Nursing Organizational Characteristics and Job Satisfaction of Nurses in a general hospital (일 종합병원 간호사의 지각된 간호조직특성과 직무만족)

  • The purpose of this study was to identify the relationship between the nursing organizational characteristics and job satisfaction. Subjects were 362 nurses at a general hospital in Seoul. Data were collected from November 1, 1999 to November 31, 1999 with the structured questionnaires and analyzed by $SPSS-PC^+$ 8.0 for descriptive statistics, ANOVA, Pearson correlation, and stepwise multiple regression. The results were as follows; 1. The average score of the nursing organizational characteristics was 3.7. The score of head nurse's leadership was highest and supervisor's was lowest. The average score of the job satisfaction was 3.1 and was similar to the results of other studies. 2. Being regarded to the nursing organizational characteristics and job satisfaction by general demographic variables, The head nurse's leadership, the supervisor's leadership and interaction, and the overall nursing organizational characteristics showed significant differences by year of career, position status, and service-department(p<.1, p<.05). The job satisfaction showed significant difference by position status(p<.01). 3. There were correlations between the nursing organizational characteristics and job satisfaction(r=0.358-0.815). 4. The most important variable in the job satisfaction was 'organizational climate' and it was explained 32.0% out of the total variance of job satisfaction and 'all of nursing organizational characteristic factors' explained 47.5%. According to the results of this study, the suggestions were as follow: 1. To improve nurses' job satisfaction, the strategy for the development of the cooperative nursing organizational climate and for the strengthening of 'the nursing manager's leadership and interaction' in the nursing department are necessary. It should to be considered the differences in the nursing organizational characteristics and job satisfaction by nursing manager and staff nurses. 2. Further study needed to analysis the organizational climate for the management of the nursing organization. Also it is necessary for further investigation regarding to the relationship between the nursing organizational characteristics and job satisfaction of pre and post-organizational change.

  • This study was to measure the awareness of the patient safety culture of medical workers in various occupations working in hospitals and tried to be used as useful data. As a result of evaluating department (ward), hospital, immediate supervisor/manager, communication & procedures and frequency of accident reports, the patient safety accidents considered to be the most dangerous, technicians showed high results in the department(ward), nurses showed high results in the immediate supervisor/manager area. Radiological technologists and physical therapists recognized falls and clinical pathologists and nurses recognized before during after the test as the most dangerous patient safety accidents. To raise awareness of patient safety culture, executives and practitioners should create an atmosphere in which practitioners can prioritize patient safety, gain and manage appropriate personnel, manage cooperative systems between workers or departments.

Analysis of the Factors and the Differences in the Awareness about the Capability Groups of the Mediator Manager in General Hospital and the Level of Performance (종합병원 중간관리자의 역량군별 중요도 인식과 수행수준 차이 및 요인분석)

  • The study has its purpose on providing basic resource to enforce the capability of the middle managers by examining the level of performance and the level of awareness about the capabilities of the managers and by understanding the significance of the difference and the reasons for the differences. The source of the study was 195 survey questionnaires that were carried out to the managers of the 9 general hospitals and the method of the analysis was the frequency analysis, analysis of the credibility, matching to sample T-test, independent sample T-test, dispersion analysis, correlation analysis, and multiple linear regression analysis using accumulated variables. The followings are the main result of the study. First, the difference between the level of awareness about the capabilities and the level of performance of the mediator managers in general hospitals had high capability in change management. The following orders were: competence in achievement and behavior, competence in management, competence in recognition, competence in influence, competence in individual effectiveness, and competence in personal relationship service. Second, as the result of the relation analysis in order to understand the correlation between awareness and performance of the mediator managers, everything had significant positive correlation. In the study about the level of importance, the cognitive capability and the management capability had the highest correlation with the correlation number of 0.88. In the study about the level of performance, the cognitive capability, individual capability, and the management capability had the highest correlation with the correlation variable number of 0.79. Third, as the result of studying the reason for the difference between the level of the awareness capability and the level of the performance, lack of the support recognition compensation in the organization level, inappropriate work environment, limit in the regulation were found as the highest reason in the order. As the result of the study, it was concluded that the creation of the efficient capability estimation model and the securement of the system that estimate the capability of the managers should be carried out in order to enforce the capability of the mediator managers in general hospitals.

