간호사의 업무수행상의 권한과 행사전략 (Nurse's Power and Tactics in Nursing Practice)

  • This study is to understand and to describe the power that a nurse experiences on nursing practice and then. to present a basic data for nurse's power-development, power improving and empowering. Ethnography was used to understand and to describe experience on exercising various powers occurred on nursing practice. and to analyze and to understand the meaning of a nurse's power. The objects was nurses. Ten nurses who have more than three year's experience were selected as objects from Cuniversity's hospital in Seoul from May of 1996 to August of 1997 through in-depth interview. participant observation, and phone interview. Instruments werw a portable recorder and field notes. I described a case appeared in a data using Agar's 'Pencil and scissors' method right after collecting materials. Then, Idescribed a theme discovered commonly. Followings are the results of the study. 1. There were three categories of relationships with main objects when nurses exercised their power on their practices: a therapeutic caring relationship with patients, a relationship of companion, vertical cooperation, and a constituent person with a doctor, and a relationship of cooperation, and a constituent person with administrative workers and medical technicians. 2. There were many types of nurse's power, tactics and various patient's responses about them. 1) Types of nurse's power to patients were giving information, controling environment, helping for cure, emotional support, and performing discretion. 2) Nurse's tatics for performing power were positive tactics neutral tactics, and negative tactics. 3) Patient's responses were appeared as compliance and noncompliance. Compliance were agreeing. taking nurse's advice, trusting, understanding, being admitted, exposuring himself, and appreciating. 3. There were types of nurse's power and performing tactics. 1) Types of power to a doctor were advice, informing, demanding and mediation. 2) Performings of tactics to a doctor were positive tactics, neutral tactics, and negative tactics. 3) Doctor's responses were appeared as accepting and unaccepting. Acceptings were taking in and appreciating, and unacceptings were denying nurse's advice and authoritative. 4. There were types of nurse's power and tactics about administrative workers and medical technicians and responses about them. 1) Types of power about administrative workers and medical technicians were suggestions and demands. 2) Power performings tactics were positive tactics.neutral tactics, and negative tactics. 3) Responses of administrative workers and medical technicians about nurse's power performing were appeared appeared as accepting and unacce pting. Acceptings were taking in, and unacceptings were denying. Therefore, it can be said that types of nurse's power and performing tactics on nursing practice and nurse's power based on responses of a patient, a doctor, an administrative worker, and a medical technicians are power or influence for agreeing, taking advice, trusting, understanding, exposuring himself, appreciating, and taking in to objects. The results of this study helped to understand nurse's power. I expect that this study will improve nure's power by using expert power, referent power, and legitimate power effectively among powers acmpanied with the origin and that nurses make ef-ort to improve professional knowledge and human nature so that they use this study as a chance to develope expert nursing practice.

일부지역 의료기관의 외래원무관리 표준화에 관한 연구 (A study on the standardization for outpatient management and adminstration process of some regional hospitals)

  • 이 연구는 일부 지역 의료기관의 외래원무관리 표준화를 위해 대한병원협회에 등록된 일부 지역 종합병원 이상 의료기관에 근무하는 원무팀 직원을 대상으로 설문조사 하였다. 이 연구의 결과를 요약하면 다음과 같다. 접수업무 프로세스는 설립주체, 병상규모, 원무팀 직원 수, 일평균 외래환자 수에 따라 큰 차이는 없었다. 그러나 진찰료 선납 여부는 병상 규모가 클수록 진찰료 선납을 받았다. 병상 예약업무 프로세스는 설립주체, 병상규모, 원무팀 직원 수, 일평균 외래환자 수에 따라 큰 차이는 없었으나, 예약 후 내원 시 진찰료 선납은 500병상 미만 의료기관이 11.8%, 500병상 이상 의료기관이 50.0%로 나타났다. 이 연구 결과 의료기관에 따라 외래원무관리 프로세스에 큰 차이는 없었으나 전산시스템, 시설관련 부분에 차이를 보였다. 이러한 업무프로세스의 차이를 극복하기 위해서는 의료기관의 경제적 부분이 지원되어야 한다. 환자에게 양질의 의료서비스를 제공하기 위해 의료기관평가인증원 조사항목과 설문을 통해 얻은 다빈도 응답을 토대로 도출한 표준화를 실제 의료기관에 적용하여 그 효과를 분석하는 것이 필요할 것으로 사료된다.

