Evaluating the Breastfeeding Information Websites for Korean Adolescents (한국청소년의 모유수유 관련 건강교육을 위한 e-정보전략)

  • Internet websites are excellent resources for younger people to gain information related to breastfeeding. So in this study, we evaluated the breastfeeding knowledge of university students, as well as the currently available website information on breastfeeding. Among the criteria, the six most important evaluation categories for the breastfeeding information on websites were selected and identified. The study participants completed the modified Williams and Hammer's tool, in order to evaluate their breastfeeding knowledge. An additional tool took into consideration the website breastfeeding information criteria, to evaluate the Korean websites. This 34 item tool was categorized into milk supply, comfort during breastfeeding, health of the baby and mother, social support, presentation of message, and products or financial motivation. Thirty-seven websites related to breastfeeding were evaluated, and the results were then discussed by nursing students. The students in this study were not knowledgeable on breastfeeding management with regard to the expression and storage of breast milk, the prevention and care of sore nipples, or use of breastfeeding aids. The breastfeeding information found on the Korean websites received positive evaluations for milk supply, comfort during breastfeeding, and health of the baby and mother. However, the educational content on social support, the message presentation, and the product areas on websites were rare or required additional information from breastfeeding specialists. In conclusion, based on our empirical survey, we need to design websites and evaluation tools for accurate website information regarding the social and economic benefits of breastfeeding, so that Korean women are better motivated to breastfeed by the information they obtain through the Internet.

Analysis of The Management of Three Tertiary General Hospital(2011 to 2013)

  • Purpose. For more effective hospital management, it analyzes the trend through general characteristics, balance sheet, income statement, and financial ratio analysis, grasps the causes of the problems, and analyzes management of the hospital in order to use the result as baseline data for development of the hospital in the future. Methods. The collected data of 3 years from 2011 to 2013 about 3 tertiary hospitals in metropolitan cities from Alio (provider of public institution information; www.alio.go.kr), Health Insurance Review & Assessment Service (www.hira.or.kr), and the website of the Ministry of Health and Welfare (www.mw.go.kr) were analyzed and general characteristics, balance sheet, income statement, and financial ratio, analysis are used as data. Results & Conclusions. From the result of data analysis from 2011 to 2013, general characteristics, balance sheet, income statement, financial ratio analysis, and pie charts could lead to conclusions as follows. In the result of comprehensive analysis, the 3 tertiary hospitals showed increase of fixed expense due to extension of the buildings and so did the scale of fund and asset. Although medical revenue increased, the margin of increase for medical expense was greater than that of medical revenue, which consequently led to loss. In prediction for the 3 tertiary hospitals based on characteristics so far, it is expected to see improved revenue structure after building extension is completed, but it is necessary to exert management effort to maintain its optimal level by enhancement in stability of management and inventory turnover through management of inventories.

Korean Translation of the CARE guidelines (CARE 지침 한국어판 제작)

  • Objectives : The aim of this study is to develop the official Korean translation of the CARE guidelines. Methods : CARE guidelines were developed in 2013 though a consensus of experts led by the CARE group. It is the first reporting guideline for case reports that includes a 13-item checklist. We contacted the main author of the original manuscript and obtained permission to develop the Korean translation. The translation process followed a systematic protocol designated by the CARE group which included an independent back translation and confirmation from the original author. Results : The Korean translation of the CARE guidelines was completed with approval from the original author. The Korean checklist is posted on the official website (www.care-statement.org) and the final translation of the manuscript is introduced in this article as an appendix. Conclusions : We hope that this will help introduce the CARE guidelines to Korean authors and other stakeholders, induce a more active use of reporting guidelines, and eventually contribute to the overall improvement of the reporting quality of case reports published in Korean literature.