  • The purpose of this study is to review in light at women's subjective. Perceptions of manifestations Premenstrual Syndrome which are known to affect their labor productivity. To this end, 364 women workers were sampled from an industrial factory located in Chonan, Chungchongnam-do and supervised by health manager, through questionnaires, and collected data were processed using SPSS package program. The result of survey can be summarized as follows: The fact that some academic circles are recently raising the issue of premenstrual syndrome. Suggests that the syndrome is being accepted in academic terms, which is contary to the position that the syndrome is just the result of women's mental imagination. However, some factors which are concived to affect woman worker' productivity and efficiency seem to be related to their subjective psychological variables involving their working environment rather than their physiological variabls. Therefore, if a health-promotion program handling their subjective psychological variables be introduced, some factors caused by the pre-menstrual syndroml may be reduced thus increasing their working productivity and. efficiency around their menstrual days.

Group Performance and the Team Learning Climate as Perceived by Hospital Nurse (임상간호사가 인지한 팀학습분위기와 집단성과)

  • Purpose: To investigate the influence of a team learning climate on group performance of hospital nurses. Method: The subjects were 386 nurses who have been working in six hospitals. The data were collected by a structured questionnaire from January 20 to April 30 of 2006. The data were analyzed by SAS version 8.2, including descriptive statistics, Pearson correlation coefficient, and stepwise multiple regression. Results: The mean score of group performance was 3.38 and team learning climate was 4.89. The group performance was positively correlated with team learning climate(r=.40, p<.0001). The team learning climate explained 15% of the variance in group performance. Conclusion: The findings showed that team learning climate was an important factor in enhancing group performance in nursing organization. Therefore, the nurse manager will establish the strategies to improve the team learning climate of the nurses in order to promote organizational performance.

The results of recognition survey for patient safety culture in a hospital (일개병원의 환자안전문화 인식도 조사결과)

  • Objectives: This study measures the level of cognition of employee's patient safety culture and evaluates the current level through comparing the results to external levels. Ultimately it is performed to construct a strategic improvement plan through the basic database for patient's safety culture. Methods: A questionnaire survey of self reporting type was carried out using structured questionnaire of the patient's safety culture for employees currently employed in a hospital. Total responders was 1,129 and a response rate was 54.6%. The survey results were calculated with a percent positive response, and the current level was evaluated by comparing with the survey results of a hospital (2009 and 2014) and the survey result of The Agency for Healthcare Research and Quality(2014). Results: Sub-dimension of high percent positive response for each area were 'teamwork within hospital units' (80%), 'feedback & communication about error' (73%) and 'supervisor/manager expectations & actions promoting safety' (67%). Meanwhile, 'teamwork across hospital units' (31%), 'hospital management support for patient safety' (29%), 'staffing' (27%) and 'non-punitive response to error' (17%) were relatively low percent positive response. Compared to the survey results of AHRQ (2014) for each area, 'teamwork within hospital units' (80%), 'feedback & communication about error' (73%), 'frequency of event reporting' (66%) were at the top 50% percentile level and the remaining sub-dimensions showed a very low level in the lower 10% percentile area. Conclusion: In order to establish a system for patient safety culture within the hospital and evaluate the effect on this, it is necessary to periodically evaluate the patient's safety culture and establish regulations on hospital safety culture to comply with this.

A Systematic Review and Meta-Analysis of Moxibustion Treatment for Knee Osteoarthritis

  • The purpose of this study was to evaluate the evidence supporting the effectiveness of moxibustion treatment for osteoarthritis. There were 9 databases used to retrieve randomized controlled trials (RCTs) that used moxibustion as treatment for osteoarthritis. The quality of methodology for the RCTs was assessed using the Cochrane Risk of Bias tool [Review Manager (RevMan) Version 5.3 Windows, The Nordic Cochrane Centre, Copenhagen, Denmark]. The inclusion criteria for this review was met by 27 RCTs. All studies were conducted in China. A 4-week moxibustion treatment period was the most common. EX-LE4 and SP10 and GB34 acupoints were most frequently selected in the treatment of osteoarthritis. The most commonly used evaluation index was the visual analog scale (VAS). All studies, including a meta-analysis showed that moxibustion treatments were statistically significantly effective at treating knee osteoarthritis. However, well-designed randomized RCTs without a high risk of bias should be designed in the future.

