The Clinical Comparison between Open Surgery and Percutaneous Endoscopic Lumbar Discectomy in Extraforaminal Lumbar Disc Herniation

  • Objective : The objective of this study is to evaluate and compare the clinical results between classical open surgery and percutaneous endoscopic lumbar discectomy[PELD] in cases of extraforaminal lumbar disc herniation. Methods : We obtained the clinical data from 47 patients with extraforaminal lumbar disc herniations who underwent the open paramedian muscle-splitting approach or percutaneous endoscopic lumbar discectomy[PELD] between January 2001 and February 2004. This study consisted of patients with soft disc extraforaminal herniations. The patients were assessed with the visual analogue scale[VAS] and MacNab's criteria before surgery, just before discharge, and postoperative 1 year. Results : There were 25 cases in the open surgery group and 22 cases in the PELD group. The average operating time was $78{\pm}36.71\;min$ in the PELD group and $110{\pm}29.68\;min$ in the open surgery group. The mean hospital stay was $2.73{\pm}2.62$ days in the PELD group and $7.68{\pm}2.59$ days in the open surgery group. VAS decreased from $8.34{\pm}0.66$ to $1.29{\pm}2.27$ in the PELD group and from $8.40{\pm}1.40$ to $1.70{\pm}1.72$ in the open surgery group at the one-year follow-up. The success rate of PELD was 86.4%, compared with 80.0% for open surgery. However, there were no statistically significant difference between two groups for success rate and VAS. Conclusion : Although the success rates were similar in both groups, PELD is less invasive, faster, and safe procedure than open surgery for extraforaminal disc herniation in selected patients.

물질주의 가치에 따른 가방 구매행동 연구 (A Study on Bag Purchase Behaviors according to Materialism Value)

  • This study aimed to investigate bag purchase behaviors according to materialism value. The subjects were 443 male and female adult consumers in their 20s to 50s. The research method was a survey, and the questionnaire consisted of questions on materialism value, bag purchase behaviors, and demographic characteristics. For data analysis, descriptive statistics, Cronbach's α, χ2 test, factor analysis, cluster analysis, ANOVA, and Duncan's multiple range test were performed. The results of this study were as follows. First, materialism value was derived from three factors (happiness pursuit, possession-oriented, and success judgment). Second, subjects could be divided into three groups (happiness pursuit group, success judgment pursuit group, and immaterialism group) based on the materialism value variable. Third, the derived groups showed many differences in bag purchase behaviors. The happiness pursuit group considered all bag evaluation criteria factors (practicality, aesthetics, economy, symbolism) and bag purchasing information sources factors (mass media and personal sources) more than other groups, and showed a tendency to prefer select shops and complex shopping malls as bag purchasing places. In addition, the average annual cost and frequency of purchasing bags of this group were higher than those of other groups. The success judgment pursuit group considered symbolism as a bag evaluation criteria more than other groups, and considered personal sources as bag purchasing information sources more than mass media sources, and preferred luxury stores and department stores as bag purchasing places. On the other hand, the immaterialism group considered practicality and aesthetics as bag evaluation criteria and placed less importance on all information sources than other groups, and preferred Internet shopping malls as purchasing places. This group had the lowest average annual purchase cost and frequency among the three consumer groups. This study suggested that materialism value is a useful variable to segment male and female adult consumer markets effectively, and to understand the bag purchase behaviors of consumer groups divided by materialism value.

  • This study focused on analyzing the relations of empowerment recognized by a five star hotel culinary team in job satisfaction, organizational commitment, and outcome. The purpose of this survey is to show a desirable culinary organization in hotel management. This survey was shared by two groups recognized as high empowerment and low empowerment groups, for job satisfaction, success, organizational commitment, and outcome measurements. This study presented the difference between recognizable high empowerment and the effects of low empowerment groups which showed positive effects on the high empowerment group compared with the low empowerment group. This means that positive effects on the three main factors of job satisfaction, success, organizational commitment, outcome measurements, and hotel culinary organizations are required empowerment for the new organization of management.

