Factors Affecting the Quality of Life in Stroke Patient at Home (재가 뇌졸중 환자의 삶의 질에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify factors affecting the Quality of Life of the Stroke Patients. Method: The subjects were 249 Stroke Patients who had visiting at the hospital and health care center. The research tools were FIM, CES-D, ATD PA, Quality of life Scale. Data were analyzed with SPSS WIN program in which frequency, percentage, Pearson's correlation and stepwise regression were used examination the factors affecting the quality of life. Result: There were positive correlation between patient's FIM and Quality of life, depression and physical disability, and negative correlation between FIM and depression and physical disability. Depression was the most important factor with influence on quality of life. Conclusion: Based on the results, Active nursing intervention to decreased depression and to improve patients physical functional status in needed.

  • This paper is addressed to Internet telephony based service implementation. It describes an implementation method which uses ARS as a gateway which combines Internet and traditional public switched telephone network for Internet information service using telephony and fax, is different to traditional Internet telephony which provide enhanced speech quality and low cost functionality. This method allows telephony and/or fax user to get Internet information without additional Internet bill, Internet infrastructure and low connection quality from low signal bandwidth connected him. Implemented system is useful to a special kind information service such as climate information of Korea etc and simpler than WAP based service as for wireless mobile telephony user. We implement job opportunity information and advertisement service supported by Home page of Choong Book Small & Medium Business Administration and e-mail service supported by Korean Society for Rehabilitation of persons with Disabilities to demonstrate the system ability. As a result of test implementation, this service system works good fur blind persons and graduated persons without job, is expected to apply for special Internet information provider via Voice and Fax.

  • As climate change is becoming the main issue, various efforts are focused on saving building energy consumption both at home and abroad. In particular, it is very important to save energy by maintenance, repair and rehabilitation of existing multi-family housing complex, because energy consumption in residential buildings is not only forming a great part of gross energy consumption in Korea but the number of deteriorated complexes is also sharply increasing. However, energy saving is not considered as a main factor in decision making on rehabilitation project. Also, any supporting tool is not appropriately prepared in existing process. As the first step for development of decision support system on rehabilitation, this paper developed a breakdown structure, which makes clusters of multi-family housing complexes. Decision tree, one of data mining methods, was used to make clusters based on the characteristics and energy consumption data of multi-family housing complexes. Energy saving and CO2 reduction will be maximized by considering energy consumption during rehabilitation process of multi-family housing complex, based on these results and following research.

  • 1. Objectives Sasang Constitutional Medicine is Mind-Body medicine and it is based on difference of Nature & Emotion(性情) & real purpose(恒心). The purpose of this study is to evaluate the effect of Hyangbujapalmul-tang for somatoform disorders and is to drive the importance of psychiatric approach home. 2. Method The subject is about 49-year-old Soeumin patient presumed by Undifferentiated somatoform disorder based on his nature & emotion, physical characteristics, symptoms, and we have prescribed Soeumin Hyangsayangyui-tang, Ceongunggyegi-tang, Hyangbujapalmul-tang. Soeumin Hyangsayangyui-tang was prescribed based on oridinary symptoms(素症), Soeumin Ceongunggyegi-tang was prescribed based on real symptoms(病症), Soeumin Hyangsayangyui-tang was prescribed based on Nature & Emotion(性情) & real purpose(恒心). 3. Result and conclusions In result, Hyangbujapalmul-tang based on Nature & Emotion(性情) & real purpose(恒心) showed best positive response. It is necessary to approach to somatoform disorders not only in clinical view but also in psychiatric view based on Nature & Emotion(性情) & real purpose(恒心)

Case Report of Physical Therapy using Proprioceptive Neuromuscular Facilitation for Activity Improvement in a Patient with Parkinson's Disease (파킨슨 환자의 활동 개선을 위한 PNF 개념을 이용한 물리치료 사례보고)

