목적 : 본 연구는 강원도 태백지역에 거주하는 초기치매환자를 대상으로 실시한 방문 인지 작업치료 프로그램의 효과를 알아보고자 하였다. 연구방법 : 초기치매환자 30명을 대상으로 작업치료학과 학생 30명과 자원봉사자들이 주 1회 총 8회기의 가정방문 인지작업치료 중재를 실시하였다. 프로그램 전후의 대상자들의 인기기능의 변화를 알아보기 위해 MMSE-K를 사용하여 측정하였다. 또한 가정방문 인지작업치료에 대한 초기치매 환자들의 인식과 만족도를 확인하였다. 결과 : 초기 치매환자에게 가정방문 인지작업치료 적용한 결과 전반적인 인지기능의 향상이 나타났으며 특히 지남력, 기억회상, 주의집중력, 언어력의 경우 통계학적으로 유의한 향상이 나타났다. 결론 : 인지기능의 향상과 일상생활능력의 향상을 위한 가정방문 작업치료 프로그램이 치매환자의 중재에 있어 유용하게 적용될 것으로 사료되며 향후 가정방문 작업치료 서비스 도입을 위한 다양한 가정방문 작업치료 중재 프로그램 개발이 필요하다.

본 연구는 순천 광양 지역에 거주하는 만18세 미만 아동의 가정방문 작업치료서비스에 대한 실태 및 만족도를 조사하였다. 가정방문 작업치료서비스에 대한 일반적인 사항과 가정방문치료 시 이용 서비스 종류, 가정방문 작업치료의 총 만족도, 아동에게 도움이 된 사항, 부모나 가정에 도움이 된 사항으로 구분하여 산술통계를 실시하였다. 그 결과 가정방문 작업치료는 장애아동 부모회를 통해서 가장 많이 알게 되었고 가정방문 작업치료에 대한 만족도는 높게 나타났다. 이 연구를 통하여 앞으로 정부의 체계적이고 안정적이며 지속적인 재활치료 지원사업을 구축하는데 기초자료를 제공하고자 하였다.

목적 : 본 연구는 재가 뇌졸중 환자를 대상으로 가정방문 작업치료를 제공한 사례를 통해 가정방문 작업치료 서비스의 효과성을 검증하고자 하였다. 연구방법 : 서울시 소재 보건지소에 가정방문 작업치료 서비스를 신청한 뇌졸중 환자 두 명을 대상으로 가정방문 작업치료 프로그램은 대상자의 일상생활동작 기능, 작업수행 기능, 인지기능, 정서 기능을 평가하고 클라이언트 중심 접근(Client-centered approach)을 기반으로 작업치료 목표를 설정하고 중재를 계획하였다. 작업치료 프로그램은 클라이언트의 주요 문제를 바탕으로 여가 활동 탐색 및 참여를 목표로 설정하여 12회기의 프로그램으로 구성하였다. 결과 : 두 사례 모두 COPM 점수에 향상이 있었고, 사례 2에서 MBI와 K-MMSE 점수의 향상이 있었다. 또한 사례 1에서 KGDS 점수의 향상이 있었다. 결론 : 가정방문 작업치료는 뇌졸중 환자의 삶의 질 향상 및 경제적 부담 등을 고려하면서 뇌졸중 환자의 일상생활활동, 인지, 정신적 기능의 변화가 있음을 확인하였다. 가정 및 지역사회 통합에 있어 기관중심재활에서 지역사회중심재활로 지속적이고 연속된 재활 서비스가 활성화가 요구되어지며, 병원에서 퇴원하는 뇌졸중 환자들의 신체적, 인지적, 정신적 및 사회적 접근의 증진을 위한 적극적인 가정방문 작업치료서비스가 필요하다.

목적 : 본 연구는 노인요양시설에서 근무하는 작업치료사의 직무 스트레스와 근무관련 특성과의 상관관계를 알아봄으로써 향후 스트레스를 측정하고 관리하는데 필요한 기초자료로 활용하고자 하였다. 연구방법 : 연구의 대상은 노인요양시설에서 근무하고 있는 작업치료사로서 2012년 5월 12일부터 6월 30일까지 온라인과 오프라인 설문방법으로 총 46부를 수집하여 분석하였다. 결과 : 직무스트레스 8개 요인 중 전문직으로서의 갈등, 전문지식과 기술의 부족, 대인관계상의 문제, 상사(국장, 과장 등)와의 관계 및 업무상 갈등, 치료의 한계에 대한 심적 부담, 부적절한 대우 및 보상 등 6개 요인이 근무연수, 보수총액, 치료 외 근무, 직장의 안정성과 통계적으로 유의한 상관관계를 나타냈다. 결론 : 본 연구는 노인요양시설에서 근무하는 작업치료사들의 직무스트레스와 그 요인들을 알아보고자 하였다. 직무스트레스 요인 평균은 3.52점으로 높은 수준이었으며, 스트레스 수준을 낮추기 위해 작업치료사들을 대상으로 꾸준한 연구가 진행되어야 할 것이다.

