음악요법이 조혈모세포 이식 전 전신 방사선조사 시 불안, 오심과 구토에 미치는 효과 (The Effects of Music Therapy on Anxiety, Nausea and Vomiting in Patients Receiving Total Body Irradiation for Hematopoietic Stem Cell Transplantation)

  • Purpose: The present study was to investigate the effect of music therapy on anxiety, nausea, and vomiting in patients undergoing total body irradiation (TBI) for hematopoietic stem cell transplantation (HSCT). Method: A untreated control group with pretest and posttest was used in this study. A total of 35 patients receiving TBI for HSCT were recruited from a medical center at a university in Korea. The music therapy taking for 15-20 minutes per a time was performed twice a day for three days while the patients in the experimental group were undergoing the TBI. Results: The scores of state anxiety, nausea and vomiting tended to increase in the control group as compare with those in the experimental group. The patients in the control group had been administerd antiemetics more often than those in the experimental group. The levels of systolic and diastolic blood pressure and the pulse rates in the experimental group tended to decrease after the experiment. However, there was no statistically significant differences in anxiety, nausea, vomiting, blood pressure, and pulse between the groups. Conclusion: It is suggested to repeat this study with a enough sample size.

신증후군 환아 어머니의 부담감, 사회적 지지 및 삶의 질 (A Study Burden, Social Support and Quality of Life in Mothers of a Child with Nephrotic Syndrome)

  • The purpose of this study was to identity the level of burden, social support and quality of life of the subjects. The subjects of this study were 68 mothers of nephrotic syndrome patients whose children were hospitalized in one pediatric ward of the University Hospital in Seoul. The data was collected using questionnaires, and the period of the data collection was from Nov. 15 to Dec. 31, 1999. The instruments used for this study were the Burden Measurement Instrument developed by Montgomery et. al(1985), social support measurement instrument designed Brandt an Weinert(1978) and Quality of life scale designed by Ro,Yoo JA(1988). The data analysis was done by SPSS, t-test, ANOVA and the Pearson correlation coefficient. The results of were as follows. 1. The level of burden showed a mean score of 54.47, the level of social support, a mean score of 86.00 and the level quality of life, a mean score of 140.20. 2. The level of burden differed according to mother's religion, patient's purpose for admission and perceived patient's condition by mothers. 3. The level of social support and the level of quality of life differed according to perceived patient's condition by mothers. 4. There was a negative correlation between burden and social support(r=-.348, p<.001). Also, burden was negatively related with quality of life(r=-3.97, p<.001). Social support was positively related with quality of life(r=.064, p<.001).

뇌손상 환자의 장애정도와 간병가족의 교육요구도 (Brain Injury Patients's Disability Degree and Educational Needs of Family Caregivers)

  • Purpose: This study was conducted in order to identify the brain injury patients's disability degree and educational needs of family caregivers. Methods: A convenience sample of 94 families with brain injury patients, who have been receiving treatment at the neurological intensive care unit and neurosurgery ward, were used. Data was collected with a self-report questionnaire from September 5 to November 28, 2011, and was analyzed using SAS program. Results: 'Defecation/urination' disability was the highest score of patient's physical disability and the next ranking was 'paralysis'. 'Memory impairment' disability was the highest score of patient's cognitive disability, and the next ranking was 'personality changes'. Overall, educational needs of family caregivers scored 4.15 out of the perfect score of 5. The factor, which scored highest, was 'information related with disease'. In addition, educational needs of family caregivers were positively related with patient's degree of. Conclusion: Educational needs of family caregivers are distinct, according to the disability degree of brain injury patient. Therefore, the study suggests the development of individualized educational program for family with brain injury patient.

