EFFECT OF THE CHLORHEXIDINE AND FLUORIDE-CONTAINING VARNISH ON THE LEVEL OF SALIVARY MUTANS STREPTOCOCCI (Chlorhexidine과 fluoride 성분의 varnish가 타액 내 mutans streptococci 수에 미치는 효과)

  • The objective of the present prospective trial was to compare the efficacy of chlorhexidine(Cervitec), Fluoride (Fluorprotector) and combination of chlorhexdine and fluoride varnishes in decreasing the level of salivary mutans streptococci. Forty healthy students of school of dentistry, Chonbuk national university were investigated to evaluate mutans streptococci(MS) counts and randomized into four groups to treat with the experimental varnishes; A) untreated group(n=10), B) chlorhexidine group(n=10), C) fluoride group(n=10), D) chlorhexidine and fluoride group(n=10). Dentocult $SM^{(R)}$ (Orion Diagnostica) strip method was used for measurement of the level of mutans streptococci in saliva. Stimulated saliva were collected at baseline for mutans streptococci counts evaluation (ms1), 12 weeks later the completion of each varnish treatment, mutans streptococci counts were re-evaluated. In varnish group with chlorhexidine, fluoride and combination of chlorhexidine and fluoride, the level of mutans streptococci was lower after 12 weeks than at baseline, but there were no significant differences in saliva(p>0.05), when compared with baseline. After 12 weeks, a remarkable reduction was still found in the subjects with high level of mutans streptococci at baseline, but not different in the low and moderate level of mutans streptococci(p>0.05).

  • Beta-hemolytic streptococci (BHS) are frequently isolated pathogens in clinical microbiology laboratories. Among them, Group C, G and F streptococci infrequently cause respiratory infections, but they often invade other sites. Patients with underlying diseases are prone to be infected by the organisms and some of them can be fatal. Therefore, combination tests of serological and biochemical tests are needed to correct the identification of non-A, B streptococci because to various species belong to this species. The aim of this study was to investigate the isolation rates of strains and distribution of serogroup C, G and F streptococci at Wonju Hospital during the period of 2003-2004. 133 clinical isolates of group C, G and F streptococci were examined. Of them, 13 (9.8%) were group C, 41 (31.8%) were group F, 79 (59.4%) were group G. The prevalent isolation of the strains by serogroup were: group C, Streptococcus constellatus (38.5%); group F, Streptococcus anginosus (56.1%), Streptococcus constellatus (26.9%); group G, Streptococcus anginosus (54.4%), Streptococcus dysgalatiae subsp. equisimilis (26.5%). Overall, the most frequently isolated BHS was Streptococcus anginosus (51.2%).

Molecular Discrimination of Mitis Group Streptococci Isolated from Koreans using RpoB Nucleotide Sequences

  • Mitis group streptococci (MGS) were classified based on the nucleotide sequences 16S rRNA gene (16S rDNA) and comprised 13 Streptococcus species. However, 16S rDNA homogeneity among MGS was too high to discriminate between clinical strains at the species level, notably between Streptococcus mitis, Streptococcus oralis, Streptococcus pneumoniae, and Streptococcus pseudopneumoniae. The purpose of this study was to discriminate between 37 strains of MGS isolated from Korean oral cavities using phylogenetic analysis of the DNA-dependant RNA polymerase beta-subunit gene (rpoB). 16S rDNA and rpoB from clinical strains of MGS were sequenced using the dideoxy chain termination method and analyzed using MEGA version 5 software. The resulting phylogenetic data showed that the rpoB sequences could delineate clinical strains of MGS at the species level. Phylogenetic analysis of rpoB is therefore a useful approach for identifying MGS at the species level.

  • A Total of 173 streptococci isolated from 536 dairy cattle in Gyeonggi Province identified by Lancefield serological reaction and other biological properties. Antibiotics sensitivity test was also examined and the results obtained were as follows: 1. A total of 173 isolates of 167 strains (96.5%) were reacted with group A,B,C,D,E and G antisera. 2. Of the 167 positive strains, 75 strains were identified to Streptooccus uberis, 56 to Streptococcus agalactiage and 26 to Streptococcus dysgactiae, respectively. Also, Sreptococcus pyogenes, group D and group G streptococci were rarely isolated. 3. In results of antibiotic sensitivity tests, the most isolates were sensitive to leucomycin and erythromycin but less sensitive to streptomycin. and kanamycin.

THE ANTICARIOGENIC EFFECTS OF XYLITOL ON MUTANS STREPTOCOCCI IN CHILDREN WITH MIXED DENTITION (혼합치열기 어린이에서 mutans streptococci에 대한 자일리톨 껌의 항균효과에 관한 연구)

  • The purpose of this study was to investigate the effect of xylitol chewing gums on mutans streptococci (MS) counts in saliva. Sixty two children 6 to 11 years old were randomly assigned into one of three groups. Stimulated saliva specimens were plated in duplicate on conventional selective culture (mitis salivarius kanamycin bacitracin agar) for mutans streptococci. Polymerase chain reaction (PCR) amplication was performed to identify MS. After the 4-month period a significant decrease of the MS counts occurred in the group B (two gum 3 times a day; P < 0.05) but not in group A (one gum 3 times a day) and control group (didn't receive xylitol gum). According to qualitative evaluations, xylitol use did reduce the levels of MS in mixed dentition children. It has been suggested that a daily intake of 2 tablet for 3 times a day (about 10g) is needed in order to obtain a clinical anticariogenic effect.

