복직근 유리피판 거상 후 합병된 대퇴 신경손상 1례 (Femoral Nerve Injury after Rectus Abdominis Muscle Slap Harvesting: A Case Report)

  • Purpose: The Rectus abdominis muscle free flap is utilized in various reconstruction surgeries due to easiness in harvesting, consistency of vascular pedicle and reduced donor site morbidity. But rarely, femoral nerve injury during rectus abdominis harvesting can be resulted. We report a case of femoral nerve injury after rectus muscle harvesting and discuss the injury mechanism with the follow-up process of this injury. Methods: To reconstruct the defect of middle cranial base after wide excision of cystic adenocarcinoma of the external ear, rectus muscle free flap was havested in usual manner. To achieve a long vessel, inferior epigastric artery was dissected to the dividing portion of femoral artery and cut. Results: One week after the surgery, the patient noted sensory decrease in the lower leg, weakness in muscle strength, and disabilities in extension of the knee joint resulting in immobilization. EMG and NCV results showed no response on stimulation of the femoral nerve of the left leg, due to the defects in femoral nerve superior to the inguinal ligament. With routine neurologic evaluations and physical therapy, on the 75th day after the operation, the patient showed improvement in pain, sensation and muscle strength, and was able to move with walking frame. In 6 months after the operation, recovery of the muscle strength of the knee joint was observed with normal flexion and extension movements. Conclusion: Rarely, during dissection of the inferior epigastric artery, injuries to the femoral nerve can be resulted, probably due to excessive traction or pressure from the blade of the traction device. Therefore, femoral nerve injury can be prevented by avoiding excessive traction during surgery.

관절계 역학적 특성의 정량적 평가방법 (A New Method for the Identification of Joint Mechanical Properties)

  • The purpose of this paper is to suggest a practical and simple method for the identification of the joint mechanical properties and to apply it to human knee joints. The passive moment at a joint was modeled by three mechanical parts, that is, a gravity term, a linear damper term and a nonlinear spring term. Passive pendulum tests were performed in 5 fat and 5 thin men. The data of pendulum test were used to identify the mechanical properties of joints through sequential quadratic programming (SQP) with random initial values. The identification was successful where the normalized root-mean-squared (RMS) errors between the simulated and experimental joint angle trajectories were less than 10%. The parameter values of mechanical properties obtained in this study agreed with literature. The inertia, gravity and the damping constant were greater at fat men, which indicates more resistance to body movement and more energy consumption fer fat men. The suggested method is noninvasive and requires simple setup and short measurement time. It is expected to be useful in the evaluation of joint pathologies.

Comparison between Isokinetic Peak Torque and Isotonic 1RM on the Knee Joint

  • Objective: Resistance exercise is a necessary element to improve quality of life, and measurement and evaluation of muscle strength provide important information for prescription and management of rehabilitation and exercise programs. This study analyzed the correlation between direct and indirect 1RM for isokinetic maximum torque of the knee joint in order to provide useful information in the field of exercise programs. In addition, the flexion-extension ratio and the difference in left-right deviation were verified. Design: A cross-sectional study Methods: The subjects of this study were 33 healthy adult men and women without medical problems who participated in the health exercise class program at S University in Seoul. The correlation between isokinetic maximum torque and direct and indirect 1RM was analyzed, and a dependent t-test was performed to analyze the flexion-extension ratio and left-right deviation. Results: There was a high correlation between the isokinetic maximum torque and direct and indirect 1RM, and no statistically significant difference was shown between the test methods in the analysis of the flexion-extension ratio and left-right deviation. Conclusions: Isokinetic muscle function measuring equipment is expensive, so it is difficult to use it in local exercise rehabilitation and training sites. Through this study, it was found that direct and indirect 1RM isokinetic maximum torque showed a high correlation, and there was no difference in evaluating muscle function such as flexion-extension ratio and left-right deviation. Therefore, it is considered that the muscle function evaluation using 1RM in general field can be usefully utilized.

