누두흉의 임상적 고찰-14례 보고- (A Clinical Study of the Funnel Chest: Report of 14 Cases)

  • Fourteen cases of funnel deformity, 11 were male and the others female, treated over a eleven-year period, are presented. The overall results with the methods described by the authors, Wada, Shannon, Adkins and Ravitch appear to be excellent. The symmetric depression was more common In children under the age of 12 years and asymmetric one Increased after the age of 12. Six cases of abnormal cardiac auscultatory findings revealed no consistent hemodynamic abnormalities. An 21-year-old female was associated with left lung agenesis and dextroversion of the heart, and a 6-year-old boy with congenital bronchogenic cyst who underwent right upper lobectomy. EKG changes associated with the anomaly were observed in all. Pulmonary function test showed the range from normal to moderate restriction which did not Improve in only one patient after operation, but the patient did not complain any restriction In activity. Some transient complications were developed which resulted in improvement.

  • 세종병원 흉부외과에서는 폐동맥 페색증, 좌측폐동맥 무형성증, 심실중격결손증, 다발성 MAPCA등을 동반한 상행 대동맥으로부터 이상기시한 우폐동맥 11례를 경험하였다. 10개월 때 1단계 수술로써 좌측 unifocalization과 함께 좌폐동맥을 만들고 우폐동맥 벤딩을 하였고, 1개월후 우측 MAPCA의 coil 색전술 단계를 거쳐, 1년후 완전 교정술을 시행하였다. 술후 경과는 양호하였고 수술 1년후 조영술과 혈역학적 검사상 만족스런 결과를 얻을 수 있었다.

  • NIRS (Near-infrared Spectroscopy) and DOI (Diffuse Optical Imaging) are relatively new, non-invasive, and non-ionizing methods that measure or image optical properties (Scattering and Absorption Coefficient) and physiological properties (Water Fraction, concentration of Oxy-, Deoxy-Hemoglobin, Cytochrome Oxidase, etc) of biological tissues. In this paper, three different types of NIRS systems, mathematical modeling, and reconstruction algorithms are described. Also, recent applications such as functional brain imaging, optical mammography, NIRS based BMI (Brain-Machine Interface), and small animal study are reviewed.

  • Digital Subtraction Angiography(DSA) technique has been widely used to detect vascular diseases and hemodynamic parameters noninvasively. However, there factors in fluencing the resultant DSA image quality. In this paper, several important factors are suggested to improve the DSA image quality based on mathematicical analysis. Experimental DSA images for different filters are shown and also dicussed the difference between original and processed image qualities.

승모판 폐쇄부전에 대한 인공판막대치술 1예 (Mitral Valve Replacement with Star-Edwards Ball Valve -Report of A Case-)

  • This is a report of a case in which mitral valve replacement was performed using Starr-Edwards ball valve [model 6230] by total cardiopulmonary bypass with total hemodilution prime and moderate hypothermia. The patient was 21 year old girl, and mitral valvular symptoms occurred 5 years ago. According to clinical features of the disease, the patient was classified as the class IV of N.Y.H. A. cla3sification. The preoperative diagnosis of mitral insufficiency was made mainly by left ventriculography of roentgenologic study. The mitral valve was highly deformed due to scar contraction of valve leaflets and shortening of chordae tendineae. After prosthetic valve replacement, no abnormal hemodynamic signs were observed. The postoperative course was uneventful.

흰쥐의 허혈심장에서의 Adenosine의 심근 보호 효과에 관한 연구 (Myocardial Protective Effect of Adenosine in Ischemic Rat Heart)

  • This study was undertaken to investigate whether adenosine administered during cardioplegic arrest could enhance myocardial protection and improve recovery of function after ischemia. Isolated Langendorff-perfused rat hearts were subjected to 40 minutes of normothermic [37oC] ischemia. Control hearts [n=10] received modified St. Thomas’ cardioplegic solution, and the remaining hearts received modified St. Thomas’ cardioplegic solution with either 20 \ulcornerM [n=10], 200 \ulcornerM [n=10] adenosine. After ischemia of 40 minutes and 30 minutes of reperfusion, left ventricular contractility was superior in all groups of adenosine-treated hearts compared with control hearts. Furthermore, there was a significant incremental increase in functional recovery with increasing dose of adenosine. Post-ischemic diastolic stiffness was significantly better in all adenosine groups compared with controls. No differences were noted in coronary flow or myocardial water content between adenosine-treated and control hearts. These data demonstrate that adenosine administered in these concentrations provides myocardial protection, preservation of myocardial ATP and creatine phosphokinase and improved post-ischemic functional hemodynamic recovery after normothermic ischemia, presumably metabolically by reducing depletion of adenosine triphosphate, inducing rapid cardiac arrest and enabling improved post-ischemic recovery.

  • 본 연구에서는 동맥경화증의 발생에 관한 가설들을 혈류역학적 관점에서 비교/검토하였다. 혈류의 와류 현상, 분리현상 등도 역시 저전단응력 위치에서 발생하게 되며 동맥경화의 발생이 되기 시작하며 동맥내 형태학적 변화로 인해 이러한 현상이 좀더 복잡한 유동을 거치게 된다. 이로 인해 동맥내의 형태적 재형성이 일어나게 되는데 유동의 변화가 역할을 하는 것으로 생각된다. 생물학적으로 동맥경화는 질병의 성격이 만성 염증성 질환으로 알려져 있는데 이러한 생물학적 변성이 발생하는데 형태적 특성에 따른 혈류역학적 변화가 그 기전에 중요한 것임을 확인되었다. 본 연구자 등은 동맥경화가 발생하기 시작하는 곳은 혈관의 만곡부, 분지부 등에서 발생하는 것을 임상적 database를 통해 확인하였고 이 위치에서 영상을 simulation model을 동생 분석하여 전단응력의 영향과 시간에 따른 역동적 변화가 큰 것을 확인하였다.

  • The estimation of the work of heart can be treated as one of the most important parameters for determining the amount of circulating blood needed for harmonious metabolism in the human body. By monitoring the work of heart, one can detect increased work load of heart and start the treatment at the early stage of CHF. Thus it is necessary to estimate the work of heart. The contractility of the left ventricle, the second important parameter for representing the motion of heart, can be estimated through information on the work of heart. In this study, the modified Windkessel model, which has been used for a measure of vascular hemodynamic impedance parameters, was adapted to estimate the work of heart.

  • Bypass anastomosis are frequently adopted for surgical treatments. After the bypass grafting, the bypass artery is often occluded due to restenosis and/or anastomotic neointimal fibrous hyperplasia phenomena. Optimal coronary bypass anastomosis should be investigated to improve the patency for the arterial bypass techniques. The objective of this study is to investigate the influence of bypass with sequential bypass effects in the stenosed coronary artery. Numerical analyses are focused on the understanding of the flow patterns for different sequential anastomosis techniques. Blood flow field is treated as two-dimensional incompressible laminar flow. The finite volume method is adopted for discretization of the governing equations. The Carreau model is employed as the constitutive equation for blood. To find the optimal sequential bypass anastomotic configurations, the mass flow rates at the outlet of different models are compared quantitatively.