  • 국내 ASP(Application Service Provider) 산업은 지난 10년 동안 정부 주도하에 중소기업을 대상으로 급 성장하였다. 그러나 최근 ASP 서비스 업체의 지속적 적자 및 이용업체의 가입 정체 등으로 ASP 산업이 침체되고 있다. 따라서 본 논문에서는 ASP 시장 규모 및 시장성장률의 BCG 매트릭스를 이용하여 핵심 서비스 군을 도출하고 이 서비스 군을 대상으로 집중 인터뷰(Focused Group Interview)를 통하여 핵심 성공 요소를 파악하여 ASP 산업의 활성화 방향과 차세대 ASP로 지칭되는 SaaS(Software as a Service)로의 발전방향을 모색하고자 한다.

교정치료 고정원으로서 Microscrew implants의 성공률에 대한 임상적 연구 (Clinical study on success rate of microscrew implants for orthodontic anchorage)

  • 치아이동의 고정원으로 사용되고 있는 miniscrew and microscrew implants의 임상 적용의 유용성을 알아보고자 73명의 환자에 식립된 180개를 대상으로 성공율을 연구하였다. 1년 기간의 전반적인 성공율은 $93.3\%$(180개의 miniscrew and microscrew implants중 168개 성공)이었고, UB군(상악 헙측치조골)은 $96.6\%$(92개중 87개 성공), LB군(하악 혐측치조골)은 $96.6\%$(58개중 56개 성공), UP군(상악 구개측 치조골)은 11개중 11개가 모두 성공하여 $100\%$를 보였고, LR군(하악 후구치 부위)은 19개중 14개가 성공하여 $73.3\%$의 성공율을 보였다. 이 연구를 통하여 볼 때 miniscrew and microscrew implants는 교정치료의 고정원으로 성공적으로 사용될 수 있을 것으로 생각된다.

한국 차량공유사업의 성공요인 사례분석 (The Case Study on the Success Factors of Korean Car Sharing Business)

  • 본 연구에서는 한국의 카셰어링 기업(쏘카, 그린카)의 핵심성공요인을 분석하고, 한국의 특수한 경제, 규제 환경 속에서 형성된 공유경제 서비스의 성장과 관련하여 자동차 제조사의 대응전략을 분석하였다. 본 연구의 결과는 다음과 같다. 쏘카와 그린카 모두 창업기에는 '적시의 시장 진입', 초기 성장기에는 '외부자원의 활용'이 핵심성공요인으로 파악되었다. 고도성장기의 핵심성공요인은 두 회사의 사업방향의 차이에 따라 각기 다르게 나타났다. 독립 벤처기업으로 B2C 사업으로 외형성장에 집중한 쏘카는 '고객밀착형 마케팅'과 '충분한 자본조달'이, 대기업 계열사로 편입되어 내부 자원활용에 집중한 그린카는 '진입전략(B2B시장 확대)'이 핵심성공요인으로 파악되었다. 고도성장기의 공통적 핵심성공요인으로 '외부자원의 활용'과 '대기업과의 공생'으로 분석되었다. 본 연구에서 분석한 현대자동차그룹의 카셰어링 대응 전략은 크게 협력, 투자 그리고 직접운영(딜카)의 형태로 나타났다. 대응전략의 목적은 단기적으로는 카셰어링 서비스 업체와의 협업을 통한 테스트베드 운영 등으로 나타났고, 중/장기적인 관점에서는 이용자 데이터 수집, 이를 통한 신규 모빌리티 서비스 기획 등과 함께 미래의 자율주행 모빌리티 서비스 시장 선점이 중요한 목적으로 파악되었다.