The Causative Organisms of Pediatric Bacterial Meningitis: A Single Center Experience from 1996 to 2005 (단일기관에서의 소아 세균성 수막염 원인균 분포 (1996~2005))

  • Purpose : We investigated the causative organisms of bacterial meningitis by age distribution from 1996 to 2005. Methods : Retrospective data were obtained from the medical records with diagnosis of bacterial meningitis or neonatal meningitis from 1996 through 2005. A case was defined by isolation of organism or detection of its antigen by latex agglutination from cerebrospinal fluid. Results : A total of 46 cases(27 neonates and 19 children) were identified. 15 of 27 episodes(55.6%) of neonatal meningitis had a concomitant-positive blood culture. Group B streptococci were the most common bacterial causes of neonatal meningitis(44.4%). Nine of 12 episodes(75.0%) were late-onset infections in neonatal meningitis caused by group B streptococci. 16 of 19 children(84.2%) with bacterial meningitis beyond the neonatal period were younger than 5 years of age(median age, 23 months). Of 19 cases, 8 infections were with Streptococcus pneumoniae, 8 were with Haemophilus influenzae and 3 were with Neisseria meningitidis. Since 2001 there was no case of meningococcal meningitis in this study. Conclusion : In neonates group B streptococci are the most common causative organisms of bacterial meningitis, especially late-onset infections. In infants and young children, the predominant causes of bacterial meningitis are H. influenzae and S. pneumoniae; meningitis caused by the former are likely to decrease after the introduction of the conjugate vaccine for H. influenzae type b.

  • 1. Purpose The accurate diagnosis and proper treatment of group A streptococcus should be emphasized concerning about possible development of late sequelae, such as acute rheumatic fever and acute glomerulonephritis. We would like to know the recover rate of beta-hemolytic streptococci by throat swab culture at the in-patient 2. Methods The throat swab cultures and filled up flow-sheets were undertaken on 619 children who had admitted to hospital, KyungHee university hospital from may 1994 to april 1995 prospectively. At the same time on admission, throat culture was performed. 3. Results The highest recover rate of BHS(Beta-Hemolytic Streptococci)and GAS(Group A Streptococci) were seen in above 10 years old, as 9.1% and 1.9%. BHS were obtained in 39 cases(6.3%) among 619 children while GAS was obtained in 3 cases (0.4%). Among 39 specimens of BHS, 33 specimens were classified as non-grouping streptococcus. 4. Conclusion The poor recovery rate of GAS inpatient compared with normal carrier rate is likely due to possible antibiotic abuse, errors in processing samples, and epidemiologic factors such as seasons and geographic areas. It is necessary to evaluate the clinical significance of non-A,B,C,G streptococcal infections and carriers.

  • Authors have experienced a case of neonatal sepsis whose organisms were group B streptococci. This case was early-onset type and the two premature babies were expired. Autopsy was done but no specific findings were shown in pulmonary parenchyma except focally mild atelectasis and dilated bronchiols and alveoli, and distictive hyaline membrane or aspirated materials are not identified. A brief review follows.

  • Identification of group A $\beta$-phemolytic streptococci is very important to provide an appropriate preventive measure of possible rheumatic fever and acute glomerulonephritis. For such purpose bacitracin susceptibility of streptococci because of its simplity has been most widely used despite of its occasional faulty results. Recently, a coagglutination technique was advocated using streptococcal group specific antibodies adsorbed to protein A-containing staphylococci. This study was conducted to evaluate the coagglutination technique using reagents prepared by ourselves. The specificity, reproducibility and stability were ascertained and the following results were obtained. 1. The identification by coagglutination technique using our own reagent gave the same results compared with the Lancefield precipitation technique. The result also agreed with the Phadebact grouping. 2. There were no variation in group A and B identification due to lot difference. However, there were a few discrepant results in group C and G identification which was conducted in different days with different lots of our reagent. 3. The stability of our reagents was less satisfactory compared to the commercial product. An effort to improve the stability was considered necessary. 4. For coagglutination, it was found convenient to use supernatant of Todd-Hewitt broth incubated for 24 hours. Both parafin-ringed slide glass and RPR card gave comparable results and the former could be used when the latter is not available.

Group B Streptococcal Disease in Korean Neonates (한국 신생아에서 B군 사슬알균 감염증)

  • Group B streptococcus (GBS) is the leading cause of neonatal sepsis and meningitis in developed countries. This article reviews the neonatal invasive GBS disease, maternal GBS colonization, and prevention strategies in the context of recent epidemiological changes in Korea. Although Korean neonates had been supposed to have low incidence of invasive GBS disease, GBS has been recently reported to be the most common cause of invasive neonatal infection after 1990s. Among Korean pregnant women, GBS carriage rate in the vagina and rectum has been reported to be much lower than that in Western countries. However, it has increased in recent studies. For decision making about preventive strategy for neonatal GBS disease in Korea, further studies are required in terms of the incidence of neonatal GBS infection and serotype distribution. In addition, studies about maternal carriage rate and serotype distribution have to be continued.
