후두 악성종양에 대한 내시경적 레이저 수술의 적용 (Endoscopic Laser Surgery in Laryngeal Carcinoma)

  • Background and Objectives: To determine if laser endoscopic microsurgery is a reliable and appropriate approach in the treatment of laryngeal carcinomas. Materials and Methods: Retrospective study of 62 patients treated with CO2 laser from June 1988 to November 2000 at Seoul National University Hospital for laryngeal squamous cell carcinoma. All patients were treated with curative intention. Fifty three untreated patients with laryngeal carcinoma (39 glottic and 14 supraglottic carcinoma patients) had primary carbon dioxide laser microsurgery. Nine radiation failure patients were treated. Postoperative radiotheray was done for 17 patients. Neck dissection was performed simultaneously for 4 supraglottic cases with cervical nodal metastasis. Mean follow-up duration was 40 months. Results: In primary laser surgery group, distribution of tumors (American Joint Committee on Cancer, 1997) were 38 cases with Tl, 13 cases with T2, 2 cases with T3. Cure rate was 88.7%(47/53) and local control rate was 92.5%(49/53). Larynx was preserved in 94%(50/53) of patients. The overall 5-year survival rate(Kaplan-Meier) was 81.5%. In radiation failure group, 56% of patients were recurred after laser surgery. Conclusion: Laser surgery could be a better treatment modality for early laryngeal cancers and selected advanced cases. Additional radiation therapy should be considered if resection margin is not satisfactory.

어깨 통증을 주증상으로 내원한 경부 해면상 림프관종 1례 (A case of cavernous lymphangioma causing shoulder pain)

  • Cavernous lymphangioma is a rare congenital malformation that usually appears in the early childhood. The most common site is head and neck area, where approximately 75% of all lymphangiomas occur. We present a cavernous lymphangioma abutting brachial plexus and causing shoulder pain. A 28-year-old male patient presented with right shoulder pain for 2 months. Neck MRI revealed a lobulated multiseptated cystic mass at the anterior superior aspect of the right neck. Inferior, medial aspect of the mass was abutting brachial plexus. Surgical excision was performed, and pathologic result with immunohistochemical analysis confirmed the diagnosis cavernous lymphangioma.

후경부에 발생한 광범위 골화를 동반한 모기질종 1예 (A Case of Pilomatricoma with Extensive Ossification Arising the Posterior Neck)

  • The pilomatricoma is a benign skin neoplasm arising from hair follicle matrix cells, and it comprises approximately 0.12% of all benign skin tumors. It occurs most commonly in head and neck. It usually tends to occur in childhood or school-age. Mainly, it exists in single subcutaneous or intradermal nodule and grows very slowly. The diagnosis is confirmed by histopathologic examination. Histopathologically, it is characterized by basaloid cells, enucleated shadow cells and calcifications in the shadow cell. However, it is rare to present extensive ossification in pilomatricoma. The treatment of choice is wide local excision. A 66-year-old man came to our clinic and he complained of two masses on right parotid tail and posterior neck. We performed excisional biopsy on posterior neck and partial parotidectomy. The pathology of former was pilomatricoma with extensive ossification. We report a rare unique case of pilomatricoma with brief review of literature.

갑상선 혈종의 보존적 치료 1예 (A case of Thyroid Hematoma Managed by Conservative Treatment)

  • Thyroid hematoma secondary to a blunt trauma is a rare problem. Despite the rarity, it can be a life-threatening condition with tracheal compression. Both surgical exploration and conservative management have been suggested for thyroid hematoma. However, there is still controversy on the optimal treatment. A 67-year old man who progressed severe dyspnea and neck swelling was transferred to the emergency department with a blunt anterior neck trauma after traffic accident. Contrast enhanced neck computed tomography scan showed huge hematoma within the right thyroid gland and slight tracheal deviation without prominent airway obstruction. One day later, anterior neck swelling was aggravated and the patient was intubated to prevent airway obstruction. After 3 days, hematoma resolution was revealed and extubation was done. We report this case with a review of literature.

이소성 갑상선에 발생한 역형성 암종 1예 (A Case of Anaplastic Carcinoma Arising Ectopic Thyroid)

  • Ectopic thyroid glands generally occur in the midline as a result of abnormal median migration, and their presence lateral to the midline is extremly rare. We report a 83-year-old male with anaplastic carcinoma admix papillary carcinoma in the extrathyroid area. We suspected left lateral neck metastasis on preoperative fine needle aspiration result and computed tomography. the patient underwent total thyroidectomy and left selective neck dissection(level II, III, IV, V). The patient was diagnosed as having an ectopic thyroid gland on the lateral neck with anaplastic carcinoma admix papillry carcinoma. The patient is alive without incident of tumor recurrence at 5month after surgery and radiotherapy.

낭포성 경부임파전이암 (Cystic Metastasis in the Neck from Pharyngeal Cancer)

  • Cystic metastasis in the neck from pharyngeal cancer has often been mistaken for either primary squamous cell carcinoma of branchiogenic origin or branchial cleft cyst. The distinctive histological and clinical features of cystic metastasis reviewed after its correct indentification can lead to the discovery of an unsuspected primary lesion and result in specific treatment options. Recendy, the authors experienced three cases of cystic metastasis in the neck from pharyngeal cancer ; one was from nasopharyngeal squamous cell carcinoma and the other two were from tonsillar squamous cell carcinomas. This report summarizes our experiences and review of the literatures.

갑상선 종양을 제외한 경부중앙 종물에 대한 임상적 고찰 (Evaluation of Midline Neck Masses Except Thyroid Tumors)

  • Midline neck masses have numerous origin and it is important to diagnose correctly for management. A clinical analysis of 29 cases of midline neck masses confirmed by histopathological examination was done retrospectively during the last 5 years. The results were followings; 1) Of 29 cases, thyroglossal duct cyst was most frequent(17 cases, 58.6%) and non-specific lymphadenopathy was the next(4 cases, 13.8%). 2) Midline neck masses were most frequent on the suprahyoid area(12 cases, 41.4%) and hyoid area was the next(7 cases, 24.1%). 3) Two thyroglossal duct carcinoma was included in 17 thyroglossal duct cyst. 4) Seventy percent of thyroglossal duct cyst was present on hyoid and infrahyoid area.

  • Liposarcoma is a malignant tumor that occurs in adipocytes, accounting for 5% of all sarcomas. Generally, it has no symptoms and it occurs in any parts of the body. There are various types of liposarcoma. Of these, 40-45% are known as highly differentiated liposarcoma, and highly differentiated liposarcoma is also referred to as atypical lipomatous tumor. We report a case of an atypical lipomatous tumor on the left neck.
