저소득 재가노인의 생활만족도, 일상생활활동, 우울 및 건강행위에 관한 연구 (Life Satisfaction, Activities of Daily Living, Depression and Health Behavior of Low Income Elderly Living at Home)

  • Purpose: This study aimed to identify the relationships among life satisfaction, activities of daily living, depression and health behavior in low income elderly living at home. Methods: Study participants were 455 elderly who were receiving home visit services from the Daegu Regional Office of Patriots and Veteran Affairs. Data were collected through personal interviews using questionnaires from July to August, 2010. Results: Average scores of ADL and IADL were 7.82 out of 21 and 12.67 out of 33 respectively, which indicate relatively independent to everyday life. Mean scores of depression, health behavior and life satisfaction were 8.61 out of 15, 88.14 out of 132 and 48.57 out of 60. There were significant relationships among the variables of life satisfaction, activities of daily living (ADL, IADL), depression and health behavior. A significant factor influencing life satisfaction was health behavior ($\beta$=.134, p=.020). Conclusion: The findings of this study would be a useful information for constructing an intervention program to care for elderly.

기혼여성들의 임신동기에 관한 연구 (A Study on Motivation for Pregnancy of Married Women in Korea)

  • Purpose: This study is to define the structural patterns of pregnancy motivation of married women. Method: Q-methodology was used on 110 statements collected from interviews with women and a literature review. 38 Q-samples were selected. The Q-samples were administerd to 30 randomized P-sample. Result: Data analysis of Q-type obtained by QUANL program shows three types of subjectivity in the perception of pregnancy motivation. Type 1, $\ulcorner$Family-connection strengthening$\lrcorner$, Pregnancy motivation is characterized by the tendency to make home, to tie family member, to connect couple firmly. Type 2, $\ulcorner$Self-satisfaction$\lrcorner$, Women of this type take pleasure and happiness from child rearing in terms of being a mother and having maternal affection. Type 3, $\ulcorner$Self-centered$\lrcorner$, This type of pregnancy motivation puts on high emphasis on self achievement and they make a family planning suitable for the maternal role. Conclusion: The stronger was women's pregnancy motivation, the more positive were women's pregnancy experience and maternal role attainment. Conclusion: Therefore, When nurses take care of pregnancy women, they should understand the women's psycho-social aspects and then apply a program of nursing intervention to the pregnant women, It is necessary to continue to study on social and psychological problems related to pregnancy.

저소득층 암환자들의 심리사회적 적응 과정 (Psychosocial Adjustment of Low-Income Koreans with Cancer)

  • Purpose: To describe psychosocial adjustment of low-income Koreans who have cancer. Methods: Data were collected during 2008 using individual in-depth interviews with 18 Korean people with cancer. The income status of the participants was low, 11 were recipients of the National Basic Livelihood Protection program. Mean age was 58.3 yr and 11 were female. Five participants had stomach cancer, five, colorectal cancer, and four, breast cancer. Data were analyzed using grounded theory methodology. Results: The core category emerged as 'bearing up alone with double suffering'. 'Poverty and cancer: A double suffering' emerged as a causal condition. The adjustment process consisted of three stages: 'forming a treatment will to live', 'practicing for the cure', and 'restructuring self and repaying favors'. Each stage indicated action-interaction strategies which were employed to bear up alone with double suffering during the illness process. Self-reflection, parental responsibility, and support from the public sector played important roles in overcoming the double suffering. Two types of consequences were identified: Transcended life and strained life suppressed by poverty and cancer. Conclusion: The results provide insights into the psychosocial adjustment process for low income Korean with cancer and can be used in developing and implementing efficient home-care services for these people.

ICNP의 후향적 방법에 의한 한국가족현상의 실무타당성 검증연구 (Validity Field Testing of Family Nursing Phenomen in Korea by the Retrospective Method of ICNP)

  • This study was conducted to test the validity of characteristics of family nursing phenomena in Korea identified in a previous study by Yoon, S. Y, et al. (1999). Data for this study were collected from 355 (first measurement 186, second measurement 169) nurses whose family nursing careers were more than one year using a cross sectional survey method. The distribution per one characteristic ranged between 0 to 5. The phenomena that had two or more characteristics and had a score of 3.0 points or more included disturbance in marital role', 'disturbance in family communication', 'inappropriate family coping', 'lack of family intimacy', 'unhealthy life style', 'deficit of financial management skill and support', 'inadequate care management sick member', and' inadequate home-making'. The essential characteristics of the 8 phenomena presented above included 27 items. Further studies on family nursing phenomena in Korea are needed to support the evidence through surveys of families in the field. A reconfirmation study, as well as qualitative research on the low validity of the characteristics, needs to be more performed in the future.

