고혈압 단계별 변증특성 분석 (Pattern Analysis in Patients with Hypertension grades)

  • To investigate and characterize basic patterns of prehypertension. Participants were divided into three groups; ideal blood pressure(n=40), prehypertension(n=62), and stage 1 hypertension groups(n=68) according to the JNC 7 standards. They answered questionnaire asking various symptoms and received clinical examination. Data were collected and analyzed in the focus of three Zheng patterns, cold-heat, deficiency-excess and four major hypertension types. Analysis of variance was used to find differences among groups. In addition, relationship between the cold-heat trends and risk factors of hypertension were analyzed using Pearson's correlation analysis. Three risk factors; age, body mass index (BMI), and fasting blood sugar (FBS) showed intergroup differences. Statistical significances were revealed in the cold-heat pattern and two hypertension subtypes. The cold scores decreased from $4.8{\pm}1.84$ to $3.9{\pm}1.88$ and $3.7{\pm}1.27$ (p=0.022), while the heat increased from $1.9{\pm}1.32$ to $2.8{\pm}1.72$ and $2.8{\pm}1.48$ (p=0.009). Additionally two hypertension subtypes, the excessive liver fire, and the yin-yang deficiency showed significant differences. Cold had negative correlations with blood pressure (both systolic and diastolic), BMI, triglyceride, and FBS. Heat had positive correlations with systolic blood pressure, BMI, triglyceride, and FBS. Prehypertension could be characterized by using the cold-heat patterns. The cold-heat are correlated with Blood pressure, BMI, blood lipids and sugar.

급성기 중풍환자에서 비만 및 혈액지표의 기허 및 화열 변증의 차이에 대한 고찰 (Study on the Obesity and Blood parameters Differences between Fire/Heat and Qi-deficiency Pattern Identification/Syndrome Differentiation among Acute Stroke Patients)

  • Object : In the present study, we investigated the obesity and blood parameters between Qi-deficiency and Fire/Heat pattern identification/syndrome differentiation (PI/SD) in acute stroke patients. Materials and Methods : A total of 391 stroke patients within 7 days after onset were consecutively recruited from 12 hospitals across South Korea from Nov. 1st, 2006 to Jun. 31st, 2009. They were diagnosed as Fire/Heat or Qi-deficiency among five PI/SD subtypes by two independent stroke experts. We investigated the differences of obesity and blood characteristics between Fire/Heat and Qi-deficiency by statistical analyses. Results : In male subjects, obesity was significantly associated with Fire/Heat PI/SD. The averaged mean BMI ($24.13kg/m^2$) and waist circumference(89.34cm) of the Fire/Heat group were higher than those of the Qi-deficiency group ($22.60kg/m^2$ and 83.43 cm, respectively). The number of obese patients was larger in the Fire/Heat group than in the Qi-deficiency group (p = 0.001). Hyperlipidemia was also related with Fire/Heat. However, obesity was not associated with PI/SD in female subjects where the number of hyperlipidemic patients was higher in the Qi-deficiency group. Among blood parameters, the levels of triglycerides and fasting blood sugar were higher in the Fire/Heat group compared with the Qi-deficiency group in male subjects. However, total cholesterol of the Qi-deficiency group was higher than in the Fire/Heat group among female subjects. Conclusion : This study shows that obesity and hyperlipidemia are significantly difference between Qi-deficiency and Fire/Heat. We suggests that PI/SD may be associated with clinical characteristics and large population study between PI/SD and clinical characteristics including blood parameters are needed.

알레르기 비염 환자의 변증별 자율신경계 특성 분석 연구 (Autonomic Conditions in Allergic Rhinitis Depending on Various Pattern Identifications)