건강한 성인의 슬개건 반사 시 무릎 감쇠효과를 고려한 대퇴사두근의 근력 및 근활성도 예측 (Identification of Muscle Forces and Activation of Quadriceps Femoris Muscles of Healthy Adults Considering Knee Damping Effects during Patellar Tendon Reflex)

  • 인체 해석모델은 주로 인간이 의식적으로 행하는 운동을 중심으로 발전해 왔다. 의식적 운동과 달리 슬개건 반사는 뇌를 거치지 않고 일어난다. 본 연구는 건강한 성인의 슬개건 반사로 인한 대퇴부의 근력과 근활성도를 해석적으로 예측하고자 하였다. 해석 모델은 시상면에서 평면운동을 하고, 앉은 자세에서 상체와 허벅지를 고정시켜 종아리만 진자 운동이 가능하도록 모델링 하였다. 무릎은 레볼루트 조인트로 모델링 하였고, 발목관절은 고정시켜 종아리와 발을 하나의 강체로 가정하였다. 근력은 Mamizuka 의 실험 결과로부터 얻은 운동학 정보를 이용하여 역동역학 해석을 통해 구하였으며, 근활성도는 Hill-type 근육 모델을 이용하여 예측하였다. 해석 결과는 실험결과를 통해 검증되었다.

인체근육 구조 인간형 로봇의 모델링 및 구현 (Modeling and Development of Human-Muscle Type Humanoid)

  • Many human-body motions such as walking, running, jumping, etc. require a significant amount of power. To achieve a high power-to-weight ratio of the humanoid robot system, this paper proposes a new design of the bio-mimetic leg mechanism resembling musculoskeletal system of the human body. The hip joints of the system considered here are powered by 5 human-like bi-and mono-articular muscles, and the joints of knee and ankle are redundantly actuated by both bi-articular muscles and joint actuators. The kinematics for the leg mechanism is derived and a kinematic index to measure force transmission ratio is introduced. It is demonstrated through simulation that incorporation of redundant muscles into the leg mechanism enhances the power of the mechanism approximately 2 times of the minimum actuation.

동적자세와 피부분절을 이용한 기능성 밀착의복 제작 프로세스 (Engineering design process of tight-fit sportswear using 3D information of dermatomes and skin deformation in dynamic posture)

  • The primary goal of this study was to provide a systematic methodology of utilizing 3D technology for tight-fit performance sportswear using information of skin deformation in various posture. Technical tools used in this study are Cyberware whole body scanner, RapidForm2004, 2C-AN 3D pattern development program, and YukaCAD. Analysis of the 3D skin deformation while knee joint was bent from $0^{\circ}$ to $60^{\circ}$ revealed that the length of dermatomes L4 was remained consistent during knee bending. Therefore, L4 was chosen as a major cutting line. To develop a highly ergonomic pattern, replicas of static and dynamic postures were developed and integrated using two methods, one is morphing method (Sqirlz Morph), and the other is AutoCAD. Experimental tight-fit garments called 'Derm-Mov Pattern' was designed using dematomes L4, L2, and inner line under knee and compared with four other patterns. As results, AutoCAD was appropriate as a integrating method of various postures. In wear test, 'Derm-Mov Pattern' was rated high (p < .001), in terms of pressure comfort especially around front crotch area. However, wear sensation was not signipicantly different in other area due to highly extensible property of materials. Pressure distribution was relatively even in these experimental garments.

여자 유도 허리후리기 기술의 운동학적 특성분석 (A Kinematic Analysis of Harai-Goshi(Hip Throw) in Judo)

  • The purpose of this study was to perform a kinematic analysis of Harai-goshi(hip throw) in superior female judo players. From our analysis, it appeared desirable for the player to shorten the duration of the technique for maximum results. It was also desirable for the player to draw the opponent backward while simultaneously maneuvering her close to her own body. A turning movement of the body toward the left side was found to be required for the technique, during which the player must lower the position of the center of gravity. During the subsequent angular change of the elbow, the player winds the elbow and, by moving the left shoulder, pulls the opponent toward her. The player lowers the body center by bending the left knee and letting the right knee extend, while turning the upper body and bending the hip joint. The player then draws the opponent in, contacts the opponent closely, turns, and stretches the right knee and hip joint rapidly and concisely, completing the move; it was also found to be desirable for the angular velocity to be increased for maximum efficacy.

