Functional Anatomy of the Temporomandibular Joint and Pathologic Changes in Temporomandibular Disease Progression: A Narrative Review

  • The temporomandibular joint (TMJ) is one of the most unique joints in the human body that orchestrates complex movements across different orthogonal planes and multiple axes of rotation. Comprising the articular eminence of the temporal bone and the condylar process of the mandible, the TMJ integrates five major ligaments, retrodiscal tissues, nerves, and blood and lymph systems to facilitate its function. Cooperation between the contralateral TMJ and masticatory muscles is essential for coordinated serial dynamic functions. During mouth opening, the TMJ exhibits a hinge movement, followed by gliding. The health of the masticatory system, which is intricately linked to chewing, energy intake, and communication, has become increasingly crucial with advancing age, exerting an impact on oral and systemic health and overall quality of life. For individuals to lead a healthy and pain-free life, a comprehensive understanding of the basic anatomy and functional aspects of the TMJ and masticatory muscles is imperative. Temporomandibular disorders (TMDs) encompass a spectrum of diseases and disorders associated with changes in the structure, function, or physiology of the TMJ and masticatory system. Functional and pathological alterations in the TMJ and masticatory muscles can be visualized using various imaging modalities, such as cone-beam computed tomography, magnetic resonance imaging, and bone scans. An exploration of potential pathophysiological mechanisms related to the TMJ anatomy contributes to a comprehensive understanding of TMD and informs targeted treatment strategies. Hence, this narrative review presents insights into the fundamental functional anatomy of the TMJ and pathological changes that evolve with TMD progression.

Reversed Cyclic Load Tests on Deep Beam-and-Exterior Column Joints (깊은보-외부기둥 접합부의 반복 횡하중 실험)

  • The most common structural system for apartment buildings in Korea is adopted to combine structural systems: for example, a moment-resisting frame will be used for lower stories and bearing wall system for the upper stories. This type of buildings have soft and/or weak stories in lower stories, and it may lead to collapse of those buildings during the large earthquake. Reversed cyclic load tests were conducted to estimate the performance and behavioral characteristics of deep beam and exterior column Joints. Experimental parameter is the amount of transverse reinforcement (designed by ACI code and Sheikh's procedure). The results of this study are as follows: (1) The required transverse reinforcement of column designed by Sheikh's procedure requires 2.9 times larger than that designed by ACI procedure. Large amount of transverse reinforcement increase the ductility of the column. (2) Most of the lateral drift in the column is due to the flexural deformation in the joint and plastic hinge region and up-lift rotation. (3) Transverse reinforcement in the exterior column shall be required not only in the hinge region but also in the joint.

An Experimental Study on the Structural Behavior of Steel-Concrete Composite Rahmen Bridge with Hinged End Supports (하단힌지 강합성 라멘교의 구조적 거동에 대한 실험적 연구)

  • The rahmen bridge is well known common type of bridge in which all members are connected rigidly. The rahmen bridge is built for several situations because it has many advantages such as no need of bridge bearing system, easy of maintenance, reduction of the cross-sectional area of superstructure, and relatively low construction cost compared with other bridge types. Recently, to lengthen the span of rahmen bridge system, steel-concrete composite beam is used for superstructure of rahmen bridge instead of normal concrete girder with slab. However, member forces are increased because of extension of span length of superstructure and substructure is designed and constructed inefficiently when steel-concrete composite rahmen bridge is designed. In this study, new-type steel-concrete composite bridge is suggested. New-type steel-concrete composite rahmen bridge is adopted hinge connection between abutment and foundation for the reduction of the bending momemt at the foundation. In this study, we present the results of experiment conducted to estimate the load carrying capacity of new-type steel-concrete composite rahmen bridge and the structural characteristics of hinge connection.