The proportion of the aged people in Korea has increased rapidly by more than 7% in 2000. A survey says that the changing rate from the aging society to the aged is the highest among the OECD countries. The prevalence rate of the aged population is on the increase. Chronic diseases such as arthritis and strokes limits their body's functions and activities of daily living. It is necessary that functional training. ADL training and continuous treatment be provided by Physical or Occupational Therapists. Discontinuance of treatment can cause the aged people to be in a bed state. The aged people need a variety of home care services at their convenience. Occupational and Physical therapist should play a major role in providing the rehabilitation-base home care services. Further they are expected to contribute to the extension of health care services such as home care and nursing home programs.

Objective : To explore the effectiveness of sensory integration program of home-based outcomes in child with sensory modulation disorder. Methods : This study used the simple case study. The subjects were three children diagnosed as developmentally delayed who 14 month and 26 month males and a 15 month female. After initial evaluation, parents were educated on sensory diet and Wilbarger protocol method for 30 minutes twice for home-based treatment and asked to make out daily treatment planning. To measure improvement of children, we used for the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999). Results : After the home-based program, the scored of the Denver Developmental Screening Test II(DDST II), Wee Functional Independence Measure(WeeFIM), hand function, functional ability, oral motor function, Sensory Profile(Dunn, 1999) were higher rather than initial evaluation after the home-based program. Conclusion : The home-based program is effective for children with sensory modulation disorder but parents had to be educated by therapist specific evaluation and treatment in sensory integration.

본 연구의 목적은 보편적 홈 디자인(universal home design)과 관련된 기존 문헌을 고찰하고, 장애인의 접근성과 독립성을 증진하기 위한 전자 및 정보통신기술로써 스마트 홈 기술을 제안하는 것이다. 환자의 가옥환경 이해는 치료와 퇴원계획에서 빠뜨릴 수 없는 부분이며 가옥평가와 변형이 작업치료 영역에서 매우 중요함에도 불구하고, 국내에서 이 주제를 다룬 연구가 매우 부족한 실정이다. 전자 및 정보통신 기술은 운동장애, 시각장애, 청각장애, 인지장애 등이 있는 다양한 경우에 기존의 보편적 디자인을 보완할 수 있는 대안이 될 수 있을 것이다.

Background: This article reviewed the current status of home-based physical therapy infrastructure in long term care insurance and then solved the problem. Method: We used two forms of data that were acquired from ⅰ) the Ministry of Health & Welfare and the Family and National Health Insurance Corporation, ⅱ) a home-visiting health care program, and ⅲ) evaluation data from the Korea Health Industry Development Institute. The home-based physical therapy program was then analyzed. Results: The role and concept of home-based physical therapy was not clearly established. There were few home-based physical therapy programs in the community. The manpower of home-based physical therapists in the home-visiting health care program was very low. The role between home-visiting nurses (caregivers) and home-based physical therapists was mixed. Research and promotion regarding home-based physical therapy was poor. Conclusion: To establish a system of legal, long-term care insurance, we must increase the manpower of home-based physical therapists and the amount of research pertaining to the demand for home-based physical therapy.

Objectives: The aim of this study was to examine the effects of home-based cognitive physico-occupational therapy(HBCPOT) on cognitive function, depression, and quality of life in dementia patients, using a cognitive impairment model. Methods: The data was analysed for Mini-Mental State Examination-Korean version (MMSE-KC) for assessing cognitive function, Quality of life-Alzheimer's Disease (QOL-AD) and Geriatric Depression Scale (GDS) in 31 dementia patients who received home-based cognitive physico-occupational therapy for one hour once a week for 12 weeks by a trained occupational therapist at the Dementia Prevention and Management Center of Y-city during 20111-2013. Results: Among these 31 patients, 18(58.1%) were female, and 25(80.6%) had Alzheimer's Disease. After HBCPOT, the mean scores of MMSE-KC and QOL-AD were significantly improved, but GDS score was not significantly decreased. Conclusions: It was suggested that HBCPOT was effective in improving cognitive function and quality of life.

목적 : 본 연구는 복지국가의 개념과 사회복지영역을 살펴보고, 우리나라 장애인복지정책의 발전을 비교 분석해 보기 위함이다. 연구방법 : 문헌고찰을 통하여 복지국가의 개념과 사회복지의 영역을 기술하였고, 우리나라의 사회복지정책과 장애인정책을 각 정부별로 비교 분석하였으며 마지막으로 노무현 정부의 장애인정책을 분석하였다. 결과 : 경제성장과 더불어 복지정책과 장애인복지정책은 '선택적 복지'에서 '보편적 복지'의 형태로 발전하였다. 사회복지정책의 영역에 있어서도 과거 이승만 정부시절의 '최소한의 보장'에서 노무현 정부의 '참여복지'에 이르기까지 영역의 확대가 이루어지고 있으며 이러한 현상은 장애인 복지정책에서도 나타나는 현상이다. 결론 : 지역사회 작업치료사는 오늘날 '보편적 사회복지'의 큰 틀에서 등록 장애인 뿐 만 아니라 지역사회의 드러나지 않는 장애를 지닌 일반인과 노인을 위한 방문재활서비스와 같은 미 충족 작업치료서비스를 발견하고 제공해야 할 것이다.