Need Assessment for Smartphone-Based Cardiac Telerehabilitation

  • Objectives: To identify the current status of smartphone usage and to describe the needs for smartphone-based cardiac telerehabilitation of cardiac patients. Methods: In 2016, a questionnaire survey was conducted in a supervised ambulatory cardiac rehabilitation (CR) program in a university affiliated hospital with the participation of heart failure or heart transplantation patients who were smartphone users. The questionnaire included questions regarding smartphone usage, demands for smartphone-based disease education, and home health monitoring systems. Results were described and analyzed according to principal diagnosis. Results: Ninety-six patients (66% male; mean age, $5{\pm}11$ years), including 56 heart failure and 40 heart transplantation patients, completed the survey (completion rate, 95%). The median daily smartphone usage time was 120 minutes (interquartile range, 60-300), and the most frequently used smartphone function was text messaging (61.5%). Of the patients, 26% stated that they searched for health-related information using their smartphones more than 1 time per week. The major source of health-related information was Internet browsing (50.0%), and the least sought source was the hospital's website (3.1%). Patients with heart failure expressed significantly higher needs for disease education on treatment plan, home health monitoring of blood pressure, and body weight (${\chi}^2=5.79$, 6.27, 4.50, p < 0.05). Heart transplantation patients expressed a significant need for home health monitoring of body temperature (${\chi}^2=5.25$, p < 0.05). Conclusions: Heart failure and heart transplantation patients show high usage of and interest in mobile health technology. A smartphone-based cardiac telerehabilitation program should be developed based on high demand areas and modified to suit to each principal diagnosis.

Status of the Development and Utilization of Critical Pathways in Medical Institutions in South Korea (국내 의료기관의 표준진료지침 개발 및 운영 현황)

  • Purpose: This study assessed the status of the Development and Utilization of critical pathways (CP) in South Korea. Methods: We surveyed 195 hospitals obtained on the Korean Hospital Association website. Data were collected using structured questionnaires for staff members in charge of CP management personnel in these hospitals. The questionnaire included CP developed by the institutions, the coverage rates and completion rates of CP in the current year, and management indicators related to CP. The questionnaire also included CP support systems and content within the institutions and questions regarding the advantages of CP utilization and obstacles associated with the CP development process. Results: Analysis of the responses from 70 hospitals (35.9% response rate) showed that a total of 1,370 CP sets were developed. The number of CP related to surgery departments was 365 (26.6%), and CP related to surgery and procedure was 1,093 (79.8%), respectively. The CP coverage rate was the most frequently used indicator to monitor the effect of CP (97.1%), followed by the completion rate (90.0%) and the length of stay in hospital (61.4%). CP managers reported that CP were highly useful for communication (3.39±0.493) and accurate information provision (3.39±0.491). The perception that CP violated doctors' autonomy in treatment was relatively low (2.87±0.645). Conclusion: It is necessary to establish an infrastructure in hospitals for CP. CP can facilitate communication and provide accurate information.

Mechanism of Wenshen Xuanbi Decoction in the treatment of osteoarthritis based on network pharmacology and experimental verification

  • To investigate the mechanism of Wenshen Xuanbi Decoction (WSXB) in treating osteoarthritis (OA) via network pharmacology, bioinformatics analysis, and experimental verification. The active components and prediction targets of WSXB were obtained from the TCMSP database and Swiss Target Prediction website, respectively. OA-related genes were retrieved from GeneCards and OMIM databases. Protein-protein interaction and functional enrichment analyses were performed, resulting in the construction of the Herb-Component-Target network. In addition, differential genes of OA were obtained from the GEO database to verify the potential mechanism of WSXB in OA treatment. Subsequently, potential active components were subjected to molecular verification with the hub targets. Finally, we selected the most crucial hub targets and pathways for experimental verification in vitro. The active components in the study included quercetin, linolenic acid, methyl linoleate, isobergapten, and beta-sitosterol. AKT1, tumor necrosis factor (TNF), interleukin (IL)-6, GAPDH, and CTNNB1 were identified as the most crucial hub targets. Molecular docking revealed that the active components and hub targets exhibited strong binding energy. Experimental verification demonstrated that the mRNA and protein expression levels of IL-6, IL-17, and TNF in the WSXB group were lower than those in the KOA group (p < 0.05). WSXB exhibits a chondroprotective effect on OA and delays disease progression. The mechanism is potentially related to the suppression of IL-17 and TNF signaling pathways and the down-regulation of IL-6.