  • Objectives : We performed a clinical study to investigate autonomic conditions in persistent allergic rhinitis depending on various pattern identifications and the availability of heart rate variability (HRV) as a pattern identification diagnostic tool. Methods : 32 patients with persistent allergic rhinitis were asked to interview with doctor of Korean Medicine and perform the four pattern questionnaires (Cold-Heat Pattern, Phlegm Pattern, Yin Deficiency pattern, bloodstasis pattern). Then, they were examined their autonomic conditions with heart rate variability test. Results : Patients were classified as three pattern groups (Lung-stomach heat, Lung qi deficiency cold, Lung-spleen qi deficiency) by doctor. In the Lung qi deficiency cold group, Total power of the HRV (TP) and the power of the low frequency component (LF) significantly higher than in the Lung-stomach heat or Lung-spleen qi deficiency group (P < 0.05). Also, Patients were classified as 8 pattern groups (Cold/Heat, Phlegm/Non-phlegm, Yin deficiency/Non-yin deficiency, Bloodstasis/Non-bloodstasis) by four pattern questionnaires. Only in the Yin deficiency group, the power of the low frequency component (LF) significantly lower than in the Non-yin deficiency group (P < 0.05). There were not any significant differences in the rest groups. Conclusions : The result may provide that HRV doesn't reflect well the differences in the various pattern groups, and the HRV's availability is low. Continuous studies are needed to develop the objective and standardized pattern identification diagnostic tool for allergic rhinitis.

치매(痴呆)의 한열허실(寒熱虛實) 변증(辨證)을 위한 지표 문항 개발에 관한 기초 연구 (Preliminary Research for Development of Instrument for Cold-Heat & Deficiency-Excess Pattern Identification of Dementia)

  • This study was performed to develop cold-heat and deficiency-excess pattern identification for dementia, as well as for standard Korean medicine diagnosis and treatment. Five experts comprised of 4 neuropsychiatrists of Korean medicine and 1 statistician to develop cold-heat and deficiency-excess pattern identification for dementia. We searched studies about pattern identification and selected 507 articles using Oasis search terms provided by the KIOM. As a result, 10 pattern identification research study were recruited. Moreover, we analyzed neuropsychological assessments for dementia that evaluate Behavioral and Psychological Symptoms of Dementia (BPSD) and cognitive function using experts conferences and we selected neuropsychological instruments using pattern identification. Six cold patterns, six heat patterns, ten deficiency patterns, and four excess patterns were identified according to the cold-heat and deficiency-excess pattern identification of dementia. We selected the Caregiver-Administered Neuropsychiatric Inventory and the Korean Mini-Mental State Examination as neuropsychological assessments of dementia, which examine behavioral symptoms and cognitive function, suspectively. We formed positive and negative correlation between Korean medicine pattern identification and neuropsychological assessments for dementia. We developed and suggested a forecast module of pattern identification for dementia. But, it is necessary to perform additional clinical trials to verify its validity and accuracy.

열처리가 냉간가공한 stainless steel wire의 복원양상에 미치는 영향 (EFFECTS OF LOW-TEMPERATURE HEAT TREATMENT ON ELASTIC MEMORY PROCESS OF COLD WORKED STAINLESS STEEL WIRE)

  • The purpose of this study was to evaluate the elastic memory process in cold worked stainless steel wire and the effect of heat treatment on it. 0.018 inch round and $0.019\times0.025$ inch rectangular wire (ORMCO stainless wire) were used in this study. Each wire type had 4 groups: non-heat treatment group, furnace heat treatment group, electric current heat treatment group, and bending after heat treatment group. Each group was consisted of 10 specimens. With the Jig, each wire was bent into v-shape uniformly, and width of two free ends of each v-shaped wire was measured by caliper (to the point of 0.1 mm correctly) at time interval of offjig, after heat treatment, 1, 2, 3, 4 hours, 1, 2, 3, 4, 5, 6 days, 1, 2, 3, 4 weeks after. The results were as follows: 1. In non-heat treatment group and bending after heat treatment group, elastic memory process was occured $60\%$ within 1 hour, and more than $90\%$ within 1 week. 2. In furnace and electric current heat treatment group, almost all elastic memory process was occured during teat treatment, and then specimen was stabilized dimensionally. 3. Magnitude of deformation by elastic memory was greater in heat treatment group than non heat treatment group and bending after heat treatment group. 4. There was no remarkable difference in deformation pattern between 0.018 inch round wire and $0.019\times0.025$ inch rectangular wire.