슬대퇴관절의 Merchant View에서 중심 X선 위치에 따른 일치각 평가 (Assessment of Congruence Angle according to the Central X-ray in the Merchant View of Patellofemoral Joint)

  • The Precise measurement of the knee's Congruence angle is required for diagnosis of patella dislocation. The purpose of this study is to consider the distortion diagram and usefulness about the test of the bilateral side and one side through the evaluating congruence angle from Merchant view of patellofemoral joint to central X-ray. We've evaluated the sulcus angle and Congruence angle following central X-ray's changes when we took Merchant view of patellofemoral joint aimed to right lower limb from human whole body phantom. The subject of the evaluation was classified as normal group and varus group, and the varus group has experimented with External rotation of legs as 15° and 30°. When normal groups result as 0, 7.5, 15 cm, it was measured as 17.25 ± 1.34°, 19.60 ± 1.41°, 20.55 ± 1.77° each. The gap between minimal and maximum angle was 3.3°, and the value was shown as getting bigger when it further away from the centeral X-ray. When Congruence angle in 15° varus group was 0, 7.5, 15 cm, it was each 16.45 ± 1.34°, 17.10 ± 0.99°, 17.80 ± 1.13°. And when Congruence angle in 30° varus group was 0, 7.5, 15 cm, it was measured each 18.35 ± 1.63°, 18.95 ± 1.06°, 19.60 ± 1.41°. The difference between minimum and maximum of angle in 15° varus group and 30° varus group was each 1.35° and 1.25°, the angles have shown as increasing the further away from the center. The patellofemoral joint showed that the congruence angle increases as the further away from Central X-ray, and also it is judged that is possible to change the congruence angle by a degree of varus knee. Thus, accurate measurement of the congruence angle is shown to advantage that methods for examining by split each side at two times than methods for examining both sides at a time. Therefore, it is expected to helpful on the diagnostic side of patella dislocation and subluxation.

불규칙적인 격자망을 이용한 생체 연조직의 곡면변형률 측정 (Measurement of Surface Strain on Soft Biological Tissues Using Irregular Grid Pattern)

  • 본 연구에서는 광학적 방법으로 곡면변형률을 측정하고자 할 때, 소재 표면이 임의의 곡면이거나 요철이 많은 경우, 부식이나 오염으로 표면이 불량한 경우, 그리고 고무나 폼, 생체 조직 등과 같이 규칙적인 격자망을 인쇄하기 어려운 경우에도 적용할 수 있는 자동 곡면변형률 측정시스템 'ASIAS-bio'를 개발하였다. 이 시스템은 시편의 재질과 표면상태, 격자 패턴과 크기, 격자 마킹 방법, 변형 정도 등 측정 조건에 관계없이 사용이 가능한 것이 특징이다. 우선 그 신뢰성을 평가하기 위하여 금속판재 성형제품의 변형률 분포를 상용 장비로 측정한 결과와 비교하였다. 또한, 돼지 어깨 관절막, 인체 무릎 피부 등 연질 생체 조직의 변형 측정에 적용함으로써 본 시스템의 유용성을 확인하였다.

능동형 대퇴의지 시스템의 설계 및 최적화 연구 (Design and Optimization of Active Transfemoral Prosthesis System)

  • 본 논문에서는 능동형 대퇴의지 시스템의 설계 및 최적화에 대한 연구를 진행하였다. 본 연구의 능동형 대퇴의지 시스템은 무릎부의 1 자유도를 통해 슬관절의 움직임을 모사하는데, 실제 다리의 기능을 대체하는 것이므로 사람의 다리 무게와 최대한 유사해야 하며, 경량화가 중요한 요소라고 할 수 있다. 본 연구에서는 기존에 개발된 3-링크 구조의 능동형 의지 시스템의 문제점을 보완하기 위해 기어 구동 방식의 능동형 대퇴의지 시스템을 설계 및 제작하였고, 사용자의 만족감 향상 및 피로도 저감을 위해 최적화를 통한 경량화를 진행하고 유한요소해석을 통해 응력 및 변위 특성을 분석하고 안정성을 검증하였다. 또한 능동형 의지 시스템이 사용자의 보행 의도에 맞추어 작동하기 위해 의지의 인공 발에 스트레인 게이지를 부착하여 보행시 발의 앞, 뒷부분에 각기 다르게 가해지는 하중을 측정, 보행 주기를 판단할 수 있도록 한다. 이 때 스트레인 게이지는 미세한 변형에도 민감하게 반응하므로 유한요소해석을 통해 적절한 부착위치를 결정하고, 실제 제작 및 실험을 통해 안정성 및 보행주기 판단 여부를 검증한다.
