다문화가족의 건강욕구 분석 및 정보접근성 향상을 위한 ICT 활용 방안 (Policies and ICT Strategies based on Health Needs for Multicultural Families)

  • 본 연구는 다문화가족의 건강욕구 및 관련 정책들을 파악하고, ICT를 활용한 건강정보의 접근성을 개선할 수 있는 방안을 모색하는 것을 목적으로 하였다. 본 연구에서 사용된 방법은 질적 연구로서, 선행문헌 검토를 통해 다문화가족의 건강실태를 1차적으로 분석한 후, 다문화가족을 대상으로 심층인터뷰를 진행하였다. 본 연구결과, 그간 다문화가족의 건강정책은 타 부문에 비해 정책우선순위가 밀려 있고, 다문화가족의 건강정보 접근성 향상을 위한 ICT 활용 논의가 미미한 것으로 분석되었다. 다문화가족을 심층인터뷰한 결과, "한국사회 진입초기 경험", "의료기관 이용시 높은 언어장벽", "다양한 이유로 인한 미충족의료 경험", "보건의료 관련 서비스 욕구가 높은 영역" 등의 4개 주제가 도출되었다. 다문화가족의 건강권 보장 및 건강정보 접근성을 향상하기 위해서 의료기관의 언어적 지원체계 강화, 정부의 다문화가족을 위한 정책적 노력, 수요자 중심의 쌍방향 ICT 플랫폼 설계 등을 제언하였다.

Interoperability between Heterogeneous Process Workflows using Interaction Ontology

  • Heterogeneity in different Health Information Systems (HIS) processes persists to be the most demanded problem to be resolved in healthcare domain. The only way to resolve this problem is to practice health standards. One of such standards is Health Level Seven (HL7), used for the communication of medical information between healthcare systems. HL7 V3 has the aim to support all healthcare workflows. It defines series of electronic messages called interactions to support healthcare workflows. These interactions are part of the Interaction Model. Different healthcare organizations can conform to different process workflows based on their requirements. The heterogeneity in workflows results in communication blockade between sender and receiver healthcare organizations. We propose Interaction Ontology in order to cater the heterogeneity in workflows and provide process interoperability.

  • 최근의 정보통신기술은 관련기술과의 융합으로 모든 시스템을 통합하는 형태로 발전하고 있다. 이런 시대의 흐름과 마찬가지로 의료산업은 근래에 많은 발전 및 응용에 관한 연구가 활발히 진행되고 있다. 의료정보시스템도 의료IT의 정보시스템들이 통합되는 방향으로 급변하게 진화해가고 있으며, 앞으로도 그 가속도는 더 할 전망이다. 따라서 본 논문에서는 국가 주도의 유비쿼터스 환경의 구축을 위한 병원 응용시스템 구축과 IT응용 서비스가 실용화하고 있는 실정에서 통합의료 정보를 위한 환자 진료의 서비스 강화를 도모하도록 통합의료정보시스템을 제안 및 설계하는 것과 동시에 통합의료정보시스템의 구축방안과 운영과제를 제시하였다.

U-Health환경에서 안전한 개인정보 관리를 위한 통합 인증스키마 설계 (Design of Integrated Authentication Scheme for Safe Personal Information Management in a U-Health Environment)

  • U-health Service는 환자와 의료진과의 언제 어디에서나 의료서비스를 제공하는 것을 지칭하며 정보통신 기술과 보건의료를 융합한 서비스로 정의하고 있다. 그러나 환자의 의료정보, 개인정보 유출과 같은 사례가 발생하고 있고, 또한 네트워크를 통하여 데이터 송수신을 계승하므로써 기존 유 무선기반의 보안위협사항을 계승하는 취약점이 있다. 그러므로 본 논문에서는 U-Health Service에서 발생하는 취약점을 보완하고자 통합 인증스키마를 설계하여 안정한 개인정보에 대한 관리 시스템을 제안하였다. 제안프로토콜에서는 ID-Based Encryption을 활용하여 사용자 등록, 의료기관과 사용자간의 인증, 데이터 통신 암 복호화, 사용자 정보 폐기에 대한 프로토콜을 설계하였으며, 이에 따른 기존시스템 및 PKI Based 기반 통신과정과 보안성과 안전성에 관하여 분석하였다.

New Obligations of Health Insurance Review and Assessment Service: Taking Full-fledged Action Against the COVID-19 Pandemic

  • In 2020, the coronavirus disease 2019 (COVID-19) pandemic has caused unprecedented disruptions to global health systems. The Korea has taken full-fledged actions against this novel infectious disease, swiftly implementing a testing-tracing-treatment strategy. New obligations have therefore been given to the Health Insurance Review and Assessment Service (HIRA) to devote the utmost effort towards tackling this global health crisis. Thanks to the universal national health insurance and state-of-the-art information communications technology (ICT) of the Korea, HIRA has conducted far-reaching countermeasures to detect and treat cases early, prevent the spread of COVID-19, respond quickly to surging demand for the healthcare services, and translate evidence into policy. Three main factors have enabled HIRA to undertake pandemic control preemptively and systematically: nationwide data aggregated from all healthcare providers and patients, pre-existing ICT network systems, and real-time data exchanges. HIRA has maximized the use of data and pre-existing network systems to conduct rapid and responsive measures in a centralized way, both of which have been the most critical tactics and strategies used by the Korean healthcare system. In the face of new obligations, our promise is to strive for a more responsive and resilient health system during this prolonged crisis.

