Purpose: This study aimed to derive the elements for effectively improving the management of emerging infectious diseases in the emergency room in large general hospitals. Methods: This research involved an analytic hierarchy process analysis of 10 experienced nurses working in a large general hospital located in the Seoul metropolitan area. The weighted values of the criteria for evaluating the importance of the management elements of patients with emerging infectious diseases in the emergency room of large general hospitals were derived using an analytic hierarchy process survey. Results: The results of the analytic hierarchy process analysis showed that experts considers responsiveness (0.291) as most important with respect to the criteria for the policy for isolation and control of infectious patients. The order of importance of the criteria for the policy for isolation and control of infectious patients were as follows: responsiveness (0.291), economical efficiency (0.257), feasibility (0.242), and sustainability (0.209). Subsequently, the weighted values of the management of the policy elements were determined based on the derived importance of the criteria mentioned above. These were, in order of priority, the construction of communication channels with the government authorities when dealing with emerging infectious diseases (15.9%), the equipment and provision of personal protective equipment to protect nurses from infection and dissemination of material (14.6%), and the operation of the isolation room and patient separation systems (14.1%). Conclusion: This study suggests critical strategies for infection control during a pandemic of emerging infectious disease.
The purpose of this study is to propose a design methodology to build temporary isolation rooms when infectious diseases suddenly occur in a certain region, such as the case of MERS-Cov in South Korea in 2015. Although most big hospitals usually have isolation rooms, they are expensive and dangerous to run such facilities on normal and typical days. To deal with these problems in this research, shipping containers are chosen as devices used to build the temporary isolation rooms near the original hospital. To do so, firstly, a prototype for the temporary isolation room was designed with the three part modules. The first part is for the medical team; the second part including the isolation rooms is for patients; the third part is for medical selection rooms to test the specimens. Secondly, the plan was compared with the MERS-Cov infection control guidelines. Finally this prototype is applied into the Yong-in Yon-sei severance hospital and then evaluated through a CFD simulation using STAR-CCM+(ver.9.06) for checking infectious bacterium movement in this prototype. The result showed that the prototype is effectively safe for patients tested as negative, patients waiting to be tested, and the medical team.
Purpose: The 2015 Middle East Respiratory Syndrome (MERS) outbreak and the recent COVID-19 pandemic have highlighted the lack of negative pressure isolation rooms and the fragility of the healthcare system. The need for healthcare facility transformation for respiratory infectious diseases has become more prominent due to COVID-19, and the purpose of this study is to provide a foundation for the rapid, economical, and safe construction of negative pressure isolation wards. Methods: This study analyzes the current status of hospitals that have been converted to negative pressure isolation rooms, and provides architectural plans and examples to provide a reference for bedroom change. Research data of this study have been obtained by analyzing the drawings of negative pressure isolation wards of nationally designated inpatient treatment beds and urgent isolation beds. In addition, the relevant literature of urgent isolation beds has been analyzed to derive bedroom change type. Result: In this study, a total of 21 isolation bed conversion methods have been presented. Implications: In order to change efficiently from a general ward to an isolation ward, it is necessary to consider the actual hospital's infectious disease transmission patterns and facility conditions.
Purpose: This study aims to provide basic data for the future construction plans of the Infectious Disease Hospitals by analyzing the area composition and required room ratios in the wards and ICU of currently under-construction infectious disease hospitals. Methods: 3 Methods have been used in this paper. 1) This study conducted a literature review on major considerations and related guidelines for hospitals specializing in infectious diseases using existing data. 2) Based on the objects and activities of the hospital space, zones and areas were set for each department according to infection control. 3) Based on the established zones and areas, basic plan drawings of three hospitals specializing in infectious diseases currently under construction were collected and architectural drawing analysis was performed. Results: 1) Infectious Diseases Hospital must have a spatial organization that can accommodate patient isolation, infection control, efficiency of medical service, and changes. 2) Zones for infection control are divided into negative pressure and non-negative pressure zones based on airborne precaution isolation. It is divided into clean and contaminated zone according to class of cleanliness by Aseptic technique. Areas are classified by objects (patients, healthcare workers, supplies) and activities (access, medical treatment, support), and a system for organizing space is established based on this. 3) By analyzing the area composition of each departmental area, each required room, and each required space in the wards and intensive care units, it provides basic data for the spatial organization for architectural planning of the infectious disease hospital. Implication: It can be used as basic data when planning related facilities by analyzing the characteristics of the space plan of the required room according to the relationship between activities, movement lines, and operation plans based on user behavior.
Purpose: In this study influenza A (H1N1) infection control performance and stress in nurses were surveyed, to identify factors influencing stress for the nurses, and to provide basic materials for promoting infectious disease control by nurses. Method: The participants were 447 nurses who had worked at isolation clinics and/or isolation rooms in hub hospitals in the Honam region during the period of the outbreak of 2009 pandemic influenza A (H1N1) virus. Data were collected in February 2010 using self-report questionnaires. Results: The performance of participants from clinics was high for 'helping patients wearing masks', but low for 'maintaining distance over 1 meter among waiting patients'. The performance of participants from hospital rooms was high for 'putting a surgical mask on a patient going out of the room', but low for 'keeping patients' family out of the room'. The participants' stress was higher in those younger and less experienced nurses, those working at a hospital with 500-999 beds, those working at both isolation clinics and rooms, and those working at a clinic longer. Conclusion: In order to lower nurses' stress from working at isolation clinics/rooms for infectious diseases, we may need to deploy experienced nurses and limit their working days to five or less.