국내 방문간호 연구동향 분석 연구 (A Study on the Research Trends in Visiting Nursing in South Korea)

  • 본 연구는 국내에서 발표된 방문간호 관련 연구의 동향을 파악하기 위하여 1993년부터 2018년까지 학술지에 게재된 논문 282편을 분석하였다. 방문간호 연구는 1993년을 기점으로 시작되어 지속적으로 증가하였고, 2014년에서 2018년에는 67편(23.8%)으로 그 수가 다소 감소하였다. 또한 우리나라 방문간호 3가지 유형 중 의료기관 가정간호 관련 연구가 154편(53.5%)으로 과반수 이상을 차지하였으며, 연구주제는 주로 방문간호 서비스(40편, 13.8%) 및 방문간호 프로그램 및 기술 개발(38편, 13.1%)과 관련된 주제가 많은 것으로 나타났다. 연구방법으로는 양적연구가 269편(95.4%)으로 질적연구 9편(3.2%), 혼합연구 4편(1.4%)에 비해 높은 비율로 수행되었으며, 양적연구 중에서도 조사연구가 177편(65.7%)으로 가장 많았다. 본 연구는 방문간호 관련 연구의 동향을 파악함으로써 후속 연구의 방향을 제시하였고, 방문간호 관련 중재연구의 기초자료로 활용될 수 있다는데 의의가 있다.

취약계층 학령전기 아동의 건강상태와 어머니의 아동 건강관리실태 (Health Status of Vulnerable Preschool Children and Their Mothers' Health Management)

  • 목적 본 연구는 취약계층 학령전기 아동의 건강상태와 어머니의 아동 건강관리실태를 파악하기 위해 수행되었다. 방법 대상자는 259명의 취약계층 가정의 어머니이며, 연구도구는 선행 연구를 기반으로 연구자가 구성한 설문지이다. 자료는 2010년 1월부터 2월까지 경상남도 20개 보건소의 방문간호사가 가정방문을 통해 수집하였으며, SAS를 이용하여 분석하였다. 결과 비만 아동이 6.5%, 저체중 아동이 19.3%였으며, 건강문제는 아토피가 14.7%였다. 손 씻기 수행율은 화장실 사용 후 69.1%, 식사 전 63.3%였으며, 아침 결식 아동이 15.8%, 규칙적으로 식사하지 않는 아동이 32.0%였다. 어머니의 아동 성교육률은 45.7%, 정기적인 치과검진율은 56.3%였고, 시력검사와 청력검사를 하는 경우가 각각 66.8%와 61.1%였다. 성장발달 촉진을 위한 가정환경은 41점 중 34.3점이었고, 질병유무와 어머니의 연령과 학력에 따라 차이가 있었으며, 사고예방을 위한 가정환경은 22점 중 17.5점이었고, 유치원 등원 여부에 따라 차이가 있었다. 결론 비만 아동과 저체중 아동의 비율이 높고, 기본적인 개인위생 습관과 식습관이 적절하지 못한 아동의 비율이 높았으며, 어머니의 아동건강관리실태는 기초적인 건강관리와 아동의 성장발달 증진과 사고예방을 위한 가정환경의 조성이 미흡한 것으로 확인되었다. 따라서 가정방문 간호사들이 취약가정 학령전기 아동의 건강과 성장발달을 증진시킬 수 있는 가정방문용 간호중재 프로그램을 개발할 필요가 있는 것으로 생각된다.

기술.가정 교과에 기초한 노년기 주생활 계획에 관한 청소년의 의식 조사 (A Survey on the Consciousness of Adolescents about the Elderly Housing Life Plan from Technology.Home Economics Textbook)

  • The purpose of this study was to grasp the consciousness of adolescents about their future in old aged housing. This research was executed with a questionnaire survey method. Data samples were collected from 255 students who finished first grade curriculum in high schools in Chonbuk province and Seoul city. The results were as followed. First, a healthcare plan was very highly recognized by students, and they wanted to live as couples or single in their old age. Second, the respondent students recognized elderly housing as a home for the aged, a silver town and nursing home. 72.1% of the respondents preferred a single detached house as an elderly house form. They preferred an elderly housing district with well provided convenient facilities for daily living. They wanted to live in the country or rest areas for the aged. They preferred an interior space composition with LDK with 2 bedrooms. 52.0% students wanted a neighborhood for their friends to live together. Third, 28.3% of the respondent students thought the appropriate time to move in elderly exclusive housing was 66-70 years old. And when their physical condition worsened or they needed care, they would. want to move into that housing. Also, they wanted the most convenient facility and a medical care facility in elderly exclusive housing. Forth, students' space design needs for elderly were high, more than 4 points. It was especially needed safety in space design, and this grade was 4.56(,53) points. Also the needs of safety and the convenient facilities.equipments were statistically significant by boys and girls.

