Farmer's Syndrome and The Accidents Due to Agricultural Machines of Some Rural Residents (일부 농촌주민의 농부증 및 농기계사고)

  • The symptoms of farmer's syndrome that occurred to Korean farmers are due to the specific characteristics of Korean agriculture, and the basic reasons are poverty and less-privileged rural cultural life originated from social and political conditions. Then the purposes of this study are to investigate farmer's syndrome of farmers and to get substantial data to make a strategy that could prevent diseases. The results are as follows: 1. The most common symptom was lumbago(83.0%) among the 7 kinds of symptoms of farmer's syndrome. And it was more common among women who deliver babies and also work house chores and 10 farms than men(p<0.05). 2. The numbers of symptoms of farmer's syndrome increased as age increases and, 71.4% of those aged 70-79 complained some of the symptoms. 3. The accident rate due to agricultural machine was significantly higher in men than women(p<0.05). By frequency contusion was the most common trauma, fall, fracture were the nexts, and the age group with highest accident rate(40%) was those aged 20-29. 4. To see the significantly correlated factors between faemer's syndrome and sociodemo graphic variables, the more the numbers of family members were, the more lumbago and the less heartburn occurred. And the less gross income was the more frequent nocturnal urinary frequency occurred, and the wider the farming land was the more frequent noctural urinary frequency occurred. 5. As result of correlation coefficient between the numbers of illnesses and sociodemographic variables, the larger the family was and the larger the farming land was the more illnesses the subjects had, and those without spouses had more illnesses.

Stress, Anxiety, and Depression of the Patients Who Complained of Functional upper Gut Symptoms (기능성 상부 위장관 증상을 호소하는 환자의 스트레스, 불안 및 우울)

  • Objective : The sensation of dysphagia, heartburn, globus hystericus, and functional dyspepsia are common symptoms of the functional upper gut disorders. This study was designed to investigate quantity of perceived stress, depression, and anxiety in the patients with functional upper gut symptoms whose esopahgeal manometry(EM) and gatroesophageal reflux (GERT) test were normal. Methods : A total of 38 patients who complained of the symptoms had been tested with 24-hour ambulatory EM and conventional GERT in our gastrointestinal clinic. Thirty patients whose tests had been normal(patients group) were assessed with Symptom Checklist-90-Revision(SCL-90-R), Beck Depression Inventory(BDI), and Spielberger Stait-Trait Anxiety Inventory(STAI) and compared with 30 patients(control group) without functional upper gut symtpoms in the hepatobiliary clinic. The two groups were also assessed by quantity of perceived stress during the last year through self-report. Results: 1) These patients tended to be predominently female, older, and possessed a lower education than control group. 79% of 38 patients who had been tested were normal. 2) Compared to the control group, the patients had significantly higher mean scores on four subscales(somatization, depression, anxiety, and positive symptom distress index). 3) The patient group had significantly more perceived stress than the control group. 4) The patients group had significantly higher levels of depression than the control gorup, but there was not any significant difference in the STAI. 5) There were significant positive correlations between the BDI score and the STAI-trait, the STAI-stait and the STAI-trait, the quantity of perceived stress and the STAI-trait. Conlusion : The patients with functional upper gut symptoms displayed more, psychological distress, sornatization, anxiety, and deperssion. Among them, patients had higer depression than control group. Functional upper gut symtoms could be more appropriately viewed as somatic symptoms of depression. These findings suggest that such patients need to have psychiatric intervention and treatment.

Awareness and Intake of Caffeine-Containing Foods among High School Students in Seoul (서울 일부 고등학생의 카페인 함유식품에 대한 인식 및 섭취 실태)