  • In the process of remarkable progress in the medical and technical field and activating the role of technology in health care services and applications, and since the safety of medical data and its protection from security violations plays a major role in assessing the security of health facilities and the safety of medical servers Thus, it is necessary to know the cyber vulnerabilities in health information systems and other related services to prevent and address them in addition to obtaining the best solutions and practices to reach a high level of cybersecurity against attackers, especially due to the digital transformation of health care systems and the rest of the dealings. This research is about what cyberattacks are and the purpose of them, in addition to the methods of penetration. Then challenges, solutions and some of the security issues will be discussed in general, and a special highlight will be given to obtaining a safe infrastructure to enjoy safe systems in return.

Current Systems of National and Regional Nutrition Surveys and Future Direction

  • The National Nutrition Survey in Japan (NNS-J) started in 1945 and has provided information on dietary intake and health status of Japanese citizens to the public and policymakers for more than half a century. We summarized several relevant issues on the survey in this report : the current framework of the NNS-J in accordance with the Nutrition Improvement Law, utilization of the survey for nutrition and health policy in Japan, the Health Promotion Law recently enacted in 2003, the national plan for health promotion and disease prevention (Health Japan 21), and possible measures to improve the survey systems under the new law. We also mentioned implementation structures of regional health and nutrition surveys, because the Health Promotion Law designates an active role of local governments on promoting health for their citizens, which will enhance the needs for appropriate assessment of health and nutrition conditions in each community as well as the monitoring at the national level. 공간역학과 환경보건: 공간위치정보 활용에 대한 고찰 (Spatial Epidemiology and Environmental Health: On the Use of Spatially Referenced Health and Environment Data)

  • Recent advances in Geographic Information Systems and spatial statistical and analytical methods, along with the availability of spatially referenced health and environmental data, have created unique opportunities to investigate spatial associations between environment exposures and health outcomes at multiple spatial scales and resolutions. However, the increased use of spatial data also faces challenges, one of which is to ensure certainty and accuracy of locational data that meets the needs of a study. This article critically reviews the use of spatially referenced data in epidemiologic studies, focusing on the issue of locational uncertainty generated from the process of geocoding health and environmental data. Primarily, major issues involving the use of spatially referenced data are addressed, including completeness and positional accuracy, potential source of bias and exposure misclassification, and implications for epidemiologic studies. The need for critical assessment and caution in designing and conducting spatial epidemiology studies is briefly discussed.

CHI서비스 활성화를 위한 협력네트워크시스템 구축에 관한 연구 (A Study on Constructing Collaborative Network Systems for Vitalizing CHI Services)

  • 건강 및 의료, 그리고 웰빙에 대한 관심의 증가로 이와 관련된 정보원의 이용률, 특히 인터넷상의 건강정보원의 이용률이 급증해 왔다. 또한 공공도서관에서의 건강 및 의료관련 참고질문 건수도 지속적으로 증가하고 있는 가운데, 공공도서관사서는 이러한 유형의 참고서비스를 수행하는데 많은 한계를 느끼고 있다. 이에 본 연구에서는 공공도서관, 의학도서관, 의료기관, 건강 및 의료관련 커뮤니티, 기타 의료정보서비스를 제공하는 모든 관련기관들이 협력하여 건강정보서비스를 제공하는 CHI서비스 활성화를 위한 협력네트워크시스템 구축모형을 제안하였다. 협력모형의 핵심 요소에 해당되는 것으로 협력의 주체, 협력대상 정보원, 협력의 내용, 건강정보원 사이트의 운영, 협력센터의 운영과 역할, 그리고 상업기관과의 협력방향까지 구체적으로 제시하고 있다.

The Intelligent Clinical Laboratory as a Tool to Increase Cancer Care Management Productivity

  • Studies of the causes of cancer, early detection, prevention or treatment need accurate, comprehensive, and timely cancer data. The clinical laboratory provides important cancer information needed for physicians which influence clinical decisions regarding treatment, diagnosis and patient monitoring. Poor communication between health care providers and clinical laboratory personnel can lead to medical errors and wrong decisions in providing cancer care. Because of the key impact of laboratory information on cancer diagnosis and treatment the quality of the tests, lab reports, and appropriate lab management are very important. A laboratory information management system (LIMS) can have an important role in diagnosis, fast and effective access to cancer data, decrease redundancy and costs, and facilitate the integration and collection of data from different types of instruments and systems. In spite of significant advantages LIMS is limited by factors such as problems in adaption to new instruments that may change existing work processes. Applications of intelligent software simultaneously with existing information systems, in addition to remove these restrictions, have important benefits including adding additional non-laboratory-generated information to the reports, facilitating decision making, and improving quality and productivity of cancer care services. Laboratory systems must have flexibility to change and have the capability to develop and benefit from intelligent devices. Intelligent laboratory information management systems need to benefit from informatics tools and latest technologies like open sources. The aim of this commentary is to survey application, opportunities and necessity of intelligent clinical laboratory as a tool to increase cancer care management productivity.