방사선사의 전직의사와 관련된 요인분석 (Factors affecting the Intention of transfering of Radiology Technologists to Different Institutions)

  • This study attempts to analyze the factors affecting the intention of transferring to another hospitals among radiology technologists. 344 cases were reviewed in 5 university hospitals and 1 general hospital. Self-administered questionaire were given to study the socioeconomic characteristics, working conditions, job satisfaction level, and the reasons for transfer among the technologists. The major findings were as follows : 1. Job position and hospital characteristics had a statistically significant relationship with the intention of transferring to another hospital. 2. Those who were not satisfied with their salaries and promotional opportunity showed a higher tendency towards to transfer. 3. Those who were less satisfied with the opportunity for developing the personal ability and had the negative attitude on their job showed a higher tendency to transfer. 4. Those who did not sustain good relationship with their superiors and co-workers scored high on the tendency to transfer. 5. In the result of mutiple regression, recognition of radiation hazard, job satisfaction, satisfaction with salary levels, job attitude were significantly related to transfer. The above indicate that besides economic incentives, job satisfaction and organizational culture to promote their ability and form a good relationship with organization members were very important to decrease the intention of transfer. Since these results represent only 6 hospitals from a limited area, more hospitals nationwide, especially small and medium-sized institutions where there is a high turnover rate of employment, need to be examined in order to investigate the various factors that affect the intention of transferring.

사회적 지지와 자아존중감이 간호사의 임파워먼트에 미치는 영향 (The Impact of Social Support and Self-esteem on Nurses' Empowerment)

  • Purpose: This study was done to measure the level of social support, self-esteem, and empowerment and to identify any effect of social support and self-esteem on the empowerment of nurses. Methods: The study design was a descriptive survey using questionnaires which were given to 381 nurses in C province. The collected data were analyzed using descriptive analysis, t-test, ANOVA, Pearson correlation coefficient, and multiple regressions. Results: The mean score for nurses' empowerment was $2.83{\pm}0.66$. Seven individual characteristics, social support(family, meaningful persons, supervisors, and co-workers) and self-esteem accounted for 23.3% of the variance in nurses' empowerment. Prediction elements influencing empowerment of nurses were salary per month, self-esteem, and social support(supervisors). Conclusion: The results indicate that it is necessary to increase nurses' empowerment. Social support by supervisors and self-esteem were confirmed as important factors to increase nurses' empowerment. In addition, raising the monthly average income would increase empowerment of nurses.

간호사의 노동조합에 대한 태도와 간호전문직관, 헌신몰입에 관한 연구 (A study on the relation among attitudes toward unions, views on nursing profession and career commitment)

  • In studies on the hospital labor-management relations, research about the attitudes of workers and management toward unions was relatively untouched in Korea. This study was investigated to identify the relation among ideal, actual attitudes toward unions, ideal, actual views on nursing profession and career commitment. Data was obtained from a convenience sample of 285 nurses of varying positions, education, career, join union or not. The results of this study were as follows. 1. There was significant differences between ideal attitude toward unions and actual (p < .001). 2. There was significant differences between ideal view on nursing profession and actual (p< .001). 3. There were no correlations between ideal and actual attitude toward unions and nursing career commitment. 4. There were correlation between ideal and actual views on nursing and nursing career commitment(r=.32, r=.46). As the results show, views on nursing profession are more important factor to inhance nursing career commitment than attitude toward union. So the findings of this study suggest that antecedents and moderating variables need to be explored for further theoretical specification and empirical evaluation.

조선수리업종 석면노출 근로자들의 흉부방사선상 석면관련 폐질환 소견의 발생양상 (Survey of Asbestos-Related Disease on Chest Radiograph of Workers Exposed to Asbestos in Ship Repair)

  • 모 조선수리업에 근무하는 1,086명 중 석면에 노출되는 작업공정에 10년 이상 근무한 근로자 118명을 대상으로 단면조사연구를 수행하였다. 연구대상자들에 대해 흥부방사선검사, 폐기능검사와 미국의 산업안전보건청 석면표준, 의학적 감시 프로그램(OSHA asbestos standard, medical surveillance program)에 따라 국내 실정에 맞게 일부 편집한 설문조사를 시행하였고 아울러 이들의 작업장에 대한 작업환경측정 자료도 분석하였다. 흉부방사선검사상 석면관련 폐질환 소견의 유병율은 118명의 근로자중 16명으로 $13.6\%$였다. 흉부방사선검사상 정상군에 비해 석면관련 폐질환 유소견군에서 유의한 요인들은 폐기능검사상 이상소견, 기침, 가래, 호흡기 질환의 과거력, 근무력이었다. 본 연구 결과를 통해 볼 때 폐기능검사 소견상 이상소견, 기침과 가래의 임상증상을 호소하거나 호흡기질환의 과거력, 20년 이상의 근무력이 있는 근로자에 대해서는 석면관련 폐질환의 엄격한 관리가 필요할 것으로 생각된다. 그리고, 본 연구에서 관련성 여부를 완전히 배제할 수 없었던 요인들에 대해서는 향후 광범위한 전향적 연구를 통하여 연관성을 규명하여 석면노출 사업장의 의학적 감시 프로그램에 적극 활용되기를 기대한다.