A COMPARATIVE STUDY ONMAGNETIC RESONANCE IMAGE AND SECTOGRAPH OF HUMAN TNJ (악관절의 자기공명영상과 시상단층 방사선촬영상에 관한 비교연구)

  • For understanding of anatomy, physiology, and diseases of human TMJ, it is required to evaluate quantitatively the movement of the disc and condyle head of mandible. The histologic section of cadaver TMJ were examined, and the magnification of the MR image and its details of anatomy were evaluated. And then a quantitative analytic method, by comparing the Sectograph and the MR image of vital human TMJ, was proposed. For this study, 15 subjects(Male, 24~35years) were selected from a prosthodontic examination randomly, and each subject’s five interocclusal rubber registration records were made on the ICP, and 5, 10, 15, and 20mmjaw opening positions. All subjects were radiographed with a Denar Quint Sectograph Image System(Denar Corp., USA), and imaged with a MRP-20EX MR Image System(0.2T, Permanent Magnet Type, Hitachi Medical Corp., Japan) using an 100mm diameter bilateral type surface coil. These images were traced on the acetate tracing paper, and analyzed In this study, the findings led to the following conclusions. 1. In comparison of the histologic section of autopsy specimen with the MR image at the same section, the size(dimension) of MR image was 70% of the real one. It was possible to recognize the shape of articular disc, anterior and posterior attachments, and adjacent soft tissues, because of the excellent reproducibility of anatomical structure. 2. When we compared the amount of joint space on MR image with that of joint space on sectograph, the amount of joint space on sectograph was significantly greater than that of joint space on MR image, except at the top of condylar head. 3. The position of minimum joint space on sectograph at intercuspal position didn't coincide with the middle position of articular disc on MR image, and was approximately in the anterior third of posterior band of articular disc. 4. The amount of condylar movement on MR image at opening movement was greater than that of articular disc movement. From Intercuspal position to 5mm jaw-opening movement, the condylar movement showed hinge one, and over the range 5mm jaw-opening it suggested hinge & translatory one. 5. In terms of area variation of articular disc measured on MR image in sagittal plane, the area of posterior band increased with increasing the amount of Jaw opening, but the area of anterior band decreased conversely.

  • For the quality control of the industrial products, an automatic hole measuring system has been developed. The measurement device allows X-Y movement due to contact forces between a hole and its own circular cone and the device is attached to an industrial robot. Its measurement accuracy is about 0.04mm. This movement of the plate is measured by two LVDT sensor system. But this system using the LVDT sensors is restricted by high cost and precision of measurement and correspondence of environment so particularly, a vision system with CCD-Camera is discussed in this paper for the above mentioned purpose. The device consists of two of two links jointed with hinge pins basically and, they guarantee free movement of the touch prove attached on the second link in the same plane. These links are returned to home position by the spring plungers automatically after each process for the next one. On the surface of the touch prove, it has a circular white mark for camera recognition. The system detect and notify the center coordinate of capture mark image through the image processing. Its measuring accuracy has been proved to be about $\pm$0.01mm through the repeated implementation over 200 times. This technique will shows the advantage of touch-indirect image capture idea using cone-shaped touch prove in various symmetrical shaped holes particulary, like tapped holes, chamfered holes, etc As a result, we attained our object in a view of the accuracy, economical efficiency, and functionality

A Study on Policy Alternatives for Major Changes in the Korea's Agricultural Energy System (우리나라 농업 에너지체계의 전환을 위한 정책대안 연구)

  • The agricultural sector's economic structure in Korea is regarded to encounter major barriers on the way toward revitalizing its economic prosperity. Among many, the energy-related problem is one of prime nuclei embedded in the country's agricultural sector. The ought-to-come structural changes in the country's agricultural energy system hinge upon the central government's policy direction as well as efforts of local governments and local farming community members. The indirect aids via 'cross subsidy' of electricity tariff rate and 'tax-exempt price' of oil fuels are two notable causes of the unsustainable energy consumption pattern in the country's agricultural sector. As measures, demand-side management(DSM) and energy-efficiency promotions are regarded to be the most attractive methods for energy conservation and economic productivity as well. Development of renewable energy sources are also receiving a great deal of attention for the long-term alternatives to the country's existing oil-based agricultural production mode. This study examines the contributive potential of DSM approaches and renewables-based technologies. With the critical evaluation on the concurrent adversities of the country's agricultural energy system, various sources of renewable energy-solar power, wind power, biomass, etc.-are examined for the purpose of technological and economical viability. As sufficient potentials of renewable energy sources are being estimated, both the system production cost and the installation cost for the county's rural areas are expected to lower in the long term. DSM options are also evaluated to be fruitful even in the short term. Both the public and civil arenas must galvanise each side's effort in order to promote these policy options and community potentials.

