  • Purpose: This study was performed to develop a valid and reliable Korean Patient Classification System for Neonatal care nurses (KPCSN). Methods: The study was conducted in tertiary and general hospitals with 1~2 grade according to nursing fee differentiation policy for NICU (neonatal intensive care unit) nurse staffing. The reliability was evaluated for the classification of 218 patients by 10 nurse managers and 56 staff nurses working in NICUs from 10 hospitals. To verify construct validity, 208 patients were classified and compared for the type of stay, gestational age, birth weight, and current body weight. Nursing time was measured by nurses, nurse managers, and nurse aids. For the calculation of conversion index (total nursing time divided by the KPCSN score), 426 patients were classified using the KPCSN. Data were collected from September 5 to October 28, 2015, and analyzed using t-test, ANOVA, intraclass correlation coefficient, and non-hierarchial cluster analysis. Results: The final KPCSN consisted of 11 nursing categories, 71 nursing activities and 111 criteria. The reliability of the KPCSN was r=.83 (p<.001). The construct validity was established. The KPCSN score was classified into four groups; group $1:{\leq}57points$, group 2: 58~80 points, group 3: 81~108 points, and group $4:{\geq}109points$ in the KPSCN score. The conversion index was calculated as 7.45 minutes/classification score. Conclusion: The KPCSN can be utilized to measure specific and complex nursing demands for infants receiving care in the NICUs.

The principal findings of this research are as below : 1. Type of leadership and job satisfaction had positive correlation in terms of both transformational and transactional leadership, with former having more relation than the latter. 2. Type of leadership and organizational commitment had positive correlation in terms of intellectual stimulus and situational rewards, having little relation statistically. 3. Type of leadership and turnover intentions had negative correlation only in individual consideration, situational rewards, and exceptional management, showing that transactional leadership had more relation than transformational leadership. 4. In terms of leadership and its effect on the organizational performance and turnover intentions, transformational leadership had more correlation than transactional leadership. But transactional leadership also turned out to have effect on organizational performance. However, both transformational and transactional leadership had negative correlation to turnover intentions. 5. Effect that type of leadership according to job classification, status, age, and academic career has on organizational performance and turnover intentions was differed by those factors; especially, in the occupation of doctor, both transformational and transactional leadership had positive correlation to turnover intentions. 6. Regression analysis on the factors of organizational performance and turnover intentions showed that higher age had positive correlation to the organizational performance in terms of socio-demographical features and that the more leadership was used for charismatic and situational rewards and the less leadership was used for intellectual stimuli, the higher organizational performance was likely. In short, hospital operators and managers should examine each manager's qualification, type, and style of leadership in the hospital organization, and they must develop lasting programs for effective education and training for developing characteristics of leadership are in needed to have managers with appropriate managerial mind to respective post and status. • Purpose: The purpose of this study was to evaluate the experience of hospital nurses regarding their intention to stay at hospital. Methods: Experiential data were collected from 10 experienced nurses through in-depth interviews. The main question was "Could you describe your experience and your work during your years at the hospital?" Qualitative data from the field and transcribed notes were analyzed using Strauss and Corbin's grounded theory methodology. Results: The core category of experience regarding hospital nurses' intention to stay was 'following a stable rather than a challenging path'. Participants used three interactional strategies: 'being encouraged via rapport with peers', 'accept reality', and 'find vitality in academic pursuits'. Conclusion: The retention of experienced nurses is critical to human resource management in nursing departments. This study found that experienced nurses have a vague uncertainty about their future in the hospital. Therefore, nursing managers should support experienced nurses by providing them with the opportunities needed to develop their careers, by managing conflicts in nursing units, and by implementing new programs to increase confidence.

  • Purpose: This research was conducted to investigate the nurses' awareness and behavior on coaching and its relation with job characteristics in a hospital. Methods: Total 300 nurses in a university affiliated hospital were surveyed with questionnaires between April and May, 2008. The data was analyzed using SAS 9.1. Results: The data showed that 88.9 % of nurses responded the need of coaching training program while only 80.9% of general nurses and 75.7% preceptor nurses would like to participate in the training. The current level of knowledge for coaching was not different by the age, education level, and career. However, coaching behavior and job characteristics were significantly higher in the nurse managers than in general nurses or preceptors. The correlation between coaching behavior and job characteristics were identified. Conclusion: The gaps between the perceived necessity of coaching program implementation and willingness to participate in the coaching program were due to worry about the burden of extra hours needed to participate the program. To introduce coaching program to a nurses' organization successfully, the efforts should be made to develop the coaching training program for nurses based on the results and to support them systematically.

  • This study was conducted to evaluate needs and intention of hospitals and clinics to join network with private health insurance, and to discover obstacles of participation of the networks. We carried out the questionnaire survey of the network managers of 236 medical institutions between December 27th, 2005 and January 25th, 2006. The result showed that the participation intention of network were different to the type of hospitals. Primary care clinics answered that participation intention and possibility were low. Secondary care hospitals was relatively affirmative regarding a network participation. Tertiary hospitals responded that they need the network with private health insurance, but participation possibility was lower than needs. The reason is that they worried about the side effect of the network with private health insurance. Depending on the type of hospitals, expected benefits from networking with private health insurance were different. We found that hospitals which already had affiliation with other hospitals answered in the affirmative regarding the network with private health insurance. In conclusion, to increase the effectiveness of network systems between hospital and private health insurance, the network is expected to consider different needs of the each hospital.