뇌경색 환자의 경동맥 초음파 검사와 한방 중풍 변증과의 관련성 연구 (Study on the Relationship between Carotid Intima-Media Thickness and Korean Standard of Pattern Identification in Ischemic Stroke Patients)

  • ■ Objectives The purpose of this study was to investigate the correlation of The Korean Standard Pattern Identification for Stroke and Intima-Media Thickness of common carotid artery(CCA-IMT). ■ Methods 100 subjects with acute ischemic stroke were recruited from the patients admitted to DongEui Medical Center from the June 2011 to January 2014. We assessed 100 patient's Common Carotid Intima Media Thickness data by B Mode ultrasonography. We investigated pattern identification of all subjects based on Korean Standard Pattern Identification for Stroke-III. then, We analyzed their characteristics, risk factor, blood test result, life style. ■ Results We analyzed indicators of Korean Standard Pattern Identification for Stroke-III by Common Carotid Intima Media Thickness difference. As a result, pale complexion, look powerless, drowsiness like to lie, heat vexation and aversion to heat were significantly higher in the CCAIMT ≧ 1.0mm group then in the CCA-IMT < 1.0mm group. ■ Conclusion According to the analysis, the Relationship between Carotid Intima-Media Thickness and The Korean Standard Pattern Identification for Stroke in Ischemic Stroke Patients were founded. To acquire more concrete conclusion on this theme, we need more research.

DITI를 이용한 갱년기 환자의 체열분포 양상 (Using DITI to examine the pattern of subcutaneous heat of the climacteric patients)

  • 1. Objectives Among many symptoms that the climacteric patients complain of, the major subjective symptom is both heat in the upper part and cold in the lower part of the body(e.g foot, knee, and leg). We use DITI as a method to prove the symptoms of climacteric patients such as both heat in the upper part and cold in the lower part of the body, diagnose these symptoms, and follow up the progress of the clinical treatments. 2. Subjects and Methods We compare the subcutaneous heat of the climacteric patients with the symptom of heat in the upper part and cold in the lower part of the body with young women without any specific disease. The age distribution of the experimental group is 28 patients in 40's, and 22 in 50's. The mean age of the experimental group is $48.5{\pm}4.25$. The age distribution of the control group is 17 patients in teen, 26 in 20's, 7 in 30's. The mean age of the control group is $23.31{\pm}6.45$. We measure the subcutaneous heat on the cheeks, upper arms, palms, thighs, knees, dorsum pedis, lower back, hip, upper abdomen, lower abdomen of both groups and analyse the results. 3. Statistical methods All the results are statiscally analysed using student T-test of Microsoft Exel program. Statistically significant value by the analysis of variance procedure is P<0.05. 4. Results 1. The subcutaneous heat of the whole body of the experimental group shows hypothermia compared with control group, and the hypothermic pattern gets severe on the periphery. ${\Delta}T$ between the right and left side of the body in experimental group is larger than that of control group. 2. ${\Delta}T$ between the cheeks and the hypogastric in the experimental group is significantly larger than that of the control group, which shows cold face. 3. ${\Delta}T$ between the upper arms and the palms in the experimental group is significantly larger than that of the control group, which shows cold hand. 4. ${\Delta}T$ between the thighs and the knees in the experimental group is significantly larger than that of the control group, which shows cold knee. 5. ${\Delta}T$ between the thighs and the feet in the experimental group is significantly larger than that of the control group, which shows cold foot. 6. ${\Delta}T$ between the hip and the lower back in the experimental group is significantly larger than that of the control group. That shows the Lower back is warmer than the hip. 7. ${\Delta}T$ between the upper and the lower abdomen in the experimental group is significantly larger than that of the control group, which shows cold hypogastric.