섬망 교육 프로그램이 중환자실 간호사의 섬망 지식, 간호중재 중요도 및 수행에 미치는 영향 (Effects of a Delirium Education Program for Nurses on Knowledge of Delirium, Importance of Nursing Intervention and Nursing Practice)

  • Purpose: The purpose of this study was to develop a delirium education program for nurses and assess its effects on knowledge of delirium, the importance of nursing intervention, and nursing practice. Method: Participants comprised 60 nurses in a university hospital. The experimental group participated in a 3-hour delirium education program. Data were collected using structured questionnaires. Chi square and independent t-tests were conducted to examine similarities in demographic and dependent variables, and an independent t-test was used to test the hypothesis. Results: The experimental group obtained significantly higher scores than the control group in knowledge of delirium(t=14.52, p<.001), importance of nursing intervention(t=13.76, p<.001), and nursing practice (t=13.71, p<.001). Conclusion: Results suggest that delirium education is an effective tool to improve knowledge of delirium, importance of nursing intervention, and nursing practice. Therefore, delirium education would be beneficial for general ward and recently graduated nurses as well as ICU and experienced nurses.

표준 기본심폐소생술 교육용 비디오를 이용한 실습교육이 간호사의 심폐소생술에 대한 지식, 태도 및 수행능력에 미치는 효과 (The Effect of Basic Life Support Education Using a Standardized Basic Life Support Video Program in Nurses' Cardiopulmonary Resuscitation Knowledge, Attitude and Performance)

  • Purpose: The study was done to identify variations in Nurses' CPR knowledge, attitude and performance before and after BLS Education. Method: This study was designed to evaluate the differences in Nurses' CPR knowledge, attitude and performance. For the study we selected 167 nurses working in Daegu K University hospital. The study group was given a test before (pre test) and after (post test) BLS education. The BLS education was given by two BLS instructors certified by the American Heart Association, using a DVD and manikin. Result: The results showed a statistically significant difference the nurse's knowledge, belief, emotion and performance but there was no significant difference in their behavior after BLS education. CPR knowledge and performance was higher among emergency room nurses compared to surgical ward nurses. Conclusion: CPR education has shown to have a positive effect on nurses' attitudes towards CPR. It also improved their related knowledge and performance. Therefore, cardiopulmonary education should be given continuously, and different steps of CPR education programs need to be developed in order to fill the knowledge deficiency gap.

응급실 간호사의 임상 등급(clinical ladder)에 따른 간호역량 및 행동지표 개발 (Nursing Competency And Indicator Development By Emergency Nurse's Clinical Ladder)

  • Purpose: This study was to develop nursing competencies, sub-competencies and behavior indicators according to the clinical ladder of emergency nurses. Method: index of content validation was used by 21 clinical experts. Results: This study had three phases to develop nursing competencies, sub-competencies and behavior indicators. In first phase: 12 nursing competencies and 33 sub-competencies were developed through the literature review on nursing competency and emergency nurses' job description. The content of 12 competencies and 33 sub-competencies were reviewed by 3 nursing professors. The 12 competencies and 33 sub-competencies were followed: clinical judgement and measures(6 sub-competencies), processing ability of ward works(2 sub-competencies), flexibility(2 sub-competencies), resources management(2 sub-competencies), confidence(3 sub-competencies), cooperation(2 sub-competencies), professional development power(2 sub-competencies), patient service orientation(3 sub-competencies), inclination toward ethical value(5 sub-competencies), influence power(2 sub-competencies), developing others(2 sub-competencies), self control(2 sub-competencies). In second phase, 132 behavior indicators were developed according to nurse clinical ladder: novice, advanced novice, competent, proficient. In Third phase, content validity was examined on 132 behavior indicators by 21 clinical experts. 126 among 132 indicators had over 70% agreement among experts and 6 indicators under 70% were revised. Conclusion: nursing competencies, sub competencies and behavior indicators can be used nurses' clinical performance as well as establishing proper directions for professional growth related to reward system.

프리셉터 간호사의 역할경험 (The Role Experience of Preceptor Nurses in Hospitals)

  • Purpose: The purpose of this study was to identify the role experience of preceptor nurses in general hospitals. Methods: Data were collected from 10 experienced preceptor nurses through in-depth interviews about their experiences. The main question was "How do you describe your experience as a preceptor in the process of role adaptation?" Qualitative data from field and transcribed notes were analyzed using Strauss & Corbin's grounded theory methodology. Results: The core category of experience in the process of role adaptation as a preceptor nurse was identified as 'being responsible to teach from beginning to end'. The participants identified five interactional strategies; 'abruptly having to take the role as a preceptor unwilling', 'difficulties in field training with variable situations', 'preceptor and new nurse with love and hate relationships', 'bring up preceptee as a child', 'redefining the concept of preceptor'. Conclusion: The results of this study provide useful information for understanding the role of preceptor nurses in general hospital who have to teach new nurses with patience and responsibility. It will be needed to establishment of preceptor's role and educational standards, and policy for their rewards by nurse manager. Additionally, making cooperative educational climate in nursing ward is important for teaching new nurse.