Purpose: The purpose of the study is to suggest the main functions and implications of public hospitals to effectively respond to the future epidemic crisis based on analyzing the accessibility to designated Coronavirus Disease 2019 (COVID-19) medical institutions of Seoul and examining the main features of the quarantine of Seoul municipal hospitals. Method: To analyze the response and function of Seoul municipal hospitals, we reviewed the Infectious Disease Control and Prevention ACT, 258 articles of Seoul Metropolitan Government press releases from January to the end of April, 48 articles of Seoul Metropolitan Government's daily newsletters, 2019 Health Bureau Budget report. We also referred to internal data of Seoul Children's Hospital, Seoul Seobuk Hospital, and Seoul Eunpyeong Hospital during the same period. Besides, the accessibility to medical institutions was analyzed by using the COVID-19 data which was announced daily basis. Results: The accessibility of COVID-19 patients living in the Southeastern part of Seoul to a medical institutions was 16.2km on a distance basis, and it was the lowest accessibility among four regions of Seoul since it took about 40 minutes by car. On the other hand, patients living in the Northeast part had the highest accessibility, as the access to medical institutions was 10.7km and 27 minutes by car. Also, the main functions of the municipal hospital of Seoul against COVID-19 were to shift the public hospital function to COVID-19 patients only hospitals, to perform the epidemiological investigation by medical doctors, and to support the operation of self-isolation facilities, community treatment centers and triage rooms of community health centers. Even though the interest to the mental hospital is recently increased, the mental hospitals are not well developed due to the pending arrangement of the concept of returning to the society and the system of local communities. Also, the hospitalism due to the isolation of patients and the long term stay in hospitals has become an issue. Despite the present circumstances, the prototype of mental health care in many field are investigated and progressively fixed. This study attempts to examine the current status of domestic and foreign mental institution and to establish the design guideline of the wards of the mental hospitals.
Purpose: This study shows a changing tendency of Japanese psychiatric hospitals, presenting "Visions in Reform of Mental Health and Medical Welfare", changed to "Hospitalized Medical Treatment to Living in the Community" and reducing the number of beds since 2004. Methods: This study uses a documentary survey, especially Jananese "Visions in Reform of Mental Health and Medical Welfare System" and a field survey on hospital built for early treatment and return to society after 2004. Results: Change tendencies of psychiatric hospitals are 1) diversification of isolation room and ward, 2) cluster of patient's room, 3) phased organization of space, 4)individualization. Implication: This study will be an important data for researching plan of Korea psychiatric hospital which needs to change by global tendency.
In the past 20 years, seismic isolation has see a variety of applications in design of structures to mitigate seismic hazard. In particular, isolation has been seen as a means of achieving enhanced seismic performance objectives, such as those for hospitals, critical emergency response facilities, mass electronic data storage centers, and similar buildings whose functionality following a major seismic event is either critical to the public welfare or the financial solvency of an organization. While achieving these enhanced performance objectives is a natural (and oftentimes requisite) application of seismic isolation, little attention has been given to the extension of current design practice to isolated buildings which may have more conventional performance objectives. The development of a rational design methodology for isolated buildings requires thorough investigation of the behavior of isolated structures subjected to seismic input of various recurrence intervals, and which are designed to remain elastic only under frequent events. This paper summarizes these investigations, and proposed a consistent probabilistic framework within which any combination of performance objectives may be met. Analytical simulations are presented, the results are summarized. The intent of this work is to allow a building owner to make informed decisions regarding tradeoffs between superstructure performance (drifts, accelerations) and isolation system performance. Within this framework, it is possible to realize the benefits of designing isolated buildings for which the design criteria allows consideration of multiple performance goals.
Purpose : This study aimed to identify the factors affecting coronavirus disease 2019 (COVID-19) infection control practices of nurses in negative pressure isolation rooms. Methods : The participants were 150 nurses working in three hospitals with negative pressure isolation rooms. Data were collected using structured questionnaires and analyzed using an independent t-test, one-way ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression analysis using the SPSS/WIN 26.0 program. Results : Factors significantly influencing the subject's COVID-19 infection control practice include Being married (𝛽=.18, p =.016), working in a ward with negative pressure isolation rooms (𝛽=.44, p <.001), working in an ICU with negative pressure isolation rooms (𝛽=.27, p =.010), COVID-19 infection control attitude (𝛽=.28, p =.001), anxiety for COVID-19 (𝛽=.30, p <.001). The explanatory power of these variables for COVID-19 infection control practice was 24.6% (F=8.67, p <.001). Conclusion : It is expected that strategies that utilize positive attitudes which believe that COVID-19 disease may be overcome by COVID-19 infection control practice will help improve emerging infectious diseases infection control practice.