  • Purpose: Patients with Parkinson's disease suffer many restrictions in daily life. This case report investigated how intervention with proprioceptive neuromuscular facilitation (PNF)-based physical therapy can affect the activities of Parkinson's patients. Methods: The subject was a 67-year-old female patient diagnosed with Parkinson's disease 3 years ago. Since the last five months, overall activities have become difficult for her, and she finally visited a hospital to take outpatient physical therapy because of the gait difficulties she suffered. The patient's medical history, system review, body structure and function, and activities were evaluated. The patient had difficulties in activities such as lying down, sitting, standing, maintaining a standing position, and walking. The PNF-based intervention was used for treating the impairments and improving the activities. The intervention was performed for 30 min a day, three times a week, for eight weeks. The qualifier of the international classification of functioning, disability, and health was used to measure the result, and the measurement was conducted before and after the intervention for eight weeks. Results: According to the result, the scores for maintaining a standing position, moving around within the home, and going to the toilet improved to "no problem" from "moderate problem." The scores for shifting the body's center of gravity, walking short distances, and washing oneself improved from "moderate problem" to "mild problem." The scores for sitting and standing improved to "no problem" and "mild problem" from "complete problem." The scores for preparing meals and doing housework improved from "severe problems" to "mild problem" or "moderate problem." The scores for walking long distances, moving around outside the home and other buildings, and using transportation did not show significant changes. Conclusion: Intervention with PNF-based physical therapy improved the activity of patients with Parkinson's disease, thus proving its effectiveness. The case report suggested that a therapist can use PNF as a physical therapy intervention for patients with Parkinson's who suffered restrictions in daily activities.

  • Objective : To identify the effect of intensive sensory integration therapy through 5 days camp programs for children with dyspraxia Methods : A comparison of quantitative date and qualitative description between before and after sensory integration therapy camp described adaptative response of children and individual home-based program was designed and therapist educated parents about it. Results : The problem of control and apraxia which children have was improved rather than before camp and adaptive response increased. Conclusion : Follow-up study is needed to observe and investigate the effect of continuous treatment after camp participation.

The Effects of a Cardiac Rehabilitation Program on Health Behavior Compliance, Cardiovascular Function, and Quality of Life for the Patients with Ischemic Heart Disease (심장재활 프로그램이 허혈성 심장환자의 건강행위 이행, 심혈관 기능 및 삶의 질에 미치는 효과)

  • This study is aimed at developing a cardiac rehabilitation program and enlightening the effects of the program on patient's health behavior compliance, cardiovascular functional capacity, and quality of life. Using a quasi-experimental approach the nonequivalent control group pretest - posttest design was accepted for this study. The subjects of this study consisted of 55 patients with ischemic heart disease at the Cardiac Center of 'G' Hospital located in Inchon from May 1, 1998 to April 30, 1999. The patients were divided into two groups: the experimental group, which participated in the cardiac program with 30 patients and 25 patients of a control group were not involved in the program. There were two phases in the cardiac rehabilitation program: the first phase was a team approach education. It focused on reducing the risk of ischemic heart problems. The second phase was individual training by using a home based exercise program, which was comprised of 8 weeks, three sessions per week, 40-60 minutes per session, and followed by consultation. Every session involved 20-40 minutes of aerobic exercise at 40-60% of heart rate reserve, 11∼13 RPE and 10 minutes of warm-up and 10 minutes of cool-down exercises. The experimental tools for the study were the health behavior compliance scale developed by Lee, Yoon-hee (1992), and quality of life scale developed by McGirr et al.(1990). RPPsubmax were measured by the treadmill. The collected data was processed by SPSS and analyzed by χ²test and t-test. The results of this study were as follows: 1. The health behavior compliance in experimental group was significantly increased (t=5.091, p=.000) when compared to the control group. 2. RPPsubmax also decreased significantly in the experimental group when compared to the control group(t=-2.109, p=.040). 3. The quality of life significantly improved in the experimental group (t=3.853, p=.000) as compared to the control group. As the above results of this study revealed, the effectiveness of the cardiac rehabilitation program of the study was confirmed. It increased the health behavior compliance for reducing the risk of further coronary events, enhanced the cardiovascular functional capacity, and eventually improved the patient's quality of life.

Systematic Review of Assessment Tools for the Housing Environment of the Old Adults Population (노년 인구의 주거환경 평가도구에 관한 체계적 고찰)

  • Objective : This study aimed to conduct a systematic review of the assessment tools used to assess the housing environment of older adults. Methods : Data were collected from January 2015 to August 31st, 2023, by searching databases including the Cochrane Library, PubMed, and ProQuest. From the 267 articles, nine assessment tools were selected for analysis based on their original instruments. These tools were categorized and systematically organized for analysis based on their frequency of use, assessment purposes, sub-domains, scales, and other relevant criteria. Results : Among the nine tools, HOME FAST and IPAQ-E were the most frequently used (20% each). The objectives of these tools are to assess friendliness, physical barriers, fall prevention, dementia-friendly environments, physical activity, and accessibility. The measurement scope encompassed various factors, such as outdoor spaces, buildings, transportation, housing, and community support. Conclusion : When considering the suitability of housing for the older adults population, providing foundational data for the rational selection of evaluation tools with logical validity is important. This includes factors such as the objectives and measurement scopes of housing environment assessment tools.