병동 간호사의 임상등급 간호역량 평가도구 개발 (Development of a Nursing Competence Measurement Scale according to Nurse's Clinical Ladder in General Wards)

  • Purpose: This methodological study was done to develop a Nursing Competency Measurement Scale based on the clinical ladders of nurses working in wards. Methods: Thirty clinical experts and 501 ward nurses evaluated the content validity of the scale. A survey using the Nursing Competency Measurement Scale was conducted with 114 nurses to evaluate reliability and applicability of the instrument. Data were analyzed using SPSS/WIN 21.0. Results: A review of the literature identified 13 components of nursing competencies and 30 core nursing competencies based on each of the 4 grade clinical ladders. Cronbach's ${\alpha}$ coefficient for the total was .92. Cronbach's ${\alpha}$ reliabilities of each clinical ladder grade were .83 for Grade I, .84 for Grade II, .81 for Grade III, and .84 for Grade IV. The Content Validity Index (CVI) of the scale with 120 individual items was 0.976~1.000 for Grade I scale, 0.986~1.000 for Grade II scale, 0.984~1.000 for Grade III scale, and 0.992~1.000 for Grade IV scale. The expert group nurses' average degree of nursing competence measured using the scale was 3.38~3.75 out of 4.0. Conclusion: Through this process, 120 final questions were confirmed to represent items of the Nursing Competency Measurement Scale based on clinical grade.

치료 중인 암환아와 치료 종료 암환아의 사회심리적 적응에 관한 비교 연구 (A Study on the Comparison of Psychosocial Adjustment of Children with Cancer between on and off Treatment)

  • Purpose: The purpose of this study was to compare psychosocial adjustment in children aged 4-11 years with cancer between on and off treatment. Method: The data was collected from parents of children with cancer through interviews with structured questionnaire at the pediatric outpatient department and inpatient ward of one university hospital located in Seoul. The instrument were the Child Behavior Check List(CBCL) developed by Achenbach(1991) and revised to standardize for Korean children by Oh et al(1997). Result: Total behavior problem score for children on treatment was greater than the score off treatment but there was no significant difference. Children on treatment reported higher levels of depression/anxiety, withdrawal, internalizing scores than children off treatment. Social competence score of children off treatment was greater than the score on treatment and the difference of school performance score of children between on and off treatment was not significant. The scores on the withdrawal, somatic complaints, social immaturity, internalizing scales in the cancer group including both on and off treatment was greater than normative findings in the general population. The scores on the school competence of children off treatment were lower than the norms for healthy children. Conclusion: Findings from this study support the importance of nursing interventions to facilitate the adjustment of children with cancer both on and off therapy.

신규간호사의 환자안전문화에 대한 인식과 안전간호활동 (Perception of Patient Safety Culture and Safety Care Activity of Entry-level Nurses)

  • Purpose: We investigated the relationship between patient safety culture and safety care activity, and identified factors for safety care activity of entry-level nurses. Methods: The subjects of the study were 204 entry-level nurses working at five general and advanced hospitals with over 500 beds located in C and D City. The data were analyzed using descriptive analysis, t-test, ANOVA, Pearson's correlation and hierarchical multiple regression with SPSS/WIN 18.0 program. Results: Perception of patient safety culture had significant correlations with safety care activity. Multiple regression analysis showed that demographic variables of quality of nursing on patient safety and health state of entry-level nurses affected safety care activity. In addition, patient safety culture in ward and communication affected safety care activity. These factors explained 38% of variance. Conclusion: Based on the results of the study, patient safety culture promotion strategies to facilitate supportive work environment and effective communication are needed.