Approximate System Reliability Analysis Under Multiple Time Varying Loads (복합 하중하에서의 구조물 체계 신뢰도 해석)

  • The evaluation of the system reliability is generally quite difficult and costly as the structure becomes large and complex, especially when it is subjected to multiple time varying loads, and for redundant structures which have many possible modes of failur, e.g., system collapse through the formation of plastic hinge mechanisms. In reality most loadings acting on the structures are random in intensity as well as in occurrence time and duration. To include the load variability in time, the loads are described in terms of stochastic processes. Based on a tri-modal upper bound, a point estimate for the system reliability has been developed for more accuracy without extensive computational effort. This tri-modal point estimate also ensures the continuity of the system reliability function, which is a necessary condition in many nonlinear programming techniques. In addition, the Load Coincidence method, by which the combined effect of time varying loads are taken into account, has been modified to suitable for cases with an always-on load.

  • The shearing process for the sheet metal is normally used in the precision elements such as a frame of TFT-LCD or lead frame of If chips. In these precision elements, the burr formation prevents the system assembly and needs the additional burr removing process. In this paper, we developed the small size NC punching system which has an aligning kinematics between the rectangular shaped punch and die. The punch is driven by an ai cylinder and the sheet metal is moving on the X-Y table system which is driven by two stewing motors. The microprocessor control the whole system and communicate with the monitoring PC by RS232C serial communication protocol. The graphic user interface program in PC monitors nil control the punching system. The cross shaped joint hinge supports the punching die and positioned by two differential screws, whose are installed in perpendicular directions. The aligning between the punch and die is performed using the sheets of half thickness(0.1mm Brass) of the real process for the frame of the TFT-LCD. Using half thickness Brass, the burr formation is magnified and we can decide the aligning direction more easily then using the real thickness(0.2mm) Aluminum. In this paper, the aligning results are measured manually using the SEM photographs and we hope to make the automated aligning procedures using some kinds of image processing techniques.

Performance based assessment for tall core structures consisting of buckling restrained braced frames and RC walls

  • In a tall reinforced concrete (RC) core wall system subjected to strong ground motions, inelastic behavior near the base as well as mid-height of the wall is possible. Generally, the formation of plastic hinge in a core wall system may lead to extensive damage and significant repairing cost. A new configuration of core structures consisting of buckling restrained braced frames (BRBFs) and RC walls is an interesting idea in tall building seismic design. This concept can be used in the plan configuration of tall core wall systems. In this study, tall buildings with different configurations of combined core systems were designed and analyzed. Nonlinear time history analysis at severe earthquake level was performed and the results were compared for different configurations. The results demonstrate that using enough BRBFs can reduce the large curvature ductility demand at the base and mid-height of RC core wall systems and also can reduce the maximum inter-story drift ratio. For a better investigation of the structural behavior, the probabilistic approach can lead to in-depth insight. Therefore, incremental dynamic analysis (IDA) curves were calculated to assess the performance. Fragility curves at different limit states were then extracted and compared. Mean IDA curves demonstrate better behavior for a combined system, compared with conventional RC core wall systems. Collapse margin ratio for a RC core wall only system and RC core with enough BRBFs were almost 1.05 and 1.92 respectively. Therefore, it appears that using one RC core wall combined with enough BRBF core is an effective idea to achieve more confidence against tall building collapse and the results demonstrated the potential of the proposed system.

  • An original sensor system based on Fiber Bragg Gratings (FBG) for the strain monitoring of an adaptive wing element is presented in this paper. One of the main aims of the SARISTU project is in fact to measure the shape of a deformable wing for performance optimization. In detail, an Adaptive Trailing Edge (ATE) is monitored chord- and span-wise in order to estimate the deviation between the actual and the desired shape and, then, to allow attaining a prediction of the real aerodynamic behavior with respect to the expected one. The integration of a sensor system is not trivial: it has to fit inside the available room and to comply with the primary issue of the FBG protection. Moreover, dealing with morphing structures, large deformations are expected and a certain modulation is necessary to keep the measured strain inside the permissible measure range. In what follows, the mathematical model of an original FBG-based structural sensor system is presented, designed to evaluate the chord-wise strain of an Adaptive Trailing Edge device. Numerical and experimental results are compared, using a proof-of-concept setup. Further investigations aimed at improving the sensor capabilities, were finally addressed. The elasticity of the sensor structure was exploited to enlarge both the measurement and the linearity range. An optimisation process was then implemented to find out an optimal thickness distribution of the sensor system in order to alleviate the strain level within the referred component.