  • The high caffeine intake by adolescents has been a concern. The purpose of this study was to examine the awareness and consumption of caffeine-containing foods among 443 high school students using a questionnaire. An analysis of the spending patterns of the students' weekly allowance showed that the amounts spent on purchasing caffeine-containing foods were higher for female students than male students (P <0.001). The scoring of the perception of caffeine was 3.1 out of 5, interest in the caffeine content of food was 2.6, consumption of caffeine-containing foods was 2.6, and usefulness of caffeine-containing foods was 2.7. The awareness of caffeine content in food was significantly higher in females (7.3 out of 11) than male students (6.7) (P<0.01). Approximately 59% of students perceived that the relationship between caffeine-containing foods and health, was harmful, and the experience of side effects after taking caffeine was significantly higher in female students than males. These side effects include heartburn (P<0.001), headache or dizziness (P<0.001), irregular heartbeat (P<0.05), and hands and feet shake (P<0.01). Caffeine-containing foods were purchased at convenience stores (62.1%). The factor considered when purchasing caffeine-containing foods was taste (72.2%), and the use of nutrition labeling for caffeine-containing foods scored 2.0 out of 5 points. When assessing the intake of caffeine-containing foods, the foods consumed more than once a week were in the order of coke, chocolate, chocolate milk, chocolate pie, and chocolate bars. These results suggest that it is necessary to prepare a caffeine-related nutrition guide improvement by sales management, and strengthen food labeling standards for the desirable recognition of caffeine and its safe intake by adolescents.

The Gastroesophageal Reflux Disease Questionnaire in Adolescents: What Is the Best Cutoff Score?

  • Background: A self-assessment questionnaire, the GERD-Questionnaire (GERD-Q) was used to determine the prevalence of GERD in adolescents, describe the related factors, and determine the impact on quality of life (QoL). Methods: The incidence of GERD was evaluated using the GERD-Q in adolescents aged 12-18 years. The Pediatric Gastroesophageal Reflux Disease Symptom Questionnaire and Quality of Life Questionnaire (PGSQ-A) for adolescents were additionally administered. Some factors considered related to GERD were also evaluated. Results: The 520 adolescents were included. The prevalence of suspected GERD, according to a GERD-Q cutoff score of ${\geq}7$ was 32.9%, and those drinking soda were 1.7 times more likely to have GERD (95% confidence interval, 1.3-2.2; p<0.001). However, soda consumption was not a risk factor for development of GERD symptoms. Applying a cutoff score of ${\geq}8$, only 10.9% of the participants had a positive GERD score, but the association with soda consumption persisted. The median PGSQ-A score in subjects suspected of GERD was 8 (range 0-37) on weekends and 1 (range 0-17) during weekdays (p<0.001) compared to those not suspected of GERD, with a median of 2 (range 0-27) during weekends and 0 (range 0-10) during weekdays. Heartburn, regurgitation, and extraesophageal symptoms correlated significantly with QoL (p<0.001). Conclusion: The prevalence of suspected GERD in adolescents was 32.9% or 10.9%, depending on the cutoff score used. There was a statistically significant difference in PGSQ-A scores between the subjects suspected or not of GERD, indicating an impaired QoL.

A Case Report on the Use of Heart Rate Variability and the Treatment Effect of Bunshimgi-eum on Autonomic Dysfunction After Cerebral Infarction (뇌경색 후 발생한 자율신경 장애에 대해 심박변이도로 살펴본 분심기음의 효과 증례보고 1례)

  • Background: The aim of this study is to investigate the use of heart rate variability on a stroke patient with autonomic dysfunction and the effectiveness of Bunshimgi-eum. Case report: The patient showed autonomic dysfunction, including chest discomfort, 煩燥 (sweating, anxiety, insomnia, nausea, lightheadedness), and abdomen discomfort (dyspepsia, heartburn) after ischemic stroke. She was treated with a herbal medicine (Bunshimgi-eum) through complex Korean medicine treatment. The effect was evaluated using heart rate variability (HRV), Mini-Mental State Examination-DS (MMSE-DS), mean pulse rate, and subjective percentage of symptoms. After treatment, the HRV score, MMSE-DS score, and subjective percentage of symptoms improved, and the pulse rate decreased from the upper boundary value. Discussion: The results suggest that HRV may be useful in evaluating stroke patients with autonomic dysfunction and that Bunshimgi-eum is suitable for treatment.