한 대학병원 종사자의 감정노동 수준과 그에 영향을 미치는 요인 (The Level of Emotional Labor among Workers in One University Hospital)

  • Purpose : The purpose of this study was to investigate the level of emotional labor and to identify affecting factors on emotional labor among one university hospital employees. Material and Method : This study was based on cross-sectional, self-administered, and Internet-based survey. The survey was conducted from Mar. 2 to Mar. 28. 2011. Total subjects were 812 employees working in one university hospital. Total response rate was 61.5%. Modified evaluation tool was used, which was originally developed by Morris and Feldman, to measure the level of emotional labor among hospital personnel. In order to identify the affecting factors on high level of emotional labor, we conducted logistic regression. The SPSS statistical software package was used to perform the statistical analysis. All statistical tests were 2-sided and a p-value<0.05 was considered statistically significant. Results : Average score of emotional labor was 3.06. Employees(3.39) who are working at the emergency department and hemodialysis room indicated the highest level of emotional labor, followed by wards(3.14), department of administration(3.14), department of ambulatory cares(3.06). The factors affecting on the high level of emotional labor were the highest level of schooling, types of department, and types of personality(p<0.05). Conclusion : The survey results showed that there was significant level of emotional labors among hospital employees. Therefore, the efforts to reduce the level of emotional labors are needed.

항만하역근로자의 직무 스트레스와 자아탄력성이 직무 스트레스 회복경험에 미치는 영향 (The Impacts of Job Stress and Ego-resilience on Recovery Experience from Job Stress in the Container Terminal Workers)

  • Purpose: This study was conducted to identify the factors affecting container terminal worker's recovery experience from job stress. Methods: The subjects were 299 workers from one S dock in P city. Data were collected from April 5 to June 5, 2015 and analyzed by SPSS/WIN 18.0 program using descriptive statistics, t-test, ANOVA, $Sheff{\acute{e}}$ test, Pearson's correlation coefficients, and logistic regression. Results: The mean scores of job stress, ego-resilience, and recovery experience from job stress were 47.18, 46.90, and 49.17 respectively. Recovery experience according to the general characteristics showed significant correlation between daily exercise. There was a significant negative correlation between recovery experience and job stress, and a positive correlation between recovery experience and ego-resilience. Recovery experience was 2.54 times higher for a high ego-resilience group than for a low ego-resilience group, and the group that exercised was 2.25 times higher, than the non-exercising group. The group with a low level of interpersonal conflict was 1.97 times higher, than a group with a high level of interpersonal conflict. Conclusion: Based on this study, intervention programs to increase ego-resilience, decrease interpersonal conflict, and encourage over 30-minute-daily exercise for in container terminal workers should be developed to improve recovery experience of job stress.

구조방정식을 이용한 종합병원 행정직원의 역할스트레스가 소진, 직무만족 및 조직몰입에 미치는 영향분석 (A Study using Structural Equations on how the Role Stress of Administrative Workers in General Hospitals Affects Burnout, Job Satisfaction, and Organizational Commitment Analysis)

  • 본 연구의 목적은 의료기관 행정직 종사자의 역할 스트레스가 소진과 직무만족, 조직몰입에 미치는 영향을 실증적으로 규명하고자 실시되었다. 본 연구는 200병상 이상의 종합병원 중 10곳의 행정직원을 대상으로 2015년 7월 13일부터 8월 14일 까지 한 달 동안 설문조사를 실시하였다. 조사된 설문은 AMOS 18.0을 이용하여 확인적 요인분석, 평균분산추출, 연구모형의 적합도 지수, 연구모형의 경로계수 추정치 분석 등을 실시하였다. 본 연구의 주요결과로는 의료기관 행정직 종사자의 역할갈등과 역할과다는 소진에 정(+)적인 영향을, 소진은 직무만족에 부(-)적인 영향을 미치는 것으로 나타났고, 조직몰입과의 관계는 유의하지 않은 것으로 나타났다. 본 연구결과는 부서간의 연계 업무 등에 있어 역할갈등을 줄이기 위해서는 업무에 대한 명확한 지침과 업무지시의 통일성 등이 필요하고, 정확한 업무 측정과 계량으로 구성원의 역할을 적정화할 때 조직에 대한 애착과 헌신도도 높아지고 궁극적으로 개인의 직무성과도 향상될 수 있음을 시사하고 있다.

병원 간호사의 만족, 직무 스트레스, 소진, 조직시민행동 및 생산성에 관한 연구 (A Study on Satisfaction, Job Stress, Burnout, Organizational Citizenship and Productivity of Hospital Nurses)

  • The health care markets are rapidly expanded due to population aging, increasing incomes, strengthening assurance of health insurance and so on, and the competition in the markets is getting serious. In the industry, hospitals have played more important role than any others, where doctors, nurses, medical technicians, administrative staff, etc cooperatively works hard. Among them, nurses capture 40 percents of specialized hospital workers and their role for patients as customers is essential. Therefore, it could be one of the most important keys to increase the productivity and wellbeing of nurses with respect to enhancing efficiency and competitiveness of operating hospitals. In this paper the impact of nurses' job satisfaction, job stress, burnout, and organizational citizenship behavior on their productivity is studied. The subjects were 576 nurses in 20 different hospitals in Seoul, Kyounggi, etc. We held hypotheses and statistically tested and analysed them, using SPSS 20.0 software. Thus, we found various significant results(p<0.05), and they will give useful suggestions for managing nurse manpower and enhancing their productivity.