Effect of relative head position on the anesthetic efficacy of inferior alveolar nerve block during endodontic treatment of patients with irreversible pulpitis

  • Background: The purpose of this prospective randomized single-blind clinical trial was to evaluate the effect of tilting the head on the anesthetic efficacy of inferior alveolar nerve block (IANB) in patients with symptomatic irreversible pulpitis. Methods: Ninety-two patients were divided into two groups: the first group received IANB and the head was tilted in the direction of the block for 15 min, whereas the second group received IANB and the head was tilted to the opposite side. Access cavity preparation was initiated after 15 min. Success was defined as no pain or faint/weak/mild pain during endodontic access preparation and instrumentation. The anesthetic success rates were analyzed by Pearson chi-square test at 5% significance levels. Results: The same side position and opposite side position yielded 41% and 30% anesthetic success rates, respectively; there was no significant difference between the two sides. Conclusions: Relative head position has no effect on the anesthetic success rate of IANB.

  • 그룹 멀티캐스트는 그룹에 속한 모든 멤버들이 그룹 전체에 데이터를 송신할 수 있는 다자간 통신 방법이며. 그룹 멀티캐스트를 위한 라우팅 알고리즘들은 QoS(예: 대역폭) 제약 조건을 만족하면서 모든 그룹 멤버를 포함하는 멀티캐스트 트리의 구성을 목적으로 하고 있다. 현재가지 제안된 방법들로 소스 트리 구성 방법과 공유 트리 구성 방법이 있는데, 소스 트리 구성 방법은 그룹 크기에 따른 놀은 트리 관리 오버헤드와 낮은 확장성 문제를 가지고 있고 공유 트리 구성 방법은 트리 구성 성공률이 낮다는 단점을 가지고 있다. 본 논문에서는 그룹 멀티캐스트를 위하여 구성되는 트리의 수가 망 부하에 따라 변화하는 휴리스틱 알고리즘을 제안한다. 시뮬레이션을 통하여. 제안하는 알고리즘이 공유 트리 구성 방법에 비하여 월등히 놀은 수준의 트리 구성 성공률을 보장할 뿐만 아니라. 소스 트리 구성 방법에 비하여 현저히 낮은 트리 관리 오버헤드로 동일한 수준의 트리 구성 성공률을 보장함을 확인할 수 있다

  • The combined use of smart materials, complementing each others' characteristics and resulting in devices with optimised features, is providing new solutions in many industries. The use of ingenious combinations of smart materials has led to improvements in actuation speed and force, signal-to-noise ratio, sensor precision and unique capabilities such as self-sensing self-healing systems and energy autonomy. This may all give rise to a revival for numerous families of smart materials, for which application proposals had already reached a stationary situation. It may also provide the boost needed for the definitive industrial success of many others. This study focuses on reviewing the proposals, preliminary studies and success cases related to combining smart materials to obtain multifunctional, improved systems. It also examines the most outstanding applications and fields for the combined use of these smart materials. We will also discuss related study areas which warrant further research for the development of novel approaches for demanding applications.

The comparison of two different intraarticular injections using a sonographic anterolateral approach in patients with osteoarthritic knee

  • Background: The intraarticular (IA) injection has become popular for the management of the osteoarthritic knee without an effusion. The success rate of IA injection would be better if it was able to be visually confirmed. We hypothesized that an anterolateral approach, which targets the synovial membrane of the lateral condyle using ultrasound, would provide an equivalent alternative to the anterolateral approach, targeting the synovial membrane of the medial condyle for IA injection of the knee. Methods: A total of 96 knees with osteoarthritis were randomized placed into the two groups, which were group I (anterolateral approach to the medial condyle) and group II (anterolateral approach to the lateral condyle). The primary outcome was to compare the success rate of the two methods of IA injection. The required length of the needle for injection was also measured and compared. Pain intensity was assessed using the Numeric Rating Scale in order to evaluate the success of injection. Results: There were no significant differences in the success rate between both groups. The success rate of group I and group II were 87.8% (95%, CI 78.7-97.0) and 91.5% (95%, CI 83.6-99.5), respectively (P = 0.549). The needle depth was $5.0{\pm}0.8$ (3.0 to 6.1 cm) in group I, and $3.0{\pm}0.8$ (1.5 to 5 cm) in group II (P < 0.001). Conclusions: The anterolateral approach to the lateral femoral condyle, using ultrasound, is an alternative method to the approach targeting the medial femoral condyle, using shorter needle.