A preliminary study on the relation of Uterine myoma focusing on General symptom (일반 증상을 중심으로 한 자궁근종 관련성에 대한 예비 연구)

  • Purpose: We intended to observe relation between general symptom and the number of uterine myoma. Methods: We surveyed 948 women from 35 to 45 years old who had received Korean oriental medical questionnaires about general symptom pattern and pelvic ultrasonography at Kyung-Hee university medical center from May, 2007 to October, 2008. After we divided women into three groups (Mono-uterine myoma, Bi-uterine myoma & uterine myoma above 3) by ultrasonography result, we compared general symptom pattern according to the number of uterine myoma. For statistics, we used Kruskal-Wallis Test, SPSS 13.0 for windows. Results: In relation to sleep pattern, severity of dreaming & awaking showed statistical significance according to the number of uterine myoma. In relation to digestion pattern, appetite & heartburn showed statistical significance according to the number of uterine myoma. In relation to evacuation pattern, hardening of stool showed statistical significance according to the number of uterine myoma. In relation to sweating pattern, severity of refreshing after sweating showed statistical significance according to the number of uterine myoma. In relation to the other patterns, hypertonicity of nape neck showed statistical significance according to the number of uterine myoma(p<0.05). Conclusion: The result showed that prevalence of uterine myoma had to be considered in respect of sleep, digestion, evacuation and sweating pattern. we need further study based on this preliminary result.

Effects of Korean Medicine Treatment at Public Health Center for Post-acute COVID-19 Patients: A Retrospective Case-series of 11 patients (일개 보건지소에 내원한 급성기 후 코로나-19 환자들의 특성 및 한의 치료 효과 분석: 사례군 연구)

  • Objectives: The purpose of this study is to determine the clinical features of post-acute COVID-19 syndrome and the effectiveness and safety of Korean medicine treatment at the Public health center. Methods: The study was conducted among 11 patients with post-acute COVID-19 syndrome who attended a single public health center from January to December 2022. We retrospectively analyzed the charts of 11 patients and collected clinical characteristics, previous treatments, Korean medicine treatments, outcome variables (Numeral Rating Scale (NRS), Leicester Cough Questionnaire (LCQ), Visual Analog Scale (VAS)), adverse events, etc. Results: Of the 11 patients, six were women, and the average age of all patients was 68.27±12.31 years. The most common symptom were cough(n=9, 81.82%) and sore throat(n=9, 81.82%), followed by sputum, fatigue, rhinorrhea, and loss of appetite, etc. All 11 patients were treated with herbal medicine, with Samso-eum(n=6, 54.55%), Yeonkyopaedok-san(n=5, 45.45%), and Haengso-tang(n=3, 27.27%) being the most commonly used. After herbal medicine treatment, the median cough NRS decreased from 5 to 1, and the median sore throat NRS decreased from 4 to 1, both of which were statistically significant. One patient reported adverse event of dyspepsia and heartburn, but it was mild. Conclusions: The study presented the clinical features of the post-acute COVID-19 syndrome and suggested that Korean medicine treatment at public health centers may be effective and safe in alleviating associated symptoms.

A Case of Advanced Gastric Cancer with Deep Vein Thrombosis Treated with Low Molecular Weighted Heparin (전이성 위암환자의 심부정맥혈전증에 대한 저분자량 헤파린 투여 사례)

  • The relationship between malignancy and venous thromboembolism(VTE) has been well established. About 20% of all VTE cases are associated with cancer and thrombotic events are the second leading cause of death in cancer patients after death from cancer itself. Effective prophylaxis and treatment will reduce morbidity and may decrease overall mortality. We report a case of VTE in a patient with advanced gastric cancer who treated with low-molecular weighted heparin (LMWH). A 49-year-old man with heartburn was admitted to our hospital. On the endoscopic and radiologic imaging, the patient was diagnosed as an advanced gastric cancer with perigastric infiltration and liver metastasis. During the combination chemotherapy, he had pain and swelling of left lower leg. Doppler ultrasonography showed left posterior tibial venous thrombosis and pulmonary embolism CT showed thromboembolism in subsegmental pulmonary artery branch in right lower lobe. He was treated with LMWH, Dalteparin once daily via subcutaneous injection, and his symptoms was subsided.

