex regional pain syndrome. We experienced a CRPS Type II patient who has sympathetically maintained pain as a major painful nature developed after right multiple iliac bone fracture, right femoral artery thrombosis and lumbosacral plexus injury. Combination treatment with L2, L3, L4 sympathetic ganglion block and continuous lower thoracic epidural block for 30 days were tried to get long term effect. The patient had signs of successful. sympathetic denervation of the right foot. After that pain relief was sustained until three month later.

  • M$\ddot{u}$ller-Weiss disease is a rare disease presenting severe deformity of navicular with osteoarthritis. This disease can be confused with Kohler's disease, but has a different entity. A 68 years-old male with painful mid-foot deformity had diagnosed as M$\ddot{u}$ller-Weiss disease, and triple arthrodesis with autogenous iliac bone graft had been performed on his left foot. Here is a report of our case with brief review of this disease.

  • The giant-cell tumor is uncommon. It occurs in the long bones and vertebral localization is much less common, particularly in thoracic spine. We present a case of a 37-year-old man suffering from severe back pain. Affected vertebral bodies were removed by transthoracic approach and the spine was reconstructed with iliac bone autografts and internal fixation device(Kaneda) between T 8 and T 11. Histologoical diagnosis was giant-cell tumor, and pertinent literature was reviewed.