A Systematic Review and Meta-Analysis on the Effects of Oral Stimulation Interventions on the Transition from Tube to Oral Feeding in Premature Infants (구강자극요법이 미숙아의 경구수유 이행에 미치는 효과에 대한 체계적 문헌고찰과 메타분석)

  • Purpose: The purpose of this study was to evaluate the effects of oral stimulation interventions on the transition from tube to oral feeding in preterm infants through systematic review. Methods: The randomized clinical trials published between 1980 and 2015 were searched using domestic and international databases, and five randomized studies were selected for this study. The quality of study was assessed by assessment tool from the SIGN and meta-analysis was performed using the Cochrane Review Manager software Version 5.3 (RevMan). Results: Oral stimulation intervention in preterm infants decreased the transition time from tube to oral feeding (Post Menstrual Age, Day of life). Especially, a significant medium effect size was found in the number of days needed on the transition (ES=-4.95, p=.02). The oral stimulation intervention also had a significant large effect on the length of stay at hospital (ES=-8.33, p<.001). Conclusion: Based on the findings, oral stimulation interventions could be useful to facilitate the transition from tube to oral feeding in preterm infants in terms of reducing the length of stay at hospital and the number of days needed on transition.

Job Analysis of the Nursing Unit Managers of Women's Hospital Using DACUM Analysis (DACUM 직무분석 기법을 이용한 여성병원 간호단위관리자의 직무분석)

  • Purpose: To analyze the job of nursing unit managers working at women's hospital, using DACUM (developing a curriculum), DACUM is a method for analyzing job-focused competency. Methods: This study involved a descriptive survey. A DACUM workshop was held to define women's hospital nursing unit managers' role and identify their duties and tasks. For the workshop, a committee was formed consisting of 5 women's hospital nursing unit managers. Finally, after validation, the developed contents were made into a survey asking about nursing unit manager's duties and tasks. Results: Sixteen duties and 83 tasks were identified on the DACUM chart. The importance, difficulty, and frequency of the tasks were ranked in terms of A, B, and C, with A being the highest degree. Eight tasks received A's all in importance, difficulty, and frequency of performance. The 8 tasks were: 'taking over', 'taking care of seriously ill patients on handover', 'ward rounding', 'analyzing and resolving demands identified during handover and patient tour', 'reporting patient status during rounding', 'promoting breast-feeding', 'uterine contraction, and training for breast-feeding'. The duty with the biggest determinant coefficient (DC) was 'patients complaint management' (DC=7.09). Based on tasks, the one with the biggest DC was 'solving patient and patient guardian's complaints' (DC=7.53), followed by 'making infection control guidelines' (DC=7.5). Conclusion: When expanding the nursing staff of the hospital, women's hospitals nursing unit managers also need to use administrative functions as intermediaries to focus on the operation management of the entire hospital rather than direct nursing to suit their role.

Status of Employment-Related Qualifications Similar to a Medical Laboratory Technology Major (임상병리기술학 전공 유사 취업연계 자격 현황)

  • This study evaluated the various qualifications that can be linked to employment similar to the medical laboratory technology major. The qualifications for employment outside the medical laboratory technology major are industrial health instructor, doping control officer, audiologist, sign language interpreter, health education specialist, claim adjuster, Korea life underwriter, administrative manager, hospital administrator, insurance review manager, and hospital coordinator. The qualifications for employment similar to the medical laboratory technology major include the clinical research coordinator, clinical research associate, anatomist, analytical chemistry analysts, hazardous materials industry engineers, biotechnologist, biosafety managers, biotechnology technicians, medical device quality officers, animal care nurse, industrial engineer hazardous material, bioprocess engineer, biosafety officer, certified technology consultant, director of medical device quality control, laboratory animal technician, animal nursing technician, and cruise medical manager. Therefore, it is necessary to conduct a social analysis survey of the various qualifications currently held by medical technologists. In addition, it will be necessary to investigate the current status of medical technologists working in other fields. In the future, medical technologists should expand the scope of their work through efforts to strengthen their individual work capacity, share cases, and strengthen their expertise.