Health in Optimal Fitness and its Related Factors in Young Korean Children Born Prematurely

  • Purpose: This study was conducted to describe health in optimal fitness (HOF) in young children born prematurely and to analyze factors affecting HOF in health status, investment resources, and anthropological values, based on HOF theory. Methods: A case-control study of 76 children with preterm births (PTB) was conducted at 24 to 42 months of corrected age. Their HOF status was evaluated based on height, weight, head circumference, and the Korean-Bayley Scale of Infant Development-II and classified as either HOF-achieved or HOF-uncertain in the domain of growth, development, and all together. Results: For growth, development, and all, 26.3%, 27.6%, and 47.4% of children, respectively, belonged to the HOF-uncertain group. Logistic regression analysis showed that longer length of hospital stay (${\geq}21days$; OR=7.8; 95% CI [1.5, 40.5]), worse scores on the Home Observation for Measurement of the Environment (HOME) (${\geq}38$; OR=0.1; 95% CI [0.0, 0.4]), having a working mother, (OR=5.7; 95% CI [1.2, 27.6]), and an older mother (${\geq}35years$; OR=8.8; 95% CI [2.1, 37.3]) were statistically significant contributors of HOF-uncertain in the domain of all. Conclusion: Findings show that young children born prematurely with prolonged stays in a neonatal intensive care unit and insufficient socioeconomic resources at home are more likely to exhibit delayed growth and development.

영아의 수유 및 보충식에 대한 조사연구 (A Survey Of Infant Feeding Practices In Seoul, 1991.)

  • A survey of infant practices was conducted to provide information on which to base planned nursing interventions. The subjects were a convenience sample of 168 mothers visiting out - patients departments of five general hospitals and one public health center in Seoul for immunizations or treatment for common colds for their infants from two to 12 months of age. Data collection was carried out from July 8th to September 30th, 1991 using a questionnaire of 84 questions, 31 on the type of feeding, 22 on supplementary feeding and 21 on demographic infer mation. The results are summarized as follows : 1) The type of feeding was primarily artificial feed-ing (63.1%), followed by mixed feeding (22%) and breast feeding (14.9%) of the 59 mothers expecting during pregnancy to breast feed, 54. 2% changed artificial feeding and 30.5% went on to breast feed as they had expected. For mothers expecting to continue breast feeding over seven months. only one infant was being breast fed for over seven months. 2) For the 106 mothers using artificial feeding, 70. 8% hed attempted breast feeding, 64% of them for less than a month. Breast milk had been suppressed by for medication (38.7%) : 34.9% had used no specia] means. 3) The major reasons for replacing breast feeding with artificial feeding were the infants' demand for more milk (47.2% ) and insufficient supply of breast milk (49.3%). 4) Most mixed feeding was started at the age of one to three months (59.5%). Only 34.4% gave an artificial feeding after breast feeding : most (46%) alternated breast feeding with artificial feeding. On the whole, the motive for mixed feeding was the lack of breast milk (70.3%). 5) Many mothers (81.8%) were adding vitamin or mineral supplements to artificial milk and 51.5% were adding something to faciliate digestion. As for the method of sterilizing milk bottles and nipples, 56% had sterilized them together in boiling water from the beginning : 27% were just washing the bottles after boiling only once initially when measuring artificial milk powder, 31. 5% of the mothers over filled the measuring spoon rather than to the level. 6) The mother's occupation was related to her way of feeding. Mothers at home full time did more breast feeding than mothers employed outside the home. (x²=5.72, p=〈0.05). 7) Most mothers began supplementary food, from three to four months (48.8%) : 11.2% began later than seven months. Supplementary food was given between milk feedings by 67.2% of the mothers : 19.2% gave it before a milk feeding. Some mothers(26.4%) made their own supplementary food : 19.2% used ready - made supplementary food products for convenience. Recommendations for nursing interventions included : 1. Prenatal education about the advantages of breast feeding and breast care, and home visits after delivery for counselling related to breast feeding. Correct preparation of artificial feeding methods need to be taught in both pre & postnatal periods. In addition, specific education about supplementary feeding needed. 2. Further research is indicated about the Perceived lack of supply of breast milk and about the effectiveness of nursing interventions to Promote breast feeding.

안전한 노인요양시설 환경을 위한 실내 바닥마감재에 관한 연구 (A Study on Interior Flooring Materials for Safe Elderly Nursing Home Environment)

  • The number of elderly who wants to access to Nursing Homes (NH) will increase due to the rapid aging society and domestic changes. Those who move into NH expect 24/7 care service in safe environment. Providing space free from danger, especially from fall, for the frail elderly is essential. The purpose of this study is to categorize performance needs of flooring materials in the aspect of safety, and analyze materials so that adequate ones for each space in NH can be suggested. Performance needs are as follows; slip resistance (dry/wet), fire resistance, resilience, water resistance, soil retardant, anti-bacterial, gloss, sound absorbtion. maintenance, durability, ease replacement, color and pattern variety, visual and tactual texture, tactile warmth, IAQ, sustainable material (before use), impact to nature (after use). They are categorized under function, economy, sensibility and sustainability. It was found that there are better materials than common ones that has been used repeatedly in NHs, such as vinyl sheets and VCT. In overall, Cork flooring and nylon carpet met all four categories, followed by wool carpet, rubber and linoleum. For bedroom, wood flooring, Cork, rubber, wool carpet, nylon carpets were suggested. In bathroom with shower, rubber, vinyl sheet and porcelain tiles were safe materials. As living/dining room and corridor floors, wool carpet, nylon carpet, cork flooring would be excellent as they are resilient and durable. The result of this paper can be used by both NH managers and material companies, resulting better quality of life of the elderly by providing safe environment.