Subjective and Functional Results after a Proximal Gastrectomy: Prospective Study for Comparison of Reconstruction Procedures (근위부 위절제술 후 재건 술식에 따른 경과 관찰)

  • Purpose: A proximal gastrectomy is performed for gastric cancer in the upper part of the stomach to preserve the function of the stomach after surgery. An esophagogastrostomy is one of the common reconstruction methods for a proximal gastrectomy, but this method results in a high incidence of reflux esophagitis. This study was undertaken to compare subjective and functional results between esophagogastrostomy and jejunal interposition reconstructions. Materials and Methods: From June 1998 to December 2002, proximal gastrectomies were performed in 33 patients with tumors in the upper third of the stomach; 8 had reconstruction using jejunal interposition between the esophagus and the remnant stomach (JI group) while 25 had reconstruction using esophagogastrostomy (EG group). The postroperative courses of the patients were reviewed in terms of symptoms, weight changes, and endoscopic findings. Results: The mean age of the patients was 59 years; 26 were men and 7 were women. There were no significant differences in general complications, operating times, or histologic features between the two groups. Fifty-two percent (52%) of the EG group complained of dysphagia, and 16% them experienced heartburn and acid belching. Twelve percent (12%) of the JI group complained of dysphagia, but heartburn and acid belching did not occur. Incidences of reflux esphagitis (36%) and balloon dilatation for anastomotic stricture (16%) were more common in the EG group than in the JI group (0% and 12%). Conclusion: To prevent or minimize complications, such as reflux esophagitis and postoperative symptoms, a proximal gastrectomy with a jejunal interposition is an alternative method as an organ-preserving surgical strategy to improve quality of life for patients. A study on characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients (협심증이 의심되는 환자에서 운동부하검사로 유발되는 흉통의 양상과 생리적 변인에 관한 연구)

  • The purpose of this study was to identify the characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients. The subjects of this study consisted of 28 inpatients and outpatients aged between 40 and 75 who underwent treadmill test at exercise testing laboratory of S-University from January 2000 to June 2000. Subjects were interviewed with questionnaire regarding sociodemography, the past health history and history related to chest pain before the exercise test. Subjects were interviewed with questionnaire concerning quality, intensity, duration of chest pain induced by walking on the treadmill(Marquette, U.S.A. 1992) according to Bruce protocol following exercise test. Systolic and diastolic blood pressure were measured before, during and after the test, heart rate was determined by ECG. The results of this study were as follows ; 1) Quality of chest pain induced by exercise test were feeling stiffness 19(67.9%), heavy 10(36.0%), exploded 9(32.1%), crushing, suffocating, tight 8(28.6%), stuffy, prickly 7(25.0%), burning 6(21.4%), clasp 5(17.9%), cleaved, tensed, piercing 3(10.7%), perfectly fitting, sore 2(7.1%), tearing, tingling, ticklish, heartburn 1(3.6%). 2) Mean score of VAS(intensity of pain) following exercise test was $5.79{\pm}2.27$ and mean duration of chest pain after the test was $7.83{\pm}5.31$ minutes. 3) Sites of chest-pain induced by exercise test were middle site 11(39.3%), left-chest 10(35.6%), right-chest 6(21.5%). Radiation site of chest-pain was neck(18.0%), right flank site 1(3.6%), left shoulder & arm 2(7.1%) and back 1(3.6%). 4) Symptoms other than chest-pain induced by exercise test were dyspnea 21(75.6%), perspiration 14(50.4%), fatigue 12(43.2%), leg-pain 11(39.6%), dizziness 7(25.2%) anxiety toward chest-pain 3(10.8%), thirst 2(7.1%), and palpation, headache and tingling sensation of hand and leg 1(3.6%). 5) Mean MET(intensity of exercise) during the exercise test was $7.64{\pm}2.57$ and mean RPE(rating of perceived exertion) was $15.89{\pm}2.36$. Mean duration of exercise was $6.79{\pm}2.88$. 6) correlation coefficients between RPE and VAS was 0.500(p=0.003), those between MET and VAS was 0.287(p=0.069) and those between either depression or elevation of ST segment and VAS was 0.236(p=0.114). 7) There was a significant difference in mean systolic pressure between before and after the test as $146.29{\pm}28.18mmHg$ and $177.96{\pm}28.82mmHg$(t=-5.640, p=0.000), a significant difference in mean diastolic blood pressure between before and after the test as $84.85{\pm}15.07mmHg$ and $88.89{\pm}13.72mmHg$(t=-2.082, p=0.047), and there was a significant difference in mean heart-rate between before and after the test as $81.89{\pm}12.22/min$ and $160.68{\pm}21.77/min$(t=-21.255, p=0.000).