Intra-operative Rectal Washout with Saline Solution Can Effectively Prevent Anastomotic Recurrence: a Meta-analysis

  • Background and Objective: Rectal washout with saline solution may theoretically prevent anastomotic recurrence in patients with resectable rectal cancer, although exact clinical effects have not yet been determined. In order to derive a more precise estimation of the relationship, the present meta-analysis was performed. Method: Relevant studies were identified by a search of Medline, Embase and Google Website with no restrictions to September 1, 2013, and included in the systematic review and meta-analysis. Results: 5 trials (642 participants) were included to assess the association between rectal washout with saline solution and anastomotic recurrence. The rate of anastomotic recurrence (AR) was 6.23% (40/642), with the pooled OR derived from the five studies being 0.32 (95 % CI=0.15-0.70, P=0.004). The pooled OR derived from the TME and radical resection subgroups were 0.72 (95%CI=0.16-3.12, P=0.66) and 0.51 (95%CI=0.13-1.96, P=0.32), respectively. Conclusion: Results from this analysis show that intra-operative rectal washout with mere saline solution largely reduces the risk of anastomotic recurrences in patients with resectable rectal cancer.

The effect of short-term particular matter2.5 exposure on asthma attacks in asthma children in Fukuoka, Japan

  • Objectives: We investigated whether asthma attacks in asthmatic children were caused by short-term exposure to particulate $matter(PM)_{2.5}$. Methods: Subjects were 411 patients who received inhalation therapy in National Fukuoka Hospital, from March to May 2013. All subjects were outpatients. We surveyed the air quality measurement results in the stations closest to the address of the patients. Data were used from the City of Fukuoka website data on air pollution. We carried out a case-crossover study and compared $PM_{2.5}$ concentration between 7 days after asthma attack occurred and the day asthma attack occurred and 1, 2 and 3 days before asthma attack occurred. Results: Highest hourly concentration of the day (OR 1.013, 95%CI 1.000-1.025) showed a significant association with 1 day before $PM_{2.5}$ concentration statistically. And 0-1 year-old infants were more vulnerable to the highest concentration of 1 day before $PM_{2.5}$ concentration(P < 0.05). Average concentration of $NO_2$ and $O_3$ and asthma attack also showed a significant association. Conclusions: Maximal daily $PM_{2.5}$ concentrations within 24 hours prior to the attack affect asthma exacerbation. 0-1 year-old infants are particularly vulnerable to $PM_{2.5}$ concentration. Asthma exacerbation is aggravated by $NO_2$ and $O_3$ concentration on the day of the asthma attack.

Development of Automatic Voice System for Foreign Patients in Testing of Magnetic Resonance Imaging (자기공명영상 검사에서 외국인 환자를 위한 자동음성 시스템 개발)

  • The purpose of this study intend to develop the automatic voice system for language communicating over foreign patients in testing of magnetic resonance imaging. Used sentences for the automatic voice system was selected with twelve sentences and five foreign languages in hospital in use. These translated texts were provided free of charge from Oddcast's website, which is recorded each language-specific voice as Wav files, The recorded audio file were produced by the two types of a power-point show of MS-office 2013(extension; ppsx) form and Web-enabled system using the PHP program to be applied in the mobile phone environment. It is considered to relieve the stress about an language barrier to medical technician and the voice system designed with the variable language will be enhanced the diagnostic information on foreign patients. Because automatic voice system designed for this study can enhance the understanding of test in between korean medical technician and foreign patients, It will be expected to utilize more usefully in clinical practice.

Analysis of Sustainability of Hospital Based on Average Achieved Rate in LEED-HC Credits (LEED-Healthcare 세부항목의 평균득점률을 통한 병원의 친환경성 분석)

  • Purpose: The purpose of the research is to find difficulty levels of LEED credits in each LEED level based on credits achievement in order to find important of LEED-HC credits to gain LEED-HC. Methods: The individual credits of LEED-HC v.2009 scorecards from USGBC website are analyzed. With achieved scores rate of LEED credits per each Level, all credits are reassembled in 4 different ranges (A~D); easy (A), easy-moderate (B), moderate-hard (C) and hard (D) to achieve scores. Results: 1) Achieved point rate in LEED-HC specific credits are low. These credits need to be reviewed carefully. 2) In 88 projects, 37 projects are gained Silver level which is more than projects in certification level. In order to encourage to gain LEED-HC level, Average Achieved rate to earn Certificate level needs to be risen. 3) Credits in range D rarely affect to gain platinum level. EAc1 (24 points) are critical to gain Silver and Gold level. However, EAc1 points are not effective to achieve Certificate level. Implications: This research will provide a fundamental back data to set up Korean Green building rating system for Healthcare.