한국형 중풍 변증 표준안-Ⅱ의 변증별 변증지표의 분포 및 타당도에 관한 연구 (Study of the Indicators of Each Pattern Identification Based on Korean Standard Differentiation of the Symptoms and Signs for Stroke)

  • The purpose of this study is to investigate clinical frequency and priority of five pattern identification settled by Korean Standard Differentiation of the Symptoms and Sign for Stroke. The present study was done over 177 hospitalized patient with stroke in the Daejeon University Oriental Medical Hospital in the period of November 2006 to July 2007. Stroke patients had been interviewed by residents and specialists who studied standard operation procedures in Fundamental study for Standardization and Objectification of Differentiation and Pattern Identification of Syndrome of Oriental Medicine for Stroke. In Heat-transformation pattern group, men showed significantly high frequency and in Deficiency of Gi pattern group, women showed significantly high frequency. In Heat-transformation pattern group, the indicators such as 'aversion to heat during fever', 'flushed face', 'full and rapid pulse', 'dryness of the eyes, 'yellow coating of the tongue', 'feverishness of the limbs', 'dryness of the eyes' showed significantly high frequency. In Dampphlegm syndrome group, the indicators such as 'white coating of the tongue', 'yellowish face', 'thick coating of the tongue', 'wheezing in the throat with sputum', 'swollen tongue', 'slippery pulse' showed significantly high frequency. In Deficiency of Gi pattern group, the indicators such as 'pale tongue', 'lassitude', 'pale face', 'weakness pulse' showed significantly high frequency. In Deficiency of Eum group, the indicators such as 'short and rapid pulse', 'mirror-like tongue' showed significantly high frequency. For more sensitive Korean Standard Differentiation of the Symptoms and Signs for Stroke, Large scale study is to be done, giving weight on the important indicators.

Total bilirubin level as a biomarker for dampness-heat differentiation in traditional Korean treatment for jaundice

  • Objectives: Classifying the pattern of jaundice during diagnosis will significantly improve the outcome of common KM interventions. This study aimed at determining an objective index for accurately diagnosing heat and dampness KM patterns in patients with jaundice. Methods: We systematically reviewed laboratory findings from case reports published in the scientific literature of Korean medicine. Cases were classified as following either the heat or dampness pattern. Biochemical indices were compared using a Bayesian factor (BF) analysis and standard t-tests. Results: The laboratory findings of 32 patients were evaluated. The heat pattern was observed in 17 patients and the dampness pattern in 15. No significant differences were observed between the 2 groups in terms of white blood cell count (BF=1.659); hemoglobin concentration (BF=2.627); platelet count (BF=1.019); or levels of direct bilirubin (BF=1.453), aspartate aminotransferase (BF=1.226), alanine aminotransferase (BF=1.340), alkaline phosphatase (BF=2.344), or gamma-glutamyl transpeptidase (BF=2.782). However, total bilirubin levels were significantly higher in the dampness pattern group (BF=0.854, P-value=0.070). Conclusions: Patients with high total bilirubin levels may predominantly follow the dampness pattern, while those with low levels may predominantly follow the heat pattern. These results are expected to be useful for the development of timely and efficient KM treatments as well as new integrative therapeutic approaches for jaundice. However, further studies are essential to fully validate the utility of total bilirubin as a biomarker for differentiating between heat and dampness patterns.

중풍(中風)환자의 5개 변증(辨證)과 혈중지질의 상관관계 연구 (Study on the Relation between Each Pattern Identification and Blood Lipid Level in Stroke Patients)

  • The purpose of this study was to investigate the relation between each pattern identification such as Dampness-Phlegm, Fire-Heat, Deficiency of Qi, Deficiency of Yin and Blood Stagnation and blood lipid level in acute stroke patients. This study was done over patients hospitalized in 13 Oriental Medical Hospitals in the period of November 2006 to Jun 2009. Patients had been interviewed by residents and medical specialists who studied standard operation procedures in Fundamental Study for Syndrome of Oriental Medicine for Stroke. Study subjects consisted of group A that was distributed to specified pattern identification by medical specialist and discriminating program, group B that was distributed to specified pattern identification by medical specialist or discriminating program, and the control group that wasn't distributed to specified pattern identification by medical specialist and discriminating program. For the purpose of obtaining suitable result, we analyzed blood lipid level of each group by univariate analysis. In this study, there was not statistically significant relation between most of each pattern identification and blood lipid level except relation between Fire-Heat pattern group B and decreasing HDL cholesterol. Based on these results, it is suggested that Dampness-Phlegm would not be the independent predictors of hyperlipidemia unlike other studies that were presented in. More prospective studies between Fire-Heat and decreasing HDL cholesterol are to be done with more clinical data.