Analyses on the Mean Length of Stay of and the Income Effects due to Early Discharge of Car Accident Patients at General Hospital (3차 병원에 입원한 교통사고환자의 평균 재원기간과 조기퇴원시의 수입증대효과 분석연구)

  • This study attempts to encourage the development of a rehabilitation delivery system as a substitute service for hospitalization such as a community based intermediate facility or home health care. We need substitute services for hospitalization to curtail the length of stay for inpatients due to car accidents. It focused on developing an estimation for early discharge based on a detailed statement of treatment from medical records of 109 inpatients who were hospitalized at General Hospital in 1997. This study has three specific purposes: First, to find the mean length of stay and mean medical expenditure. Second, to estimate the mean of early discharge from the mean length of stay. Third, to analyize the income effect per bed from early discharge. In order to analyze the length of stay and medical expenditure of inpatients the author conducted a micro and macro-analysis with medical expenditure records. To estimate the early discharge we examined with a group of 4 experts decreases in the amount of treatment after surgery, in treatments, in tests, in drug methods. We also looked their vital signs, the start of ROM exercise, the time removel, a patient's visitations, and possible stable conditions. In addition to identifing the income effect due to an early discharge, the data was analyzed by an SPSS-PC for windows and Excell program with a regression analysis model. The research findings are as follows: First, the mean length of stay was 47.56 days, but the mean length of stay due to early discharge was 32.26 days. The estimation of early discharge days was shown to depend on the length of stay. The longer the length of stay, the longer the length before discharge. For example, if the patient stayed under 14 days the mean length of stay was 7.09 while an early discharge was 6.39, whereas if the mean length of stay was 155.73, the early discharge time was 107.43. The mean medical expenditure per day of car accident patients was found to be 169,085 Won, whereas the mean medical expenditure per day was shown to be in a negative linear form according to the length of stay. That is the mean expenditure for under 14 days of stay was 303,015 Won and the period of the hospitalization of 15 days to 29 days was 170,338 Won and those of 30 days to 59 days was 113,333 Won. The estimation of the income effect due to being discharged 16 days was around 2,350,000 Won with a regression analysis model. However, this does not show the real benefits from an early discharge, but only the income increasing amount without considering prime medical cost at a general hospital. Therefore, we need further analysis on cost containments and benefits incending turn over rates and medical prime costs. From these research findings, the following suggestions have been drawn, we need to develop strategies on a rehabilitation delivery system focused on consumers for the 21st century. Varions intermediate facilities and home health care should be developed in the community as a substitute for shortening the length of stay in hospitals. In home health care cases, patients who want rehabilitation services as a substitute for hospitalization in cooperation with private health insurance companies might be available immediately.

Comparative Research of the Medical Cost, The Quality of Life, The Family burden of the Mentally III before and after the Community Mental Health Service (지역사회 정신보건서비스 제공 전$\cdot$후 정신장애인의 의료비용, 삶의 질, 가족 부담감 비교연구)

  • Community mental health management system emphasizing on the rehabilitation and the return to the community has been established and carried out for many years. The study has been demanded to prove that the decreasing rate of the recurrence of the mentally ill resulted to lower their medical costs, to enrich the quality of life, and to reduce the psychological burden of their family. This study tried to prove that the mental health services to the mentally ill which were registered in community mental health center of A city have an influence on the medical cost, the quality of their lives. the family burden. The subject group of this study were 39 home-based mentally ill patients and their 37 family members, totally 76 people registered in mental health center of A city and participated in its program. This research had been measured twice, the first before the intervention and the second after at least a year. The measuring tools in the research were the medical cost measurment tools developed by the researcher, the quality of life index by Yoo ja, Noh(1988) and the family burden by Montgonery(1985). The methods were modified and supplemented in this study. This research made use of SPSS Win 10.0. The results of this study are the same as followings. 1) There were the significant difference in the medical cost before and after the mental health service delivery. 2) The quality of lives of the mentally ill, after the mental health services delivered were significantly higher than before. 3) The family burden were significantly reduced after the delivery of community mental health services. Community mental health services brought out efficient results to the social return and rehabilitation. And these results means that the mentally ill changed highly the quality of life and their burden of family and medical cost were reduced. So the public organization and the private society should help positively the mentally ill and their family through mental health policy and social service agency to live healthy lives and to be valuable member of society.
